신체 장애 여성노인들의 침상 재활운동 후 일상생활수행능력 변화 연구 (A Study on the Change of ADL in the Elderly Disability Women after the Self Rehabilitation Exercise at Bedside)

  • The improving in physical activity of Disability elder is the most important and effective way of prevent and treatment. Especially, It improves ADL(Activities Daily of Living) by decreasing restrictive function and dependence. This study was researched on change by comparing before and after about ADL of Women elders who were instructed self rehabilitation exercise in bedside and have been take that. The whole subjects are 92 persons. Elders aged 65 to 69 are 32 persons. elders aged 70 to 74 are 22 persons 38 persons are over the age of 75. This study shows that the whole subjects have been improved ADL. especially, "Take a bath" in detail items of ADL comes to the most improvement rate. Next, "Sit to stand on the bed and transfer" comes to the second highest rate. According to findings upon the age group. "Take a bath" is also the highest improvement rate. Elders aged 60 to 65 show "Sit to stand on the bed and transfer" is the second highest rate of improvement" and elders aged 70 to 74 show that "control urine" is the second highest rate of improvement. and elders over the age of 75 shows that "sit to stand on the bed and transfer" is the second highest rate of improvement. This study have some problems on having possibilities of results because hospitalized persons have answered imperfect statement and being compare progressed for short term with rehabilitation exercise. so, There are some causes that is early to be generalized.

보건진료소를 이용하는 농촌노인의 상실감과 자살생각과의 관계: 우울의 매개효과 (The Relationship between Sense of Loss and Suicidal Ideation among Rural Elders use Primary Health Care Post's Services : Mediating Effect of Depression)

  • 본 연구는 지역사회 보건진료소를 이용하는 농촌노인의 상실감과 자살생각과의 관계에서 우울의 매개효과를 확인하기 위한 서술적 조사연구이다. 연구대상자는 전남지역 보건진료소의 건강증진프로그램을 3년 이상 꾸준히 이용하고 이는 65세 이상의 노인으로 총 345명으로, 자료분석은 SPSS 21.0을 이용하여, 기술통계, 상관분석, 위계적 회귀분석, Sobel test 방법으로 분석하였다. 본 연구결과, 지역사회 보건진료소를 이용하는 노인의 상실감과 우울 및 자살생각은 모두 정적상관을 나타내었다. 또한 지역사회 보건진료소를 이용하는 노인의 상실감과 자살생각과의 관계에서 우울의 부분매개효과가 있음을 확인하였다. 본 연구결과에 근거하여 지역사회 노인의 자살생각을 감소시키기 위해서는 보건진료소를 이용하는 노인들의 상실감과 우울을 감소시킬 수 있는 중재방안이 필요할 것으로 보인다. 이에 본 연구자는 보건진료소에서 제공하고 있는 건강증진프로그램과 함께 우울을 감소시킬 수 있는 정신건강프로그램을 결합하여 확장시키는 것이 바람직할 것으로 사료된다.

재미한인 노인들을 위한 너싱홈의 물리적 주거환경에 관한 사례연구 - 재미한인 노인 거주자 관점을 중심으로 - (A Case Study Regarding Physical Environments of Nursing Homes for Frail Korean American Elders - Focused on Korean American Elderly Residents' Views -)

  • The purpose of this study was to describe the overall physical environment's features of nursing homes for Korean American elders in the United States. This investigation was conducted in Korean American nursing homes on the East Coast and in the Midwest with multiple qualitative data collection methods, such as interviews, observations, document review, and field notes. The findings of this study are as follows: 1) Although almost all respondents were unable to speak and understand English at all, most written materials in nursing homes were in only English; 2) The nursing homes were placed near or within the Korean communities; thus, it contributed to close ties with religious Korean American groups and other groups in the ethnic communities; 3) Whether the facility's physical features looked like those of a hospital or a home, many residents did not consider a nursing homes as a real home; 4) A fenced garden in the nursing home in the Midwest was the residents' favorite public space; 5) Due to being forced to room with residents who had dementia and were bedridden, they were more likely to have conflicts with their roommates and feel uncomfortable living in their room; and 6) The facilities seemed to be designed without consideration to protect residents' privacy.

