상상훈련이 아급성뇌졸중환자의 상지기능 및 일상생활수행능력에 미치는 영향 (Imagery training effects of Upper limb function and Activities of daily living in Subacute stroke patients)

  • 본 연구는 아급성뇌졸중환자의 상지기능과 일상생활수행능력에 대한 상상훈련의 효과를 알아보기 위해 시행되었다. 연구대상자들은 총 16 명으로 상상훈련군과 대조군에 8 명씩 무작위로 할당되었다. 상상훈련군은 4주 동안 주 5회, 매일 30의 상상훈련과 30분의 과제지향훈련을 시행하였고 대조군은 4주 동안 주 5회, 매일 30분간 과제지향훈련을 시행하였다. 측정은 상지기능의 변화를 알아보기 위해 울프운동 기능검사(Wolf motor function test, WMFT)와 Fugl-meyer 운동기능평가(Fugl-Meyer motor function assessment, FMA)를 측정하였고, 일상생활수행능력의 변화를 알아보기 위해 수정된 바델 지수(modified Barthel index, MBI)를 사용하여 측정하였다. 본 연구의 결과는 상상훈련이 대조군에 비해 모든 검사에서 더 유의한 향상을 보였다(p<.05). 그리고 훈련 전후의 효과크기는 상상훈련을 시행하였을 때 WMFT와 FMA는 각각 1.59, 2.02로 큰 효과를 나타냈으며, MBI는 0.37로 최소의 효과를 나타났다. 이러한 결과는 상상훈련이 상지기능과 일상생활수행능력 향상에 도움이 될 수 있으며, 상상훈련의 임상적용 가능성을 지지한다.

뇌졸중 환자를 위한 일상생활활동 평가도구에 관한 체계적 고찰 (A Systematic Review of Assessment Tools of Activities of Daily Living for Stroke Patients)

  • 목적: 본 연구는 뇌졸중 환자를 위한 일상생활활동 평가도구에 관해 체계적 고찰함으로써 임상 및 연구에서 자료로 활용되고자 하였다. 연구방법: 2001년부터 2016년까지 학회지에 게재된 논문을 검색하기 위하여 Pubmed, Embase, Cochrane, NDSL 데이터베이스를 사용하여"stroke AND activities of daily living AND occupational therapy AND assessment OR evaluation OR measurement"를 검색하여 체계적 고찰을 하였다. 평가도구의 종류와 사용빈도를 분석한 후 이에 대한 경향을 파악하였다. 또한 국제 기능 장애 건강 분류(International Classification of Functioning, Disability and Health; ICF) 모델의 분류 기준에 맞추어 활동과 참여에 대한 평가도구를 분류하였다. 결과: 총 111개의 논문이 최종적으로 분석되었으며, 30개의 평가도구가 확인되었다. 최근 들어 뇌졸중 환자를 대상으로 한 연구 게재편수가 증가함에 따라 평가도구의 종류와 사용빈도가 증가하였다. 가장 많이 사용되는 평가도구는 기능적 독립성 측정(Functional Independence Measure; FIM), 바델 지수(Barthel Index; BI)와 캐나다작업수행측정(Canadian Occupational Performance Measure; COPM)이었다. ICF 모델의 분류 기준에 따라 분석한 결과 대부분의 도구는 활동에 대한 평가도구로 참여에 대한 평가도구 수는 매우 적었다. 결론: 본 연구는 뇌졸중 환자의 일상생활활동 수행능력을 측정하기 위해 사용된 평가도구를 체계적으로 고찰하였다. 연구 결과 FIM, BI와 같은 특정한 도구가 많이 사용되었다. 앞으로는 평가도구의 특성에 맞춰 적절한 평가가 이루어 질 수 있도록 다양한 평가도구를 사용한 연구가 진행되어 근거기반의 작업치료를 뒷받침 해 주어야 한다. 또한 ICF 모델의 분류 기준 중 참여에 대한 평가도구의 개발이 이루어져야 할 것이다.

