Neurotization from Two Medial Pectoral Nerves to Musculocutaneous Nerve in a Pediatric Brachial Plexus Injury

  • Traumatic brachial plexus injuries can be devastating, causing partial to total denervation of the muscles of the upper extremities. Surgical reconstruction can restore motor and/or sensory function following nerve injuries. Direct nerve-to-nerve transfers can provide a closer nerve source to the target muscle, thereby enhancing the quality and rate of recovery. Restoration of elbow flexion is the primary goal for patients with brachial plexus injuries. A 4-year-old right-hand-dominant male sustained a fracture of the left scapula in a car accident. He was treated conservatively. After the accident, he presented with motor weakness of the left upper extremity. Shoulder abduction was grade 3 and elbow flexor was grade 0. Hand function was intact. Nerve conduction studies and an electromyogram were performed, which revealed left lateral and posterior cord brachial plexopathy with axonotmesis. He was admitted to Rehabilitation Medicine and treated. However, marked neurological dysfunction in the left upper extremity was still observed. Six months after trauma, under general anesthesia with the patient in the supine position, the brachial plexus was explored through infraclavicular and supraclavicular incisions. Each terminal branch was confirmed by electrophysiology. Avulsion of the C5 roots and absence of usable stump proximally were confirmed intraoperatively. Under a microscope, neurotization from the musculocutaneous nerve to two medial pectoral nerves was performed with nylon 8-0. Physical treatment and electrostimulation started 2 weeks postoperatively. At a 3-month postoperative visit, evidence of reinnervation of the elbow flexors was observed. At his last follow-up, 2 years following trauma, the patient had recovered Medical Research Council (MRC) grade 4+ elbow flexors. We propose that neurotization from medial pectoral nerves to musculocutaneous nerve can be used successfully to restore elbow flexion in patients with brachial plexus injuries.

뇌졸중 환자의 마비쪽에 따른 골반의 기울임과 보행의 상관관계 분석 (Correlation Analysis of Pelvic Tilt and Gait according to the Paralytic Side of Stroke Patients)

  • Objective: This study investigated the effect of pelvic tiltng according to the paralytic side on gait in stroke patients during a 10 m functional movement timed up and go (TUG) test. Method: In this study, gait parameters were measured using a gait analyzer for 20 stroke patients and their gait was analyzed during a 10 m TUG test. For statistical analysis, an independent sample t-test were performed for age, height, and weight among general characteristics of subjects and homogeneity was tested by performing a chi-square test for gender, paralysis side, period of onset, and K-MMSE score. In order to understand the relationship between each variable, Pearson correlation analysis was performed on the variables. Results: First, the right-hand paralyzed group showed correlations in cadence and gait velocity in the up and down tilt of the pelvis, and the left-hand paralyzed group showed correlations in cadence and step length in the anterior and posterior tilt of the pelvis. Second, the tilt of the pelvis was correlated with the Sit to stand, walk forward, walk backwards, turn around at the end point, sit on a chair and the total TUG time in the right hemiplegic group compared to the left hemiplegic group. Conclusion: In this study, a significant correlation was confirmed as a result of gait analysis of right-handed stroke patients divided into a right paraplegic group and a left paraplegic group. In the future, it is suggested that treatment for improving gait of stroke patients should be treated differently for the right and left paralyzed side.

골프스윙 방법에 따른 체중이동 패턴에 관한 연구:숙련자와 비숙련자의 케이스 스터디(I) (Weight Transfer Patterns Under the Different Golf Swing Types: a Case Study Involving a Low Handicap Player and a High Handicap Player (I))

