Purpose: The purpose of this study was to analyze the concept of self-care in elders with dementia through a review of nursing literature and to provide more understanding of the definition and perspectives of the concept of self-care notion in elders with dementia. Methods: The technique developed by Walker and Avant was used as a guide in analyzing the concept of self-care. Results: Attributes of self-care in dementia may include a single or group of actions needed for sustaining life, a personal effort to maintain functional independence while minimizing other's assistance, an outcome behavior from the person's interaction with inter-personal and/or contextual environment, and a functional ability that may decline in parallel to cognitive impairment. Antecedents of self-care in dementia may include at least presence of a certain degree of cognitive appraisal for the self-care needs, self-willingness for the self-care action, spatial and visual orientation, cultural pre-conception of the self-care behavior, presence of environmental context/equipment available for self-care, and sufficient time available. The consequences may include sustaining of life, feel of satisfaction, achieving independence, extended life expectancy, increased self-confidence, decreased caregiver distress and/or burden, savings in health care costs. Discussion: Defining attributes and antecedents and consequences of self-care in dementia identified in this study provided empirical ground of a middle-range theory of self-care for a clinical population with dementia and generated possible hypotheses to be tested in future studies.
Purpose: This study was done to identify effects of a fall prevention program on physical fitness and psychological functions in community dwelling elders. Methods: A quasi-experimental study was carried out with a nonequivalent control group pre & post-test design. The program, which included exercises and education, consisted of a 12-week group program and an 8-week self-management program using a health calendar. An experimental group (32) and a control group (21) participated. Results: There were significant differences in SPPB (t=-3.92, p<.001), TUG test (t=4.94, p<.001), standing with right leg (t= -3.60, p=.001), standing with left leg (t=-3.74, p<.001), front and rear maximum step length test (t=-4.34, p<.001), right-left maximum step length test (t=-2.65, p=.011), and fall efficacy (t=-2.42, p=.019). Fall efficacy, fear of falling and depression showed significant differences following the 12-week exercise program and 8-week self-management program in the experimental group. Conclusion: Study findings indicate that the fall prevention program is an effective nursing intervention to enhance physical fitness and psychological functions for elders. Using a health calendar, the self-management program was more effective for psychological functions compared to only the group program. Therefore, health providers should develop diversified fall prevention programs which include motivation plans to encourage clients in participating.
Purpose: The purpose of this study was to identify family resiliency, ADL in elders with dementia, and symptom for families with an elder with dementia. Method: The participants were 102 people who were caregivers to elders with dementia and who visited the out-patient department at S. hospital for follow up care. Data were collected from August to October 2003 using a questionnaire. The collected data were analyzed using descriptive statistics and t-test aided by the SPSS/PC. Result: Family resiliency for the whole sample was within normal limits but when the sample was quarter-divided by good and poor family resiliency, based on established cut-off scores, there were differences for some characteristics according to good/poor family resiliency. These differences were for perceived health state of caregivers (t=-2.78, p=.008), economic state of family (t=-3.34, p=.002), and ADL (t=-2.56, p=.014). Conclusion: Future research should focus on the way in which family resiliency can be enhanced and on the effects of interventions for those caregivers who report poor family resiliency.
Purpose: This study has been conducted to develop and examine a health promotion empowerment program using a lay health leader for frail elderly. Methods: The research was organized in a nonequivalent control group pre-post test design. Data collection was performed from August 18 to October 8, 2015. The subjects included 76 frail elders aged over 65 registered in home visiting services (Experimental group=39, Control group=37). A health promotion empowerment program using a resident volunteer as a lay health leader was run for 8 weeks. Health factors (health promotion behavior, perceived health status and frailty) and empowerment factors (empowerment, social participation) were assessed. The data were analyzed by using the SPSS/WIN 18.0 program. Results: After the program, health promotion behavior, perceived health status and social participation increased in the experimental group more significantly than in the control group, but frailty decreased in the experimental group greater than in the control group. Conclusion: The health promotion empowerment program using a resident volunteer as a lay health leader was effective. Therefore, the health promotion empowerment program needs to be expanded to other frail elders. Also, a health leader should be recommended as a public health resource and systematically managed.
Purpose: This study was conducted in order to identify the effect of a community based health promotion program for elders in Korea. Methods: The PICO-SD (Participants, Intervention, Comparison, Outcome, and Study Design) strategy was established. A search of the electronic bibliographic database of NDSL, RISS, KMbase, and KoreaMed etc. was performed and 28 articles met inclusion criteria. Results: Both exercise and health education were in most programs and were more effective than one of the two. The health education included stress management, mental health, health promotion lifestyle, chronic disease, and medication. Various exercises such as walking, stretching, gymnastics, rhythmic activity, muscle strength, and joint exercise were performed. The programs for elders showed an association with significantly improved muscular strength, flexibility, depression, quality of life, subjective health status, satisfaction with life, self-efficacy, and health promotion behavior. Conclusion: Intervention programs including both exercise and health education are effective in improving health promotion behavior and physical and psychological health status in elders. Therefore, these results could provide useful guidelines for development of effective health intervention programs for the elderly.
