Background: Extracardiac pericardial-flap lateral tunnel Fontan operation has theoretical advantage of growth potentiality of the extracardiac tunnels. The mid-term results of this technique and morphologic change of the lateral tunnel were studied. Material and Method: Clinical data was reviewed in 42 patients who underwent extracardiac pericardial-flap lateral tunnel Fontan operation between November 1993 and December 2004. The age was $2.8{\pm}1.5$ years and the body weight was $12.3{\pm}3.2$ kg. Extracardiac tunnel was constructed using the pedicled pericardium with the base undetached. By reviewing the follow-up cardiac angiograms, the diameter and the cross-sectional area of the lateral tunnel was compared to those of inferior vena cava. Result: There were four operative mortality cases (9.8%) and the causes of death were low cardiac output for all four cases. Postoperatively, five patients had prolonged pleural effusion longer than two weeks and one patient required a permanent pacemaker due to complete heart block. Follow-up was possible in 37 patients and the follow up duration was $3.8{\pm}2.2$ years. During that period, one patient died, of upper gastrointestional bleeding combined with heart failure and one patient died a sudden death of unknown cause. Two patients required reoperation due to subaortic stenosis and anastomosis site stenosis between inferior vena cava and lateral tunnel. In one patient, bradyarrhythmia was anew but there was no thromboembolic complication. The lateral tunnel showed growth in proportion to the size of the inferior vena cava. Conclusion: Extracardiac pericardial-flap lateral tunnel Fontan operation is relatively simple and safe. The mid-term result was favorable and the extracardiac tunnel showed potential for growth.
Background: Central nervous system complication after coronary artery bypass grafting(CABG) is one of the major prognostic determinants and the use of the cardiopulmonary bypass(CPB) may increase the incidence of this devastating complication. In this study, the outcomes after off-pump CABG were studied and compared with those following the conventional CABG using CPB. Material and Method: Among the consecutive isolated CABG's performed in SNUH during Feb. 1995 and Jun. 1999, 338 coronary artery bypass grafting were divided into two groups. 223 patients underwent CABG using the CPB(Group I), and 115 patients underwent CABG without CPB(OPCAB)(Group II). All patients enrolled in this study received extensive preoperative examinations including thorough neurologic examination before and after surgery, transcranial doppler study, carotid duplex ultrasonography, and magnetic resonance angiography if necessary. Central nervous system(CNS) complications were defined as stroke, seizure, metabolic or hypoxic encephalopathy and transient delirium after surgery. Result: There were 61 cases(27.3%) who developed postoperative CNS complication in Group I, whereas 8 cases(7.0%) of CNS complications developed postoperatively in group II(p<0.05). Statistically significant predictors of postoperative CNS complications in group I were age and the use of cardiac assist devices perioperatively. Conclusion: This study suggested that omitting the use of CPB in CABG resulted in significant decrease of the postoperative CNS complications. OPCAB should be more widely applied especially to the elderly who have preexisting cerebrovascular disease.
Background: Compression of the left common iliac vein by the overriding common iliac artery is frequently combined with acute deep vein thrombosis in patients with May-Thurner Syndrome. We evaluate the results of treatment with thrombolysis and thrombectomy followed by stenting in 34 patients with May-Thurner Syndrome combined with lower extremity deep venous thrombosis. Material and Method: The authors retrospectively reviewed the records of 34 patients (mean age: $65{\pm}14$ year old) who had undergone stent insertion for acute deep vein thrombosis that was caused by May-Thurner syndrome. After thrombectomy and thrombolysis, insertion of a wall stent and balloon angioplasty were performed to relieve the compression of the left common iliac vein. Urokinase at a rate of 80,000 to 120,000 U/hour was infused into the thrombosed vein via a multi-side hole thrombolysis catheter. A retrieval inferior vena cava (IVC) filter was placed to protect against pulmonary embolism in 30 patients (88%). Oral anticoagulation with warfarin was maintained for 3 months, and follow-up Multi Detector Computerized Tomography (MDCT) angiography was done at the date of the patients' hospital discharge and at the 6 months follow-up. Result: The symptoms of deep venous thrombosis disappeared in two patients (4%), and there was clinical improvement within 48 hours in twenty eight patients (82%), but there was no improvement in four patients (8%). The MDCT angiography at discharge showed no thrombus in 9 patients (26%) and partial thrombus in 21 (62%), whereas the follow-up MDCT at $6.4{\pm}5.5$ months (32 patients) revealed no thrombus in 23 patients (72%), and partial thrombus in 9 patients (26%). Two patients (6%) had recurrence of DVT, so they underwent retreatment. Conclusion: Stent insertion with catheter-directed thrombolysis and thrombectomy is an effective treatment for May-Thurner syndrome combined with acute deep vein thrombosis in the lower extremity.