한국노인의 우울, 자살생각 및 삶의 만족도 영향 요인 (Influencing Factors on Depression, Suicidal Thought, and Life Satisfaction of Korean Elders)

  • Purpose: The purpose of this study was to identify the factors that influence depression, suicidal thought, and life satisfaction of Korean elders. Method: The subjects for this study were 219 Korean elders living in three different provinces. The data was collected using structured questionnaires from October through December, 2007. Result: There were statistically significant differences in the levels of depression, suicidal thought, and life satisfaction according to age, health state, sleep state, economic state, feeling of loneliness, feeling of abuse, and having friends or neighbors. Suicidal thoughts correlated less with the above factors than depression did. Depression significantly correlated with age, life satisfaction, loneliness and suicidal thought. Suicidal thought was influenced significantly by depression. Life satisfaction was influenced significantly by depression, health state, and economic state. Conclusion: It could be concluded that nursing interventions for reducing depression and suicidal thought should be developed to improve life satisfaction and the health state of Korean elders as well as to relieve their feelings of loneliness.

Translation and Psychometric Testing of the Korean Versions of the Spiritual Perspective Scale and the Self-transcendence Scale in Korean Elders

  • Purpose: The purpose of this study was to translate the Spiritual Perspective Scale (SPS) and Self-transcendence Scale (STS) into Korean and test the psychometric properties of the instruments with Korean elders. Methods: A cross-sectional survey design was used to implement the three stages of the study. Stage I consisted of translating and reviewing the scales by six experts. In Stage II, equivalence was tested by comparing the responses between the Korean and English versions among 71 bilingual adults. Stage III established the psychometric properties of the Korean versions SPS-K and STS-K among 154 Korean elders. Results: Cronbach's alpha of the SPS-K and the STS-K .97, and .85 respectively with Korean elders. Factor analysis showed that the SPS-K had one factor; the STS-K had four factors with one factor clearly representing self-transcendence as theorized. Both scales showed good reliability and validity for the translated Korean versions. However, continued study of the construct validity of the STS-K is needed. Conclusion: Study findings indicate that the SPS-K and the STS-K could be useful for nurses and geriatric researchers to assess a broadly defined spirituality, and to conduct research on spirituality and health among Korean elders. Use of these scales within a theory-based study may contribute to further knowledge about the role of spirituality in the health and well-being of Korean people facing health crises.

노인의 건강정보이해능력, 건강 관련 위험인식과 건강행위 (Health Literacy, Health Risk Perception and Health Behavior of Elders)

  • Purpose: The aim of this study is to clarify the level of health literacy, health risk perception and health behavior of Korean elders and to determine the impact of their health literacy and health risk perception on their health behavior. Methods: A descriptive correlation study was conducted with 188 elders aged 65 or older in senior welfare centers in Busan. Questionnaires were used to measure levels of health literacy, health risk perception, and health behavior. Descriptive statistics, t-test, ANOVA, Kruskal Wallis test, Pearson's correlation, and hierarchical regression were performed. Results: 43.6% of the subjects had low levels of health literacy. There was a negative relation between health literacy and health risk perception, and between health risk perception and health behavior. There was a positive relation between health literacy and health behavior. Health concern, health literacy affected health behavior. Health literacy independently accounted for 24% of health behavior. Health risk perception didn't affect health behavior. Conclusion: Many of the Korean elders had low levels of health literacy and health literacy was independently associated with health behavior. These findings show that interventions for improving health literacy are necessary to enhance health behavior of the elderly.

장기 요양 시설 거주 치매노인의 식사행동 특성 (Characteristics of Eating Behavior in Elders with Dementia residing in Long-Term Care Facilities)

  • Purpose: The purpose of this study was to explore characteristics of eating behavior according to level of functional status of elders with dementia (EWD), and to examine feeding time, change in food intake and body mass index (BMI) according to eating behavior. Methods: Participants were 149 EWD residing in long-term care facilities located in Seoul or Gyeonggi province and evaluated using the Mini-Mental State Exam-Korean version, Korean version-Activities of Daily Living, and Eating Behavior Scale (EBS). Feeding time, change in food intake, and BMI were also measured. Data were analyzed using SPSS 17.0, specifically descriptive statistics, ANOVA, and Chi-square test. Results: Participants' mean EBS score was $10.43{\pm}6.01$ and half of them (54.4%) needed moderate or total assistance while eating. The EBS score was significantly lower for elders with severe dementia compared to those with mild or moderate dementia; and elders with severe ADL dependence compared to those with mild or moderate ADL dependence. Lower EBS scores were related to longer feeding time, a greater the rate of participants with decreased food intake and 'underweight' BMI. Conclusion: Nursing intervention programs which are designed for EWD are needed to maintain functional eating skills and prevent negative consequences in this population.

