구급대원 재교육 및 평가를 위한 학습모형 연구 (A Study on Learning Model for Re-education and Valuation of EMT)

  • Nowadays, the whole world is putting in the knowledge information society by the internet which is connecting the global network. The internet enable us to see the life of the other people in real time. Most of health care professional obtain and update their new knowledge and skills through continuing education. EMTs who works five or ten shifts per day are not easy to attend continuing education session usually offered during their off-times. One of solutions to this problem would be to develop a education program which does not limit time and place. There has been increasing interest in the use of computers in medical education, but all designated facilities and teachers have no sufficient preparations for the education and training of EMT. There has been less attention to the rationale for the use and selection of applications. This article discusses an approach to the selection of medical informatics applications for the undergraduate medical curriculum. The aim of this study is to develop and evaluate a web-based continuing education program, to help EMT improve their knowledge of transfusion therapy and medical law, and to enhance their ability to contribute to the quality of patient care. The program generally was well received by EMTs. The program is a useful adjunct to the existing continuing education program. It also could be used in EMT student education and other health sciences.

부모의 게슈탈트적 언어 중재가 유창성장애인의 유창성 개선에 미치는 영향 (Parent's Gestalt Speech Intervention for Fluency Development of Fluency Disorder he Subject of Essay)

  • 본 연구는 부모의 게슈탈트적 언어중재를 통한 유창성장애인의 유창성 개선에 미치는 영향을 알아보기 위해 실시하였다. 부모의 게슈탈트적 언어중재 과정은 유창성장애인의 말더듬 행동에 대한 이해, 자각, 변화, 적응 그리고 유지단계로 실시하였다. 연구 대상은 신경학적 손상이나 다른 언어 및 발달 장애가 없는 유창성장애인 6명(여성 2명, 남성 4명)으로 하였다. 연구의 결과는 다음과 같다. 부모의 게슈탈트적 언어중재 후 유창성장애인의 말더듬 빈도는 감소하였다. 대상자 모두 비정상적인 유창성 중 단어부분 반복이 치료 전에는 가장 많이 빈도를 나타내었으나 치료 후에는 가장 적은 빈도를 나타내어 개선을 보였다. 그러므로 부모의 게슈탈트적 언어중재는 유창성장애인의 말더듬 빈도를 감소시키며 행동을 긍정적 방향으로 개선시키데 효과가 있었다.

데카르트의 심신론이 의학에 미친 영향 (The impact of Rene Descartes′s Mind-Body Theory on Medicin)

  • A purpose of this study is to study on Rene Descartes's mind-body theory in medical aspect. Though Rene Descartes was not so much a doctor as a philosopher, he had health and medical science at heart. When he came into the world in 1596, he was in poor health. Therefore, he suffered from his bad health. Descartes's ideas absolutely colored Western thought for three hundred years, especially, his mind-body theory, mechanistic life-view, and reductionism had important effect on medical study and science of public health. As a rule, we know that his mind-body theory was applicable to mind-body dualism, and his mind-body dualism was connected with biomedical model of medicine. But by this study, his mind-body theory was not only mind-body dualism but also mind-body monoism. And he asserted mind-body interaction too. In other words, he advocated mind-body dualism in scientific aspect, but he knew mind-body monoism from his experence. He confessed this fact to Princess Elizabeth of Bohemia, he wrote mind-body interaction in $\boxDr$Discours de la methode$\boxUl$, $\boxDr$Meditationes de prima philosophia$\boxUl$, and $\boxDr$Traite des passions de 1'ame$\boxUl$ etc. However, only mind-body dualism of his mind-body theories was written in our medical text book, morever mental realm was excluded from the persuit of learning Descartes advocated a mechanistic world-view and mechanistic life-view, he regarded human body as a machine part. And a paticent corresponds to a troubled machine, a doctor deserves a repairman. But this point of view made holistic understanding of man impossible. Descartes divide the whole into basic building blocks, we named the approach Reductionism. Reductionism led to ontological concept in medical science, bacteriology established 'specific cause-specific disease-specific therapy'. We examined medical influence of Descartes's thought, we need to draw out a philosophic basis of medical science and science of public health by a close study of his records.

부모의 자녀애착, 양육 참여 및 스마트기기 사용지도가 유아의 스마트기기 과다 사용에 미치는 영향 (Effects of Parent-Child Attachment, Parental Involvement in Child-Rearing and Instruction for Children's Effective Use of Smart Devices on Young Children's Smart-Device Overuse Behavior)

  • This study examined how parent-child attachment, parental involvement in child rearing and instruction for children's effective use of a smart device reflect on young children's smart device overuse behavior. We collected questionnaires from 196 parents with children attending early childhood education institutions for the survey. Frequency analysis as well as Person correlation coefficient and regression analysis were conducted using IBM SPSS 21.0 statistics. The results showed that 91% of targeted young children were using a smart device and 78% first used them before age 5. As for time of use hours, 43.9% used their smart device from 30 minutes to 1 hour on average. In addition, parent-child attachment and father's involvement in child rearing were found to be inversely correlated to young children's smart device overuse. The result of hierarchical regression analysis on parent factors influencing young children's smart device overuse behavior indicated that mothers' contact-seeking behavior to young children, mother's involvement in learning and instruction for children's effective use of a smart device at home had beneficial effects. This study analyzed parent factors that influenced young children's smart device overindulgence. In addition, the baseline data of this study will be utilized to develop programs for the prevention and therapy to solve the smart device overindulgence as well as to establish young children's guidelines for using a smart device.

