만성 가쪽위관절융기염 환자에서 아래팔 스트랩 적용이 손목폄근 활성도, 악력 및 통증에 미치는 영향 (Effects of Forearm Strap on Wrist Extensor Activity, Grip Strength, and Pain in Patients with Chronic Lateral Epicondylitis)

  • The purpose of this study was to investigate changes in wrist extensor activity, pain, and grip strength before and after wearing a forearm strap in their 20s who complained of chronic lateral epicondylitis for more than 6 weeks. As a result of the study, there was an increase in wrist extensor activity on both the injured and non-injured sides after wearing the forearm strap, but this was not statistically significant. There was a statistically decrease in pain after wearing the forearm strap on the injured side, and a statistically significant increase in grip strength on both. After applying the forearm strap with pad, there was an increase in muscle activity of ECRB (extensor carpi radialis brevis) on both the injured and non-injured sides. This is thought to be due to the strap pad acting as a compressive force, reducing pain, and increasing muscle recruitment ability due to stability in wrist extension. However, considering that the number of study subjects is insufficient to generalize the results, additional supplementary research is deemed necessary.

주관절 내, 외상과염에 대한 체외충격파 치료의 임상적 결과 (Extracorporeal Shockwave Therapy for Medial or Lateral Epicondylitis of the Elbow)

  • 목적: 주관절 내상과염 및 외상과염은 주관절 통증의 가장 흔한 원인 중의 하나로 이에 대한 치료방법으로 최근 체외충격파 치료가 임상에서 점점 많이 시도되고 있다. 이에 본 연구에서는 내, 외상과염에 대한 체외충격파 치료를 시행한 후 그 임상적 결과를 알아보고자 하였다. 대상 및 방법: 2005년 11월부터 2009년 1월까지 본원에서 주관절 내, 외상과염으로 진단받은 환자 중 72명 74예(남자 11명, 여자 61명)를 대상으로 하였으며 평균 연령은 52.4세였다. 모든 환자는 타 병원에서 국소 주사, 물리 치료, 약물치료, 주관절 석고 고정 등의 보존적 치료방법을 시행받았으나 증상이 호전되지 않아 본원을 방문한 경우로 치료 효과의 판정은 통증의 개선 정도를 통해 평가하였는데 휴식 시와 작업 시의 통증의 정도를 각각 치료 전과 치료 후 6주, 12주, 24주에 visual analogue scale (VAS)를 이용하여 측정하고 이와 함께 통증으로 수면 중 깨어나는 횟수를 조사하여 이를 같은 방식으로 치료 전, 후를 비교하여 평가하였다. 결과: 치료 24주 째 최종 추시에서 휴식시 통증의 경우 우수 70예, 양호 4예였으며 불량은 없었고 작업시 통증의 경우 우수 42예, 양호 14예, 보통 8예였으며 불량은 없었다. 이에 따라 작업시 VAS를 바탕으로 한 치료 성공률은 약 89.2%로 확인되었다. 휴식시 통증의 VAS 평균값은 치료 전 4.60에서 치료 6주째 2.01, 12주째 0.43, 24주째 0.16으로 개선되었고 작업시 통증의 경우 평균값이 치료 전 7.20에서 치료 6주째 4.05, 12주째 2.01, 24주째 0.36으로 유의하게 개선되었다. 수면 중 깨어나는 빈도 역시 치료 전 1주일에 평균 9.64회에서 치료 6주, 12주, 24주가 되면서 각각 1.21회, 0.08회, 0.09회로 유의한 치료 효과를 보여주었다. 결론: 체외충격파를 이용한 주관절 내, 외상과염의 치료는 보존적 치료에 반응하지 않는 환자들에서 통증의 감소와 근력 증진을 위한 효과적인 치료법으로 사료된다.

외측상과염의 도침 치료에 대한 체계적 문헌고찰 및 메타분석 (Effect of Acupotomy Treatment for Lateral Epicondylitis: A Systematic Review and Meta-Analysis)

  • Objectives The purpose of this study is to observe the effectiveness of acupotomy treatment for lateral epicondylitis by comparing it with various control groups. Methods We searched 11 domestic and international databases for systematic reviews and meta-analysis. The subjects were studies published from January 1, 2017 to September 1, 2023, and only randomised controlled trials were included. Results 208 studies were searched, of which 21 studies were finally selected. Among the studies published after 2017, the largest number of studies was published in 2019. The average number of participants per study was 72.28±20.26 and the average age was in the 40s. The most frequent intervention in the study was acupotomy alone, and the treatment most often mentioned as a control group was local nerve block. The most used evaluation tool is efficiency. Acupotomy+manipulation had statistically better effect than that of local nerve block in terms of pain (standard mean difference -1.87, 95% confidence interval, -2.18 to -1.57, p<0.00001) and elbow joint function (standard mean difference 2.25, 95% confidence interval, 1.65 to 2.86, p<0.00001). Conclusions As a result of the meta-analysis, the effect of acupotomy added manual therapy treatment was statistically significant compared to the local nerve block frequently used for lateral epicondylitis. Based on these results, it appears that more research on combination treatments other than acupotomy treatment will be needed. Also, it appears that more large-scale randomized controlled studies that strictly adhere to the standards for reporting interventions in controlled trials of acupuncture, risk of bias 2 criteria will be needed.

