A Clinical Study on Peripheral Paralysis of the Facial Nerve (말초성 안면신경마비에 대한 임상적 고찰)

  • This study has done to show the analysis and ressult of their recovery in 35 cases of peripheral paralysis of the facial nerve. The research was from Jan. 1, 1994 to Dec. 31, 1996 at Ulsan Dong Kang Hospital. The results were as follows ; 1. There were 19 males(54.3%) and 16 females(45.7%). Age distribution of patients showed above 20 years old in 31 patients(88.6%). 2. 18 cases(51.4%) were on the right and 17 cases(48.6%) on the left side. 3. Unknown of etiology was 23 cases(65.7%). 4. Grade of injury was zero in 21 cases(60.0%), trace in 8 cases(22.9%). 5. The period of treatment was $2{\sim}3$ weeks in 23 cases(65.7%), $1{\sim}2$ months in 3 cases(8.6%). 6. On electromyography facial nerve action potential amplitude was above 1.0 mv in 22 patients(62.9 %) after treatment. Facial nerve conduction latency using orbicularis oris was below 6.0 msec in 23 patients(65.7%) after treatment. 7. Recovery of patient was normal in 25 cases(71.5%) and fair in 10 cases(28.5%).

Acute Osteomyelitis in the Hand Due to Dog Bite Injury: A Report of 3 Cases

  • As many people keep small dogs as pets, dog bites are common injuries, accounting for approximately 80%-90% of all animal bite injuries. These injuries usually occur on the upper extremities, most commonly on the fingers. Most of these injuries appear as simple lacerations or abrasions of the skin. Common symptoms include inflammatory reactions of the soft tissue, such as pain, swelling, erythema, and cellulitis. However, the complications of small dog bites may include joint or cartilage injuries, including acute osteomyelitis. Once osteomyelitis develops, it is difficult to diagnose since it has a latency period of approximately 2 weeks. Plain radiography, magnetic resonance imaging, and 3-phase bone scans should be performed when acute osteomyelitis is suspected, and broad-spectrum empiric antibiotic treatment should be administered for approximately 8-12 weeks. We report 3 very rare cases of acute osteomyelitis that occurred after a dog bite injury.

  • In this paper, for the purpose of constructing LAN(Local Area Network) with Token ring type Star-wired ring method, we implemented an in board network adapter and a concentrator using microprocessor. Using cconcentrato's communication controlling function, the concentator prevents a break of communication by by-passing the fault network adapter. Because, in the data transfer period, a concentrator recognized the network adapter at transmit and receiving side to construct single by -pass line and reduces ring latency time at both sides, this method can keep a high throughput in high and low load. Being considered the result of performance evaluation for the method our study employed, high throughput can be obtained.

Increased Response of Hypogastric Nerve Fibers to Bradykinin by Mustard Oil-Induced Uterine Inflammation in the Rat

  • It is well known that the inflammation of somatic tissues, bladder and colon can alter the sensitivity of primary afferents innervating these tissues. To see if uterine afferents also show altered sensitivity, we examined their responses to the algesic agent bradykinin before and after induction of uterine inflammation. Inflammation was induced by injecting the mustard oil into the uterine lumen of adult female rats. After induction of inflammation, the response latency to bradykinin did not change, but the duration and peak of the response and integrated impulse discharges during the response period increased significantly. Furthermore, after inflammation, the level of resting discharges of the afferents was much higher. These results are consistent with the idea that the inflammation can sensitize the uterine afferents.

Maintenance of Wakefulness and Occupational Injuries among Workers of an Italian Teaching Hospital

  • Background: To assess in a laboratory setting the ability to stay awake in a sample of workers of an Italian hospital and to investigate the association between that ability and the risk of occupational injury. Methods: Nine workers at the University Hospital of Udine who reported an occupational injury in the study period (cases), and seven noninjured workers (controls) underwent a polysomnography and four 40-minute maintenance of wakefulness tests (MWT). Differences in sleep characteristics and in wakefulness maintenance were assessed using Wilcoxon's rank sums tests and Fisher's exact tests. Results: Controls had greater sleep latency, lower total sleep time, fewer leg movements, and a higher percentage ratio of cycling alternating pattern, were more likely not to fall asleep during the MWT and were less likely to have two or more sleep onsets. Although not all the differences reached statistical significance, cases had lower sleep onset times in Trials 1-3. Conclusion: In the literature, the evidence of an association between MWT results and real life risk of accidents is weak. Our results suggest a relationship between the MWT results and the risk of injury among hospital workers.

New Two-Level L1 Data Cache Bypassing Technique for High Performance GPUs

  • On-chip caches of graphics processing units (GPUs) have contributed to improved GPU performance by reducing long memory access latency. However, cache efficiency remains low despite the facts that recent GPUs have considerably mitigated the bottleneck problem of L1 data cache. Although the cache miss rate is a reasonable metric for cache efficiency, it is not necessarily proportional to GPU performance. In this study, we introduce a second key determinant to overcome the problem of predicting the performance gains from L1 data cache based on the assumption that miss rate only is not accurate. The proposed technique estimates the benefits of the cache by measuring the balance between cache efficiency and throughput. The throughput of the cache is predicted based on the warp occupancy information in the warp pool. Then, the warp occupancy is used for a second bypass phase when workloads show an ambiguous miss rate. In our proposed architecture, the L1 data cache is turned off for a long period when the warp occupancy is not high. Our two-level bypassing technique can be applied to recent GPU models and improves the performance by 6% on average compared to the architecture without bypassing. Moreover, it outperforms the conventional bottleneck-based bypassing techniques.

