  • Objective : Unilateral facet dislocation of the cervical spine occurs by flexion and rotation injuries and cannot be easily reduced by axial traction. We analyzed 14 consecutive patients with unilateral facet dislocation of the cervical spine to increase knowledge about anatomical reduction of locked facet and factors for successful reduction. Methods : Fourteen patients [10 men and 4 women] with unilateral facet dislocation of the cervical spine were retrospectively analyzed. Plain X-ray, computerized tomography scan, and magnetic resonance imaging were performed. All patients underwent manual reduction and surgery with anterior interbody fusion and plate fixation. The manual reduction was performed by neck flexion and rotation to the opposite side of dislocation, followed by rotation and flexion of the head toward the side of dislocation and extension with relaxation of traction. Mean follow-up period was 17 months. The level of spine, amount of subluxation, combined facet fracture, and time from injury to initial reduction were analyzed using the data obtained from medical records. Results : Thirteen [93%] patients were reduced successfully. Immediate reduction was achieved in 7 patients but failed in 7 patients. Seven patients underwent delayed closed reduction under general anesthesia, and successful reduction was achieved in 6 patients. Only one patient with bone chips between articular facets failed to achieve anatomical reduction. Conclusion : In order to reduce the locked facet more easily and safely, we recommend manipulative traction with anterior interbody fusion and plate fixation under general anesthesia after being aware of spinal cord injury with magnetic resonance imaging.

  • Purpose: This study aimed to investigate the potential factors that could affect the reduction rate of odontogenic cysts following decompression using cone-beam computed tomography (CBCT) for 3-dimensional volumetric analysis. Materials and Methods: The study sample consisted of CBCT images of 41 individuals who underwent decompression of odontogenic cysts at the Department of Oral and Maxillofacial Surgery, Faculty of Dentistry, Chulalongkorn University, between 2010 and 2022. Preoperative and postoperative CBCT results were collected, and a volumetric analysis was conducted to evaluate the differences in the reduction rate and the percentage of volume reduction of cystic lesions based on different parameters. Correlations between these parameters were analyzed to determine associations. Results: In this study, the average time of decompression for odontogenic cysts was 316 days. Males demonstrated a higher reduction rate than females (P<0.05). The reduction rate was directly proportional to initial cyst volume, with higher reduction rates for cysts with large initial volume than those with small initial volume (P<0.05). Spearman's rank correlation coefficient indicated a weak positive correlation between the initial cyst volume and the duration of decompression. Additionally, a strong positive correlation was observed between the initial volume and the reduction rate. Conclusion: Knowledge of the reduction rate of odontogenic cysts is vital for surgeons to evaluate the duration of decompression before enucleation and to determine a definitive treatment plan. Sex and initial lesion volume had significant effects on the reduction rate.

  • Management of perimenstrual discomfort could be an effective strategy for women's health promotion. This study was designed to develop and test the nursing intervention program for the reduction of the perimenstrual discomfort. The data were collected from a group of 76 female university students (34 in the intervention group, 42 in the control group) during November 20, 1997 and March 10, 1998. Measurements were taken concerning perimenstrual discomfort, self care behavior, and perimenstrual discomfort from both groups, at pre and post intervention. Intervention program involved education, support, counseling, and relaxation training, via individual and group levels over a twelve-week period. Listed are the summarized results : 1. The intervention group had more knowledge con corning perimenstrual discomfort(t=2.290, p=.025), self care behavior(t=3.198, p=.000), and lower perimenstrual discomfort score (t=-4.446, p=.000) than the control group at post intervention. 2. The intervention group showed an enhancement on the knowledge on perimenstrual discomport (t=2.35, p=.025), and decreased the perimenstrual discomfort score(t=-7.36, p=.000). However change of self care behavior was not significant during the intervention. According to this study a developed nursing intervention program is effective for reduction of the perimenstrual discomfort. For a future research, it is necessary that experiments concerning menstruation, and a correlational study of perimenstrual discomfort and stress ill be performed.

