Change of activation of the supplementary motor area in motor learning: an fMRI case study (운동학습에 따른 대뇌 보조운동영역의 활성화 변화: fMRI 사례연구)

  • Purpose: The contribution of the supplementary motor area (SMA) to the control of voluntary movement has been revealed. We investigated the changesin the SMA for motor learning of the reaching movement in stroke patient using functional MRI. Methods: The subject was a right-handed 55 year-old woman with left hemiparesis due to an intracerebral hemorrhage. She performed reaching movement during fMRI scanning before and after reaching training in four weeks. The motor assessment scale and surface EMG were used to evaluate the paretic upper limb function and muscle activation. Results: In the fMRI result, contralateral primary sensorimotor cortex (SM1) was activated before and after training. SMA was only activated after training. In addition, muscle activation of the paretic upper limb was similar to that of the unaffected upper limb after training. Conclusion: These findings suggest SMA is related to the execution of a novel movement pattern resulting in motor learning in stroke patients.

Change of Fractional Anisotropy in the Left Inferior Frontal Area after Motor Learning (운동학습에 의한 왼쪽 하전두영역의 분할비등방성의 변화)

  • Purpose: This study was to delineate the structural change of neural pathway after sequential motor learning using diffusion tensor imaging (DTI). Methods: The participants were 16 healthy subjects, which were divided by training (n=8) and control (n=8) group. The task for the training was the Serial Reaction Time Task (SRTT) which was designed by Superlab program. When the 'asterisk' shows up in the 4 partition spaces on the monitor, the subject presses the correct response button as soon as possible. The training group participated in the training program of motor learning with SRTT composed of 24 digits pattern in one hour per daily through 10 days during 2 weeks. Results: In the behavioral results the training group showed significant changes in the increase of response number and the reduction of response time than those of the control group. There was significant difference in the left inferior frontal area in the fractional anisotropy (FA) map of the training group in DTI analysis. Conclusion: Motor sequential learning as like SRTT may be needed to the learning of language and visuospatial processing and may be induced for the experience-dependent structural plasticity during short period.

An Effect of Group Art Therapy on Adolescents' with Learning Disabilities - Focusing on Improvement of Self-esteem and Sociability - (집단미술치료가 학습장애 청소년에게 미치는 효과 -자아존중감과 사회성 향상을 중심으로-)

  • Purpose. This study examines the effects of improving students self-esteem and sociability by conducting a Group Art Therapy Program for students with learning disabilities. This study focused on three students with learning disabilities, attending B middle school special education class located in K county. The study was conducted for two hours once a week (Friday) at B middle school during special education class from May 2, 2014 to July 18, 2014. Methods. For the tools of studying, Self-esteem and social strip forms/rubrics were filled, by students', before and after and data processing was inspected, paired t-Test using the SPSS / WIN (Ver.18.0) for quantitative analysis. Also, on the basis of content, analyzed data of the individuals' self-esteem social changes are shown in graph format. In addition, this qualitative study shows before and after of dynamic house, tree, person (K-H, T, P), dynamic family (KFD), dynamic school life of search phase (4-6 sessions) significant improvement of self-esteem and sociability in adolescents' with learning disabilities influenced by conducting a Group Art Therapy Program. Results. The result of this study can be summarized as follows. First, Group Art Therapy Program shows a statistically significant difference (p<.05) showing that self esteem average is 2.04 before Group Art Therapy Intervention but self esteem average is 2.92 after Group Art Therapy Intervention. Second, as a result of social strip pre-post, there wasn't statistically significant difference compared to self esteem pre-post result, although, post examination's average level was enhanced after Group Art Therapy Program in difference examination.(p>.05). Third, as a result of analysis the indicated pre-post change of Students' pre-post dynamic house, tree, person (HTP), dynamic family (KFD), a dynamic school daily life (KSD), students' family and interpersonal relationships, self-concept shows statistical significant changes. Conclusion. As the result of this study, Group Art Therapy Program shows effective improvement in students' self-esteem and social skills learning disabilities. However, a future study on larger and more diverse group of students' with learning disabilities are advised to be conducted in order to better understand the significance of the Group Art Therapy Program.

