간호대학생과 병원간호사의 가상현실기반 환자안전을 위한 팀 의사소통과 팀워크 기술에 대한 교육요구의 차이: 혼합연구설계 (Nursing Students and Clinical Nurses' Awareness of Virtual Reality(VR) Simulation and Educational Needs of VR-based Team Communication and Teamwork Skills for Patient Safety: A Mixed Method Study)

  • 본 연구의 목적은 간호대학생과 간호사를 대상으로 가상현실기반 환자안전을 위한 의사소통과 팀워크 기술 교육요구를 파악하기 위함이다. 본 연구는 간호대학생 60명과 간호사 123명에게 질문지 조사를 시행하고, 간호사 14명에게 포커스 그룹 인터뷰를 시행한 혼합연구설계를 사용하였다. 자료 분석은 SPSS/WIN 25.0 및 포커스 그룹 인터뷰 후 귀납적 내용분석방법을 이용하였다. 연구결과는 대상자의 가상현실인식과 팀 의사소통 및 팀워크 기술 교육요구는 보통수준 이상이었고 간호대학생에서 가상현실인식과 교육요구가 더 높았다. 팀워크 지식은 보통수준으로 두 군 간에 유의한 차이가 없었다. 질적 자료 분석에서 간호사는 병원에서 의사 또는 타부서 직원과의 의사소통 어려움을 일으키는 4개 주제와 결과요인 2개 주제가 도출되었다. 본 연구는 대상자들의 가상현실에 대한 인식은 긍정적이었으며, 환자안전을 위해서 팀 의사소통과 팀워크 교육의 필요성을 보여주었다. 따라서 대상자들의 교육요구를 반영한 가상현실기반 시뮬레이션 프로그램을 개발하여 간호대학생과 간호사에게 요구되는 환자안전을 위한 팀 의사소통과 팀워크 기술 역량을 향상할 필요가 있다.

간호대학생의 환자안전관리(Patient Safety Management) 수행에 미치는 영향요인 (Factors affecting on the Practice of Patient Safety Management (PSM) in Nursing College Students)

  • 본 연구의 목적은 간호대학생의 환자안전관리에 대한 태도, 지식, 수행자신감 및 중요도 인식이 환자안전관리 수행도에 미치는 영향을 확인하기 위함이다. 연구대상은 D시 소재 간호대학에 재학 중인 3,4학년 139명을 대상으로 하였으며, 자료수집은 2018년 11월 26일부터 2018년 11월 30일까지 시행하였다. 자료분석은 Descriptive statistics, Paired t-test, Independent t-test, One-way ANOVA, Scheffe 사후검증, Pearson's correlation coefficient, Multiple regression을 이용하였다. 연구결과 환자안전관리 수행도는 5점 만점에 평균 $4.25{\pm}0.48$점이었다. 환자안전관리 수행도는 태도(r=.39, p<.001), 수행자신감(r=.43, p<.001), 중요도 인식(r=.54, p<.001)과 양의 상관관계를 보였다. 환자안전관리 수행도에 미치는 영향요인은 중요도 인식(${\beta}=.43$, p<.001)과 수행자신감(${\beta}=.26$, p=.001)으로 환자안전관리 수행도에 대한 설명력은 38%로 나타났다. 따라서 간호대학생의 환자안전관리 수행도 증진을 위하여 환자안전관리에 대한 중요도와 수행자신감을 포함시켜 교육 프로그램을 개발 할 필요가 있다.

Laser Application and Nursing in the Field of Gynecology

  • The recent development of new surgical techniques using lasers has increased the opportunities for open surgery involving minimal manipulation and faster and more accurate removal of lesions. The increasing use of laser technology requires nurses to play an extensive role. As assistants, nurses play an important role in maintaining the efficacy and safety of the laser device. In addition, they are also responsible for providing pre-and post-operative care to patients. Therefore, nurses should be aware of how to proceed with operative laser treatment for all surgical procedures and the steps for maintaining safety prior to, during, and after laser treatment. This review provides in-depth knowledge for nurses undertaking continuing education on lasers and patient care in the field of gynecology.

