Parenchymal-sparing anatomical hepatectomy based on portal ramification of the right anterior section: A prospective multicenter experience with short-term outcomes

  • Backgrounds/Aims: Parenchymal-sparing anatomical hepatectomy (Ps-AH) based on portal ramification of the right anterior section (RAS) is a new technique to avoid unnecessarily transecting too much liver parenchyma, especially in cases of major anatomical hepatectomy. Methods: We prospectively assessed 26 patients with primary hepatic malignancies having undergone major Ps-AH based on portal ramification of the RAS from August 2018 to August 2022 (48 months). The perioperative indications, clinical data, intra-operative index, pathological postoperative specimens, postoperative complications, and follow-up results were retrospectively evaluated. Results: Among the 26 patients analyzed, there was just one case that had intrahepatic cholangiocarcinoma The preoperative level of α-Fetoprotein was 25.2 ng/mL. All cases (100%) had Child-Pugh A liver function preoperatively. The ventral/dorsal RAS was preserved in 19 and 7 patients, respectively. The mean surgical margin was 6.2 mm. The mean surgical time was 228.5 minutes, while the mean blood loss was 255 mL. In pathology, 5 cases (19.2%) had microvascular invasion, and in the group of HCC patients, 92% of all cases had moderate or poor tumor differentiation. Six cases (23.1%) of postoperative complications were graded over III according to the Clavien-Dindo system, including in three patients resistant ascites or intra-abdominal abscess that required intervention. Conclusions: Parenchymal-sparing anatomical hepatectomy based on portal ramification of the RAS to achieve R0-resection was safe and effective, with favorable short-term outcomes. This technique can be used widely in clinical practice.

Associations between income and survival in cholangiocarcinoma: A comprehensive subtype-based analysis

  • Backgrounds/Aims: Socioeconomic determinants of health are incompletely characterized in cholangiocarcinoma (CCA). We assessed how socioeconomic status influences initial treatment decisions and survival outcomes in patients with CCA, additionally performing multiple sub-analyses based on anatomic location of the primary tumor. Methods: Observational study using the 2018 submission of the Surveillance, Epidemiology, and End Results (SEER)-18 Database. In total, 5,476 patients from 2004-2015 with a CCA were separated based on median household income (MHI) into low income (< 25th percentile of MHI) and high income (> 25th percentile of MHI) groups. Seventy-three percent of patients had complete follow up data, and were included in survival analyses. Survival and treatment outcomes were calculated using R-studio. Results: When all cases of CCA were included, the high-income group was more likely than the low-income to receive surgery, chemotherapy, and local tumor destruction modalities. Initial treatment modality based on income differed significantly between tumor locations. Patients of lower income had higher overall and cancer-specific mortality at 2 and 5 years. Non-cancer mortality was similar between the groups. Survival differences identified in the overall cohort were maintained in the intrahepatic CCA subgroup. No differences between income groups were noted in cancer-specific or overall mortality for perihilar tumors, with variable differences in the distal cohort. Conclusions: Lower income was associated with higher rates of cancer-specific mortality and lower rates of surgical resection in CCA. There were significant differences in treatment selection and outcomes between intrahepatic, perihilar, and distal tumors. Population-based strategies aimed at identifying possible etiologies for these disparities are paramount to improving patient outcomes.

  • Backgrounds/Aims: Pancreaticoduodenectomy (PD) is the only radical treatment for periampullary malignancies. Superior mesenteric artery (SMA) first approach combined with total meso-pancreas (MP) excision was conducted to improve the oncological results. There has not been any previous research of a technique that combines the SMA first approach and total MP excision with a detailed description of the MP macroscopical shape. Methods: We prospectively assessed 77 patients with periampullary malignancies between October 2020 and March 2022 (18 months). All patients had undergone PD with SMA first approach combined total MP excision. The perioperative indications, clinical data, intra-operative index, R0 resection rate of postoperative pathological specimens (especially mesopancreatic margin), postoperative complications, and follow-up results were evaluated. Results: The median operative time was 289.6 min (178-540 min), the median intraoperative blood loss was 209 mL (30-1,600 mL). Microscopically, there were 19 (24.7%) cases with metastatic MP, and five cases (6.5%) with R1-resection of the MP. The number of lymph nodes (LNs) harvested and metastatic LNs were 27.2 (maximum was 74) and 1.8 (maximum was 16), respectively. Some (46.8%) patients had pancreatic fistula, but mostly in grade A, with 7 patients (9.1%) who required re-operations. Some 18.2% of cases developed postoperative refractory diarrhea. The rate of in-hospital mortality was 1.3%. Conclusions: The PD with SMA first approach combined TMpE for periampullary malignancies was effective in achieving superior oncological statistics (rate of MP R0-resection and number of total resected LNs) with non-inferior short-term outcomes. It is necessary to evaluate survival outcomes with long-term follow-up.

