상악골 Le Fort I 골절 환자에서 경구 기관 내 삽관 하에서의 악간고정 및 정복: 증례보고 (Intermaxillary Fixation under Oral Intubation in a Patient with Le Fort I Fracture: a Case Report)

  • In order to reduce jaw fracture accompanied by basal skull or nasal fracture, submental intubation could be generally performed. Albeit submental intubation has been widely accepted, it could develop complications such as nerve injury, glandular duct injury, and orocutaneous fistula. Here, we suggest oral intubation for overcoming complications and providing more stable surgical environment in emergency case. Under oral intubation maintaining in retromolar triangle and buccal corridor space, intermaxillary fixation was successfully underwent in 38-years-old female patient with Le Fort I fracture accompanied by pneumocephalus.

중안면골 골절에 대한 임상통계학적 연구 (A CLINICOSTATISTICAL STUDY ON MIDFACIAL BONE FRACTURE)

  • 저자등은 1992년 1월 1일부터 1996년 12월 31일까지 최근 5년간 전남대학교병원 구강악안면외과에 입원하여 치료받은 환자중 추적이 가능한 중안면골 골절 환자 234례에 대해 임상통계학적 비교 분석하여 다음과 같은 결과를 얻었다. 남녀비는 5.3 : 1로 남자에서 호발하였고, 연령별로는 20대, 30대, 10대순으로, 월별로는 8월, 5월, 9월과 10월 순으로 발생하였다. 발생원인은 자동차사고, 추락사고, 오토바이 사고의 순이었고, 골절부위로는 관골-상악골 복합골절이 86례로 가장 많았다. 연관 손상으로는 안면열상이 가장 많았으며 신경외과적 손상, 하악골 골절순이었다. 수상후 수술시 까지 경과시간은 1주일이내가 55%였다. 사용된 금속판수는 관골 또는 상악골 단독골절시에는 2개, 관골-상악골 복합 골절시에는 3개, Le Fort I, Le Fort I, II ; II, III ; I, III ; Le Fort I,II,III 골절시에는 각각 4개, 5.5개, 7개였다. 술후 합병증으로는 감염 7례, 안면비대칭 6례, 안구함몰증 5례, 부정교합 2례로 모두 20례(8%)가 발생하였다. 이상의 결과는 중안면골 골절시에 골절 부위와 환자의 전신상태 및 연관 손상에 대한 신속한 진단과 치료가 필요하고 타과와의 협동 치료가 이루어져야 함을 시사하였다.

광학추적항법장치를 이용한 르포씨 제1형 골절단 가상 수술의 정확성에 대한 연구 (Accuracy of simulation surgery of Le Fort I osteotomy using optoelectronic tracking navigation system)

  • Introduction: The aim of this study was to demonstrate that the simulation surgery on rapid prototype (RP) model, which is based on the 3-dimensional computed tomography (3D CT) data taken before surgery, has the same accuracy as traditional orthograthic surgery with an intermediate splint, using an optoelectronic tracking navigation system. Materials and Methods: Simulation surgery with the same treatment plan as the Le Fort I osteotomy on the patient was done on a RP model based on the 3D CT data of 12 patients who had undergone a Le Fort I osteotomy in the department of oral and maxillofacial surgery, Seoul National University Dental Hospital. The 12 distances between 4 points on the skull, such as both infraorbital foramen and both supraorbital foramen, and 3 points on maxilla, such as the contact point of both maxillary central incisors and mesiobuccal cuspal tip of both maxillary first molars, were tracked using an optoelectronic tracking navigation system. The distances before surgery were compared to evaluate the accuracy of the RP model and the distance changes of 3D CT image after surgery were compared with those of the RP model after simulation surgery. Results: A paired t-test revealed a significant difference between the distances in the 3D CT image and RP model before surgery.(P<0.0001) On the other hand, Pearson's correlation coefficient, 0.995, revealed a significant positive correlation between the distances.(P<0.0001) There was a significant difference between the change in the distance of the 3D CT image and RP model in before and after surgery.(P<0.05) The Pearson's correlation coefficient was 0.13844, indicating positive correlation.(P<0.1) Conclusion: Theses results suggest that the simulation surgery of a Le Fort I osteotomy using an optoelectronic tracking navigation system I s relatively accurate in comparing the pre-, and post-operative 3D CT data. Furthermore, the application of an optoelectronic tracking navigation system may be a predictable and efficient method in Le Fort I orthognathic surgery.

