The Result of Radiation Therapy of Glottic Laryngeal Carcinoma for 20 Years (성문암(聲門癌) 방사선치료(放射線治療) 20년(年) 성적(成績))

  • To evaluate the result of radiation therapy for twenty years experience, a total of 115 cases of pathologically proven glottic carcinoma had been analyzed according tot survival respectively. All the patients had been treated with radiation therapy in curative intent using Co-60 teletherapy machine. The results are as follows: 1) Accoridng to sex, 5YSR & 10YSR were 58.7% and 50.4% in male patients, and 80.0% & 72.0% in female. 2) According to T staging, 5YSR & 10YSR were 83.4% & 83.4% in T1, 69.2% & 60.0% in T2, 34.3% & 21.8% in T3, 32.0% & 0% in T4. 3) According to N staging, 5YSR & 10YSR were 63.8% & 59.2% in node-negative groups, and 30.7% & 15.4% in node-positive groups. 4) According to the histologic grade, 5YSR & 10YSR were 66.8% & 57.6% in G1, 61.3% 54.3% in G2, and 35.0% 35.0% in G3. 5) According to AJC staging, 5YSR & 10YSR were 83.4%% 83.4% in stage I, 72.0% & 62.7% in stage II, 36.8% & 28.3% in stage III, and 14.3% & 7.1% in stage IV. 6) In summary, 5YSR & 10YSR wre 60.4% & 52.8% in glottic carcinoma.

PARENT-ADOLESCENT AGREEMENT IN THE ASSESSMENT OF BEHAVIOR PROBLEMS OF ADOLESCENTS:COMPARISON OF FACTOR STRUCTURES OF K-CBCL AND YSR (문제행동에 대한 청소년 자신과 부모 평가간의 관계:K-CBCL과 YSR의 하위요인 구조 비교)

  • The present study compared the self report and parental report on the behavior problems of adolescents as a way to explore similarities and differences in the ways that adolescents and their parents conceptualize behavior problems of adolescents. Specifically, K-CBCL and YSR data from 3271 adolescents between the ages of 12 and 17 were subjected to factor analyses. Five factors;Depression/Anxiety/Withdrawal, Aggressiveness, Somatic Symptom, Disruptiveness, Attention Getting were obtained from the YSR data with the first factor, Depression/Anxiety/Withdrawal explaining 14.23% of the total variance. K-CBCL data yielded somewhat different factor structure with Aggression/Delinquency as the first factor explaining 14.08% of the total variance, followed by Somatic Symptoms, Social Withdrawal, Disruptiveness, and Depression/Anxiety. Total K-CBCL and YSR score showed a moderate correlation(r=.51), and correlation between pairs of comparable K-CBCL and YSR factor scores were also moderate. Regression analyses of the variables contributing to the total problem score of the K-CBCL and YSR suggested that social competence and academic achievement are two important sources of influence on the evaluation of behavior problems both in self-report and parental report. However, externalizing problems such as aggressiveness/delinquency appeared to be more salient for parents, while adolescents themselves appeared to be more concerned with internalizing problems such as depression/anxiety. Implications of these subtle differences for assessment of adolescent behavior problems were discussed.

GENDER DIFFERENCES IN EMOTIONAL AND BEHAVIORAL PROBLEMS OF KOREAN ADOLESCENTS (청소년기 정서 및 행동문제의 성차)