상완골 외상과염(Tennis elbow)에 대한 봉독약침치료와 일반 침치료의 비교연구 (The Comparative Study on the Bee-Venom Therapy and Common Acupuncture Therapy for the Lateral Epicondylitis (Tennis Elbow))

  • Objective : The purpose of this study is to compare the effect of Bee-Venom Therapy and common acupuncture therapy for the lateral epicondylitis. Methods : This study has been carried out for 24 cases of ankle sprain patients who have visited Dr. An Keun Hyeong's Oriental Clinic from april 1, 2004 to august 30, 2004. We have treated 12 cases of them by Bee-Venom therapy(group I) and the other 12 cases by common acupuncture therapy(group II). And we have compared those two group. Results : 1. There were statistical significance on the pain and grip strength with bee-venom therapy group. 2. There were statistical significance on the pain and grip strength with common acupuncture therapy group. 3. Bee-venom therapy group was more statistical significance than common acupuncture therapy group on the pain. 4. There were no statistical significance between bee-venom therapy group and common acupuncture therapy group on the grip strength.

초음파와 테이핑이 외측상과염 환자의 통증과 악력에 미치는 효과 (The Effects of Ultra sound and Taping Treatment on the Pain and Grip Strength in Lateral Epicondylitis)

  • Taping treatment is frequently used in the management of musculoskeletal pain The purpose of this study was to assess the effectiveness of ultrasound and taping treatment on the pain and grip strength with lateral epicondylitis. Pain and grip strength were assessed prior, after first, after second, after third, after fourth, and after fifth treatment. All 6 times calculated pain and grip strength. To find out the effectiveness of taping treatment, we were divide two groups. the one group was consisted of 20 patients that treated with H/P, electrical therapy and ultrasound, and the other group was consisted of 20 patients that treated with H/P, electrical therapy and taping. The results were as follow: 1. There were statistical significance on the pain and grip strength with ultrasound group(P<0.05). 2. There were statistical significance on the pain and grip strength with taping group(P<0.05). 3. Taping group was more statistical significance than ultrasound group on the pain(P<0.05). 4. There was no statistical significance between ultrasound group and taping group on the grip strength(P>0.05).

Experiences of the Specified Symptoms from the Selected Work-Related Musculoskeletal Syndrome and Posture and Action of the Dental Hygienists When Work-Related Musculoskeletal Diseases of Dental Hygienists Patient Care Procedure

  • Objective: The purpose of this study is to examine about dental hygienists' myofascial pain syndrome, lower back pain, carpal tunnel syndrome (CTS), medial and lateral epicondylitis, hand-arm vibration syndrom and work-related musculoskeletal syndrome (WMSD) experience and hygienists' posture, motion. Methods: The self-administered questionnaire was surveyed from June 1 to September 30 of 2018 targeting 280 dental hygienists in Gyeongnam province and 266 dental hygienists' answers were analyzed. Results: The average daily working hours of a dental hygienist was more than eight hours 59.0%, with an average of 33 patients per day. The average number of patients who receive treatment for more than 30 minutes is 15. The angle of motion of the subjective evaluation was above 60%. Medical position and form of movement were more than 50% above the standard level. Symptoms of posture and motion that cause WMSD were hand-arm vibration syndrome 68.1%, myofascial pain syndrome 58.6%, lower back pain 51.1%, CTS 50.4% in order. Experience WMSD related symptoms which dental hygienists experience were myofascial pain syndrome 92.9%, CTS 57.9%, lower back pain 56.4%, medial and lateral epicondylitis 37.2%, hand-arm vibration syndrome 24.4%. Conclusion: The above results showed the posture and motion of dental hygienists and were found that the experience rate which dental hygienists experience WMSD of myofascial pain syndrome, Lower back pain, and CTS was significantly more than 56%.

Pragmatic Combinations of Acupuncture Points for Lateral Epicondylalgia are Unreliable in the Physiotherapy Setting

  • This study describes the reliability of pragmatic combinations of acupuncture points for lateral epicondylalgia (LE) as prescribed by physiotherapists who were experts in acupuncture. Raters (n = 14; 33-59 years) independently prescribed acupuncture points for 30 simulated human patients with LE who were surveyed via a printed questionnaire. The frequency and cooccurrence of acupuncture points prescribed for patients with lateral epicondylitis were assessed. Absolute agreement and Light's kappa (${\kappa}_{Light}$) with 95% confidence interval (CI) were used to quantify the interrater agreement. Raters prescribed 103 unique acupuncture points in different combinations with a median (min-max) of 5 (0-11) acupuncture points. The most prescribed acupuncture point was LI-11 (297 of 420; 71%), and the most common cooccurring acupuncture points were LI-11 and LI-4 (160 of 420; 38%). The absolute agreement for prescribing the acupuncture points ranged from 70% (point GB-20) to 0% (points LI-10, SP-6, LI-11, GB-34, LI-12, and LI-4). Point LR-3 showed the highest interrater reliability for prescribing the acupuncture points [${\kappa}_{Light}=0.112$, 95% CI = (0.055-0.194)], whereas point LI-4 showed the lowest reliability [${\kappa}_{Light}=-0.003$, 95% CI = (-0.024 to 0.024)]. These findings suggest that pragmatic prescriptions of acupuncture points for LE are unreliable among physiotherapists who are experts in acupuncture. Explicit, high-level evidence-based rules for prescribing and teaching combinations of acupuncture points for LE are warranted.