보건계열 대학생의 학과만족도, 진로의사결정유형이 진로준비행동에 미치는 영향 (The Effects of Satisfaction in their Departments of Health College Students and the Types for Career Decision on Career Preparatory Behavior)

  • 본 연구는 보건계열 대학생을 대상으로 일반적 특성에 따른 학과만족도, 진로의사결정유형, 진로준비행동의 차이를 파악하고, 변수들 간의 관계와 진로준비행동에 미치는 영향을 파악하여, 진로지도 프로그램을 개발하는데 기초자료를 제공하고자 시행하였다. 연구의 대상자는 B와 G광역시에 소재한 3년제 학과인 응급구조과, 물리치료과, 보건행정과, 치위생과 대학생을 임의선정하고 편의표집 한 450명으로 하였다. 조사기간은 2013년 11월 12일부터 15일까지이었고, 배부된 설문지 450부 중 418부를 최종 분석하였다. 자료분석은 SPSS 14.0 for Window 프로그램을 사용하였다. 연구결과, 변수들 간의 상관관계를 보였고, 학과만족도는 진로준비행동에 가장 큰 영향은 적성학습이었고, 다음으로 대인관계, 진로직업이었다. 진로의사결정유형의 합리적 유형은 진로준비행동에 정(+)의 영향을 보였고, 의존적 유형, 직관적 유형은 진로준비행동에 부(-)의 영향을 보였다.

PNF 다리 굽힘 패턴 시 복부 드로잉-인 기법 동시적용이 동측 몸통과 다리의 근활성도에 미치는 효과 (Effect of PNF Leg Flexion Pattern on Muscle Activity of Ipsilateral Trunk and Leg with and without Abdominal Drawing-in Maneuver)

  • Purpose: The purpose of this study was to investigate the effect of the simultaneous abdominal drawing-in maneuver (ADIM) on the muscle activity of the ipsilateral trunk and leg during proprioceptive neuromuscular facilitation (PNF) leg flexion, adduction, and external rotation with knee flexion (D1) patterns. Methods: The participants were 20 healthy adult males and females (18 males and 2 females). The maneuvers were performed by a physical therapist who fully understands the PNF leg patterns (D1) and their application in clinical practice. The participants were trained and allowed to practice for 15 minutes prior to applying ADIM, to ensure adequate learning as evidenced by the pressure biofeedback unit. In this study, we measured the muscle activity of the trunk and leg when the PNF leg pattern (D1) was performed by the physical therapist either sustaining or releasing the ADIM. Muscle activity was measured on the right transverse abdominis muscle (TRA), the external abdominal oblique muscle (EO), the internal abdominal oblique muscle (IO), the erector spinae muscle (ES), the vastus medialis oblique muscle (VMO), the vastus lateralis oblique muscle (VLO), and the tibialis anterior muscle (TA) and compared using the mean values from averaging three repeated measurements. Results: The muscle activity of the transversus abdominis, the external abdominal oblique, the internal abdominal oblique, the vastus medialis oblique, and the vastus lateralis oblique was significantly greater (p < 0.05), and the muscle activity of the erector spinae was significantly less (p < 0.05) during PNF leg pattern (D1) when the ADIM contraction was sustained compared to when it was not. Conclusion: These results suggest that sustaining ADIM during PNF leg pattern (D1) training increases the trunk and leg muscle activity, resulting in more effective training.

Induced neural stem cells from human patient-derived fibroblasts attenuate neurodegeneration in Niemann-Pick type C mice

  • Background: Niemann-Pick disease type C (NPC) is caused by the mutation of NPC genes, which leads to the abnormal accumulation of unesterified cholesterol and glycolipids in lysosomes. This autosomal recessive disease is characterized by liver dysfunction, hepatosplenomegaly, and progressive neurodegeneration. Recently, the application of induced neural stem cells (iNSCs), converted from fibroblasts using specific transcription factors, to repair degenerated lesions has been considered a novel therapy. Objectives: The therapeutic effects on NPC by human iNSCs generated by our research group have not yet been studied in vivo; in this study, we investigate those effects. Methods: We used an NPC mouse model to efficiently evaluate the therapeutic effect of iNSCs, because neurodegeneration progress is rapid in NPC. In addition, application of human iNSCs from NPC patient-derived fibroblasts in an NPC model in vivo can give insight into the clinical usefulness of iNSC treatment. The iNSCs, generated from NPC patientderived fibroblasts using the SOX2 and HMGA2 reprogramming factors, were transplanted by intracerebral injection into NPC mice. Results: Transplantation of iNSCs showed positive results in survival and body weight change in vivo. Additionally, iNSC-treated mice showed improved learning and memory in behavior test results. Furthermore, through magnetic resonance imaging and histopathological assessments, we observed delayed neurodegeneration in NPC mouse brains. Conclusions: iNSCs converted from patient-derived fibroblasts can become another choice of treatment for neurodegenerative diseases such as NPC.