  • 본 연구는 뇌졸중환자를 대상으로 동작관찰 디지털콘텐츠 활용교육과 동작관찰 없이 과제 지향적 작업치료를 실시하여, 뇌졸중 환자의 상지기능과 일상생활수행력에 미치는 영향에 대해 알아보았다. 뇌졸중 환자 30명을 대상으로 무작위로 실험군(동작관찰)과 대조군(과제 지향적 작업치료)으로 각각 15명씩 나누어 1주일에 5회 총 30분씩 5주간 실시하였다. 치료 중재 전과 후의 마비측 상지기능을 알아보기 위해, Jebsen-Taylor Hand Function Test (JTHFT)과 Box and Block Test (BBT)를 실시하였고, 일상생활수행을 비교하기 위해 Korean-Version of Modified Barthel Index (KMBI)를 실시하였다. 실시 결과 두 그룹 모두에서 상지기능 및 일상생활수행은 증가하였고, 그룹간의 차이는 보이지 않았다. 그러므로 동작관찰 디지털콘텐츠 활용교육과 과제 지향적 작업치료는 뇌졸중환자의 상지기능 및 일상생활수행에 긍정적인 영향을 미친다는 것을 알 수 있었다.

독거노인의 외로움과 수면의 관계 (Association between Sleep and Loneliness in the Elderly Living Alone)

  • 연구목적 외로움과 수면의 질 저하는 노인에게서 흔히 나타나는 증상이다. 하지만 우리나라에서 노인의 외로움과 수면의 관계에 대해서는 연구된 바는 거의 없다. 본 연구는 독거노인의 외로움과 수면의 질의 연관성을 보고자 하였다. 방 법 본 연구는 천안 지역에 거주하는 독거노인 1,090명을 대상자로 시행되었다., 훈련된 간호사의 면담과 외로움 척도, 피츠버그 수면의 질 지수(Pittsburgh Sleep Quality Index, PSQI), 인구사회학적 정보, 신체 건강상태(Cumulative illness rating scale, CIRS), 인지기능, 단축형 노인우울척도 등의 자가설문지를 통해 조사하였다. 통계분석은 인구학적 변인과 외로움 척도 간의 관계를 탐색하기 위해 기술적 통계분석을 시행하였으며 외로움과 수면의 질의 연관성을 확인하기 위해 단변량 및 다중회귀분석을 사용하였다. 결 과 외로움군의 평균 PSQI 점수(9.2±4.2)는 정상군(7.3±3.7)에 비해 통계적으로 유의하게 높았다(student-t test, p<0.001). 성별, 기분장애 및 상관분석에서 유의성을 보인 변수를 통제한 다중회귀분석에서 외로움은 전체 PSQI 총점(standardized β=0.065, p=0.025), 수면방해(standardized β=0.086, p=0.005), 수면제 사용(standardized β=0.065, p=0.034)과 유의한 연관성을 보였다. 결 론 외로움은 수면문제와 연관성을 보였으며 이는 독거노인의 수면의 질 저하에 외로움이 위험요인이 될 수 있음을 의미한다. 독거노인의 외로움을 줄이기 위한 개입은 수면의 질을 호전시키고 신체 및 정신 질환을 줄일 수 있을 것이다. 따라서 독거노인의 외로움을 예방하고 줄일 수 있는 지역사회 기반의 보건복지 정책이 필요함을 시사한다.

C.M.I.간이법에 의한 노인들의 건강수소율 (Health Complaints of Elderly Persons Using a Modified C.M.I.)

  • The explosion of the aging population is changing the social environment of today's older people. Traditionally in Korea, a large percentage(over 90%) of elderly parents have lived with their married first son. But today, the number of elderly who live with their married sons has decreased(65.6%) The number of those who live in a different situation such as with their married daughter, unmarried offspring, with a spouse or alone has increased (34.4%) We can expect that the number of the elderly who have to live in institution will increase. The objective of this investigation was to determine differences in the number of health Complaints of older people according to their living situation with a view to planning more effective health care. The sample consisted of 231 persons over 65 years of age, 60 living in an Old Age Home ana 171 living in their own home in Kwangju. Data were gathered from July 9 to 26, 1983 by nursing students using a Questionnaire which was a modified form of the Cornell Medical Index. The data were categorized according to the subjects, living, sex, educational level, previous occupation, hobbies and sexual activity. The date were analyzed for statistical significant differences using F and X²tests. findings included the following: 1. There was a higher number of health complaints from persons who live in the institution than those living at home, but the difference was not significant. 2. The highest number of health Complaints were from persons who live alone, followed by those living with their daughters, and then by those living in the institution. Persons who live with their sons had the least Complaints. The difference in the number of Complaints accord-ing to with whom they were living was significant. 3. Women had signincantly more Complaints than men. Persons who were not living with their spouses had significantly more complaints than those living with their spouses.4. The higher eductional level the persons had, the less health Complaints they had. The number of Complaints accoraing to educational level was significantly different. 5. The highest number of health complaints were from persons who had involved in Commerce and industry, followed by those in Agriculture. Persons who were civil servant had the least 6. There were more complains from persons who had no hobby than those with hobbies. The complaints. The difference was significant. difference was significant. 7. Persons who said they were sexually inactive had significantly more complaints than those who said they were sexually active. As age increased, sexual activity significantly decreased. Those who lived with their spouse were significantly more sexually active. 8. The highest number of Somatic Complaints were eye fatigue, followed by nocturnal frequency, lumbago, cramps in extremities, vertigo, stiffness in Shoulder, tinnitus, common cold and constipation. The order of Psychic Complaints from higher to lower were anger. sensitivity, anxiety, depression and loneliness. 9. This group of Elderly persons said they valued Health the most, followed by Harmony, Religion, Money and Honor.

