A Case Study of Poisoning Symptoms Caused by Misusing Xanthii Fructus, Treated with Korean Medicine (도꼬마리 열매 복용으로 발생한 중독증상(中毒症狀)의 한방치료 1례)

  • This is a clinical report of one patient with mental disorientation, abnormal behavior, spasm, and other symptoms caused by poisoning with Xanthii Fructus, who recovered completely with treatment. The patient was treated with Korean medical treatments, such as herbal medicine and acupuncture. After treatment, various symptoms caused by the Xanthii Fructus poisoning disappeared completely and laboratory findings, such as liver function tests (aspartate transaminase and alanine transaminase), were also close to the normal ranges. This case shows that Korean medicine therapy can be applicable in treating various symptoms caused by Xanthii Fructus poisoning. It is considered that further research is needed to confirm the safe use of Xanthii Fructus.

  • Aminolysis, hydrazinolysis and alkylation of 4-methoxy and 4,9-dimethoxy-6-cyano-7-thione-5-methyl-7H furo [3,2-g] [1] benzopyridine (1 a-b) yielded 7N-substituted furobenzopyridine derivatives (2 a-e or the possible isomers 3 a-e and 4 a-b), (5 a,b and 6 a,b) and the ester (8 a,b). Hydrolysis of (la) with acetic acid gave the corresponding pyridone derivatives (7). Furobenzopyridinyl-7-thioacetyl hydrazide (9 a,b) have been prepared via alkylation of furobenzopyridine thione (1 a-b) with ethyl chloroacetate followed by condensation with hydrazine hydrate. Schiff base (11) was prepared by reacting (9a) with p. N,N-dimethyl aminobenzaldehyde in boiling ethanol. Treatment of (8a) with anthranilic acid gave the corresponding 7-substituted-4H-3,1-benzoxazine-4-one (10). We found that compound (11) increased bleeding, coagulating time, the total count of white blood cells, blood glucose level (cause hyperglycemia), enzymes (GOT, GPT) activities, concentration of urea and creatinine. On the other hand it decreased red blood cells number, haemoglobin content and haematocrite value.

Severe Diarrhea-induced Acute Kidney Injury and Its Consequence in an Elderly

  • Methods: This study presents a comprehensive case study of an elderly male diagnosed with acute kidney injury (AKI) resulting from severe dehydration, supported by an extended follow-up with laboratory findings. Results: An 83-year-old male patient experienced severe diarrhea overnight, leading to hospitalization due to symptoms of dehydration and hypotension. His laboratory results displayed a typical AKI pattern, including a significant increase in creatinine levels (5.19 mg/dL) and the presence of hyperkalemia and hyponatremia. Following general treatments, including the administration of an herbal drug (Bulhwangeumjeonggi-san), the estimated glomerular filtration rate (eGFR) improved from 10 ml/min (Stage 5) to 34 ml/min (Stage 3) within five days when he was discharged. Although subsequent eGFR tests, conducted one and two months later as an outpatient, revealed an improvement of 42 ml/min, the patient still experienced mild chronic dysfunction as a consequence. Conclusion: This study presents a noteworthy case of acute kidney injury attributed to severe dehydration, emphasizing the importance of medical awareness regarding diarrhea-induced kidney function impairment, especially in the elderly population.

A Case Report on the Effect of Short-term Intensive Fat Treatment on an Obese Patient with Metabolic Syndrome (대사증후군 환자의 단기 집중 비만치료 프로그램의 효과에 대한 증례 보고 1례)

  • Objectives: This case study reports on a case of rhabdomyolysis that occurred after spinning exercise. Methods: A patient diagnosed with rhabdomyolysis received Korean medical treatment and Western medical treatment for 6 days. We observed the patient for 13 days. Clinical symptoms were evaluated with the Numerical Rating Scale (NRS) and laboratory tests that included Liver Function Test, Renal Function Test, creatinine phosphokinase (CPK), myoglobin, and urine. Results: The clinical symptoms improved after treatment. In this case, acute renal failure has not occurred. The Laboratory results, including AST, ALT, CPK, and LDH, improved. Conclusions: Rhabdomyolysis can be treated with Korean medical and Western medical cooperative treatment.

