Effects of Multipath Electrical Stimulation on the Functional Recovery of Early Stage Patients of Total Knee Arthroplasty

  • PURPOSE: This research was intended to investigate the influence to function recovery at the early stage after surgery, by conducting Multipath Electrical Simulation and isometric exercise treatment as early stage medical treatment method for Total knee arthroplasty patients. METHODS: The subject of 30 patients having Unilateral Total knee arthroplasty over age 65, Multipath Electrical Simulation and isometric exercise (experiment group I), Conventional Electrical Simulation and isometric exercise (experiment group II) and isometric exercise (control group). The intervention was performed in 5 times per a week and 60 minutes per a day during 4 weeks. We performed research by conducting Neuromuscular Electrical Stimulation and isometric exercise together and measured pain, range of motion, muscle strength and gait ability before and after intervention. RESULTS: The result showed therapeutic improvement in experiment group I, experiment group II and control group, but Multipath Electrical Simulation and isometric exercise showed significant improvement in function recovery of early stage compared to Conventional Electrical Simulation and isometric exercise, only isometric exercise. CONCLUSION: Based on research result, in order for early state function recovery of Total knee arthroplasty patients, when conducting neuromuscular electrical stimulation and isometric exercise together, especially when applying Multipath Electrical Stimulation, we could know that it showed more significant improvement to function recovery after surgery. Also, we suggest that Multipath Electrical Simulation may become a useful tool as a method for intervention and performing in various diseases for weakening of Quadriceps muscle.

  • In the total knee arthroplasty (TKA), kinematic benefic of a mobile-bearing total knee prosthesis is still arguing. Main reasons for implant failure are loosening and polyethylene wear and should be solved with new designs with mob ile bearings. The kinematics of the knee prosthesis also affects the implant failure. Recently, a second generation of p rostheses with a mobile-bearing was developed. The current study aimed to assess the kinematic path of the 2nd generation mobile knee prosthesis compared to the normal knees. Using 3D/2D registration method, CT-derived 3D knee models were fitted to sequential 2D X-ray images during knee flexion. 3D kinematics of the femur and the tibia were analyzed. The 2nd generation mobile-bearing TKA prosthesis (e.motion, Aesculap, Germany) knees showed less external rotation and knee flexion range compared to the normal knee, but the trend of external rotation was similar each other.

Factors Influencing Depression Among Patients with Chronic Degenerative Arthritis after Total Knee Arthroplasty (인공슬관절 전치환술을 받은 만성 퇴행성관절염 환자의 우울 영향요인)

  • Purpose: This study was to identify the factors influencing depression among patients with degenerative arthritis after total knee arthroplasty. Methods: The subjects were 108 patients who admitted or visited K hospital in K city after total knee arthroplasty. Data were analyzed using SPSS WIN 18.0 program. Results: The level of depression was 2.72 with a possible range of 1 to 5. Social support was 3.71 out of a total score 5. Self-efficacy was 64.47 ranged from 10 to 100. Self-esteem was 2.59 ranged from 1 to 5. The associated factors with depression were marital status, length of illness, perceived health status, pain, social support, self-efficacy, and self-esteem. Marital status, length of illness, and perceived health status accounted for 5.8% of depression. Next, all variables including pain, social support, self-efficacy and self-esteem accounted for 66.4% of depression. Conclusion: The level of depression among the subjects significantly be related to marital status, length of illness, perceived health status, pain, social support, self-efficacy and self-esteem. It indicates a need to develop nursing interventions for them to decrease depression and develop quality of life during recovery.

Effect of rheumatoid arthritis on primary total knee arthroplasty and revision arthroplasty (류마티스관절염이 슬관절치환술과 슬관절재치환술에 미치는 영향)

  • The demand for total knee arthroplasty (TKA) is rapidly increasing worldwide. The most common indication for TKA is osteoarthritis (OA); however, some patients with rheumatoid arthritis (RA) also undergo TKA. This study aimed to investigate the effects of RA on TKA. Our findings revealed that patients with RA underwent TKA at a younger age than did patients with OA. However, contrary to the findings of pre-21st century studies, the average age of TKA among patients with RA was not significantly different from that of patients with OA. Additionally, patients with RA had a 1.5-fold higher risk of undergoing TKA. Although not statistically significant, patients with RA had a higher revision TKA rate, a shorter time until revision TKA, and underwent more revision TKAs due to infections than did patients with OA. An analysis of factors that affect revision TKA revealed that the risk of revision increased if the erythrocyte sedimentation rate and C-reactive protein levels were increased at the time of TKA. This study showed that patients with RA have a slightly higher risk of undergoing TKA than did patients with OA. Furthermore, the presence of inflammation at the time of TKA increases the risk of revision; therefore, inflammation should be adequately controlled before performing TKA.

