An Analysis of the Existing Guidelines and Clinical Trials for the Development of the Guidelines of Clinical Trials with Herbal Medicinal Products for Gastroesophageal Reflux Disease (GERD) (위식도역류질환 한약제제 임상시험 가이드라인 개발을 위한 관련 국내 가이드라인 분석 및 기존 한약임상시험과의 비교)

  • Objectives: This study aimed to learn what should be considered in the Guidelines of Clinical Trials with Herbal Medicinal Products for Gastroesophageal Reflux Disease (GERD) by analyzing the existing guidelines and clinical trials.Methods: The development committee searched the existing guidelines for herbal medicinal products or GERD. Then, clinical trials related to GERD using herbal medicine were selected. The chosen trials were analyzed in terms of their inclusion and exclusion of participants, intervention, comparators, outcome, and trial design. Then, we compared the results of the analysis according to the regulations and guidelines of the Ministry of Food and Drug Safety to suggest the issues that we will have to consider when developing the Guidelines of Clinical Trials with Herbal Medicinal Products for Gastroesophageal Reflux Disease (GERD).Results: As a result, few guidelines for GERD and clinical trials with herbal medicinal products were located in the national institution homepage. In addition, 8 articles were found using the following combination of search terms: “Gastroesophageal reflux disease”, “GERD”, “herbal medicine”, “herbal therapy”, “Korean Medicine”, “Traditional Chinese Medicine”, and “TCM”. Even though all trials had their own unique research questions, all studies were performed using a randomization method. Most trials included participants with reflux esophagitis, but two trials targeted proton pump inhibitor-refractory GERD. The type of intervention varied, such as decoction, granules, and capsules. Additionally, individualized herbal medicines were used in two studies. Comparators were diverse, such as placebo, Western medicine, and electro-acupuncture. The most frequently used outcome for efficacy was the effectiveness rate. In addition, the outcome for evaluating quality of life, esophageal mucosa and pressure, esophageal acid reflux, and recurrence rates were used. Safety was investigated by recording adverse events and carrying out laboratory tests.Conclusions: We identified some issues by reviewing the existing guidelines and comparing them with clinical trials for GERD and herbal medicinal products. These results will be utilized for developing the Guidelines of Clinical Trials with Herbal Medicinal Products for Gastroesophageal Reflux Disease (GERD).

Evaluation of the Microbiological Status of Raw Beef in Korea: Considering the Suitability of Aerobic Plate Count Guidelines

  • This study was conducted to analyze the microbiological contamination status of raw beef distributed in Korea, and evaluate the suitability of current aerobic plate count (APC) guidelines. We analyzed five years (2010-2014) of microbiological monitoring data obtained from the Ministry of Food and Drug Safety and investigated the microbiological status of raw beef collected from meat packing centers and meat shops in the Seoul/Gyeonggi, Gangwon, and Chungcheong regions in August 2015. From 2010-2014, most raw beef (>94%) displayed APC levels of < $1.0{\times}10^6CFU/g$. However, raw beef samples collected from all three regions in August 2015 had comparatively higher APC levels than those reported in previous years. To evaluate the relationship between the APC level and quality, changes in beef loin were evaluated during cold storage for 15 days at $4^{\circ}C$. On day 11, the mean APC level ($4.7{\times}10^6CFU/g$) conformed to current guidelines in Korea ($1.0{\times}10^7CFU/g$) and the pH value was 5.82. However, the sensory evaluation score for color and overall acceptability was under 3.0, meaning that the beef loin was not acceptable for eating. These results suggest that current APC guideline for raw beef should be lowered to $1.0{\times}10^6CFU/g$ to improve both the microbiological safety and palatability of raw beef.

Long-term results of unilateral cleft lip repair with multiple infantile hemangiomas including one involving the cleft side of the upper lip

  • Infantile hemangiomas have a growth phase and an involution phase. For this reason, serial observation has generally been recommended as the treatment for uncomplicated infantile hemangiomas. Recently, however, individualized approaches have been emphasized. Although cleft lip and infantile hemangioma are common congenital diseases, infantile hemangiomas on the cleft side (i.e., in the operative field of the cleft lip) are extremely rare, and no clear guidelines have been established for their treatment. We experienced a case in which a patient with a cleft lip had an infantile hemangioma on the cleft side. In accordance with general treatment guidelines, cleft repair was performed 3 months after birth. The Millard rotation-advancement technique, which involves the use of a lower small triangular flap, was used for the repair. No intraoperative complications, such as massive bleeding, or postoperative complications were noted. The patient has received regular follow-up for the past 18 years, and other than a reddish scar on the lower lip, he currently has no related issues. Therefore, this case demonstrates that cleft lip repair performed according to cleft lip treatment guidelines produces good outcomes, even in cases involving a hemangioma on the cleft side.