뇌졸중(腦卒中) 환자(患者)의 기능평가방법(機能評價方法)에 대(對)한 연구(硏究) (Study on function evaluation tools for stroke patients)

  • Our conclusions for function evaluation tools of Stroke patients are as follows. 1. Evaluating tools of Activities of Daily Living, Katz Index, Barthel Index, Modified Barthel Index have high validity and reliability because of ease of measuring, high accuracy, consistency, sensitivity and sufficient stastistics, but they mainly measure motor function except sense, mentation, language, and social conception. Therefore cerebrovascular disease and brain injury in trauma patients with lacked acknowledgement and sensation, we are not able to apply these tools. 2. PULSES Profile is a useful scale for measuring the patient's over-all status, upper and lower limb functions, sensory components, excretary functions, and intellectual and emotional adaptabilities. It is recognized as a good, useful tool to evaluate patient's whole function. 3. Motor Assessment Scale was designed to measure the progress of stroke patients. The scale was supplemented with upper arm function items. We believe that the Motor Assessment Scale could be a useful evaluation tool with inter-rater reliability ,test-retest reliability. 4. The existing evaluation tools, Katz Index, Barthel Index, Modified Barthel Index, PULSES Profile, Motor Assessment Scale, mainly measured the rehabilitational motor function of sequela of cerebrovascular patients. On the other hand CNS & INH stroke scale can measure cerebrovascular disease patient's neurologic deficits and over-all stautus, which are recognition ability, speech status, motor function, sensory function, activities of daily living. Those scales have been recognized as useful tools to measure function of cerebrovascular disease patients and have increased in use. 5. Every function evaluation tool was recognized to have some validity and inter-rater, test-retest reliability in items of each evaluation tool and total scores of each evaluation tools, but it is thought that none of these scales have been fully validated and proved reliable. Therefore afterward, the development of a highly reliable rating system may best be accomplished by a careful comparison of several tools, using the same patients and the same observers in order to choose the most reliable items from each. 6. Ideal evaluation tools must have the following conditions; (1) It should show the objective functional statues at the same time. (2) It should be repeated consecutively to know changed function status. (3) It should be easy to observe the treatment program. (4) It should have the same result with another rater to help rater exchange information with treatment team members. (5) It should be practical and simple. (6) The patient should not suffer from the observer.

QscanTM을 이용한 노인 구강보건교육의 효과 (The effect of oral health education for the elderly using QscanTM)

  • Objectives: The purpose of the study is to investigate the effect of oral health education for the elderly using $Qscan^{TM}$. Methods: This study was a quasi-experiment design of nonequivalent control group pretest-posttest design carried out by oral health education from January 17 to March 7, 2015. The subjects were 64 elderly people over 65 years old who had more than one remaining teeth in the anterior teeth and canines living in Hongseong-gun and assigned to 33 control group and 31 intervention group. The intervention group was measured only by $Qscan^{TM}$. The educational effect between two groups were evaluated using oral health behavior, gingivitis index, and plaque index. After Institutional Review Board from Namseoul N University, the elderly people participated in the study. Results: The intervention group showed lower plaque index of percent reduction than the control group. The two groups showed a significant difference in gingival index after the oral health education(p<0.05), but did not show a significant difference in oral health behavior after the oral health education(p<0.05). Conclusions: The effect of oral health education through the motivation of $Qscan^{TM}$ was very effective in the comparison of oral health behavior, gingival index and plaque index.