Effects of Electrical Stimulation Biofeedback on Motor Learning of Quadriceps Isometric Exercise of Total Knee Replacement (전기 자극을 이용한 피드백의 형태가 무릎성형 수술 환자의 넙다리 네갈래근 등척성 운동 학습에 미치는 영향)

  • The purpose of this study was to determine the effect of electrical stimulation biofeedback on motor learning of quadriceps muscle isometric exercise in 3 patients who have undergone total knee replacement surgery. A multiple baseline design across subjects was used. The electrical stimulation biofeedback was provided with each patient during quadriceps isometric exercise, which last 10 to 14 sessions with 10 repetitions each sessions. After training patients received 4 retention tests. Maximum muscle activity was measured pre- and post- electrical stimulation biofeedback training and retention test to evaluate the effect of biofeedback training. Maximum isometric muscle activity of quadriceps was increased after electrical stimulation biofeedback training in all subjects. The results indicate that a electrical stimulation biofeedback training is a useful method to improve motor learning of quadriceps isometric exercise in total knee replacement.

  • Purpose : The purpose of this study is to investigate the effects school business hospital-based integrated health education on learning transfer factor and level. Methods : This study conducted a questionnaire survey of 60 students at D college using metastatic diagnostic tool who took the integrated health education curriculum, statistical analysis utilized the SPSS 17.0 for window version. Results : On comparison of the details 5 clauses, 29 questions using LTSI, this study found that the integrated health education based on the school business hospital is effective for learning transfer. Conclusion : What the integrated health education based on clinic practice system at D college to overcome the limitations of health and medical line is effective for learning transfer and it will be useful to cultivate professional.

A Phenomenological Study on Students' Experiences of Flipped Learning-Based Class of Sensory Integration Therapy (대학생의 플립드 러닝 기반 감각통합치료 수업 경험에 관한 현상학적 연구)

  • Objective : The purpose of this study is to investigate the meaning of students' experience participating in the flipped learning based instruction in sensory integration, and to understand the demand and introspection of participants. Methods : This study used a phenomenological approach of qualitative study. The participants were 10 students in 3rd year of the occupational therapy program in K Univeristy. Data information was collected by one-to-one interview and analyzed through phenomenological research method. Results : Through the interview, 20 units of meaning, 8 central meanings, and 3 themes were drew. The information collected were analyzed into three themes; Learning Experiences in Online and Offline Courses, Request and Introspection of Learners on Flipped Learning. The result showed that online courses brought learners convenience and satisfaction with repeatable learning in every time and space the learner want. However, the learners appealed issues of communication and concentration due to the absence of face-to-face instruction by their instructor. For the offline courses, students showed interest in various practical classwork of sensory integration and changes in their attitude to actively engage in the practical classes. Conclusion : Flipped learning based instruction was effective for the sensory integration classes which require practice in terms of time securement and immersion in practice. The learners requested for adopting flipped learning based instruction to other subjects in occupational therapy curriculum, and introspected that they needed to actively engage in classes through the experience of flipped learning-based classes of sensory integration. The results of this study can be used as a basic resource when flipped learning classes are planned in occupational therapy education.

Effects of Motor Learning Guided Laryngeal Motor Control Therapy for Muscle Misuse Dysphonia (운동학습이론에 기초한 발성운동조절법이 근오용성 발성장애의 음성에 미치는 효과)

  • Muscle misuse dysphonia (MMD) is defined as a behavioral voice disorder resulting from inappropriate contractions of intrinsic and/or extrinsic laryngeal muscles. The purpose of this study was to investigate the effect of motor learning guided laryngeal motor control therapy (MLG-LMCT) which is designed to improve an existing LMT and further the effective voice treatment on people with muscle misuse dysphonia. Forty-six people with MMD (M:F=16:30) participated in this study. The voice samples of the participants were recorded to investigate the effect of MLG-LMCT before and after the voice therapy. Voice samples were analyzed via electro-glotto-graph (EGG). Contact quotient (CQ), speed quotient (SQ), and waveform were reported. In addition, perceptual and acoustical evaluation were conducted to determine the change of voice improvement after treatment. The experimenter massaged the tensioned muscles around the neck. In order to find more proper phonation the experimenter showed the subjects their EGG wave forms as to whether or not they are moving the vocal folds to the appropriate position. Therefore, the EGG wave forms were used as a type of visual feedback. With the wave form, the experimenter helped subjects move the vocal folds and laryngeal muscles to find more proper voice production. The sensory stimuli from the experimenter gradually faded out. A paired dependent t- test revealed that there was significant differences in CQ between pre- and post-therapy. Perceptually, overall, rough, breathy, strain, and transition were significantly reduced. Acoustically, there were significant differences in Fo, jitter, shimmer, and NHR. After using MLG-LMCT, most of the subjects showed improvements in voice quality. The results from this study led us to the following conclusions: Motor learning guided laryngeal motor control therapy (MLG-LMCT) has reduces muscle misuse dysphonia. These results may occur because a visual feedback from EGG wave form can maintain the effect of the muscle tension reduction from laryngeal manual therapy. In case of people with MMD who reduced muscle tension from the therapy (LMT) but, not appropriately manipulating the location of larynx or adducting the vocal folds, MLG-LMCT might be an alternative therapy approach.

