Dilemmas Within the Korean Health Insurance System

  • The health insurance system in Korea is well-established and provides benefits for the entire national population. In Korea, when patients are treated at a hospital, the hospital receives a partial payment for the treatment from the patient, and the remaining amount is provided by the health insurance service. The Health Insurance Review and Assessment Service (HIRA) assesses whether the treatment was appropriate. If HIRA deems the treatment appropriate, the doctor can receive payment from the health insurance service. However, this system has several drawbacks. In this study, we aimed to provide examples of the problems that can occur in relation to HIRA assessments in Korea through actual clinical cases.

  • The Syndrome Differentiation(辨證) means to make diagnosises and to classify symptoms based oriental medicine theory. Questionnaires and clinical decision support system would assist the doctor checks up symptoms of a patient. We analyze about domestic and foreign patents and papers of these diagnosis tools and catch the trend. In patents, the system examines by telemedicine and offers medical information and prescriptions to patient. Papers was itemized fields ; theory, methods of analysis, clinical application, questionnaire, clinical decision support system. The results of this research can be applied to develop the high-quality tool to support syndrome differentiation.

국가 암 비용 감소를 위한 환자중심 진료의 적정성 확보 전략 (Strategies for Appropriate Patient-centered Care to Decrease the Nationwide Cost of Cancers in Korea)

  • In terms of years of life lost to premature mortality, cancer imposes the highest burden in Korea. In order to reduce the burden of cancer, the Korean government has implemented cancer control programs aiming to reduce cancer incidence, to increase survival rates, and to decrease cancer mortality. However, these programs may paradoxically increase the cost burden. For examples, a cancer screening program for early detection could bring about over-diagnosis and over-treatment, and supplying medical services in a paternalistic manner could lead to defensive medicine or futile care. As a practical measure to reduce the cost burden of cancer, appropriate cancer care should be established. Ensuring appropriateness requires patient-doctor communication to ensure that utility values are shared and that autonomous decisions are made regarding medical services. Thus, strategies for reducing the cost burden of cancer through ensuring appropriate patient-centered care include introducing value-based medicine, conducting cost-utility studies, and developing patient decision aids.

  • A decision support system (DSS) was developed to support doctor's decision-making in diagnosing hearing loss. The final diagnosis encompassed 41 diseases with the problem of hearing loss. The system was developed by integrating model-oriented DSS technique and artificial intelligence technology. The system can be used as both diagnosis tool and teaching tool for medical students. Furthermore, the AI technology obtained from this study may also be used in developing DSS for hospital management.

실손의료보험에 대한 한의사 인식도 조사 (Survey on Private Health Insurance Awareness of Korean Medicine Doctor)

  • Objectives : This study aimed to investigate the recognition of coverage for Korean medicine treatments in private medical insurance among Korean medicine doctors (KMDs). Methods : Questionnaire reviewed by experts was performed to KMDs who were registered in the Association of Korean Medicine. The survey targeted awareness of private health insurance for Korean medicine, appropriate coverage, cost, and frequency. Results : Data were collected from 932 respondents out of 28,234 Korean medicine doctors. Most KMDs were aware of coverage for Korean medicine in private medical insurance, and they responded that pharmacopuncture and herbal decoction were covered first. KMDs also responded that the coverage should be provided without limited number of times, except herbal medicine. Conclusion : Most KMDs responded that Korean medical private medical insurance was essential. To promote the developing insurance for Korean medicine, survey for public and insurance company will be performed.