This study is to derive a project model based on potential demand for Korean-style houses, focusing on new town detached housing sites that LH supplies and to test validity of the derived model and to present the direction and supply methods of the projects. The existing high-class new town Korean-style housing developments that have been considered were found to have little business value due to problems in choice of location and discordance of demand, so 6 types of projects were established through the methods of changes in planned scale, combined use, and subdivision of plot of land based on the results of survey. The type that has the highest business value among the project models was block-type multifamily houses, and this can be interpreted as the increase in total construction area leading to increase inrevenues of allotment sales due to economies of scale. The feasibility of mass housing model in which small-scale Korean-style houses are combined with amenities was found to be high, and if the same project conditions as those of the block-type multifamily houses are applied, the business value of the Korean-style tenement houses was found to be high. Besides, the high-class housing models within block-type detached housing areas are typical projects that the private sector generally promotes, and the construction cost was found to be most expensive with 910 million won per house. In order to enhance the business value of the Korean-style housing development, collectivization such as choice of location, diversification of demand classes, optimization of house sizes, and combination of uses is needed. And in order to adopt Korean-style houses in the detached housing sites, the adjustments and division of the existing planned plots are needed, and the strategies to cope with new demand through supplying Korean-style housing types of sites can be suggested. Also breaking away from the existing uniform residential development methods, the development method through supplying original land that is natural land not yet developed besides basic infrastructures (main roads and water and sewage) can be considered, and as the construction of more than 1~2 stories building is impossible due to the structure of Korean-style house roof and furniture. So it can be suggested that original land in the form of hilly land is considered to be most suitable to large-scale development projects.

Stem length, main root length and main root diameter showed rapid increasement in two-years-old and three-years-old peony. And there after, the increasement was almost finished. Occurrence of disease was increasing every cultivated year, especially the development of leaf spot was most serious. In propagation by root dividing method, the radix yield (kg/10a) was increasing every cultivated year. However, the difference of radix yield at four and five-years-old peony were not significant. The radix yield of four-years-old peony was higher 26% than three-years-old one. As cultivated year goes by, content of paeoniflorin was increasing then the content was highest at four-years-old peony (4.06%). In 1995, the content, had no certain tendency, was highest at three-years-old peony (3.14%). At hot air drying, browing of peony radix was increasing every cultivated year. If we consider radix yield and color, three-years-old peony was proper object of harvesting.
The morphological, physical, and chemical properties of Sonjeong series derived from acidic crystalline rocks are presented. Also it deals with the genesis and classification of the Songjeong series. Morphologically these soils have brown to dark brown loam A horizons and yellowish red to red clay loam Bt horizons with moderate, medium subangular blocky structure and thin patchy clay cutans on the ped faces. C horizons are very deep, yellowish red to yellowish brown fine sandy loam or sandy loam with original rock structure. Physically distribution of particle size indicates that clay increases with depth up to argillic horizons but below the argillic horizons clay content decrease. The moisture holding capacity is fairly good in Songjeong soils. Chemically soil reaction is strongly to very strongly acid throughout the profile and content of organic matter is less than 1 per cent except A horizons. Cation exchange capacity ranges from 5 to 9 me/100g of soils and base saturation is less than 35 per cent throughout the profile. The natural fertility of Songjeong soils are usually low. It needs lime, organic matter, and heavy application of fertilizer for the crop land. These soils occur temperate and humid climate under coniferous, deciduous, and mixed forest vegetation. Songjeong soils are classified as Red-Yellow Soils. Characteristically Songjeong soils are similar to Red-Yellow Podzolic soils in the United States but lack of A2 horizons and are quite liket Red-Yellow Soils of the Japan. According to new classification system which is 7th approximation of USDA Songjeong soils can be classified as fine loamy, mesic family of Typic Hapludults and in the FAO/UNESCO project World Soil Map as Orthic Acrisols.
Enormous amount of oyster-shell waste has been illegally disposed at oyster farm sites along the southern coast of Korea and then made a serious problem in environmental side. To increase the consumption of oyster shell meal as a soil amendment, the effects of oyster shell meal on soil properties and spring Chinese cabbage productivity were evaluated in silt loam soil to which 0, 4, 8, 12 and $16Mg\;ha^{-1}$ of oyster-shell meal fertilizer were added. Hydrated lime treatment ($2Mg\;ha^{-1}$) was selected as a control. Oyster-shell meal fertilizer, which made by a simple crushing and has high alkalinity as a calcium materials, had significant effects on neutralizing acid soil and on supplying calcium element. Spring Chinese cabbage yields increased with shell meal application. Plant uptakes of macro-nutrients such as P and C, and micro-nutrient such as B were significantly increased by oyster-shell meal application and then contributed to promote Chinese cabbage growth. The highest yield was achieved following the addition of $8Mg\;ha^{-1}$ shell meal application, and the same yield with that in lime treatment was at the $4Mg\;ha^{-1}$. Oyster-shell meal had more substantial effect on suppling calcium and on improving soil pH than that of lime. In conclusion, oyster shell meal fertilizer could be a good supplement to other inorganic soil amendments to improve nutrient balances in upland soils.