A Study on the Elementary Computer Education Invigorating Policy based on Analysis of the Computer Textbooks (초등학교 컴퓨터 교과서의 분석을 통한 컴퓨터 교육의 활성화 방안에 관한 연구)

  • The "Information and Communication Technology Education Guidelines" were revised in December, 2005. However, the revised guidelines are not propagated in the elementary school. One of the reasons is that the textbook is not faithful to the revised guidelines. We analyzed the elementary computer textbooks published on and after June 2006. In the result, the contents of many textbooks are not based on the "Information and Communication Technology Education Revised Guidelines". Especially, "Understanding of the Information Processing" and "Comprehensive Activities" areas are not faithful to the revised guidelines. Therefore, the systematization of the textbook screening system, participation of the computer education expert, development of the detailed curriculums, and training of the elementary teachers are essentials for invigorating of the elementary computer education.

Patterns and Interrelationships for Meeting Guidelines of Physical Activity, Sedentary, and Dietary Behavior in Korean Adolescents (한국 청소년의 신체활동, 좌식행동, 식이행동에 대한 가이드라인 실천 패턴 및 상호관련성)

  • The purpose of this study was to analyze the patterns and the interrelationships for meeting guidelines of three health behaviors including physical activity(PA), sedentary behavior(SB), and dietary behavior(DB) for Korean adolescents using 2007 KYRBS(Korea Youth Risk Behavior Survey) data. A cross-sectional design was used to assess 73392 middle-high school students. PA(moderate physical activity${\geqq}$5times/week, or vigorous ${\geqq}$3times/week), SB(watching TV, computer, DVD, video, etc. < 3 hr/day), and DB(servings of fruits ${\geqq}$1 times/day, or vegetables ${\geqq}$ 3 times/day) were categorized into two levels (meeting guidelines or not). Chi-squared tests were conducted to compare the prevalence of students not meeting these three health behaviors between boys and girls, and logistic linear regression was used to determine the interrelationships of three health behaviors. Students meeting guidelines for PA, SB, and DB were 31.0%(boys 42.6%, girls 18.0%), 74.7%(boys 75.4%, girls 74.0%), and 38.5%(boys 38.6%, girls 38.4%), respectively. In addition, there were significant differences between both genders for PA and SB (PA; $x^2(1)$=35175.11, p<.0001, SB; $x^2(1)$=19.44, p<.0001). Only 10.9% of students met all three guidelines and 12.4% did not meet all three. Pattern that simultaneously did not meet PA and DB were high in both boys(27.1%) and girls(37.2%). Students who did not meet DB were at greater risk of not meeting PA(Boys; OR 1.49, 95% CI 1.43 to 1.55, Girls; OR 1.42, 95% CI 1.34 to 1.50), and not meeting SB(Boys; OR 1.41, 95% CI 1.34 to 1.48, Girls; OR 1.36, 95% CI 1.29 to 1.43) compared with students who met DB. The findings of this study supported further evidence for the need of a multiple behavior approach considering gender and interrelationships among three behaviors.

Most trials included gastric cancer participants with medical history of operation. The type of intervention was various such as decoction, granules, and fluid of intravenous injection. Comparators were diverse such as placebo, conventional treatment including chemotherapy and nutritional supplement. The most frequently used outcome for efficacy was quality of life. Besides, the symptom score, tumor response, and survival rate were used. Safety was investigated by recording adverse events. Conclusion: We found out some issue by reviewing the existing guidelines and comparing it with clinical trials for gastric cancer and herbal medicinal products. These results will be utilized for developing [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer].

Association between Compliance with Dietary Guidelines and Dyslipidemia among Koreans (한국 성인의 식생활 지침 실천도와 이상지혈증의 관련성)

  • While metabolic syndrome(MS) is rapidly expanding and dietary pattern, the known risk factor of MS, goes through heavy transition to western diet, not many researches have been done on the association between dyslipidemia and dietary factors in Korean adults. The purpose of this study was to investigate the association between compliance with dietary guidelines and dyslipidemia among Koreans. The subjects of 399 adults who visited health examination center were classified into dyslipidemia(n=180) and control(n=219). Diagnosis of dyslipidemia was based on NCEP-ATPIII criteria(triglyceride>=150mg/d, HDL-C<50mg/dl for male, HDL-C<40mg/dl for female). A questionnaire based interview was done to collect information on compliance with dietary guidelines, general characteristics and health related behaviors. Anthropometric variables were measured during the survey. Mean compliance score of dietary guideline was significantly lower in dyslipidemia group than in control. It was associated negatively with waist circumference and positively with serum HDL-cholesterol(p<0.05). Risks of dyslipidemia were significantly decreased in the group with highest dietary guideline score, high serum triglyceride levels(OR=0.484, 95% CI=0.268-0.875), abdominal obesity(OR=0.296, 95% CI=0.159-0.553), and dyslipidemia(OR=0.481, 95% CI=0.266-0.869). These results indicated that increasing compliance with dietary guidelines could be an effective strategy to lower the risk of dyslipidemia among Koreans.

