도시지역 여성의 요실금, 하부요로증상과 일상 및 성생활과의 관계 (A Study on the Urinary Incontinence, Lower Urinary Tract Symptoms, Lifestyle and Sexual Matters of Women in an Urban Area)

  • Purpose: The purpose of this study was to identify the frequency of urinary incontinence and lower urinary symptoms, to identify lifestyle and sexual matters for UI of adult women in an urban area. Method: The sample consisted of 364 educated women from a women's health education program in a public health center. For data collection, the Bristol Female Lower Urinary Tract Symptoms Questionnaire developed by Jackson, et. al.(1996) and the demographic questionnaire were used. For data analysis, SAS 8.2 program was used. Result: The results were as follows: 1. the prevalence rate of UI was 73.63%. 45.88% of them were stress incontinence and 27.75% of them were urge incontinence. 2. Remarkable differences were found between the type of UI and LUTS in the case of urgency($x^2=42.0585$, P<0.001), unexplained incontinence($x^2=59.0585$, P<0.001), noctural incontinence($x^2=18.8080$, P<0.001) and catetherization($x^2=10.7207$, P= 0.004), burning sense($x^2=27.7400$, P<0.001). 3. Remarkable differences were found between the type of UI and lifestyle matters in the cases as follows fluid intake restriction($x^2=31.0532$, P=0.008), interference in physical activity($x^2=36.7481$, P=0.001), interference in relation with others($x^2=22.2729$, P=0.034). 4. Remarkable differences were found between the type of UI and sexual matters with difficulty of sexual intercourse($x^2=16.1898$, P=0.002), and urine leakage during sexual intercourse($x^2=17.9752$, P=0.001). Conclusion: Based on the results, developing and carrying out an adequate UI care and education program is needed.

공통 자기 상관기를 이용한 효율적인 디지털 위성 방송 프레임 동기부 회로 구조 (Efficient Frame Synchronizer Architecture Using Common Autocorrelator for DVB-S2)

  • 본 논문은 위성방송 표준인 DVB-S2 (Digital Video Broadcasting via Satellite, Second generation) 에 적용 가능한 공통 자기상관 연산기를 사용한 효율적인 프레임 동기부 회로를 제안한다. 열악한 채널 상태 환경에서의 안정적인 성능을 달성하고 구현된 기능 동기블록의 하드웨어 자원을 효율적으로 활용하기 위해 본 논문에서는 새로운 구조의 효율적인 공통 자기상관기 구조를 제안한다. 제안한 동기부 회로는 병렬 구조를 취함으로써 프레임, 주파수 동기부 회로의 성능을 개선하여 프레임 동기부의 복잡도를 현저히 감소시킬 수 있었다. 따라서 제안한 동기부 회로는 직접 구현한 방식과 비교하여 약 92%의 곱셈기 개수와 81%의 덧셈기 개수를 줄일 수 있었다. 또한 FPGA 보드와 R&STM SFU 방송 테스트 장비를 이용하여 제안된 구조를 검증하였으며 총 LUTs는 XilinxTM Viertex IV LX200 칩의 29,821을 차지하였다.

  • SIFT(Scale Invariant Feature Transform) 알고리즘은 영상 데이터로부터 객체의 꼭지점이나 모서리와 같이 색상 성분의 차가 심한 영역에서 특징점을 찾아 벡터성분을 추출하는 알고리즘으로, 현재 얼굴인식, 3차원 객체 인식, 파노라마, 3차원 영상 복원 작업의 핵심 알고리즘으로 연구 되고 있다. 본 논문에서는 SIFT 알고리즘을 임베디드 환경에서 실시간으로 처리하기 위해 가장 연산량이 많은 특징점 위치 결정 단계를 Verilog HDL 언어를 이용하여 FPGA로 구현하고 그 성능을 분석한다. 하드웨어는 100MHz 클럭에서 $1,280{\times}960$영상기준 25ms, $640{\times}480$영상기준 5ms의 빠른 연산속도를 보인다. 그리고 Xilinx Virtex4 XC4VLS60 FPGA를 타겟으로 Synplify Pro 8.1i합성툴을 이용하여 합성시 약 45,792LUT(85%)의 결과를 나타낸다.