수술실의 간호오류 및 과오 예방을 위한 E-learning 실무교육 프로그램의 개발 및 평가 (Development of an E-learning Education Program for Preventing Nursing Errors and Adverse Events of Operating Room Nurses)

  • Purpose: This study was to develop, implement, and evaluate an e-learning education program for improving practical knowledge and preventing nursing errors and adverse events of nurses working in the operating room (OR). Method: The e-learning program was developed and evaluated according to the following processes: 1) preparation phase 2) implementation phase 3) evaluation phase. In evaluation phase, the effectiveness was analyzed based on the Kirkpatrick's model. Results: The e-learning program consisted of OR basic nursing skills and techniques and nursing activities' manual based on the categories of nursing errors: surgical operation preparation, nursing skills and techniques, environment management, patient safety and comfort, and patient monitoring. The program was provided through on-line, http://cafe.daum.net/pnuhorn, for 4 weeks. The mean score(percent) of participants' satisfaction was $21.24{\pm}1.71$(68.2%). Their total knowledge level was significantly improved(Z=-3.00, p=.003) and specifically in the category of environment management(Z=-3.77, p<.001) and patient monitoring(Z=-2.46, p=.014). The occurrence of nursing errors or adverse events was a little decreased, but not statistically significant(Z=-3.10, p=.756). Conclusion: E-learning for nurses is one way of effective and efficient teaching-learning strategies. For better e-learning, it is important to develop the vital content of the education and objective measures for detecting nursing errors and adverse events.

급성기 의료기관의 일혈 발생 현황 및 임상간호사의 일혈 예방 및 관리에 대한 지식과 태도에 관한 연구 (Incidence of Extravasation in Acute Care Hospitals and Nurses' Knowledge and Attitude toward Extravasation Prevention and Management)

  • Purpose: Extravasation of diagnostic and therapeutic materials might occur when the intravascular solution leaks into the surrounding tissues. Injury associated with extravasation depends on various factors. It may range from mild skin reaction to severe necrosis. However, the incidence rate for extravasation is largely unknown because of the limited reporting in Korea. Therefore, this study was conducted to identify the incidence of extravasation and nurses' attitude and knowledge of extravasation for providing high-quality nursing care. Methods: Three acute care hospitals were surveyed to estimate the occurrence of extravasation. Knowledge and attitude toward extravasation were investigated from 793 nurses working in six hospitals. Results: The incidence rate of extravasation was 0.5%. Extravasation commonly occurred in elderly patients aged 66 or older (59.9%) and internal medicine (48.2%), and it happened 13.73±20.68 days after hospitalization on average. It mostly occurred in the forearm site (52.9%) and was mainly caused by parenteral nutrition (33.6%). The mean scores of nurses' knowledge and attitude were 14.63±2.86 and 28.91±36.00, respectively. There was a significant negative correlation between the subjects' knowledge and attitude (r=-.11, p=.002). Conclusion: It is necessary to have a reporting system that can accurately monitor the occurrence of extravasation for patient safety management. In addition, it is necessary to develop a protocol that can be applied to clinical practice and a nurse education program.

  • 3-dimensional information for anatomic stucture plays a role as integral part in clinical aspect of dental practice. CBCT(cone beam computed tomography) has been accepted as useful diagnostic tool offering Volume data and images for evaluating teeth and jaws in lower radiation dose than conventional CT. CBCT equipment is essential for the quality assurance of it to ensure continued satisfactory performance and result of adequate images. Dental practitioner and oral and maxillofacial radiologist should have a responsibility and critical thinking to deliver this technology to patients in a responsible way, so that diaganostic value is maximised and radiation doses kept as low as resonably achievable. CBCT imaging modality should be used only after a review of the patient's health and imaging history and the completion of a thorough clinical examination. Clinical guidelines are systematically developed statements to assist practitioner and patient decisions about appropriate health care for specific clinical circumstances Dental practitioners should prescribe CBCT imaging only when they expect that the diagnostic yield will benefit patient care, enhance patient safety or improve clinical outcomes significantly. Knowledge of patient dose is essential for clinicians who are making the decision regarding the justification of the exposure. There are some limitation in the measurement of patient dose in CBCT for the approval and adaptation of conventinal methodolgy in CT. It is also important to ensure that doses are optimised and in line with any national and international guidelines. The higher radiation doses of CBCT compared with conventional radiography, mean that high standards must be maintained. The Quality Assurance(QA) programme should entail surveys and checks that are performed according to a regular timetable. QA programme should be maintained by staff to ensure adherence to the programme and to raise its importance among staff.