Development and Psychometric Evaluation of a Quality of Life Scale for Korean Patients with Cancer(C-QOL) (한국 암 특이형 삶의 질 측정도구(C-QOL) 개발 및 평가)

  • Purpose: The purpose of this study was to develop and evaluate a quality of life scale for Korean patients with cancer (C-QOL). Methods: The C-QOL was developed and validated as follows, item generation, pilot study, and psychometric tests. A total of 337 patients diagnosed with stomach, liver, lung, colon, breast, or cervix cancer were recruited. The patients were asked to complete the preliminary questionnaire comprising the content-validated items, the SF-36, and the ECOG performance status. The obtained data was analyzed using descriptive statistics, factor analysis, multidimensional scaling (MDS), multitrait/multi-item matrix, ANOVA, t-test, and Cronbach's alpha. Results: Preliminarily twenty-six items were generated through content validity and a pilot study. Factor analysis and MDS extracted a total of 21 items with a 5-point Likert-type scale (C-QOL). The C-QOL included five subscales: physical status (6 items), emotional status (6 items), social function (3 items), concern status (2 items), and coping function (4 items). The C-QOL established content validity, construct validity, item convergent and discriminant validity, known-groups validity, reliability, and sensitivity. Conclusion: The Newly developed C-QOL is an easily applicable instrument which established psychometric properties and reflected Korean culture. It is recommended for further study to examine the responsiveness of the C-QOL using a longitudinal research design.

A Case Report of Add-On Traditional Korean Medical Treatments in a Patient with Uncontrolled Type 2 Diabetes Mellitus with Conventional Oral Hypoglycemic Agents (기존의 경구용 혈당강하제로 혈당조절이 불량한 제2형 당뇨병 환자에 대한 한양방 병용 치료 1례)

  • This study presents a case of add-on traditional Korean medical treatments in a 77-year-old male patient with uncontrolled type 2 diabetes mellitus with conventional oral hypoglycemic agents. Chief complaints were uncontrolled glucose level and weight loss. The patient was treated with electroacupuncture (on ST36, CV4, and CV12), herbal medication, and lifestyle advice. The conventional oral hypoglycemic medications were maintained. After 12 weeks, the glycated hemoglobin A1c (HbA1c) decreased from 11.0% to 8.4%. There were significant abnormalities in the liver function and renal function tests, but no adverse event was reported. Further studies are needed to confirm the findings of this case.

An analysis of liver function test of preemployment screening for office workers (사무직 채용 신검에 있어서 간기능 검사에 대한 고찰)

  • This is to analyze and compare the distribution of the liver function test and its abnormal rates of the preemployment screening for office workers in asymptomatic young age groups between female and male. Liver function test(SGPT and SGOT) of 8,184 young adults(2,633 in female and 5,551 in male ) were examined during the period from Jan.1, 1994 to Dec.31, 1994. The results were as follows; 1. Mean level of SGPT was $9.1{\pm}7.6(IU/L)$ in female, $21.0{\pm}27.9(IU/L)$ in male, and that of SGOT was $15.1{\pm}6.0(IU/L)$ in female, $20.5{\pm}26.5(IU/L)$ in male. There were significant differences(p<0.01) between female and male in both SGPT and 5G07 And also there was significant increasing trend(p<0.05) by age groups in male for SGPT, decreasing trend(p<0.01) in female for SGOT, 2. In the abnormal rates of liver function test by the level of cut-off value, there were significant differences up to twice between the lowest and the highest cut-off value in both female(0.4% vs 0.7%) and male(6.5% vs 32.4%) 3. Abnormal rate of SGPT was 0.4% in female and 6.3% in male, and that of SGOT was 0.2% in female and 1.2% in male with significant differences between female and male in both tests.

Effect of Aspartate and Asparagine on Metabolism and Central Nervous System Effect of Alcohol in Healthy Male Volunteers (Aspartate 및 Asparagine 투여가 알코올 대사 및 중추신경계 효과에 미치는 영향)

  • Background; To explore the efficacy of aspartate as NAD regenerating agent for ethanol and acetaldehyde oxidation, we performed crossover challenge in two groups of volunteers by coadministration of various doses of aspartate, asparagine and ethanol. Methods; 18 healthy male volunteers were randomly divided into two groups. 6 volunteers of the first group were administered 5 gm monosodium aspartate(MSA), 5 gm asparagine or placebo with 100 ml of $40^{\circ}$ whiskey by the 3 way-crossover design, while 12 volunteers of the other group were administered placebo, 1, 2 or 5 bottles of $Aspar^(circledR)$ containing 1 gm of MSA per bottle with 100 ml of $40^{\circ}$ whiskey by the 4 way-crossover design. Ethanol(and acetaldehyde) concentrations in venous blood drawn at 0, 0.25, 0.5, 1, 2, 4, 8th hour after ethanol ingestion were analysed by gas chromatogaphy. Subjective symptoms, liver function tests and psychomotor function tests were also performed during the study periods. Result; Plasma concentration and AUC of acetaldehyde in asparagine and MSA trials on ethanol ingestion were significantly lower than those of placebo trial in the 1st group. Plasma ethanol concentration of 5 bottle $Aspar^(circledR)$ trial was significantly lower than that of placebo trial in the 2nd group. Improvement of subjective symptoms or psychomotor performance by the treatment was not statistically significant. Conclusion; Aspartate and asparagine may be prospective candidates for acceleration of ethanol metabolism and prevention of ethanol toxicity.

