Incarcerated Hiatal Hernia with Perforation after Laparoscopic Total Gastrectomy with Roux-en-Y Reconstruction: a Case Report

  • The occurrence of hiatal hernia after total gastrectomy with Roux-en-Y reconstruction is rare. We report the case of a 76-year-old man who presented with dyspnea, vomiting, and fever around 8 days after total gastrectomy with Roux-en-Y reconstruction. Abdominal computed tomography revealed a hiatal hernia containing part of the small intestine in the left thoracic cavity. Emergent reduction and repair of the hiatal hernia were performed later. Operative findings revealed that the Roux limb was incarcerated in the left pleural cavity. Esophagojejunostomy leakage, perforation of the small intestine with transient ischemic change, and pyothorax were also found. Thus, feeding jejunostomy, thoracoscopic decortication, and diversion T-tube esophagostomy were performed. Considering that the main cause of hiatal hernia is blunt dissection with division of the phrenoesophageal membrane, approximating the crus with 1 or 2 figure-8 sutures, according to the size of the defect, to prevent the incidence of hiatal hernia after total gastrectomy may be performed.

Suggestions for the Design of Trousers for Women with Lower Extremity Disabilities (여성 하지 지체장애인을 위한 바지 디자인 제언)

  • The purpose of this study was to analyze the reality of clothes-wearing among physically disabled women and propose trouser designs. As a research method, an in-depth interview method was adopted for 10 women in their 30s and 50s with lower limb disabilities who could wear trousers and use wheelchairs. The study results identified the following factors that should be considered in the design of trousers. First, a semi-casual design that could hide physical deficiencies, was not different from ready-made clothes and enabled various looks was desirable. Second, the proper material would have little transformation in the form, have remarkable durability and laundry fastness, two-directional stretch, and hide deficiencies with a sense of thickness and density. Third, an "L-frame" pattern in a sitting position, and a straight fit were desirable. Finally, pockets can impose pressure according to their form, size, position, and sewing method, highlighted the need for a sewing approach to minimize seams in the fabric. For adjustment, an elastic band would be a good choice in terms of type, length, position, and material as it could satisfy both the convenience of putting on and taking off and comfort in wearing at the same time. Decorative design details can also impose pressure according to a sense of thickness, which means that they should be avoided in parts that are close to the body or can impose pressure according to posture.

Treatment of Quarter Crack Using Polymethyl Methacrylate Composites with a Reshaped Gluing Shoe in a Warmblood Foal

  • A 5 day-old Warmblood foal was presented with a quarter crack due to the dam accidentally stepping on his left hind foot. Physical examination revealed the vertical crack included the coronary band of the medial side of the left hind hoof (quarter crack) with the lower pastern avulsion wound. Different from adult horses, the foot of the 5 day-old foal lacked adequate hoof wall thickness for stainless steel wire insertion and hoof size for a reinforcing fabric to be applied. Therefore, the authors decided to stabilize the edges of the cracked wall only by the polymethyl methacrylate (PMMA) composites without wiring and a reinforcing fabric application; and gluing a plastic shoe that was reshaped to protect the lower pastern and to spread pressure to other parts of the hoof when bearing weight. Disinfection and a supportive bandage were applied daily for 2 weeks. A month after the hoof crack accident, lameness had diminished. The quarter crack in this foal was treated successfully by the application of the composites three times with a reshaped gluing shoe, with approximately a month interval over the 4 months. After 4 months, it presented a successful functional recovery and needed no further treatment. When followed up 8 months after the accident, it revealed successful outcomes without hoof shape distortion and unbalanced hoof/limb growth.

Effectiveness of Transcranial Direct Current Stimulation(tDCS) on Upper Extremity Function in Stroke Patients : A Systematic Review and Meta-Analysis (뇌졸중 환자의 상지기능에 대한 경두개 직류자극술 효과 : 체계적 고찰 및 메타분석)

  • Objective : The purpose of this article was to analyze the effects of tDCS on the recovery of upper limb function in stroke patients. Methods : We searched for papers published in journals between 2009 to 2018, using NDSL and RISS. A total 14 experimental research papers were selected for analysis. The quality of the 14 articles was evaluated using the PEDro scale and 12 articles were analyzed through the Comprehensive Meta Analysis 3.0 program. Results : All of the 14 articles that were systematically reviewed in this study were published in foreign journals. The effect sizes for upper extremity(U/Ex) strength and U/Ex motion were 0.19(small size effect) and 0.49(medium size effect) respectively. Furthermore, the effect sizes of anode mode and cathode mode were 0.71(large size effect) and 0.41(medium size effect), respectively. The effect size of U/Ex motion and the anode mode were statistically significant(p<0.05). Conclusion : We identified that tDCS can be a useful rehabilitation technique for stroke patients with limited upper body function. These findings are expected to help with suggestions for basic data on new rehabilitation techniques for stroke patients and the planning of effective interventions.