The Effect of Project Based Learning(PBL) Application Using Social Networks on Problem Solving Ability of Occupational Therapy Students (소셜네트워크를 활용한 프로젝트기반학습(PBL) 적용이 작업치료과 대학생의 문제해결능력에 미치는 영향)

  • The purpose of this study is to analyze the problem solving ability of occupational therapy students by applying PBL classes using social networks. Data were collected from 35 students in the third year of occupational therapy at K University from March to June 2019. The results were as follows: First, PBL application using social networks showed a significant improvement in problem solving ability. Seconds, there was a significant difference in problem solving ability according to the preference of mobile device use and major satisfaction. Therefore, as a learning tool for improving the problem-solving ability of occupational therapy students, we proposed a way to increase satisfaction with learning by activating PBL using social networks.

Deep Learning Frameworks for Cervical Mobilization Based on Website Images

  • Background: Deep learning related research works on website medical images have been actively conducted in the field of health care, however, articles related to the musculoskeletal system have been introduced insufficiently, deep learning-based studies on classifying orthopedic manual therapy images would also just be entered. Objectives: To create a deep learning model that categorizes cervical mobilization images and establish a web application to find out its clinical utility. Design: Research and development. Methods: Three types of cervical mobilization images (central posteroanterior (CPA) mobilization, unilateral posteroanterior (UPA) mobilization, and anteroposterior (AP) mobilization) were obtained using functions of 'Download All Images' and a web crawler. Unnecessary images were filtered from 'Auslogics Duplicate File Finder' to obtain the final 144 data (CPA=62, UPA=46, AP=36). Training classified into 3 classes was conducted in Teachable Machine. The next procedures, the trained model source was uploaded to the web application cloud integrated development environment (https://ide.goorm.io/) and the frame was built. The trained model was tested in three environments: Teachable Machine File Upload (TMFU), Teachable Machine Webcam (TMW), and Web Service webcam (WSW). Results: In three environments (TMFU, TMW, WSW), the accuracy of CPA mobilization images was 81-96%. The accuracy of the UPA mobilization image was 43~94%, and the accuracy deviation was greater than that of CPA. The accuracy of the AP mobilization image was 65-75%, and the deviation was not large compared to the other groups. In the three environments, the average accuracy of CPA was 92%, and the accuracy of UPA and AP was similar up to 70%. Conclusion: This study suggests that training of images of orthopedic manual therapy using machine learning open software is possible, and that web applications made using this training model can be used clinically.

  • Purpose : In this study, we selected three research subjects to attempt to clear learning motivation of college students is what impact the stress coping style and stress test. First, age, gender, the future career after graduation, the presence or absence of part-time job is, its impact on learning motivation. Second, learning motivation is what effect the stress of the test. Third, based on the motivation of learning, how deal pursuing efforts form the social support, the center of the problem-solving approach, seeking to avoid the reaction to stress how different form. Methods : K University occupational therapy and one, two, three grade 100 students (male 22 people, female 78) to target age, sex, and after graduation, part-time status, motivation, stress, stress coping style questionnaire for distribution and was written. Results : First of all, women's social support form graduation course, more robust than pursue blank after the synchronization uncertainty and stress, and graduated from the trading center and avoid the use of career, more form. Second, motivation and stress test, a difference between the notice could not see. Third, the higher the motivation of learning, problem-solving, Action form to the center to use as many as you, but avoid using too much in the center form is addressed. Conclusion : As a result of the study that came out of the course after graduating from ensure that learning motivation is high, the more the uncertainty, the more to cope with stress in the center of the form to avoid form address was used. Because of this, the student careers after graduation, to make sure that can help you to compare efforts over is believed to be necessary.