  • Objectives : The purpose of the present study was to investigate the gender differences in behavior problems and social competences of Korean Adolescents. Methods : K-YSR (Korean-YSR) data from 6570 Korean adolescents (2549 boys and 4021 girls) and K-CBCL (Korean-CBCL) data from 2373 parents (boys 1173 and girls 1200) between the ages of 12 to 17 were analysed. Results : The results showed that boys reported higher mean scale scores on the Social Problems, Delinquent Behavior, whereas girls showed higher mean scale scores on the Withdrawn, Somatic Complaints, Anxious/Depressed and Aggressive Behaviors as well as Internalizing Problems and Total Behavior Problems in K-YSR. Girls also reported higher scores on the Social. as well as Total Competence Scale. Also parents reported higher mean scale scores on the Attention Problems, Delinquent Behavior in their boys, whereas reported higher mean scale scores on the Withdrawn, Somatic Complaints, Anxious/Depressed and Aggressive Behaviors as well as Internalizing Problems in their girls in K-CBCL. Parents reported no differences between boys and girls in social competence scales. Conclusion : In both K-YSR and K-CBCL, girls consistently showed higher scores in the Internalizing Problem scales and there was no significant difference between boys and girls in the Externalizing Problem scores, but girls showed higher mean score in the Aggressive Behavior scale.

Affective Factors That Contribute to the Quality of Life of Juvenile Inmates with Attention-Deficit/Hyperactivity Disorder: A Focus on Items from the Korean Youth Self Report

  • Objectives: This study investigated quality of life in Korean juvenile inmates with attention-deficit/hyperactivity disorder (ADHD) and the impact of behavioral and emotional problems on quality of life. Methods: In total, 200 inmates were evaluated using the Korean version of the Mini-International Neuropsychiatric Interview (K-MINI) and the Korean version of the Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime (K-SADS-PL-K). We extracted the inmates with ADHD and evaluated their quality of life, behavioral problems, and emotional problems with the Pediatric Quality of Life Inventory (PedsQL) and the Korean Youth Self Report (K-YSR) scale. Descriptive statistics, Pearson correlation analysis, and multiple regression analysis were conducted. Results: Among the 200 total inmates, 68 were diagnosed with ADHD by the K-SADS-PL-K. Most of the correlations between PedsQL scores and K-YSR items were significant. Multiple regression analysis showed that PedsQL could be predicted by affective problems (among the DSM-oriented scales of the K-YSR) and attention problems (among the syndrome scales of the K-YSR). Conclusion: Our results demonstrate that, among juvenile inmates with ADHD, quality of life was negatively correlated with most behavioral and emotional problems. Meanwhile, the significant influence of affective and attention problems on inmates' quality of life suggests the necessity of comprehensive treatments for this group.

The Efficiency of Radiation Therapy in the Treatment of Intracranial Oligodendrogliomas : Factors Influencing the Prognosis (뇌내 희돌기교종의 방사선치료 성적 및 예후인자)

  • Purpose : Oligodendrogliomas (ODG) are a rare, slow growing, tumor in the brain, which can be cured by complete surgical resection, but as yet it is not known if postoperative adjuvant radiation therapy (RT) is essential, We analyzed the treatment results of patients with irradiated ODG to investigate the efficacy of RT in terms of survival rates and other influencing prognostic factors. Methods and Materials : Between March 1983 and December 1997, 42 patients with ODG were treated with RT at our hospital. The RT was peformed dally at a dose of $1.8\~2.0\;Gy$, at 5 fractions per week, to a total dose of between 39.6 Gy and 64.8 Gy (mean 53.3 Gy). The ages of the patients ranged between 5 and 62 years, with a median age of 39 years. The mean follow-up period was 63.4 months (8-152 months). The Kaplan-Meier method was used to assess the survival, and 5 year survival rates (5-YSR). Log rank tests and Cox regression analyses were used to define the significance of prognostic factors. Results : The majority of ODG in this study were located in the cerebral hemisphere $(83.3\%)$. ODG are slightly more common in men than women, and commonly occurs in middle age, between the 3rd and 4th decades. It has been recommended that RT is commenced within 4 weeks following surgery (5-YSR; $86\%\;vs.\;49\%;\;p<0.03$). Histologically well differentiated, as opposed to poorly differentiated, tumors were found to have a more favorable prognosis (p<0.02). The actuarial 5-YSR was $65.3\%$ (median survival 90 months). 5-YSR for the various extents of surgical excision, followed by external RT, was superior to that of biopsy only followed by external RT $(69.9\%\;vs.\;25.6\%,\;p<0.01)$. Tumor size and location, overall elapsed irradiation days, age, sex, whole brain irradiation as a course of treatment and chemotherapy, had no influence on the 5-YSR (p>0.05). Conclusion : A local involved field irradiation with conventional fractionation, commencing within 4 weeks following surgical excision of the tumor, was beneficial for the 5-YSR, but a total radiation dose exceeding 60 Gy did not improve the 5-YSR.