A Study on the Repeat Tests for Diagnosis at a Tertiary Hospital in Taegu City (3차진료기관(3次診療機關)과 환자의뢰기관간(患者依賴機關間)의 중복검사(重複檢査))

  • This study was conducted to investigate the repeat test rate for diagnosis at a tertiary hospital for the outpatients who were referred themselves to the hospital by the clinics and other medical facilities. The study population consisted of 498 patients who visited outpatient department of internal medicine, general surgery, orthopedic surgery and neurosurgery in the hospital between March 16 and April 11, 1992. This study was surveyed by the questionnaire about the tests for diagnosis at first level medical facilities, and then, was investigated by the medical record about the tests for diagnosis at a tertiary hospital. The proportion of test among the patients who utilized the first level medical facilities was 20.9% for the X-ray test, 10.6% for the urinalysis, 9.0% for the electrocardiogram, 3.4% for the computer tomogram and 6.4% for the ultrasonogram. At the tertiary hospital, the X-ray test was 45.2%, the liver function test was 24.1%, the urinalysis was 19.1%, and the electrocardiogram was 15.7%. The proportion of patients who possessed results of test for diagnosis at the first level medical facilities was 76.5% for the computer tomogram, and 31.3% for the ultrasonogram. As the repeat test rate between the first level medical facilities and the tertiary hospital, the thyroid function test was the first rank as 71.4%, the second rank was the routine CBC as 67.9%, and the third rank was the X-ray test as 64.4%. But among the patients that brought the result for tests at the first level medical facilities, the repeat test rate was as follow : the routine CBC was 75.0%, the liver function test was 72.1%, and the computer tomogram was 15.4%.

Diagnosis of Fatty Liver Complicated by Simple Obesity in Children: Serum ALT and Its Correlation with Abdominal CT and Liver Biopsy (소아의 단순성 비만증에 의한 지방간의 진단: ALT치와 복부 전산화단층촬영 및 간생검 소견간의 상관관계)

  • Purpose: The purpose of our study is to provide useful information for diagnostic methods of fatty liver by childhood simple obesity and to provide correlation between serum alanine aminotransferase (ALT) for screening test and abdominal computerized tomography (CT) and liver biopsy for confirmative diagnostic methods of fatty liver. Methods: Among 78 obese childrens who visited our hospital, CT was carried out in 26 childrens. Of these, liver biopsy was carried out in 15 childrens who had high obesity index or severe elevated ALT. Based on the level of serum ALT, 26 cases were classified into 3 groups, and compared with physical measurements and degree of fatty infiltration on CT and liver biopsy. Results: 1) Correlation between ALT and physical measurements: Of 26 obese children, ALT was abnormally elevated (>30 IU/L) in 17 cases (67.4%) but there was no significant correlation between ALT and physical measurements (p>0.05). 2) Correlation between degree of fatty infiltration on CT and ALT: Of 26 cases, 13 cases (50%) revealed fatty liver on CT. The degree of fatty liver on CT had significant correlation with elevation of ALT (p<0.05). 3) Correlation between the degree of fatty infiltration on liver biopsy and ALT: Liver biopsy was performed in 15 cases of which 14 cases revealed fatty liver. But one case had normal hepatic histology with severe obesity and normal ALT. Fourteen fatty liver cases on liver biopsy were classified into 3 groups by the degree of fatty infiltration and analysed with obesity index and ALT. The histologic hepatic steatosis had no significant correlation with obesity index (p>0.05), but significant correlation with ALT (p<0.05). 4) Correlation between CT and liver biopsy finding: Both CT and liver biopsy were performed in 15 cases of which 6 cases revealed normal finding on CT and 9 cases manifested fatty liver. There was significant correlation between CT and liver biopsy findings (r=0.6094). Conclusion: The results of our study suggest that abdominal CT and liver biopsy are useful and accurate methods of estimating fatty liver in the childhood obesity. But biochemical abnormalities of routine liver function tests dot not correlate well with severity of the fatty liver and liver injury.