Carbon monoxide poisoning-induced encephalopathy in a carbon dioxide arc welder: a case report

  • Background: It is widely known that carbon dioxide (CO2) arc welding generates carbon monoxide (CO). However, to the best of our knowledge, no case reports have been published regarding CO poisoning in CO2 arc welders. Therefore, we aimed to report a case of CO poisoning-induced encephalopathy in a CO2 arc welder in the Republic of Korea to inform about the dangers of CO exposure among CO2arc welders. Case presentation: A 40-year-old man working as a CO2 arc welder for 15 years visited a local hospital with a tremor, involuntary urination, and speaking gibberish, on April 9, 2019. He stated that he had intermittent headache and forgetting symptoms for the last 5 years, and had been lost on the way to work several times. On April 9, 2019, he was diagnosed with CO poisoning-induced encephalopathy through brain magnetic resonance imaging. He received hyperbaric oxygen therapy, and some of his symptoms improved. According to the exposure assessment of his work environment, he was continuously exposed to high concentrations of CO for 15 years while operating CO2 arc welding machines. Conclusions: After evaluating the patient's work environment and evaluating his medical history, we concluded that his encephalopathy was caused by CO exposure during CO2 arc welding. Thus CO2 arc welders must be aware of the risk of CO poisoning and strive to avoid CO exposure.

화학 공장에서 근로자의 사건 보고 의지에 영향을 미치는 인자 (Factors Affecting Workers' Willingness to Report Incidents in Chemical Plants)

  • Lack of incident reporting culture has been pointed out as a potential risk factor across industries. Consequently, comprehensive understanding of the factors affecting the willingness to report (WTR) is necessary. However, our knowledge on the related studies are not enough to date and even most parts of the existing studies have focused on patient safety, which makes it difficult to universally apply the factors to all industries. Therefore, this study aimed to identify WTR factors in chemical plants and evaluate the influence degree of the factors. To do this, 45 candidate factors were summarized throughout the previous studies and surveying plant safety staff. A questionnaire survey was conducted for 614 all employees from 9 chemical plants which belong to one company, and finally 32 WTR factors were identified throughout 520 responses. Of these, 19 factors were consistent with the previous studies and 13 factors were newly identified. The most influential factor was 'Views on the necessity of incident reporting', and 'Reporting practice by outcome severity', 'Fear of vilification, conflicts, blame, or sanctions' were followed. This result not only suggests various WTR factors suitable for chemical plants, but also shows need to derive specific factors that are appropriate to each industry. An empirical study could be expected to increase incident reporting by using these factors and verify its effectiveness on injury rate.

우리나라 전공의 수련교육 구성 및 운영에 대한 제안 (The Proposal for Residency Educational Programs)

  • In clinical clerkships, residents function as trainees, workers, and teachers for other medical students. Although residents care for patients in harsh environments and encounter precarious patient-safety situations, they are working towards becoming competent specialists. Residency education programs are very important in cultivating specialists able to adapt to the rapidly-changing medical environment, and are also necessary to improve the quality of specialist training. Competent specialists not only need clinical competency, but also a wide range of abilities including professionalism, leadership, effective communication, cooperation, and attention to continuous professional development/continuing medical education activities. Each Korean association of specialties has its own educational goals and standardized education programs to help residents learn specific techniques and competencies related to medical care for patients, though the training environment of each residency is different within each trainee hospital. Although it is also important to evaluate residency education programs, currently there is only an examination of knowledge and assessment of skills based on mini-clinical evaluation exercises or direct observation of procedural skills. In order to develop an objective and estimable evaluation tool that can assess the overall achievement level within each training course, it is necessary to evaluate the knowledge, skills, and attitudes of residents. Residency education programs need further attention and reform.

A long-term subacute sclerosing panencephalitis survivor treated with intraventricular interferon-alpha for 13 years

  • Subacute sclerosing panencephalitis (SSPE) is a rare, progressive, and fatal central nervous system disorder resulting from persistent measles virus infection. Long-term data are scarce, with a maximum follow-up period of 10 years. Interferon-alpha ($IFN-{\alpha}$) is a protein that exerts its antiviral activity via enhancement of cellular immune response and is reported to be an effective drug for the treatment of SSPE. However, there is currently no consensus regarding the optimal duration of $IFN-{\alpha}$ therapy. Here, we present a case report of a patient with SSPE treated with long-term intraventricular $IFN-{\alpha}$ therapy, which facilitated clinical improvement and neurological stabilization without causing serious adverse effects. To the best of our knowledge, this is one of the longest follow-up studies investigating a patient with SSPE receiving intraventricular $IFN-{\alpha}$ treatment. Further studies are necessary to validate the benefits and safety of long-term intraventricular $IFN-{\alpha}$ treatment in patients with SSPE.