노인의 학대경험과 대처양식이 자살생각에 미치는 영향 (The Effect of Abuse Experience and Coping Styles on Suicidal Ideation in the Elderly)

  • Purpose: The aim of this study is to determine the impact of abuse experience and coping styles on suicidal ideation in Korean elders. Methods: Data were collected from the 2011 Korean National Survey on the Elderly. The participants were 10,537 elders aged 65 or older who had replied to the question on suicidal ideation. Socio-demographic and health related factors, abuse experience, and coping style were included as the influence factors of suicidal ideation. Data were analyzed using ${\chi}^2$-test and logistic stepwise regression with the SPSS/WIN 21.0 program. Results: The prevalence of abuse experience among the elders was 12.7% and that of suicidal ideation was 11.2%. Elders with abuse experience (OR=2.79, 95% CI=2.39~3.26) were more likely to have suicidal ideation but those with an active coping style (OR=0.69, 95% CI=0.57~0.84) were less likely to have suicidal ideation even with other influence factors controlled. Conclusion: Abuse experience of the elderly was a major risk factor that increased suicidal ideation, and active coping style was a protective factor that reduced suicidal ideation. These findings show that careful attention to elderly abuse and education for active coping style are necessary to reduce suicidal ideation among the elderly.

중국 연변지역 조선족 노인들의 의료정보 이해능력과 건강상태 (Health Literacy and Health Status of Korean-Chinese Elderly People Living in Yanbian, China)

  • Purpose: This descriptive study was done to identify the relationship between health literacy and health status and to provide basic data for developing nursing interventions for Korean-Chinese elders living in Yanbian, China. Methods: For data collection, intentional sampling of 300 elders was used. The questionnaire was composed of 5 items based on "Ministry of Health, the People's Republic of China(2008)" to measure health literacy, 33 health status items from the "Korean Health Status Measure for Elderly People" developed by Shin(2002), revised for use in China, and 9 general characteristics. Data were analyzed using SPSS Win 13.0 program. Results: Total level of health literacy was relatively high(68.7%). Elders had high scores for taking medicines according to doctor's instruction, but lower ones for full comprehension through communication with doctors. Health status was high for emotional, physical, and social function in that order. There were significant differences between general characteristics and health status for gender, age, marital status, education, family, smoking, and alcohol consumption in that order. Results of multiple regression analysis for factors influencing health status showed that self-report health level was the most influential, followed by health literacy, age, gender. Conclusion: Health literacy is the main factor affecting health promotion among minority elders indicating a need to develop health promotion programs for elders who have low health literacy.

노인 외로움의 영향요인 분석 - 지역사회 거주 노인을 중심으로 - (Factors Affecting Loneliness in Community Dwelling Korean Elders)

  • Purpose: This study was done to investigate the relationship of family function, self-esteem, life satisfaction, and general characteristics to loneliness in community dwelling Korean elders and identify factors affecting loneliness. Method: With a cross-sectional causal-relationship design and a convenience sample, 205 elders residing in three districts of the city of Seoul, S. Korea were recruited. Participants were assessed using the Family APGAR Score, Self-Esteem Scale, Life-Satisfaction Scale, and Revised UCLA Loneliness Scale. Results: The mean score for degree of loneliness (Mean=39.61, SD=10.09) was just below the mean for the scale (Possible range 20-80). Loneliness had significant negative correlations with family function (r= -.400, p<.001), self-esteem (r= -.399, p<.001), and life satisfaction (r= -.644, p<.001). Other general characteristics that had significant or nearly significant relationships with loneliness were perceived current financial and health status, whether doing any exercise or physical activities, degree of close relationship with family members, and length of living in current residence. Among variables, life satisfaction (Standardized ${\beta}\;=\;-.589$, p<.001) and length of living in current residence (Standardized ${\beta}\;=\;-.136$, p<.05) significantly predicted degree of loneliness. Conclusion: Findings of this study allow a comprehensive understanding of loneliness and related factors among community dwelling elders in Korea. However, further studies with a larger random sample from various living environments are necessary.