Analysis of Treatment Results of Malignant Melanoma (악성 흑색종의 치료결과 분석)

  • Purpose: This study was designed to evaluate the treatment results of malignant melanoma and to analyze the factors influencing prognosis. Materials and Methods: Thirty one cases of malignant melanoma were included in this study. They were treated in our hospital surgically, medically and immunologically from January 1996 to December 2005, and were followed more than 5 years. We compared 5 year survival rate (5YSR) according to the age, gender, anatomical site, depth of tumor, TNM stage, involvement of lymph node and immuno-chemotherapy. Results: Overall 5YSR was 80.6%. 5YSR of the age group below 65 years was 89.7% and 66.7% for the age group over 65 (p=0.033). 5YSR for men was 75% and 90.9% for women. 5YSR according to the site of occurrence showed 66.7% in upper extremities, 89.5% in lower extremities, and 66.7% in other site. 5YSR was 100% for the Clark level below III and 62.5% for the level above IV (p=0.032). 5YSR was 53.8% for lymph node metastasis group and 100% for non-lymph node metastasis group (p=0.021). Conclusion: We concluded that early diagnosis and wide excision was the most important in treatment of malignant melanoma. The prognostic fractors of malignant melanoma were age, depth of tumor (Clark's stage) and metastasis of lymph node.

The Results of Combined External Radiotherapy and Chemotherapy in the Management of Esophageal Cancer (식도암의 방사선-항암화학 병용치료결과)

  • Purpose : To evaluate the role of combination therapy of external radio-therapy and chemotherapy in the management of advanced esophageal cancer as a primary treatment compared with radiation therapy alone. Materials and Methods : A retrospective review of evaluable 55 esophageal cancer patients referred to the Department of Therapeutic Radiology, Paik Hospital for the external radiotherapy between Jul, 1983 and Dec. 1994 was undertaken. Combined therapy patients (A group) were 30 and radiation alone patients (B group) were 25. Median age was 60 years old in A group (ranges : 42-81) and 65 years old in B group (ranges : 50-81). The male patients were 53. The fifty patients had squamous cell carcinomas. Radiation doses of 2520-6480c0y were delivered over a period of 4-7weeks, using 4MV LINAC. Chemotherapy was administered in bolus injection before, after, or during the course of external radiotherapy. The local control rate and patterns of failure according to both treatment modalities and 1, 2 year survival rates according to prognostic factors (stage, tumor length, radiation dose etc.) were analysed. Resuts : Median follow up Period was 7 months (range : 2-73 months). Median survival was 7.5 months (20 days-29 months) in A group and 5 months (20 days-73 months) in B group. The 1, 2 YSRs were $26.7\%$, $8.9\%$ in A group, $12.7\%$, $4.3\%$ in B group (p>0.05), respectively. The 1, 2 YSRs according to stage(II/III), tumor length (5cm more or less). radiation dose (5000cGymore or less) of A and B group were analyzed and the differences of survival rates of both treatments were not statistically significant. But among group B, patients who received 5000cGy or more showed significant survival benefits (p<0.05). The treatment response rates of A and B group were $43.8\%$. $25.0\%$, respectively. Complete response rate of $25.0\%$ in A and $8.3\%$ in B were achieved. The local failure and distant metastsis were $52.4\%$. $23.8\%$ in A group, $64.3\%$, $14.3\%$ in 8 group, respectively. The combination therapy revealed more frequent leukopenia and nausea/vomiting than radiation alone group, but degree of side effects was only mild to moderate. Conclusion : The combined external radiotherapy and chemotherapy for advanced esophageal cancer appears to improve the response rate, local control rate and survival rate, but the improvement was not statistically significant. The side effects of combined modalities were mild to moderate without significant morbidity. Therefore it may be worthwhile to continue the present combined external radiotherapy and chemotherapy in the management of advanced esophageal cancer to confirm our result.