  • The purpose of this study was to analyze the weight transfer patterns under the different golf swing types which are full swing control swing and putting stroke. Two women golfers participated in this study, one(165cm, 94.3kg)being classified as a low-handicap(LH)player, the other(165cm, 54.5kg) being classified as a high-handicap(HH) player. Both players are right-handed. Two force plates(Kistler, 9286AA) were synchronized with a motion capture system(Qualisys ProReflex MCU240). Anteriorposterior, mediolateral, and vertical forces were used as an indicator of the pattern of swing. Four discrete positions which are address, top of backswing impact, and finish were identified as an event and three phases which are backswing downswing, and follow-through between he events were also identified. The results showed that, at impact, the total force was 1.24BW ring the full swing 1.17BW during the control stroke, 1.00BW during the putting stroke. Depending on the golf swing types, the differences are existed. At impact, the distribution of forces is different with a low-handicap(LH) player and a high-handicap(HH) player. A LH player has 26% in right foot and 74% in left foot during the full swing 49% in right foot and 51% in left foot during the control swing 49% in right foot and 51% in left foot during the putting stroke. A HH, on the other hand, has 74% in right foot and 26% in left foot during the full swing 62% in right foot and 38% in left foot during the control swing 54% in right foot and 46% in left foot during the putting stroke. From address to top of backswing the amount of vertical forces are changed 43:57(right foot: left foot) to 76:24 during the full swing 47:53(right foot: left foot) to 75:25 during the control swing 50:50(right foot: left foot) to 54:46 during the putting stroke. The biggest weight transfer pattern took place in full swing and the control swing is next, and the putting stroke is the final.

우성안에 따른 길이식별 인지능력 차이에 관한 연구 (The Study on Difference in Length Cognition Ability in Dominant Eye)

  • Background: Human body is formed of symmetric bilateral structures that are comprised of eye, upper arm, lower arm and etc. but, we are used only dominant components. The purpose of this study was to analysis length cognition ability in dominant eye. Methods: Total 88 persons (male 18, female 70) were participated in this study. They were tested with ‘hole in the card’ test for identification of dominant eye's side, then the length cognition ability was measured in right & left axillary level by describing 10cm line. Results: The results by independent t-test were as follows. In difference of length cognition ability in right axillary level between right dominant eyed group & left dominant eyed group, right dominant eyed group was superior to left dominant eyed group, but significant difference was not existed statistically(p>.05). In left axillary level, right dominant eyed group was superior to left dominant eyed group, but significant difference was not existed statistically(p>.05). Conclusion: These result can be applied to the learning of palpation & observation skill in physical therapy, although this study was not identify a relation between dominant eye & dominant hand.

한국 전통복식에 투영된 좌우 개념 (The Concept of Right-and-Left in Korean Traditional Costume)

  • Underlying principles on the origin and establishment of the concept fo Right-and-Left (R-&-L) in Korean traditional costume is studied in this paper. Among Korean traditional costumes, ordinary and funeral costumes are considered in this study. Thed concept of R-&-L expanded into different elements of daily life among Korean people is also considered as a process of conceptual development : starting from astron-omy principle, then developed to pragmatism principle, and expanded to abstraction principle. No evidence on the case where the astro-omy principle was directly applied to was found in this study. In most cases, pragmatism and abstraction principles are observed as underly-ing principles. In ordinary costumes, pragmatism principle is the fundamental base for R-&-L through right-hand-use rule. Even though abstraction prin-ciple was found in some ordinary costumes, we believe that this was generated for adding more meanings in addition to the pragmatism principle. In funeral costumes, pragmatism principle was almost totally ignored unlike in ordinary costumes because the funeral rite was performed though formality with abstract thoughts. The abstraction principle was exercised with the form of simple-discrimination rule based on the dual system along with Yin-Yang related cultural rules such as‘Yang-left-and-Yin-right’or‘father-left-and-mother-right’. However, the case where‘left-male-and-right-female’rule is applied to was not found in this study.