디스플레이포트 인터페이스의 AUX 채널 설계 (A Design of DisplayPort AUX Channel)

  • 본 논문은 디스플레이포트 v1.1a 표준에 적합한 AUX(Auxiliary) 채널 구현에 대한 논문이다. 디스플레이포트는 영상 및 음성을 전달하기 하기 위해 메인 링크, AUX 채널, 핫 플러그 검출 라인을 사용한다. 등시적 전송 서비스를 제공하기 위해서 소스 디바이스는 메인 링크를 통해 전달될 영상 및 음성 신호를 특정 형태로 변환하여 재구성하고 싱크 디바이스로 전달한다. AUX 채널은 메인 링크를 구성하고 유지하기 위해 링크 서비스를 제공한다. 그리고 디스플레이 장치가 소스 디바이스에서 전송된 데이터를 정상적으로 나타낼 수 있는지 파악하기 위해 디바이스 서비스를 제공한다. 핫 플러그 검출 라인은 두 디바이스간의 연결을 확인하기 위해서 사용한다. 본 논문은 AUX 채널 구현을 목표로 설계하였으며 설계된 시스템은 SoC Master3를 이용하여 검증을 수행하였다. 합성 툴은 Xilinx ISE 9.2i를 사용하여 3315개의 LUTs와 1466개의 Flip Flops을 사용하였고 최대 168.782MHz 동작 속도의 결과를 얻었다.

Diagnostic Role of Serum Free-to-Total Prostate Specific Antigen (PSA) Ratio in Prostate Cancer with Serum Total Concentration of PSA below 4 ng/mL

  • Purpose: To examine the effectiveness of serum free-to-total prostate specific antigen ratio (%fPSA) for the detection of prostate cancer (PCa) in men with different serum total PSA (tPSA) categories. Materials and Methods: From January 2010 to December 2013, a total of 225 patients with lower urinary tract symptoms (LUTS) underwent tPSA and %fPSA measurements. Histological examination with calculation of Gleason score and whole body bone scans were performed in identified cases of PCa. Results: PCa was diagnosed in 44 (19.6%) patients and the remaining 181 patients had benign prostate disease. PCa was detected in 5 (23.8%), 13 (8.7%) and 26 (47.3%) cases with tPSA level ranges ${\leq}4ng/ml$, 4 to 10 ng/ml and >10 ng/ml, respectively. The average Gleason score was $7.2{\pm}0.2$. Some 6 (13.6%) out of 44 PCa patients had bone metastases. The sensitivity was 80% and specificity was 81.3% at the cut-off %fPSA of 15% in PCa patients with a tPSA level below 4 ng/mL. A lower %fPSA was associated with PCa patients with Gleason score ${\geq}7$ than those with Gleason score ${\leq}6$ ($11.7{\pm}0.98$ vs. $16.5{\pm}2.25%$, P=0.029). No obvious relation of %fPSA to the incidence of bone metastasis was apparent in this study. Conclusions: The clinical application of %fPSA could help to discriminate PCa from benign prostate disease in men with a tPSA concentration below 4 ng/mL.

Is the Neutrophil-Lymphocyte Ratio an Indicator of Progression in Patients with Benign Prostatic Hyperplasia?

  • Purpose: The aim of this study was to evaluate inflammation parameters and assess the utility of the neutrophil-lymphocyte ratio (NLR) as a simple and readily available predictor for clinical disease activity in patients with nenign prostate hyperplasia BPH. We also aimed to investigate the relationship between inflammatory parameters with ${\alpha}$-blocker therapy response, and evaluate the potential association between NLR and the progression of benign prostatic hyperplasia (BPH). Materials and Methods: We examined 320 consecutive patients (July 2013-December 2013) admitted to our outpatient clinic with symptoms of the lower urinary tract at Bozok University. The mean age was 60 (range, 51-75) years. Complete blood count (CBC), prostate-specific antigen (PSA), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) were assessed. Correlations between PSA, CRP, ESR, prostate volume, International Prostate Symptom Score (IPPS), maximum urinary flow rate (Qmax), and NLR were assessed statistically. Patients were divided into two groups: high and low risk of progression. Results: NLR was positively correlated with IPSS (p=0.001, r=0.265), PSA (p=0.001, r=0.194), and negatively correlated with Qmax (p<0.001, r=-0.236). High-risk patients a had a higher NLR compared with low-risk patients, based on IPSS (p<0.001), PSA (p=0.013), and Qmax (p<0.001); however, there were no significant differences between the groups in terms of age (p>0.05), and prostate volume (p>0.05). Conclusions: NLR can predict BPH progression. We propose that increased inflammation is negatively associated with clinical status in BPH patients and suggest that NLR can give information along with LUTS severity which may be used as a readikly accessible marker for patient follow-up.

