ORTHODONTIC TRACTION OF HORIZONTALLY ERUPTED LOWER LATERAL INCISOR ON THE LINGUAL SIDE (설측으로 수평 맹출한 하악 측절치의 교정적 견인)

  • Tooth eruption is the movement of the tooth from the developing place in the alveolar bone to the functional position in the oral cavity. The permanent incisors originate from the dental lamina on the lingual side of preceding deciduous tooth and erupt to the level of the occlusion through the well developed gubernacular cord. Ectopic eruption is a developmental disturbance in the eruption pattern of the permanent dentition. Most of the ectopically erupted lower incisor has been found in lingual side. The ectopically erupted tooth could be repositioned by orthodontic force in the early mixed dentition, which could help preventing the problems of loss of space and the lingual tilting of the lower anterior teeth. An eight-year-old girl visited the department of pediatric dentistry, Yonsei Dental University Hospital, for the evaluation and the treatment of the lower right lateral incisor, which was horizontally erupted in the lingual side, parallel to the mouth floor. Her tongue was placed on the labial side of that tooth. There was no previous dental history of dental caries or trauma on the pre-occupied primary incisor. Clinical and radiographic examinations including the computed tomography(CT), showed no evidence of dilacerations on root. Therefore, we decided to start active orthodontic traction of the lower right lateral incisor. We designed the fixed type of buccal arch wire and the lip bumper with hook for the traction. Button was attached to the lingual side of the ectopically positioned tooth. Elastic was used between the appliance and the button on that tooth. After the tooth become upright over the tongue level, appliance was change to the removable type and periodic check-up with occlusal guidance was followed to monitor the position of the tooth. In this case using the fixed appliance with modified form of lip bumper and hook embedded in acrylic part instead of extraction was very efficient up-righting the ectopically erupted tooth toward the occlusal plane.

  • Along with traditional goal of 'straightening teeth', today's orthodontics put more and more emphasis on facial esthetics. Naturally, the importance of soft tissue, which dictates one's facial form, is noticed. There have been many cephalometric studies on growth of hard tissue ; however, only a few on that of soft tissue. In Korea, various methods of sampling, measuring and analysing were used to study soft tissue changes, and yet, soft tissue changes along with process of normal growth have not been studied. The author carried out this study as a part of semi-longitudinal study on craniofacial growth and development of Korean children from ages of six to seventeen. 409 boys and 436 girls who'd had no systemic disease for the last three years and shown normal developmental and occlusal status were chosen as subjects. Cephalometric X-rays were taken of each subject, and facial form, lip position & form, nose form, and lip thickness were measured. Mean values and standard deviations were calculated according to age and gender, and figures and tables were drawn accordingly. The following results were obtained: 1. In respect to facial form, boys showed growth for longer period compared to girls. Also, lower face showed higher growth rate than upper face. 2. There was not much change in thickness of upper and lower lips with age; however, they appeared more prominent compared to caucasian children. 3. Nose grew horizontally with age. 4. Horizontal thickness of lower face increased with age, and upper lip thickness at nose base[A-Sn(FH)] showed more growth than anywhere else.

Morphological characteristics and genetic diversity of Calanthe species native to Korea (한국 자생 새우난초의 형태적 특성 및 유전적 다양성)

  • This study was conducted to research the morphological characteristics and analyze the genetic diversity by using RAPD in Calanthe species native to Korea. Nine samples were selected by flower color and 19 morphological characteristics. In the length and width of leaf, dorsal sepal, the lateral sepal, the petal, the central lip, and the lateral lip, C. discolor was the shortest and narrowest, respectively, but C. sieboldii was the longest and the widest, respectively. The flower stalk length was the shortest in C. discolor, and the longest in C. sieboldii. Three variants were the intermediate between C. discolor and C. sieboldii in the above morphological characteristics, but spur length was the longest in C. discolor, the shortest in C. sieboldii, and intermediate in the variants. The ovary length in C. discolor was shortest and C. sieboldii and variants were similar with each other. The flower color of C. discolor were brownish red, the value of CIE Lab was between 40 and 50. The flower color of C. sieboldii was yellowish, the value of CIE Lab was between 110 and 130. And variants had various colors between 50 to 70 in the value of CIE Lab. By analyzing multiple band patterns of PCR products, 154 bands were selected as polymorphic RAPD markers. The analysis of genetic similarity of Calanther species using RAPD showed that C. discolor and C. sieboldii are more distant from each other than variants, and these results demonstrated that genetic position of variants located between C. discolor and C. sieboldii.