Effect of topical ozonated sunflower oil on second intention wound healing in turtles: a randomised experimental study

  • Background: Ozone is an antimicrobial agent that in experimental and case-control studies has been found to exert a positive effect on wound healing. Wild and pet chelonians frequently present insidious wounds exhibiting secondary infections and/or delayed healing. Objectives: Evaluate the effects of topical ozonated sunflower oil on second-intention healing of acute experimental skin wounds in red-eared sliders (Trachemys scripta elegans). Methods: Randomised within-subject controlled study; Group 1 (n = 24) was used to assess clinical healing features; Group 2 (n = 12) was used for histological evaluation in which two sets of wounds were biopsied at 2, 7, 14, 21, 28 and 42 days over the course of the cicatrisation process. A single 6 mm diameter wound was made on each rear limb and topical ozonated (950 peroxide value) and non-ozonated sunflower oil were applied daily for one week on treated and contralateral control wounds, respectively. Results: Mean wound size was significantly lower in the ozone-treated group at day 28 (p < 0.0001) with differences of clinical relevance (74.04% vs. 93.05% reduction of initial wound size). Histologically, the acute inflammatory reaction was enhanced in treated wounds, with significantly higher numbers of heterophils (p = 0.0016), lymphocytes (p < 0.001) and fibroblasts (p < 0.001). Conclusions: Daily topical application of ozonated sunflower oil over the course of one week improved the healing of acute, full-thickness skin wounds in chelonians. This clinical outcome was histologically correlated with an enhanced acute inflammatory reaction, as well as the production and remodelling of collagen fibres.

Effects of Virtual Reality Program on Recovery of Functional in individuals Stroke: A Systematic Review and Meta Analysis (뇌졸중 환자 대상의 가상현실 운동프로그램이 기능회복에 미치는 효과: 체계적 문헌고찰 및 메타분석)

  • This study was carried out to evaluate the efficacy of Virtual Reality(VR) program on recovery of functional in Stroke through systematic literature review and meta-analysis. The following databases were used to search the literature: RISS, KISS, KMbase. Keywords included 'stroke', 'CVA' 'virtual reality', 'rehabilitation', 'virtual reality program' and the evaluated articles were published from 2009 to November 2018. The statistical was used R program, the effect size of upper limb, balance and ADL were calculated by random-effects model. As a result, 9 RCT studies were meta-analyzed and were evaluated for the risk of bias by the RoB of Cochrane Collaboration; the overall risk of bias was low. The effect sizes of VR program was balance(g=.77), ADL(g=.80) as indicated by a "large effect size". Therefore, VR program is large effective in reducing balance and ADL in strokes. It is necessary to develop and apply a virtual reality-based program using more efficient program development.

Evaluation of pedicled flaps for type IIIB open fractures of the tibia at a tertiary care center

  • Background Soft tissue coverage plays a vital role in replacing the vascularity of the underlying bone in Gustilo type IIIB fractures. The aim of this article was to evaluate the feasibility of local pedicled flaps in type IIIB fractures at a tertiary care center. Methods We included all cases of open Gustilo-Anderson type IIIB fractures of the tibia treated with local flap coverage from January 2017 to February 2019. We carried out a retrospective analysis to investigate the relationships of complications, hospital stay, and cost-effectiveness with the choice of flap, infective foci, site and size of the defect, and type of fixation. Results Out of 138 Gustilo type IIIB fractures analyzed in our study, 27 cases had complications, of which 19 (13.76%) involved flap necrosis, four (2.89%) were infections, three (2.17%) involved partial necrosis, and one (0.72%) was related to bone spur development. Flap complications showed a statistically significant association with the perforator flap category (propeller flaps in particular) (P=0.001). Flap necrosis showed a significant positive correlation with cases treated within 3 weeks after trauma (P=0.046). A significant positive correlation was also found between defect size and the duration of hospital stay (P=0.03). Conclusions Although local flaps are harvested from the same leg that underwent trauma, their success rate is at least as high as microvascular flaps as reported from other centers. Amidst the local flaps, complications were predominantly associated with perforator flaps.