Development and Evaluation of Evidence-Based Nursing Protocol for Extracorporeal Membrane Oxygenation to Critically Ill Patients (체외막산소화장치 적용 중환자를 위한 근거기반 간호 프로토콜 개발 및 효과 평가)

  • Purpose: This study aimed to develop an evidence-based extracorporeal membrane oxygenation (ECMO) nursing protocol for critically ill patients receiving ECMO treatment by using an adaptation process, and to verify the effects of the protocol. Methods: The protocol was developed according to the adaptation guidelines. A non-randomized controlled trial was conducted to test the protocol's effects. Data were collected between April 2019 and March 2021. The differences in physiological indicators and complication rates between the two groups were investigated using a chart review to evaluate patient outcomes. The nurses' outcome variables were evaluated using a questionnaire. Results: First, after reviewing 11 guidelines by appraisal of the guidelines for research and evaluation collaboration II, 5 guidelines with a standardization grade of over 50 points were selected. An ECMO nursing protocol was developed based on these guidelines. Second, there were no statistically significant differences in physiological indicators between the two groups of patients. However, the experimental group showed a statistically significant decrease in the infection rate (p = .026) and pressure injury rates (p = .041). The levels of satisfaction with ECMO nursing care, and empowerment and performance of the nurses who used the ECMO nursing protocol were higher than those of nurses who did not (p < .001). Conclusion: This protocol may help prevent infections and pressure injuries in patients, and improve nurses' satisfaction and empowerment. The nursing protocol developed for critically ill patients receiving ECMO treatment can be utilized in evidence-based nursing practice.

Relationship between adhering to dietary guidelines and the risk of obesity in Korean children

  • BACKGROUND/OBJECTIVES: Dietary guidelines for Korean children were released in 2009. The goal of the present study was to examine diet quality in terms of adherence to these dietary guidelines as well as explore the association between guideline adherence and risk of obesity in Korean children. SUBJECTS/METHODS: Children aged 5-11 years (mean age = 8.9 years old, n = 191, 80.6% girls) were recruited from a university hospital in Seoul, Korea. Adherence to dietary guidelines for Korean children was calculated using the Likert scale (1-5), and children were then categorized into low, moderate, and high groups based on adherence scores. Obesity or being overweight was determined based on an age- and gender-specific percentile for body mass index (BMI) of the 2007 Korean National Growth Charts. Diet quality was evaluated from 3 days of dietary intake data. RESULTS: Children in the high adherence group were characterized by significantly lower BMI percentiles and paternal BMIs as well as higher percentages of fathers with a high level of education and higher household incomes compared to those in the low or moderate group. Children in the high adherence group consumed significantly higher amounts of milk and dairy products, were less likely to consume lower than the EAR of phosphorus and iron, and had higher NARs for calcium, phosphorus, iron, zinc, and MAR than those in low groups. The ORs for obesity ($BMI{\geq}95^{th}$ percentile) or being overweight including obesity ($BMI{\geq}85^{th}$ percentile) were significantly lower in the high adherence group compared to the low adherence group (OR: 0.33, 95% CI = 0.13-0.82, P for trend = 0.019; OR: 0.26, 95% CI: 0.11-0.61 P for trend = 0.002). CONCLUSIONS: Korean children who adhered to dietary guidelines displayed better diet quality and a reduced risk of obesity.

Stress and Self Care Guideline Adherence in Students with Influenza A (H1N1) (신종인플루엔자 감염학생의 스트레스와 자가치료지침 수행)

  • Purpose: This study examined levels of stress and adherence to self care guidelines in elementary and middle school students who were infected with the influenza A (H1N1) virus in 2009. Method: A total of 649 students from J city participated in the survey. Stress was measured with the Impact of Event Scale-Revised which was developed by Weiss and Marmar (1997) and translated by Eun and colleagues (2005). Adherence to self care guidelines was measured with the scale developed by the authors based on the self care recommendations from Korea Centers for Disease Control and Prevention. Result: Levels of stress and adherence to self care guidelines were higher in elementary school students. However, the severity of participants' stress was less than moderate. The adherence level was significantly different depending on status of taking an antiviral agent, health education and self care education regarding the virus infection. Conclusion: Findings suggest that effects of health education under the national crisis situation due to influenza A (H1N1) virus pandemic were helpful. More in-depth study is needed to understand and to improve middle school students' self care behaviors.