  • $T4K^{TM}$(Myofunctional Research Co, Australia) is one of the myofunctional appliance developed to be used in children of mixed dentition. Myofuncitonal appliance stimulate the facial, masticatory and tongue muscle and help to balance the muscular force. Labial bow included in the device exerts strength in excessively labial inclineded upper jaw, Lip bumper blocks strength of the mouth to prevent abnormal strength exerted in lower jaw, Tongue tag secures proper position of tongue, and additional exercise is not required for child patients. For the more, simpler design and softer texture of device prmoted cooperation of patients during use. This case report is to present the satisfactory results gained by using $T4K^{TM}$ on Class II patients. Comment 1. $T4K^{TM}$ was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. 2. Excessive overjet, overbite were improved. 3. Main effects are regarded to have been achieved by development of lingual slant of upper jaw, labial slant of lower jaw, and lower part of jawbone. 4. Bad habits, such as mouth breathing, can also be adjusted.

DENTOFACIAL CHANGES IN CLASS I PROTRUSION PATIENTS TREATED WITH PREMOLAR EXTRACTIONS (제 1 소구치 발치가 수반된 Class I전돌 증례의 치료 전후 변화)

  • The purpose of this study was to evaluate the dentofacial characteristics and the fost-treatment dentofacial changes of those treated by four premolar extractions and to investigate the factors affecting extraction decision. The sample consisted of 35 patients (27 females, and 8 males) with no more than 7.0mm crowding, diagnosed as Class I protrusion. Pre-treatment and post-treatment lateral cephalograms were evaluated. Computerized statistical analysis was carried out using SPSS/PC+ program. The results were as follows. 1. There was no significant change in skeletal pattern after treatment while there was significant change in dentoalveolar and soft tissue pattern. 2. In pre-treatment skeletal pattern, a tendency toward vertical discrepancy was found. 3. In pre-treatment dental pattern, interincisal angle was $113.11^{\circ}$, U1 to FH was $117.78^{\circ}$ and L1 to A-Pog was 7.94mm. Pre-treatment upper and lower lip position was 2.88mm and 5.43mm to E line. 4. After treatment, interincisal angle increased $14.46^{\circ}$ and upper and lower lip moved back 2.45mm and 3.2mm to E line.(p<0.001) 5. The EI was 138.71 before treatment and 148.2 after treatment.

  • It is commonly assumed that nasorespiratory function can exert a dramatic effect upon the development of the dentofacial complex. Specially, it has been stated that chronic nasal obstruction leads to mouth breathing, which causes altered tongue and mandibular positions. If this occurs during a period of active growth, the outcome is development of the "adenoid facies". Such patients characteristically manifest a vertically long lower third facial height, narrow alar bases, lip incompetence, a long and narrow maxillary arch and a greater than normal mandibular plane angle. But several authors have reported that so-called adenoid facies is not always associated with adenoids and mouth breathing, and that a particular type of dentition is not always found in mouth breathers with or without adenoids. Some authors have believed adenoids lead to mouth breathing in cases with particular facial characteristics and types of dentition. We assumed that the ability to adapt to individual's neuromuscular complex is various. So, we compared the difference of influence of mouth breathing between childrens who have different facial types. This study included 60 patients and they were divided into three groups by Rickett's facial type. Their dentition and tongue position were compared. The results are as follows. 1. There is a significant difference in arch width of upper molars between different facial types. Especially dolichofacial type patients have narrowest arch width. 2. There is a significant difference in tongue position between different facial types. Especially dolichofacial type patients have lowest positioned tongue.

Treatment effects of mandibular total arch distalization using a ramal plate

  • Objective: The purpose of this study was to evaluate treatment effects after distalization of the mandibular dentition using ramal plates through lateral cephalograms. Methods: Pre- and post-treatment lateral cephalograms and dental casts of 22 adult patients (11 males and 11 females; mean age, $23.9{\pm}5.52years$) who received ramal plates for mandibular molar distalization were analyzed. The treatment effects and amount of distalization of the mandibular molars were calculated and tested for statistical significance. The significance level was set at p < 0.001. Results: The mandibular first molar distalization at the crown and root were 2.10 mm (p < 0.001) and 0.81 mm (p = 0.011), respectively. In the evaluation of skeletal variables, there was a significant increase in the Wits appraisal (p < 0.001). In the evaluation of the soft tissue, there was no significant effect on upper lip position, but the lower lips showed a significant retraction of 2.2 mm (p < 0.001). Conclusions: The mandibular molars showed a significant amount of distalization accompanied by limited extrusion and mesiobuccal rotation of the crowns. A ramal plate may be a viable device for mandibular total arch distalization in Class III patients who are reluctant to undergo orthognathic surgery.