Le Fort I 골절단술을 통한 상악의 후상방 회전에 따른 상순과 비부의 연조직 변화 (Soft tissue changes of upper lip and nose following posterosuperior rotation of the maxilla by Le Fort I osteotomy)

  • Introduction: This study evaluate the soft tissue changes to the upper lip and nose after Le Fort I maxillary posterosuperior rotational movement. Materials and Methods: Twenty Skeletal class III patients, who had undergone bimaxillary surgery with a maxillary Le Fort I osteotomy and bilateral sagittal split ramus osteotomy, were included in the study. The surgical plan for maxilla was posterosuperior rotational movement, with the rotation center in the anterior nasal spine (ANS) of maxilla. Soft and hard tissue changes were measured by evaluating the lateral cephalograms obtained prior to surgery and at least 6 months after surgery. For cephalometric analysis, four hard tissue landmarks ANS, posterior nasal spine [PNS], A point, U1 tip), and five soft tissue landmarks (pronasale [Pn], subnasale [Sn], A' Point, upper lip [UL], stomion superius [StmS]) were marked. A paired t test, Pearson's correlation analysis and linear regression analysis were used to evaluate the soft and hard tissue changes and assess the correlation. A P value <0.05 was considered significant. Results: The U1 tip moved $2.52{\pm}1.54$ mm posteriorly in the horizontal plane (P<0.05). Among the soft tissue landmarks, Pn moved $0.97{\pm}1.1$ mm downward (P<0.05), UL moved $1.98{\pm}1.58$ mm posteriorly (P<0.05) and $1.18{\pm}1.85$ mm inferiorly (P<0.05), and StmS moved $1.68{\pm}1.48$ mm posteriorly (P<0.05) and $1.06{\pm}1.29$ mm inferiorly (P<0.05). The ratios of horizontal soft tissue movement to the hard tissue were 1:0.47 for the A point and A' point, and 1:0.74 for the U1 tip and UL. Vertically, the movement ratio between the A point and A' point was 1:0.38, between U1 tip and UL was 1:0.83, and between U1 tip and StmS was 1:0.79. Conclusion: Posterosuperior rotational movement of the maxilla in Le Fort I osteotomy results in posterior and inferior movement of UL. In addition, nasolabial angle was increased. Nasal tip and base of the nose showed a tendency to move downward and showed significant horizontal movement. The soft tissue changes in the upper lip and nasal area are believed to be induced by posterior movement at the UL area.

생체흡수성 고정장치를 이용한 상악골 이동술 후 장기간 결과 (STABILITY OF MAXILLARY POSITION AFTER LEFORT I OSTEOTOMY USING BIODEGRADABLE PLATES AND SCREWS)

  • Introduction: In orthognathic surgery, internal fixation has been usually done with titanium plates and screws. Recently, Biodegradable plates and screws have been frequently used but the reports of long term results of postoperative stability are rare, especially after maxillary reposition in orthognathic surgery. Objective: In order to clarify the clinical utility of self-reinforced bioresorbable poly-70L/30DL-Lactide miniplates & screws in maxillary fixation after LeFort I osteotomy, this study examined the postsurgical changes in maxilla and complications of biodegradable plates and screws. Study design: Nineteen patients who had undergone maxillary internal fixation using biodegradable plates and screws were evaluated radiographically and clinically. A comparison study of the changes in maxilla position after surgery in all 19 patients was performed with 1-week, 1-month, 3-months, 6-months and/or 1-year postoperative lateral cephalograms by tracing. Complication of the biodegradable plates and screws was evaluated by follow-up roentgenograms and clinical observation. And one-way ANOVA test was used for statistical analysis. Results: The position of the maxillary bone was stable after surgery and was not changed significantly from 1 week to 1 year after operation. And we could not find any complication of biodegradable plates and screws. Conclusions: Internal fixation of the maxilla after LeFort I osteotomy using self-reinforced biodegradable plates and screws is a reliable method for maintaining postoperative position of the maxilla after LeFort I osteotomy.