  • This study mainly evaluated the responses in growth performance of growing pigs to different energy systems and energy levels in diets. Subsequently, we compared the nutrient digestibility and digestible nutrient concentrations of each energy level diet. In experiment 1, a total of 144 growing pigs with an average initial body weight (BW) of 26.69 ± 7.39 kg were randomly allotted to six dietary treatments (four pigs/pen; six replicates/treatment) according to a 2 × 3 factorial arrangement resulting from two energy systems (metabolizable energy [ME] and net energy [NE]) and three energy levels (low [LE], recommended [C], and high energy [HE]). Pigs were fed the experimental diets for 6 weeks and were allowed free access to feed and water during the experimental period. In experiment 2, 12 growing pigs with an average initial BW of 27.0 ± 1.8 kg were randomly allotted to individual metabolism crates and fed the six diets in a replicated 6 × 6 Latin square design. The six dietary treatments were identical to those used in the growth trial. Pigs were fed their respective diets at 2.5 times the estimated energy requirement for maintenance per day, and this was divided into two equal meals provided twice per day during the experimental period. Differences in energy systems and energy levels had no significant effect on the growth performance or nutrient digestibility (except acid-hydrolyzed ether extract [AEE]) of growing pigs in the current study. However, the digestible concentrations of ether extract, AEE, and acid detergent fiber (g/kg dry matter [DM]) in diets significantly increased (p < 0.05) with increasing energy levels. Additionally, there was a tendency (p = 0.09) for an increase in the digestible crude protein content (g/kg DM) as the energy content of the diet increased. Consequently, differences in energy systems and levels did not affect the BW, average daily gain, and average daily feed intake of growing pigs. This implies that a higher variation in dietary energy levels may be required to significantly affect growth performance and nutrient digestibility when considering digestible nutrient concentrations.

Monoclonal antibody-based enzyme-linked immunosorbent assay for quantification of majonoside R2 as an authentication marker for Nngoc Linh and Lai Chau ginsengs

  • Background: Recent years have witnessed increasing interest in the high amount of ocotillol-type saponin in Panax vietnamensis, particularly in relation to majonoside R2 (MR2). This unique 3%-5% MR2 content impart Ngoc Linh and Lai Chau ginsengs with unique pharmacological activities. However, in the commercial domain, unauthentic species have infiltrated and significantly hindered access to the authentic, efficacious variety. Thus, suitable analytical techniques for distinguishing authentic Vietnamese ginseng species from others is becoming increasingly crucial. Therefore, MR2 is attracting considerable attention as a target requiring effective management measures. Methods: An enzyme-linked immunosorbent assay (ELISA) was developed by producing monoclonal antibodies against MR2 (mAb 16E11). The method was thoroughly validated, and the potential of the immunoassay was confirmed by high-performance liquid chromatography with ultraviolet spectroscopy. Furthermore, ELISA was applied to the assessment of the MR2 concentrations of various Panax spp., including Korean, American, and Japanese ginsengs. Results and conclusions: An icELISA using mAb 16E11 exhibited linearity between 3.91 and 250 ng/mL of MR2, with detection and quantification limits of 1.53 and 2.50 - 46.6 ng/mL, respectively. Based on this study, the developed icELISA using mAb 16E11 could be a valuable tool for analyzing MR2 level to distinguish authentic Ngoc Linh and Lai Chau ginsengs from unauthentic ones. Furthermore, the analysis of the samples demonstrated that Ngoc Linh and Lai Chau ginsengs exhibit a notably higher MR2 value than all other Panax spp. Thus, MR2 might be their ideal marker compound, and various bioactivities of this species should be explored.