두 가지 케겔 운동 자세를 통한 최대 수의 환기량과 배 근육 두께의 효과 비교 연구 (A Comparative Study on the Effects of Maximum Voluntary Ventilation and Abdominal Muscle Thickness through Two Kegel Exercise Postures)

  • Purpose : The purpose of this study was to investigate a comparative study on the effects of maximum voluntary ventilation (MVV) and thickness of the abdominal muscles through two Kegel exercise postures. Methods : Twenty eight (male= 7, female= 21) subjects participated in this experiment. They performed Kegel exercise in sitting and hooklying. The order of exercise was conducted in the order chosen by the subjects to exclude the learning effect. The MVV was measured using a spirometer. The thickness of the abdominal muscles were measured by ultrasound. The MVV and thickness of the abdominal muscles were measured according to the manual in a sitting and hooklying position before the experiment. After each exercise, the MVV and thickness of the abdominal muscles were also measured in the same way. A one way repeated measures analysis of variance (ANOVA) was used to compare the MVV and thickness of the abdominal muscles according to two postures and post hoc analysis, Bonferroni was used. Results : As a result of this study, significant differences in the MVV were observed after exercise than before exercise (p<.05). However, as a result of post hoc analysis, there was no difference in the MVV according to the postures (p>.05). Significant differences in thickness of the abdominal muscles were observed after exercise than before exercise (p<.05). However, as a result of post hoc analysis, there was no difference in thickness of the abdominal muscles according to the postures (p>.05). Conclusion : Based on the results of this study, the MVV and the thickness of the abdominal muscles were confirmed in the sitting and hooklying posture after Kegel exercise. However, further studies on vital capacity and abdominal muscles according to Kegel exercise postures should be conducted.

임상 물리치료사의 업무환경 및 전문성 제도화에 대한 태도가 직업만족도에 미치는 융복합적 영향 (Convergence Influence of Clinical Physical Therapist's Attitude toward Work Environment and Professionalization on Job Satisfaction)

  • 본 연구는 임상 물리치료사들의 업무 환경 및 전문성 제도화에 대한 태도가 직업 만족도에 미치는 영향을 알아 보고자 실시하였다. 이 연구는 설문지를 이용한 횡단적 연구로서 병원에서 근무하고 있는 물리치료사 356명을 대상으로 하였다. 결과에서는 20대, 여성, 임상 경력이 낮은 물리치료사, 병원급에서 근무하는 물리치료사가 직업 만족도가 낮았다. 대부분의 물리치료사들의 전문성을 위한 제도가 필요하다고 하였고 직업 만족도가 낮을수록 전문 제도 도입의 필요성에 대한 태도가 높았다. 직업 만족도에 영향을 미치는 변인은 학력과 연봉이었다. 따라서, 물리치료사들의 직업 만족도를 향상시키기 위하여서 연봉의 개선과 전문성을 위한 학습이 직업 만족도 향상에 중요한 역할을 하고 있었다. 전문성을 갖추기 위한 제도 도입을 통한 자기 계발과 업무의 전문화는 물리치료사의 직업 만족도를 향상시키기 위하여 중요한 것으로 보인다.

해부 실습 비교과프로그램이 보건의료계열 대학생의 전공효능감 및 만족도에 미치는 영향 (The Effects of Anatomy Extracurricular Program on Efficacy of Major and Satisfaction level of Medically Inclined College Students)

  • 본 연구에서는 해부 실습 비교과프로그램이 의료보건계열 학생의 전공효능감 및 만족도에 미치는 영향을 알아보았다. 연구 도구는 전공효능감 조사와 프로그램 만족도 조사를 활용하였으며, 전공효능감 조사는 해부학 실습 참여 전-후 향상도, 해부학 그림 참여 전-후 향상도를 비교 분석하였으며, 비교과프로그램 종료 후 해부학 실습과 해부학 그림 간 만족도조사를 비교 분석하였다. 두 그룹간 비교 분석 결과, 전공효능감에 미치는 영향은 프로그램 진행 전보다 후에 유의한 결과가 나타났으며, 만족도의 경우 해부학 실습 그룹의 만족도가 해부학 그림 그룹의 만족도보다 높게 나타난 것을 알 수 있었다. 이와 같은 연구 결과를 통해 해부 실습 비교과프로그램을 운영할 때, 해부학 그림 작성 보다 해부학 실습 프로그램을 운영하는 것이 전공효능감과 만족도 점수를 향상시키는 것으로 생각된다.