  • In an attempt to retrieve useful information for an efficient decision in the large knowledge system, it is generally necessary and important for a refined feature selection. Rough set has difficulty in generating optimal reducts and classifying boundary objects. In this paper, we propose quick reduction algorithm generating optimal features by rough entropy analysis for condition and decision attributes to improve these restrictions. We define a new conditional information entropy for efficient feature extraction and describe procedure of feature selection to classify the significance of features. Through the simulation of 5 datasets from UCI storage, we compare our feature selection approach based on rough set theory with the other selection theories. As the result, our modeling method is more efficient than the previous theories in classification accuracy for feature selection.

  • Floods are the most common natural disasters and are annually causing severe destructions worldwide. Human activities, along with expected increased extreme precipitation patterns as a result of climate change enhance the future potential of floods. There are proven evidence that infrastructure based responses to flood disaster is no longer achieving optimum mitigation and have created a false sense of security. Nature-based solutions(NBS) is a widely accepted sustainable and efficient approach for disaster risk reduction and involves the protection, restoration, or management of natural and semi-natural ecosystems to tackle the climate and natural crisis. Adoption of NBS in decision-making, especially in developing nations is limited due to a lack of sufficient scenario-based studies, research, and technical knowledge. This study explores the knowledge gap and challenges on NBS adoption with case study of developing nation, specially for flood management, by the study of multiple scenario analysis in the context of climate, land-use change, and policies. Identification and quantification of the strength of natural ecosystems for flood resilience and water management can help to prioritize NBS in policymaking leading to sustainable measures for integrated flood management.

  • Fuel lean reburning method is very attractive way in comparison with conventional reburning method for reducing NOX. Meanwhile, the knowledge of the how flue gas re-circulated, temperature distribution and species concentration is crucial for the design and operation of an effective fuel lean reburning system. For this reason, numerical analysis of fuel lean reburning system is a very important and challenge task. In this work, the effect of fuel lean reburn system on NOX reduction has been experimentally and numerically conducted. Experimental study has been conducted with a 15kW lab scale furnace. Liquefied Petroleum Gas is used as main fuel and reburn fuel. To carry out numerical study, the finite-volume based commercial computational fluid dynamics (CFD) code FLUENT6.3 was used to simulate the reacting flow in a given laboratory furnace. Steady state, three dimensional analysis performed for turbulent reactive flow and radiative heat transfer in the furnace.

  • Purpose: The purposes of this study were to develop an educational program to reduce the use of physical restraints for caregivers in geriatric hospitals and to evaluate the effects of the program on cargivers' knowledge, attitude and nursing practice related to the use of physical restraints. Methods: A quasi experimental study with a non-equivalent control group pretest-posttest design was used. Participants were recruited from two geriatric hospitals. Eighteen caregivers were assigned to the experimental group and 20 to the control group. The data were collected prior to the intervention and at 6 weeks after the intervention through the use of self-administered questionnaires. Descriptive statistics, $\chi^2$ test, Fisher's exact probability test, and Mann-Whitney U test were used to analyze the data. Results: After the intervention, knowledge about physical restraints increased significantly in experimental group compared to the control group. However, there were no statistically significant differences between the groups for attitude and nursing practice involving physical restraints. Conclusion: Findings indicate that it is necessary to apply knowledge acquired through educational programs to nursing practice to reduce the use of physical restraints. User friendly guidelines for physical restraints, administrative support of institutions, and multidisciplinary approaches are required to achieve this goal.

  • Purpose: The purpose of this study was to identify factors influencing neonatal pain management by nurses in order to provide evidence-based data for the development of more efficient neonatal pain care programs. Methods: This study used a descriptive research design to survey 204 registered nurses working in neonatal intensive care units and nurseries in Busan. Data collection was done from July to September 2010. General knowledge of pain and pain scale, pain intervention, neonatal pain management, and barriers to pain management were measured. Data were analyzed using stepwise multiple regression with SPSS 17.0. Results: it was found that among the factors affecting neonatal pain management by nurses, the unit where the nurse worked was the most important influential factor, followed by knowledge of pain management, education level, and barriers from medical personnel in that order. The explanatory power of these four variables accounted for 42.0% of neonatal pain management. Conclusion: Study results indicate that it is necessary to develop education programs to increase knowledge of neonatal pain management for nurses caring for neonates. Also, plans for reduction of obstacle factors in medical personnel should be prepared.