The nutritional status and chronic disease occurrence of the elderly living in local areas is affected by certain factors, including low energy intakes, low food diversity, poor nutritional quality and living alone. Moreover, elderly people who live alone may have low incomes and be socially isolated. Thus, we have provided them adjustable and balanced menus using standard recipes. In this study, we investigated aspects of nutritional status and living conditions in the elderly status, in relation to food, activities of daily living(ADL), nutrition risk index, average intake of calories and nutrients, the mini dietary assessment index score, depression score, menu satisfaction, menu demand, satisfaction with menu offerings. The result indicated significant nutritional improvements by providing menus to the elders and suggest that by providing adjustable and balanced menus using standard recipes, the nutritional status of isolated elders can be changed and improved.
Purpose: The purpose of this study was to investigate the importance of nurses' caring behaviors and to identify influencing factors on nurses' caring behaviors for elders with dementia. Methods: Data were collected through questionnaires from 156 nurses in several demented elderly setting and analyzed by applying descriptive statistics, t-test, ANOVA, Scheffe-test, Pearson's correlation coefficient and stepwise multiple regression with SPSS/WIN 16.0. Results: The average score for importance of nurses' caring behaviors was 3.55 and the 'attentive to other's experience' factor was the highest level. The mean score of nursing professional value was 8.46 and job satisfaction, 3.86. Importance of nurses' caring behaviors had a significant positive correlation with nursing professional value, job satisfaction and nurse's age. Four significant variables influencing importance of nurses' caring behaviors were job satisfaction, nursing professional value, working area of nurses and cognition dysfunction of elderly. Conclusion: With the results of this study, importance of nurses' caring behaviors can be improved by intervening the factors affecting this importance. Based on the findings of this study, further nursing practice and research for nurses' caring the elders with dementia should be focused on job satisfaction, nursing professional value, working area of nurses and cognitive dysfunction of elderly.
Purpose: The purpose of this study was to evaluate the effects of BeHaS exercise program on pain, balance, and falling efficacy in elderly with osteoarthritis. Method: A convenience sample of 52 elderly people with osteoarthritis who lived in C city were recruited through the public health center. For this quasi-experimental study design, 31 of the elders were assigned to the experimental group which received the BeHaS exercise program twice week for eight weeks, and 24 were assigned for the control group (no intervention). Outcome measures were numeric pain scale, standing on one leg for balance, fall efficacy scale. Analysis was performed with SPSS version 12.0 program. Results: Pain was decreased significantly in the experimental group as compared to the control group, Balance and fall efficacy were improved significantly in the experimental group as compared to the control group. Conclusion: The BeHaS exercise program can be effective in improving pain, balance, and fall efficacy in elders with osteoarthritis.
Purpose: The purpose of this study was to identify the factors that affect quality of life in elderly people with dementia. Methods: The participants for this descriptive correlational study were 97 elderly patients with dementia who were attending a daycare center or were being cared for at home. Data were collected from the participants using the Mini-Mental Status Examination-Dementia Scale, the Geriatric Depression Scale (Short Form), Activity of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) Scale. Results: There were statistically significant differences in quality of life of elders with dementia according to mobility, using dementia day-care center, and cohabitation. In multiple regression analysis, depression, activity of daily of living, using dementia day-care centers and cohabitation were significant predictors of quality of life and the four variables explained 60% of the variance in quality of life. Conclusion: Findings indicate that to enhance the quality of life for elders with dementia, depression needs to be decreased and regular use of daycare service recommended. Further, various programs enhancing physical ability need to be developed and used with these elders.
Purpose: This study was done to provide statistical data for developing client-needs based welfare services in community welfare facilities. Method: The participants were 270 senior citizens, who visited a community welfare center in Gyunggi province, during the month of October 2005. They were asked to answer a structured questionnaire. Descriptive statistics, $X^2$ test and Fisher's exact test were used to analyze the data from 221 of the elders. Results: 1. More than 50% of the respondents were women aged between 60 and 75, who had education levels above high school and who lived in apartments. Among the respondents, 54.3% lived separately from their children. 2. 70.0% of the respondents considered their health status as "good", even though they had more than one disease. Also 41% of the respondents were supported by their children, and 76.8% wanted jobs. 3. The respondents pointed out several programs such as languages, computer, singing class, physical activities (dances, billiards, Tai-chi and table tennis), needed to be included in the welfare center programs. Conclusions: Elders aged 60-75, educated above high school, living in an apartments, live alone or with spouse, and having frequent contact with children, were the most frequent utilizers of the welfare center. They want the welfare center programs in variety, including languages, computer, singing class, and physical activities.
