  • Purpose: This study was to examine the effects of sleep promoting program on sleep and the immune response in the elderly with insomnia. Methods: The study was designed as a quasi-experimental, nonequivalent control group pre-post test design. Thirty two elders who suffered from insomnia (16 experimental and 16 control subjects) were selected among the elders who had been enrolled in two churches. The subjects in the experimental group participated in a sleep promoting program which was composed of sleep hygiene education and progressive muscle relaxation for 4 weeks. Data were analyzed using the SPSS/WIN. Results: The experimental group showed higher sleep score than that of the control group (t=7.86, p=.00). The experimental group showed higher sleep satisfaction score than that of the control group (t=7.61, p=.00). The sleep promoting program was not effective in enhancing immune response. Conclusion: The sleep promoting program increased the sleep score, sleep-satisfaction score and B cell in immune response of elderly people suffering from insomnia. Therefore, sleep promoting program can be applied as an effective nursing intervention to promote sleep quality and sleep satisfaction.

The questionnaire consisted of demographical characteristics and subjective symptoms of periodontal diseases by healthy life habits, dietary life habits, and oral healthy life habits. Data were analyzed by SPSS 18.0. Results: Smokers tended to have higher subjective symptoms of periodontal diseases than the non-smokers because smokers ingested more alcohol than the nonsmokers. The smokers tended to consume less meat, fish, beans and vegetables. The smokers did not brush their teeth frequently. They did not visit the dental clinics frequently for the scaling services. Conclusions: The development of oral health improvement program is very important and necessary for the elderly people and the program will enhance the health-related quality of life in the elderly.

  • Purpose: The purpose of this research is to identify factors affecting suicidal ideation of the low-income elderly. Methods: The study subjects were lowest-income(basic living allowance) and lower-income elders who were registered for home visiting services at five District Health Centers in Seoul. Interview data were collected from 280 elders and analyzed. Using SPSS/WIN 21, $x^2$-test and t-test were conducted to analyze the relationship between the subjects' characteristics and their suicidal ideation, and logistic regression was done to identify factors affecting suicidal ideation. Results: Of the subjects, 48.6% had suicidal ideation for the last one year. Self-evaluated psychological status, quality of sleep, depression, and loneliness were affecting factors to the subjects' suicidal ideation. Conclusion: Psychological and social factors are strongly related to the suicidal ideation of the elderly who are living alone in metropolitan areas. Therefore, home visiting services should supply physical, psychological and social cares continuously to the low-income elderly, especially those who live alone in metropolitan areas.

  • This study is aimed at identifying and evaluating the current operating model of elderly group homes in Korea, which were introduced with the amendment to the Welfare of the Aged Act in 2008, based on a survey of 25 managers of such homes. There are several key themes identified in the evaluation of their operation in terms of human resources, service, space and finance. The number of employees at the homes complied with the applicable law, with the majority having more employees than the minimum legal requirement for the care of residents. A wide variety of service programs were offered for residents. Typically located within detached houses purchased on the first floor, the homes varied in size from 73 square meters to 560 square meters, with each having a distinctly residence-like atmosphere. The greatest challenge such homes face was a shortage of financial resources. Many struggled to operate because they have no other source of revenue than payments from residents. Consequently, to help promote elderly group homes, there needs to be state-level support for the view that providing assistance for self-sufficient elders can save social costs in the long run by delaying their transition to a physical state requiring constant nursing.

  • Purpose: This study was performed to investigate the influencing factors of cognitive function and depression in elderly. Method: From 3 provinces, 282 elders who resided in community and facilities completed structured questionnaires, including cognitive functions, depression, self-esteem, ADL, and IADL. Data were analyzed by applying ${\chi}^2$-test, ANOVA, partial correlation coefficient, and stepwise multiple regression analysis with SAS 8.12. Results: Community dwelling aged people showed higher scores in MMSE-K, self-esteem, ADL and IADL, and showed lower scores in depression than facility elders. MMSE-K was positively correlated in self-esteem, ADL, and personal cognition of health, except depression. Major factors that affect cognitive function of elderly were residual type, age, and IADL. In addition, major factors that affect depression of elderly were self-esteem, personal cognition of health, and marital status. Conclusions: Based on the results above, it is necessary to identify the status of cognition and depression in the elderly, and to develop nursing intervention programs, which improve cognitive function and reduce depression for aged, especially for the facility admitted aged.

Method: The subjects consisted of 371 elderly who has at least one adult child classifying two groups(living with children and not living with children). The data were collected by a structured questionnaire that included general characteristics, Geriatric Leisure Activity Scale, Geriatric Life Satisfaction Scale, Geriatric Depression Scale, from March to December, 2004. The collected data were analyzed by SPSS program including descriptive statistics, ${\chi}^2-test$, t-test, Pearson Correlation Coefficient and Hierarchical Regression. Result: In hierarchical regression, the elders who live with their children showed more life satisfaction than elders who lived by themselves. However, living arrangement showed no effect on the level of depression of the elderly parents. Significant leisure activity interaction effect was found on the depression among the elderly: The elderly with no leisure activity reported lower levels of depression when they lived with their adult child. Conclusion: It is necessary to explore further the various relationship among living arrangement and life satisfaction of the elderly, their preferences and expectations regarding inter-generational obligations and living arrangements.

  • Purpose: This study was conducted to evaluate the effects of foot reflexologic massage on blood pressure and sleep of elderly with essential hypertension. Method: This study was employed non-equivalent control group pretest-posttest quasi-experimental design. The subjects were divided into two groups: 20 elders for experimental group and 18 elders for control group. For experimental group, foot reflexologic massage was done three times at three-day intervals. The data analyzed by SPSS version 10.0 program. Result: The systolic and diastolic blood pressure after foot reflexologic massage did not show a significant difference between the two groups. In experimental group, there was a significant difference in the systolic(t=3.559, p=.001) and the diastolic blood pressure(t= 3.048, p=.004) that measured after foot reflexologic massage. There was a significant difference(t=2.665, p=.011) in sleep between the two groups. Conclusions: The results suggest that foot reflexologic massage was partially effective to reduce the degree of blood pressure and improve sleep of the elderly with essential hypertension. A follow up research is needed to compare and confirm its long-term effects.

  • Purpose: This study was conducted to contribute on nursing practice for elderly people by identifying the effects of hand moxibustion and hand press pellet therapy on the alleviation of constipation. Methods: The data were collected from October to November, 2009, and the subjects included 39 elders (experimental group: 20, control group: 19) from a senior citizens center in B City who reported constipation. Experimental group received an intervention of hand moxibustion and hand press pellet therapy three times a week for six weeks (total: 18 times). Data about the number of defecations per week and constipation degree scores were collected one week after each treatment. Results: 1. The number of defecation in the experimental group were different from that of the control group (F=228.26, $p$ <.001). 2. The degree of reported constipation in the experimental group differed from the control group (F=170.59, $p$ <.001). Conclusion: Hand moxibustion and hand press pellet therapy was shown to be effective in alleviating the constipation of elderly people by increasing the number of defecation per week and reducing the constipation degree scores. These two interventions can thus be used for alleviating the constipation of elderly people, replacing the stool softener and enema.