Gamma Knife Radiosurgery for Trigeminal Neuralgia : Review and Update

  • Accurate diagnosis of trigeminal neuralgia (TN) is the starting point for optimal treatment. Gamma knife radiosurgery (GKRS) is currently regarded as one of the first-line treatment options for medically refractory TN. GKRS is a less invasive treatment with a low risk of complications than other surgical procedures that provides a favorable pain control Barrow Neurological Institute (BNI) I-IIIb rate of >75% at short-term follow-up. Drawbacks of GKRS include the latency period before pain relief and higher recurrence rate compared with microvascular decompression. Therefore, repeat treatment is necessary if the initial GKRS was effective but followed by recurrence. The concept of dose rate and the biologically effective dose of radiation has been actively studied in radiation oncology and is also applied in GKRS for TN to achieve high safety and efficacy by prescribing the optimal dose. Recent progress in functional imaging, such as diffusion tensor imaging, enables us to understand the pathophysiology of TN and predict the clinical outcome after GKRS. Here, we review TN, GKRS, and recent updates, especially in the concepts of radiation dose, diffusion tensor imaging studies, and repeat treatment in GKRS for TN.

AUGMENTATION OF MAXILLARY ANTERIOR AREA USING DISTRACTION OSTEOGENESIS : CASE REPORT (골신장술을 이용한 상악 전치부 수복의 치험보고)

  • Objective : This is to report the criteria of success of intraoral distraction osteogenesis for alveolar augmentation in the severely atrophied alveolar defects through clinical result of 2 cases. Subjects and Methods : Anterior segmental osteotomy was performed and alveolar distractors (Martin and Leibinger, Germany) were applied each in 2 patients with severely defected anterior maxillary area. The osteomized alveolar segments were distracted by 1mm a day after latency period. After the consolidation period implants were installed with removal of distractor. The implants were evaluated clinically and radiographically. Results : In Case I, the distracted bone was directed to the palatal side, and another augmentation treatment - block bone graft, guided bone regeneration - was needed. In Case II, the successful alveolar bone augmentation was achieved. Dental implant was placed on distracted alveolar bone, and showed good osseointegration and good function without any complication. Conclusion : Distraction osteogenesis can be a good choice for alveolar ridge augmentation of severely atrophied ridges. However, the anterior esthetic prosthetics relies on the control of the vector, the kind of distractor, the healing capacity of patient and the etiology of atrophy. Therefore another study of each category would be needed.

CLINICAL EVALUATION OF ALVEOLAR DISTRACTION OSTEOGENESIS FOR IMPLANT INSTALLATION (임플란트 식립을 위한 치조골 증강술의 임상적 분석)

  • Objectives: Distraction osteogenesis has recently evolved a challenging technique to overcome the limitations of conventional augmentation procedures. The aim of this report was to evaluate the clinical result of alveolar distraction osteogenesis for implant installation. Methods: Twenty five patients with alveolar ridge deficiencies were treated with vertical alveolar distraction osteogenesis by intraoral device (total 27 devices: 25 extraosseous and 2 intraosseous devices). After the latency periods of 5-7 days, activation of the device was started. The distraction rhythm and rate was 0.75-1.0 mm a day with 2 or 3 times a day. After 3-4 months, dental implants were placed with removing the distractor simultaneously. Results: On average, a vertical gain of $9.8{\pm}3.4\;mm$ was obtained by distraction osteogenesis. Total 84 implants were installed. Average follow up period was $13.5{\pm}7.5$ months. No implant was removed during the follow up period. Three patients showed infection during the distraction osteogenesis. Three devices were broken and 2 devices among them were replaced with new one. Conclusion: Relatively larger amount of alveolar bone augmentation could be obtained with distraction osteogenesis. For the ideal anatomically and functionally ideal regeneration of alveolar bone to install dental implant, the complication of distraction should be controlled.

  • Objective : The aim of this study was to assess protective effects of Whangryonhaedoktang on the chronic mild stress (CMS) animal model of depression. Method : Male Sprague-Dawley rats were used for this study. The subjects were divided into 3 groups (CMS-drug group: Whangryonhaedoktang was administered during CMS procedure, CMS-vehicle: water was administered during CMS procedure, normal control group: without CMS procedure). After 4 weeks of CMS procedure, Morris water maze (MWM) test and open field test were executed and tyrosine hydroxylase (TH) was measured in ventral tegmental area (VTA) and locus coeruleus (LC) of rat brain. Result : 1. CMS procedure induced defects of spatial learning in early period of MWM test. 2. CMS Whangryonhaedoktang group showed shorter escape latency in comparison with CMS control group in MWM test on the first day of the test. 3. CMS Whangryonhaedoktang group and CMS control group showed no significant difference of activities and emotional behaviors in comparison with normal control group in open field test. 4. CMS Whangryonhaedoktang group showed significant inhibition effects of TH expression in VTA and LC areas in comparison with CMS control group. Conclusion : These results suggest that Whangryonhaedoktang may have inhibition effects to early period defects of spatial learning and protective antidepressant effects in CMS model rats.