A PHOTOMETRIC STUDY OF MAXILLOFACIAL NORMALS IN $14{\sim}15$ YEAR-OLD KOREAN. (($14{\sim}15$세 한국인 안모의 실물사진계측분석에 의한 연구)

  • The primary method of diagnosis in craniofacial deformity patient is used by gross inspection. The study for objective analysis and evaluation in Korean maxillofacial soft tissue portions is rare and our normals have been based on Caucasian such as American. But it is difficult to apply it to ours so we studied the frontal photometric measurements for maxillofacial Korean normals. The photograph were taken with the subject sitting in head position parallel with interpupillary plane to the floor. During taking pictures, facial analyser by Obwegeser in Martin was used for correct measurements. 54 male and 57 female ages 14 to 15 years were selected from a group of 111 healthy middle school students in Inchon. 1. The vertical measurements in glabella-subnasale, subnasale-stomion and stomion-menton in the male were higher than in the female subjects in $14{\sim}15$ years old Korean, so middle and lower third face were higher than those of female(p<0.05). 2. The horizontal measurements of female was higher than those of male in the rt. and It. eye width but smaller in nose width and lip width(p<0.05). 3. There was no significant difference in gender compared with soft cheek and mention angle measurements When our reports were compared with american white population, the vertical proportions in Korean normals were similar to american and the line measurements in middle facial length, nose width and intercanthal distance were higher than those of american.

Comparison of treatment effects between four premolar extraction and total arch distalization using the modified C-palatal plate

  • Objective: The purpose of this study was to compare the skeletal, dental, and soft-tissue treatment effects of nonextraction therapy using the modified C-palatal plate (MCPP) to those of premolar extraction (PE) treatment in adult patients with Class II malocclusion. Methods: Pretreatment and posttreatment lateral cephalographs of 40 adult patients with Class II malocclusion were retrospectively analyzed. The MCPP group comprised 20 patients treated with total arch distalization of the maxillary arch while the PE group comprised 20 patients treated with four PE. Fifty-eight linear and angular measurements were analyzed to assess the changes before and after treatment. Descriptive statistics, paired t-test, and multivariate analysis of variance were performed to evaluate the treatment effects within and between the two groups. Results: The MCPP group presented 3.4 mm of retraction, 1.0 mm of extrusion, and $7.3^{\circ}$ lingual inclination of the maxillary central incisor. In comparison, the PE group displayed greater amount of maxillary central incisor retraction and retroclination, mandibular incisor retraction, and upper lip retraction (5.3 mm, $14.8^{\circ}$, 5.1 mm, and 2.0 mm, respectively; p < 0.001 for all). In addition, the MCPP group showed 4.0 mm of distalization and 1.3 mm of intrusion with $2.9^{\circ}$ distal tipping of the maxillary first molars. Conclusions: These findings suggest the MCPP is an effective distalization appliance in the maxillary arch. The amount of incisor retraction, however, was significantly higher in the PE group. Therefore, four PE may be recommended when greater improvement of incisor position and soft-tissue profile is required.

First report of a bacteriovorus nematode, Diploscapter coronatus (Cobb, 1893) Cobb, 1913 (Rhabditida: Diploscapteridae), in Korea

  • Diploscapter coronatus (Cobb, 1893) Cobb, 1913 was first reported on oriental melon (Cucumis melo vars. makuwa Makino) roots from plastic film houses, Gyeongsangbuk-do, Andong, Korea (latitude $N36^{\circ}33^{\prime}$. longitude $E128^{\circ}29^{\prime}$). Diploscapter coronatus is most similar to D. pachys. Species in the genus Diploscapter have a visibly annulated cuticle. The stoma of D. coronatus is 1.5 times longer than the lip region width. The pharyngeal corpus is clearly separated from the isthmus and the vulva is situated around the mid-body. However, it differed by higher ratio of "a", filiform tail and had relatively blunt labial hooks. The Korean population is well matched and within the range of D. coronatus as described from Iran and Ethiopia in de Man's of L, b, c, c', and V. Female length, however, varied between populations: the Ethiopia population female length is longer than in the Korea population (396.4 vs 427.0) while females in the Iran population are smaller than in the Korea population (396.4 vs 350.0). Nematode size may vary due to environmental conditions such as food sources. The position of excretory pores in the Korean population were shorter 53.9-72.5 than in other populations (67-82 and 70-89). Males were uncommon. Males in the Korean population are smaller than females but larger(356.0) than males in the Indian population (306.0).