  • Recently, South Korea senile patients to undergo total knee arthoplasty surgery along with the aging of the population has increased. Hence, this descriptive investigational study is to identify the correlation among the variables including the pain, depression, social support, and the degree of life satisfaction in the senile patients with total knee arthroplasty, and to analyze the factors to affect the degree of life satisfaction. The study subjects were 115 the senile patients, who had been hospital in less than one year or visited the hospital on an out patient basis after getting surgery of total knee arthroplasty at the orthopedics of S hospital located in G Metropolitan City. Data collection period was from September 25, 2015 to October 31, 2015, they were collected using structured questionnaire with the researches describing the research objectives and methods.

Clinical and radiologic study of total knee replacement arthroplasty using Doujet GF bone cement (liquid-powder bone cement containing gentamicin) (Doujet GF 골 시멘트를 사용한 인공 슬관절치환술의 임상적, 방사선적 고찰)

  • Gentamicin-loaded bone cement used in total joint arthroplasty is indispensable, as it provides stability by directly binding the surfaces of implants and bones. Depending on multiple factors, including the material of the bone cement used, common complications, such as aseptic loosening, osteolysis, and infection can occur postoperatively. In clinical practice, Doujet bone cement is easy to handle (pre-packed all-in-one system), and has shown low failure rates and non-inferior results compared with similar available products. We conducted a retrospective comparative study to analyze the clinical and radiological results of each bone cement group to establish the safety and usefulness of Doujet bone cement. From July 2020 to July 2022, we enrolled 198 patients in this study after an average follow-up period of 37 months (range, 6-48 months). In 99 patents, Doujet® bone cement (Injecta, Gunpo, Korea) was used for total knee arthroplasty (TKA), while Refobacin® bone cement (Biomet, Warsaw, IN, USA) was used in 99 patients. The average range of motion (ROM) of the knee increased by 2.4° (from 127.0° preoperatively to 129.4° postoperatively) in the Doujet group, and by 0.1° (from 128.7° to 128.8°) in the Refobacin group (P=0.701). The Western Ontario and McMaster Universities (WOMAC) osteoarthritis index scores decreased from 44.1 to 7.8 in the Doujet group, and from 44.2 to 6.3 in the Refobacin group (P=0.162). Complications, such as osteolysis or post-operative wound infection, did not occur in more than two cases in both groups. The WOMAC and ROM of the knee in both groups had no clinical differences. Both Doujet and Refobacin similarly showed low complication rates after TKA.

Death due to Postoperative Ileus after Total Knee Arthroplasty (슬관절 전 치환술 후 장폐색으로 인한 사망)

  • The incidence of postoperative ileus (POI) after colonic and abdominal surgery is high. On the other hand, the reported incidence after lower extremity reconstruction ranges from 0.3% to 4.0%. This paper reports an 81-year-old woman who expired due to POI at six days after primary total knee arthroplasty. The risk factors, diagnosis, preventive methods, and treatment of POI were also investigated through literature reviews.

  • It is necessary to have a model that describes the feature of the knee Joint with a sufficient accuracy. Koreans, however, do not have their own knee joint model to be used in the total knee replacement arthroplasty. They have to use European or American models which do not match Koreans. Three-dimensional visualization techniques are found to be useful in a wide range of medical applications. Three-dimensional imaging studies such as CT(computed tomography) and MRI(magnetic resonance image) provide the primary source of patient-specific data. Three-dimensional knee joint models were constructed by image processing of the CT data of 10 subjects. Using the constructed model, the dimensions of Korean knee joint were measured. And this study proposed a three-dimensional model and data, which can be helpful to develop Korean knee implants and to analyze knee joint movements.