The model equations including scaling factors to estimate the soil water characteristics curve(SWCC) without direct measurement of soil water tension were developed. Scaling were applied to a data set of soil water content, soil water tension, particle size distribution, and OM contents of the 134 soil samples with the 10 soil textural classes. The capability of the model equations was tested on another 205 soil samples. The parameter, ${\theta}^*$, of soil water contents was used by scale transformation as follows : ${\theta}^*=[{\theta}i-{\theta}(1.5MPa)]$/$[{\theta}(10KPa)-{\theta}(1.5MPa)]$ Using ${\theta}^*$ a model equation to estimate SWCC, which was applicable to all textural classes, was developed as follows: $H(0.1MPa)=0.13{\cdot}({\theta}^*)^{-2.04}$. Other model equations to estimate the water content at the soil water tension of 10KPa [${\theta}(10KPa)$] and 1.5MPa [${\theta}(1.5MPa)$], which are required to ${\theta}^*$ were developed by using scale factors of sand(S) and silt(Si) content and organic matter content(OM) as foilows : ${\theta}(10KPa)=26.80-3.99ln[S]+2.36{\sqrt{[Si]}}+2.88[OM]$ ($R=0.81^{**}$) ${\theta}(1.5KPa)=15.75-2.86ln[S]+0.55{\sqrt{[Si]}}+0.70[OM]$ ($R=0.76^{**}$) The measured and estimated values of ${\theta}(1/30MPa)$ on the 205 soil samples were highly correlated on 1 : 1 corresponding line with $R=0.85^{**}$.
This experiment was carried out to improve the utilization of liquid pig manure (LPM) for rice at the two textures of valley soil in 2000 and 2001. The soil textures were coarse loamy and fine loamy in Sachon and Jisan series, respectively. Treatments consisted of no fertilized plot, chemical fertilized plot, LPM 150%, LPM 100%, LPM 100%+NK (top dressing) 30%, LPM 70%+NK 30%, LPM 50%+NK 50% plot. LPM was applied as basal fertilizer compare to nitrogen of chemical fertilized plot. Total N contents in the LPM were 6.0 and $4.5g\;kg^{-1}$ in 2000 and 2001, respectively. After the experiment, P and K contents of soils were not difference between chemical and LPM application plots. But heavy metal contents in soils were slightly higher in LPM application plots than in chemical fertilized plot. Immediately after LPM application, ammonia gas content was $18mg\;kg^{-1}$ in LPM 150% plot, but it was $3mg\;kg^{-1}$ in LPM 50% plot. Two days after LPM application, ammonia gas content was 3 times higher in coarse loamy than in fine loamy soil. After rotary tillage, ammonia gas was not detected at all LPM treatments. This result suggests that rotary tillage can reduce the nasty smell of LPM quickly. Inorganic nitrogen, $NO_3$ and $NH_4$, contents in water of paddy was higher at coarse loamy soil from rice transplanting to tillering stage. After that season, inorganic nitrogen contents of water were not different according to soil texture and treatments. Content of $NH_4-N$ in soil solution was higher at LPM plots than chemical fertilizer plot. Total nitrogen contents in rice plant after harvesting were higher at chemical fertilization plot than LPM application plot, but K contents showed an opposite tendency. Rice yield was decreased only in LPM plots at two soil textures. But yield was not significantly difference between chemical fertilizer and LPM+top dressing plots at coarse loamy soil and increased 5% at LPM 50%+NK 50% plot at fine loamy soil in 2001.
In order to establish environmentally friendly phosphorus(P) and potassium(K) fertilizer application rates for rice cultivation in Korea, a field experiment was conducted in two different characterized paddy soils, higher and lower P and K content in soil from 2005 to 2006 in Suwon, Korea. Chuchong which is typical rice variety in Korea, was selected for experiment. Rice yields in base P-K fertilizer level ($30-30kg\;ha^{-1}$) and no P-K fertilizer level were 97~98% and 93~97% of standard P-K fertilizer level ($45-57kg\;ha^{-1}$), respectively. At the lower content of P and K in immatured paddy soil, the yield of no P plot was decreased by 7% compared to that of standard P-K plot. Significant difference in rice yield and yield characteristics was found neither in standard P-K fertilization plot nor in base P-K fertilization plot. There was no significant difference in P and K uptake at the harvest stage between standard P-K fertilizer plot and base P-K fertilizer plot. Phosphorus and K uptake was slightly decreased in no fertilizer plots compared to standard fertilizer plot. Phosphorus and K use efficiency of rice plant in base fertilizer plot was higher than that in standard fertilizer plot. Available phosphorus and exchangeable potassium content in soil at the harvest stage were increased in both standard and base fertilizer plot. But no significant difference of P and K content in ? was found between standard fertilizer plot and base fertilizer plot. Rice quality characteristics such as proportion of brewers rice, damaged kernel, broken rice, and content of amylose and protein were lowered in the base fertilizer plot than the standard fertilized plot, while the proportion of perfect kernel increased in the base fertilizer plot From the results, we generally recommend the base P-K fertilization ($30-30kg\;ha^{-1}$) instead of standard P-K fertilization ($45-57kg\;ha^{-1}$) for rice cultivation in Korea.