Unplanned Excision of Soft Tissue Sarcoma: Patient Profile and Treatment Outcomes (연부 조직 육종에 대한 무계획적 절제술: 환자의 임상적 특징 및 치료 결과)

  • Purpose: Unplanned excision of a soft tissue sarcoma is defined as the operation performed for gross removal of a soft tissue sarcoma without regard for preoperative imaging or the necessity to removal a margin of normal tissue covering the cancer. We report our experience of treating primary soft tissue sarcoma after an unplanned excision. Materials and Methods: We retrospectively reviewed 31 patients referred to our hospital after unplanned excision at other hospitals for treatment of a STS. The clinical information was reviewed with a focus on the patient's age, gender, tumor location, tumor size, tumor depth, presumptive diagnoses at the previous surgery, refer hospital, definitive diagnosis, interval between the initial and additional surgery and local recurrence. Results: There were 19 males and 12 females with a median age of 48 years (range, 17-75 years) at the time of referral. Seventeen patients (54.8%) had tumors in their lower limb, 6 (19.4%) had tumors in their upper limb, and 8 (25.8%) had tumors in their trunk. Tumor depth could be determined for 8 patients (25.8%), with superficial and 22 deep tumors (71%). The medial interval between unplanned excision to re-excision ranged from 2 weeks to 1 year (median, 5 weeks). Local recurrence was detected in 2 patients. All patients were alive without metastasis at last follow up. Conclusion: Even in upper class general hospital, many unplanned excision had been performed, which is considered to be avoided. When the relatively huge mass located in deeper layer it requires enough preoperative imaging studies and biopsy.

The Usefulness of the Two-Staged Pedicled Latissimus Dorsi (LD) Flap in Fourth-Degree Burns of Upper Extremity (상지의 4도 화상에서 두 단계의 유경 광배근 피판술의 유용성)

  • Purpose: A large defect by fourth-degree burns in the upper limb requires flap reconstruction. Since severe vascular damage and decrease in blood circulation after vascular anastomosis can occur in defects caused by fourth-degree burns. Because of the disadvantages, it is difficult to apply free flap surgery to fourth-degree burns. We reconstructed a upper extremity using the pedicled Latissimus Dorsi (LD) flap in two stages. The purpose of our study is to review our experience and suggest two-staged pedicled Latissimus Dorsi (LD) flap in fourth-degree burns of upper extremities. Methods: A retrospective review was performed from 2016 to 2019, on a total of 12 fourth-degree burn patients undergone two-staged pedicled LD flap surgery as reconstruction of upper extremities in our hospital. We reviewed the location of the injury, etiology, TBSA (%), size of burns requiring flap surgery, period from 1st surgery to secondary division surgery, complications. Results: Using two-staged LD flap as a primary reconstruction, the outcome is satisfactory. This flap preserves the elbow joint and maintains the length of the forearm. We obtain low donor-site morbidity, simplicity and a small incision in the donor site. Conclusion: Using two-staged LD flap in fourth-degree burns of upper extremity is effective, such as preserving elbow joint and maintaining the length of the forearm. Successful reconstruction was achieved with excellent cosmetic results with reducing a postoperative scar, donor-site morbidity. Due to these advantages, two-staged pedicled LD flap can be an optimal option for reconstruction of fourth-degree burns in the upper limb.

Sexual Size Dimorphism of Lacertid Lizards from Korea (한국산장지뱀과의 성적이형)

  • This study was conducted to find the sexual size dimorphism(SSD) in morphometric characters of three species, Eremias argus, Takydromus amurensis and T. wolteri in lacertid lizard from Korea. Six external traits, snout-vent length, tail length, head length, head width, forelimb length and hind-limb length were focused to compare two sexes among the each three lizards. Student's t-test was used to compare the adult SVL between the sexes for each species. For the other parts of the body, a one-way analysis of covariance(ANCOVA) with SVL as the covariate was used. In all these species, males have longer tail and head than females like general lacertid lizard. In addition, male E. arugus have broader head and longer forelimbs and hind-limbs than conspecific females. Likewise, male T. wolteri have broader head than females. In the three lacertid lizards from Korea, snout-vent length did not significantly differ between the sexes. To understand the causes for SSD in SVL, ecological information, such as the presence/absence of male-male combat and the correlation between the SVL of female and litter size, is required. Therefore, further ecological study on the three species of lacertid lizard from Korea will make it possible to explain the reason SSD is not found in SVL.