  • Plasmodium vivax variant interspersed repeats (vir) refer to the key protein used for escaping the host immune system. Knowledge in the genetic variation of vir genes can be used for the development of vaccines or diagnostic methods. Therefore, we evaluated the genetic diversity of the vir genes of P. vivax populations of several Asian countries, including Pakistan, which is a malaria-endemic country experiencing a significant rise in malaria cases in recent years. We analyzed the genetic diversity and population structure of 4 vir genes (vir 4, vir 12, vir 21, and vir 27) in the Pakistan P. vivax population and compared these features to those of the corresponding vir genes in other Asian countries. In Pakistan, vir 4 (S=198, H=9, Hd=0.889, Tajima's D value=1.12321) was the most genetically heterogenous, while the features of vir 21 (S=8, H=7, Hd=0.664, Tajima's D value =-0.63763) and vir 27 (S =25, H =11, Hd =0.682, Tajima's D value=-2.10836) were relatively conserved. Additionally, vir 4 was the most genetically diverse among Asian P. vivax populations, although within population diversity was low. Meanwhile, vir 21 and vir 27 among all Asian populations were closely related genetically. Our findings on the genetic diversity of vir genes and its relationships between populations in diverse geographical locations contribute toward a better understanding of the genetic characteristics of vir. The high level of genetic diversity of vir 4 suggests that this gene can be a useful genetic marker for understanding the P. vivax population structure. Longitudinal genetic diversity studies of vir genes in P. vivax isolates obtained from more diverse geographical areas are needed to better understand the function of vir genes and their use for the development of malaria control measures, such as vaccines.

Characteristics of newly bred oak mushroom cultivar 'K815' for Sawdust Cultivation (표고 톱밥재배용 'K815' 육성 및 특성)

  • A new oak mushroom cultivar 'K815' was bred from monokaryotic strains of 'Jadam-2' and 'LE191732-118'. The optimal temperature for mycelial growth of 'K815' was 25℃, and the temperature for primordia formation and fruiting body development was 15~20℃. The fruiting body morphology is hemispherical and the fruiting form is sporadic. Total cultivation period of the new cultivar, from inoculation to its first harvest, was 133 days, which was 3 days longer than control cultivar 'Jadam' (129 days), with 32 days for spawn running, 89 days for browning, 4 days for primordia formation, and 8 days for fruiting body development. In the fruiting body, the thickness and diameter of pileus of the new cultivar were 32.2 mm and 58.3mm, respectively, and the length of stipe was 35.6 mm. Compared to the control cultivar ('Jadam', 'L808'), the pileus of the new cultivar was thicker and smaller and the length of stipe was shorter. The total number of fruiting bodies of 'K815' was 58.9, which was higher than the control varieties and total yield of 2 crop cycle of 'K815' was 580 g (per 3 kg substrate), which was similar to the control variety.

  • Efforts to expand the installation of devices that assist visually impaired individuals in their mobility are ongoing, but there are significantly fewer devices installed indoors compared to outdoors, causing considerable inconvenience for indoor navigation. Therefore, this paper aims to address these issues by applying the results of machine learning using YOLO(You Only Look Once) to a Raspberry Pi and by researching techniques to reduce errors through the trilateration method of UWB(Ultra-Wideband) sensors, applying it with a Kalman filter. The research results implemented an object recognition algorithm with a comprehensive accuracy of 91.7% using YOLO technology. Based on this object recognition, the direction (left, right, or front) was determined using the distance difference between two ultrasonic sensors set at an angle difference of 15 degrees. A distance of up to 1.5m was accepted through an infrared sensor to output a warning message according to the distance. The distance between the user's tag and the fixed three anchors was measured indoors through a UWB sensor, and the user's location was also measured indoors by linking the distance value with the three-side positioning technique.

  • According to the 4th and 5th assessment of the Intergovernmental Panel on Climate Change (IPCC), global climate has been rapidly changing because of the human activities since Industrial Revolution. The perceived changes were appeared strongly in temperature and concentration of carbon dioxide ($CO_2$). Global average temperature has increased about $0.74^{\circ}C$ over last 100 years (IPCC, 2007) and concentration of $CO_2$ is unprecedented in at least the last 800,000 years (IPCC, 2014). These phenomena influence precipitation, evapotranspiration and soil moisture which have an important role in hydrology, and that is the reason why there is a necessity to study climate change. In this study, Asia region was selected to simulate primary energy index from 1951 to 2100. To predict future climate change effect, Common Land Model (CLM) which is used for various fields across the world was employed. The forcing data was Representative Concentration Pathway (RCP) data which is the newest greenhouse gas emission scenario published in IPCC 5th assessment. Validation of net radiation ($R_n$), sensible heat flux (H), latent heat flux (LE) for historical period was performed with 5 flux tower site-data in the region of AsiaFlux and the monthly trends of simulation results were almost equaled to observation data. The simulation results for 2006-2100 showed almost stable net radiation, slightly decreasing sensible heat flux and quite increasing latent heat flux. Especially the uptrend for RCP 8.5 has been about doubled compared to RCP 4.5 and since late 2060s, variations of net radiation and sensible heat flux would be significantly risen becoming an extreme climate condition. In a follow-up study, a simulation for energy index and hydrological index under the detailed condition will be conducted with various scenario established from this study.