항암화학요법으로 유발된 만성 말초신경병증 환자의 한의학적 치험 1례 : 2년 이상 지속된 항암화학요법 유발 말초신경병증 치험 (A Case Report of Chronic Chemotherapy-Induced Peripheral Neuropathy Treated by Korean Traditional Medicine)

  • Objective: This study reports the effect of Korean traditional medicine on persistent chemotherapy-induced peripheral neuropathy (CIPN). Methods: The patient was treated with Korean traditional medicine that included acupuncture, moxibustion, and herbal medicine. The effectiveness of the treatment was evaluated by the Quality of Life Questionnaire-Core 30 (QLQ-C30), the Quality of Life Questionnaire Chemotherapy-Induced Peripheral Neuropathy (QLQ-CIPN20), the Medicinae Doctor Anderson Symptom Inventory (MDASI), and a numeral rating scale (NRS). We also used the Beck Depression Inventory (BDI) to evaluate the patient's mood change after relieving the paresthesia. Results: After treatment, the symptoms were improved. Paresthesia decreased 80%p after Korean medicine treatment. Conclusions: According to this study, Korean traditional medicine can be an effective treatment for paresthesia in patients suffering from CIPN over 2 years.

한의사의 직무스트레스와 직업만족도에 관한 설문지 연구 (Questionnaire Study of Job Stress and Job Satisfaction of Oriental Medical Doctors)

  • This study was designed to investigate job stress, job satisfaction and related factors of oriental medical doctors. The survey used administered questionnaire, was conducted from June 10 to June 30, 2010 in Jeonbuk province. Questionnaire was composed of general characteristics, socioeconomic status perception, job stress scale and job satisfaction scale. Job stress was related to sex, age, marriage, working form, working time, clinical career. Job satisfaction was related to age. Working time, working form and self-status perception influenced job stress. Self-status perception influenced job satisfaction. Especially, it was supposed that working time was most important factor to job stress.

갑상선 기능 항진증 변증 도구 개발 기초 연구 (Development of a Basic Standard Tool for Pattern Identification in Hyperthyroidism)

  • Objective: The purpose of this study was to develop a basic standard tool for pattern identification in hyperthyroidism. Method: It was based on a review of the published literature in China and Korea. The advisory committee on this study included 8 internal medicine professors from Korean Medicine Schools and 2 who had a doctor's degree and specialized in internal medicine. We were advised about the importance of pattern identification, weight gain as a symptom, the importance of treatment, changes in symptoms, rare changes in symptoms, and frequency of prescriptions regarding hyperthyroidism. Results: A Korean instrument of pattern identification was completed. Conclusion: We sincerely look forward to improving the instrument through continuous clinical studies.

Cervical Spondylotic Myelopathy 환자에게 소시호탕(小柴胡湯) 투여 후 발생한 약물유해반응에 관한 임상 보고 (Adverse Drug Reactions of Patient with Cervical Spondylotic Myelopathy Treated by Soshiho-tang)

  • Objective : The purpose of this paper is to report the adverse drug reactions of patient with cervical spondylotic myelopathy(CSM) treated by herb medicine, Soshiho-tang. Methods : The patient was prescribed Soshiho-tang herbal medicine for 9days. The patient complained of dizziness, difficulty of concentration and elevation of blood pressure on the 6th day. The doctor made the patient stop to take the herbal medicine and every symptoms and abnormal blood pressure are normalized after 6 days. The abnormal responses were assessed by WHO-UMC Causality Categories and LDS scale. Results : The WHO-UMC Causality is 'probable/likely' and LDS scale is 5, it means 'moderate' severity. Conclusions : The herbal medicine, Soshiho-tang is relevant to abnormal responses of the patient with CSM. This is a first case report of hypertension induced by herbal medicine, Soshiho-tang in Korea.

Nurse-perceived Patient Adverse Events and Nursing Practice Environment

  • Objectives: To evaluate the occurrence of patient adverse events in Korean hospitals as perceived by nurses and examine the correlation between patient adverse events with the nurse practice environment at nurse and hospital level. Methods: In total, 3096 nurses working in 60 general inpatient hospital units were included. A two-level logistic regression analysis was performed. Results: At the hospital level, patient adverse events included patient falls (60.5%), nosocomial infections (51.7%), pressure sores (42.6%) and medication errors (33.3%). Among the hospital-level explanatory variables associated with the nursing practice environment, 'physician-nurse relationship' correlated with medication errors while 'education for improving quality of care' affected patient falls. Conclusions: The doctor-nurse relationship and access to education that can improve the quality of care at the hospital level may help decrease the occurrence of patient adverse events.