Le Fort I 상악골전진술 후 안정성에 관한 연구 (THE SKELETAL STABILITY OF LE FORT I MAXILLARY ADVANCEMENT)

  • The purpose of this retrospective study was to evaluate relapse, comparing large and small maxillary advancements with four-plate rigid fixation and without bone grafting. All patients had skeletal class III malocclusion, and underwent bimaxillary surgery. Standardized cephalometric analysis by one examiner was performed on serial radiographs of 14 patients immediately before surgery, and within 1 week and at least 6 months postoperatively (mean 10 months). The group was divided into two subsets to determine whether the magnitude of relapse. In group 1 ($\leq$ 5 mm, n = 8), the average advancement was $4.0{\pm}0.9\;mm$, with a mean relapse of $0.1{\pm}0.5\;mm$. In group 2 (6-8 mm, n = 4), the average advancement was $6.8{\pm}0.9\;mm$, with a mean relapse of $0.7{\pm}0.4\;mm$. There was no statistical difference in the measured relapse among the groups. Maxillary advancement with a 1-piece Le Fort I osteotomy is a relatively stable procedure.

  • Purpose: To compare the case of widespread maxillary osteomyelitis treated through different approach. Method: We report the two case of osteomyelitis. In a case, we had gradually removed the small amount of sequestrum for several times. In the other case, intended Le Fort I fracture was done to approach the lesion. Result: In the gradual sequestrectomy case, bone formations were found after 4 months. In the other case, after intended Le Fort I down fracture, sequestrectomy of Lt. maxilla was done with the labiobuccal alveolar bone preserved for the prosthodontic treatment.

  • 저자들은 영남대학교 영남의료원 치과로 교통 사고후 부정유합으로 인한 부정교합으로 저작 곤란을 호소하며 내원한 환자를 상악골 수평 및 후방 분절 골절단술을 시행해 다음과 같은 결과를 얻었다. 1. 교통 사고후 부정유합된 상악골을 수평 및 후방 분절 골절단술을 이용하여 전방 치열의 반대 교합과 구치부의 개교 및 scissor's bite를 해소했다. 2. 술후 감염등 특기할 만한 합병증은 발생하지 않았으며 술후의 종창은 48시간이 지나면서 서서히 감소했다. 3. 수술후 악간고정은 8주간 시행했으며 악간 고정 제거 1주후 정상적인 개구가 가능했다. 4. 절단된 상악골 고정시 확고한 고정과 회귀성향을 줄이기 위해 miniplates로 고정했다.

Surgical Treatment for a Huge Maxillary Ameloblastoma via Le Fort I Osteotomy: A Case Report

  • Ameloblastomaa are odontogenic benign tumors with epithelial origin, which are characterized by slow, aggressive, and invasive growth. Most ameloblastomas occur in the mandible, and their prevalence in the maxilla is low. A 27-year-old male visited our clinic with a chief complaint of the left side nasal airway obstruction. Three-dimensional computed tomography showed left maxillary sinus filled with a mass. Except for the perforated maxillary left edentulous area, no invaded or destructed bone was noted. The tumor was excised via Le Fort I osteotomy. The main mass was then sent for biopsy and it revealed acanthomatous ameloblastoma. The lesion in the left maxillary sinus reached the ethmoidal sinus through the nasal cavity but did not invade the orbit and skull base. The tumor was accessed through a Le Fort I downfracture in consideration of the growth pattern and range of invasion. The operation site healed without aesthetic appearances and functional impairments. However, further long-term clinical observation is necessary in the future for the recurrence of ameloblastoma. Conservative surgical treatment could be the first choice considering fast recovery after surgery and the patient's life quality.