류마티스성 관절염 환자의 일상생활활동 조사연구 (A Study of Activities of Daily Living(ADL) in Patients with Rheumatoid Arthritis)

  • This study attempts to provide the rudimentary data that might help patients with Rheumatoid Arthritis manage an independent life by the analysis of their activities of daily living(ADL). The subjects were forty patients who had been diagnosed Rheumatoid Arthritis and under drug therapy from RA clinics of the two university hospitals in Pusan. Data were collected by performing an individual interview for each of them during June 15th through July 15th 1998. Questionaires were imployed to investigate the general characteristics of patients. the traits of Rheumatoid Arthritis, and actual conditions of their activities of daily living. The questionaires utilized especially for the study of ADL is the version of Lee Eun Ok's tool which modified and complemented Katz scale (Katz, Down, Cogh and Grots 1970) and Bathel index(Bathel. 1973). It's consisted of 31 questions that each question is measured by 0-3points. The high score in the questionaire signifies the high independency in the ADL. The data were analyzed by SPSS/Win in terms of frequency, percentage, mean, and standard deviation, close connection with the contents of the personal interview. Total mean of the ADL shows $1.55{\pm}0.68$, indicating the overall activities of daily living is generally dependent. It also indicates that patients, in the condition of dependent activity. tends either to have recourse to drugs or to take negative strategy, getting help from inside or outside family member. Hence, it is necessary that patients are exposed to the introduction and instruction of self-help devies that might help them manage a comparatively independent ADL, easing their paychological burden.

요추 추간판 탈출증으로 인한 요통 및 생활기능저하에 대한 한방 집중치료 치험 1례 (Intensive Korean Medicine Treatments for Low Back Pain and Decreased Living Function due to Herniation Intervertebral Discs in the Lumbar Spine: A Case Study)

  • Background: This case study suggests the potential use of Korean medicine treatments as a conservative management for low back pain and decreased living function due to herniation of intervertebral discs in the lumbar spine. Case Summary: The patient suffered low back pain and decreased living function due to herniation of intervertebral discs of the lumbar spine. Korean medicine treatments, including herbal medicine, acupuncture performed on BL23, BL40, SP6, and GB39, and pharmacupuncture containing Sinbaro, were applied for eight days. The numeric rating scale (NRS) results for low back pain decreased from 6 to 2 and the Oswestry Disability Index (ODI) low back pain scores decreased from 75.56 to 31.11. Walking time also increased from less than one minute to 20 minutes, with an increase in range of motion (ROM) from "Uncheckable" to about normal motion range. Conclusion: Korean medicine treatment can be considered to be an effective conservative management option for pain and decreased living function in patients with herniation of intervertebral discs in the lumbar spine.

익산지역 대학생의 주거형태에 따른 영양소 섭취량과 식사의 질, 식습관 및 식태도 비교 (Comparing the Nutrient Intake, Quality of Diet, Eating Habit Scores and Dietary Behaviors of University Students in Iksan, according to Their Type of Residence)

  • The aim of this study was to compare the dietary patterns and behaviors of university students according to their type of residence. The subjects consisted of two thousand two hundred fifty students from Wonkwang University. They were divided into three groups : those who live in their family homes (393 men, 392 women) ; those who live in university dormitories (371 men, 401 women) : and those who live in houses with cooking facilities (345 men, 348 women). This cross-sectional survey was conducted using a self-administered questionnaire. The nutrient intake data collected from a three-day recall were analyzed using the Computer Aided Nutritional Analysis Program and the diet quality was estimated using the Nutrient Adequacy Ratio, Mean Adequacy Ratio and Index of Nutritional Quality. The results showed that the mean daily intakes of calcium were lower than the Korean Recommended Daily Allowance (RDA) in all groups. There was a significant difference in the nutrient intake, dietary quality and dietary behaviors of the three groups. The nutrients intake and dietary quality of the men were poorer than those of the women in all three groups. The subjects living in dormitories and houses with cooking facilities seemed to have lower levels of nutrient intake and dietary quality. However, the concern about nutrition and interest in health information was higher among those living in dormitories and houses with cooking facilities than among those living in their family homes. Male students living in houses with cooking facilities had more dietary problems than students living in their family homes or in dormitories possibly because they might have had a lower ability in meal management. These results suggest that the type of residence affects the nutrient intake levels, and dietary quality of university students in Iksan. Nutritional education is essential if university students are to practice optimal nutritional habits, including the eating of well-balanced diets and selecting of foods of high quality. Therefore, nutritional education for university students is needed so as to improve their health and a nutritional education program should be developed to meet the various needs of these students.