Utility of Digital Rectal Examination, Serum Prostate Specific Antigen, and Transrectal Ultrasound in the Detection of Prostate Cancer: A Developing Country Perspective

  • Purpose: To determine the utility of digital rectal examination (DRE), serum total prostate specific antigen (tPSA) estimation, and transrectal ultrasound (TRUS) for the detection of prostate cancer (PCa) in men with lower urinary tract symptoms (LUTS). Materials and Methods: All patients with abnormal DRE, TRUS, or serum tPSA >4ng/ml, in any combination, underwent TRUS-guided needle biopsy. Eight cores of prostatic tissue were obtained from different areas of the peripheral prostate and examined histopathologically for the nature of the pathology. Results: PCa was detected in 151 (50.3%) patients, remaining 149 (49.7%) showed benign changes with or without active prostatitis. PCa was detected in 13 (56.5%), 9 (19.1%), 26 (28.3%), and 103 (74.6%) of patients with tPSA <4 ng/ml, 4-10 ng/ml, 10-20 ng/ml and >20 ng/ml respectively. Only 13 patients with PCa had abnormal DRE and TRUS with serum PSA <4 ng/ml. The detection rate was highest in patients with tPSA >20 ng/ml. The association between tPSA level and cancer detection was statistically significant (p<0.01). Among 209 patients with abnormal DRE and raised serum PSA, PCa was detected in 128 (61.2%). Conclusions: The incidence of PCa increases with increasing serum level of tPSA. The overall screening and detection rate can be further improved by using DRE, TRUS and TRUS-guided prostate needle biopsies.

공유 메모리 기반 시스토릭 어레이 FFT 프로세서 설계 및 구현 (Design and Implementation Systolic Array FFT Processor Based on Shared Memory)

  • 본 논문에서는 base-4 시스토릭 어레이 FFT 프로세서에서 사용되는 여러 메모리를 하나의 메모리로 공유함으로써 기존 보다 작은 메모리 면적의 FFT 프로세서의 설계 및 구현 결과를 제시한다. 메모리를 공유하여 면적이 줄어드는 장점이 생겼으며, 데이터의 입출력이 하나의 메모리에서 진행되므로 데이터의 흐름이 단순해졌다. 제시한 FFT 프로세서를 FPGA 디바이스 상에서 구현 및 검증하였으며, 구현 결과 4096-point FFT 기준 51,855개의 CLB LUT, 29,712개의 CLB registers, 8개의 block RAM tile과 450개의 DSP로 구현되었고, 최대 동작 주파수는 150MHz 인 것을 확인했으며 특히, 기존 base-4 시스토릭 어레이 구조 대비 메모리 면적이 65% 감소 가능함을 확인하였다.

  • Alfuzosin, an Alphal-adrenoceptor antagonist is used for the treatment of patients with voiding and in a lesser extent storage lower urinary tract symptoms (LUTS) associated to benign prostatic hyperplasia (BPH). The objective of this study was to formulate sustained release alfuzosin HCl granules and assess their formulation variables. The $Eudragit^{(R)}$ as a polymer, sustained release membrane, and dibutyl sebacate (DBS) as a plasticizer were used. Multi-coated alfuzosin HCl delivery systems composed of sugar sphere, various excipients, $Eudragit^{(R)}$ and HPMC (hydroxy propyl methyl cellulose), Cellulose Acetate were prepared by fluid-bed coater. Membrane layer were used $Eudragit^{(R)}$ RS PO and NE 30D. And the alfuzosin HCl coated beads were coated immediate release drug layer for initial burst. Its dissolution test was carried out compared to conventional products ($XATRAL^{(R)}$ XL). The release rate of drug from coated beads was higher than that from $XATRAL^{(R)}$ XL in pH 6.8.

Bluetooth 응용을 위한 MUSIC 알고리즘 기반 DoA 추정기의 설계 (Design of MUSIC-based DoA Estimator for Bluetooth Applications)

  • 본 논문에서는 multiple signal classification(MUSIC) 알고리즘을 바탕으로 Bluetooth 저전력 응용 기술에 적용 가능하도록 설계한 각도 추정기를 제안하고, 이를 FPGA로 구현한 결과를 제시한다. MUSIC 알고리즘은 높은 정확도에 따른 많은 연산량이 요구되므로 이를 하드웨어 고속설계 하였고, 실내 시스템의 다양한 해상도의 요구에 대응하기 위해 snapshot 가변을 가능하게 설계하였다. Xilinx zynq-7000으로 구현한 결과 9,081개의 LUTs로 구현함을 확인하였고, 100MHz의 동작주파수로 동작이 가능하도록 설계하였다.