  • Eigenstructure (eigenvalues/eigenvectors) assignment has been shown to be a useful tool for flight control system design. In the sense of the eigenstructure assignment, the effectiveness and disturbance suppressibility of a controller depend mainly on the left eigenstructure (eigenvalues/left eigenvectors) of a system. On the other hand, the disturbance decouplability is governed by the right eigenstructure (eigenvalues/right eigenvectors) of the system. In this paper, in order to obtain a disturbance decouplable as well as effective and disturbance suppressible controller, a concurrent assignment methodology of the left and right eigenstructures is proposed. The biorthogonality condition between the left and right modal matrices of a system as well as the relations between the achievable right modal matrix and state selection matrices are used to develop the methodology. The proposed concurrent eigenstructure assignment methodology guarantees that the desired eigenvalues are achieved exactly and the desired left and right eigenvectors are assigned to the best possible(achievable) sets of eigenvectors in the least square sense, respectively. The proposed design methodology is applied to designing a lateral flight control system for an L-1011 aircraft with disturbances.

상태되먹임에 의한 동시고유구조 지정 기법 (A concurrent eigenstructure assignment method by state feedback)

수부에서 수지 신경의 신경내 결절종 - 증례 보고 - (Intraneural Ganglion of the Digital Nerve of the Hand - A Case Report -)

  • Intraneural ganglia in the upper extremity are rare, and the involvement of the digital nerve of hand has not been reported. The following case report demonstrates a 57-year-old woman with a symptomatic nodular mass on the thenar area of the left hand. Magnetic resonance images showed a lobulated, homogeneous mass of high signal intensity on T2-weighted images and low signal intensity with peripheral enhancement on T1-weighted images. Excisional biopsy and histopathologic examination revealed an intraneural ganglion of the digital nerve of the thumb. Her symptom disappeared immediately after the surgery, and has remained free of abnormal sensation and parestheia for the 3-year follow-up period.

한국(韓國)의 음주예법(飮酒禮法)에 관한 고찰(考察) (Studies on Drinking Manner in Korea)

  • The drinking manners of the modern generaton were reviewed based on historical records. First, for reasons of health, we should reject the drinking from the same cup as others in reciprocation. Second, if the other people are older or younger brothers above the same level, pouring with one hand is all right, but if 5 years older or more, or stranger, grasp the bottle in two hands in the following manner. With the right hand under the bottle, and the left hand supporting its side, carefully pour wine not to show the palm of the hand. Third, ladies, also, can drink freely at the same table, but if people of the opposite sex are sitting together, they should pour with two hands to convey the meaning of courteous treatment. Fourth, because the level of our living is not two high, we must not order more them we can use, so that when we are finished, there will be no remainder.

양손으로 물체 옮기기 과제 수행 시 우세손이 옮기는 물체의 종류와 목표점의 위치 변화가 비우세손의 팔뻗기 동작에 미치는 영향 (The Influence of Different Objects and Target Locations of Dominant Hand on the Non-Dominant Hand Movement Kinematics in Bimanual Reaching)

  • The purpose of this study was to investigate the effects of different objects and target location of dominant hand on the non-dominant hand movement kinematics in a bimanual reaching task. Fifteen right-handed volunteers were asked to reach from same starting point to the different target point of right and left hand with grasping the objects of different size. Independent variables were 1) three different object types (small mug cup, name pen, and PET bottle), and 2) three different target locations (shorter distance, same distance, and longer distance than the non-dominant hand) of the dominant hand. Dependent variables were movement time (MT), movement distance (MD), movement mean velocity ($MV_{mean}$), and movement peak velocity ($MV_{peak}$) of the non-dominant hand. Repeated measures two-way analysis of variance (ANOVA) was used to test for differences in the non-dominant hand movement kinematics during bimanual reaching. The results of this study were as follows: 1) MT of the non-dominant hand was increased significantly when traveling with grasping the mug cup and reaching the far target location, and was decreased significantly when traveling with grasping the PET bottle and reaching the near target location of the dominant hand. 2) MD of the non-dominant hand was significantly increased during reaching the far target location, and significantly decreased during reaching the near target location with dominant hand. 3) $MV_{mean}$ of the non-dominant hand was increased significantly when traveling with grasping the PET bottle, and was decreased significantly when traveling with grasping the mug cup of the dominant hand. Therefore, it can be concluded that the changes of the ipsilateral hand movement have influence on coupling of the contralateral hand movement in bimanual reaching.