A Comparative Study on the Mental Health of Adolescents Residing in Child Care Institutions (Orphanages) and Youth Shelters (아동양육시설과 청소년 쉼터에 거주중인 청소년들의 정신건강 수준 비교)

  • Objectives : This study aims to compare the mental health of adolescents living in child care institutions (orphanages) and youth shelters, to provide basic data for appropriate psychiatric interventions for each facilities. Methods : The sample consisted of 157 adolescents in child care institutions and 31 adolescents in Youth Shelters who completed the Korean version of the Youth Self Report (K-YSR). We compared the K-YSR scores between the two groups and also examined factors related to suicidality. Results : Adolescents in youth shelters were older and had higher rates of drinking (p<0.001), smoking (p<0.001), and "abnormal sleep" (p=0.007) than adolescents in child care institutions. They also showed higher 'suicidality' (p=0.004) and K-YSR scores. On the other hand, adolescents in child care institutions have lower 'suicidality', but they have higher 'suicidality' when they have 'abnormal sleep' (OR 4.194; p=0.006), 'abnormal diet' (OR 2.503; p=0.010), and 'concerns about weight gain' (OR 2.503; p=0.010). Additionally, 17.6% of participants in the study showed 'suicidality'. The 'suicidality' group had a higher overall K-YSR score than the non-suicidal group, and had higher 'abnormal sleep' (OR 3.766; p=0.014) and 'concern about weight gain' (OR 2.864; p=0.032). Conclusions : Adolescents in youth shelters appear to have more serious clinical difficulties than adolescents in child care institutions, so prompt psychiatric intervention is needed. Additionally, it is necessary to consider mental health assessments including suicidality when adolescents in child care institutions complain of sleep, eating, or weight gain problems.

The Relationship between Childhood Trauma Experience and Parent-Adolescent Reports of Problem Behavior : Comparison of Delinquent and General Adolescent (아동기 외상 경험과 문제행동에 대한 청소년 자신과 부모 평가 간의 관계 : 비행청소년과 일반청소년 비교)

  • Objectives : The aim of this study was to investigate correlation of childhood trauma experience and parent-adolescents' assessment reports on problem behavior through comparison of delinquent and general adolescents. Methods : First, delinquent adolescents and general adolescents were asked to complete the Childhood Trauma Questionnaire (CTQ). Then, 71 pairs of delinquent adolescents and their parents and 133 pairs of general adolescents and their parents were asked to complete the Korean Youth Self-Report (K-YSR) and the Korean Child Behavior Checklist (K-CBCL). Finally, responses from 410 people were used for the analysis. Results : First, childhood trauma experience score was significantly higher for delinquent adolescents, compared to general adolescents. Second, t-test showed a greater difference between K-YRS and K-CBCL for general adolescents than for delinquent adolescents. Third, in the case of delinquent adolescents, the correlation analysis of K-YSR filled out by adolescents and K-CBCL filled out by parents showed significant correlation in certain areas, including social immaturity, delinquent behaviors, internalization issues, and externalization issues. On the other hand, the correlation analysis of K-YSR and K-CBCL of general adolescents showed significant correlation in all sub-categories. Fourth, the correlation analysis of delinquent adolescents' CTQ and K-YSR showed minimal yet significant correlation in social adaptation, withdrawing, depression/anxiety, and delinquent/aggressive behaviors, however, no correlation was observed between CTQ and K-CBCL. The correlation analysis of general adolescents' CTQ and K-YSR, and CTQ and K-CBCL showed significant correlation in all sub-categories, although the degree of correlation varied. Conclusion : Delinquent adolescents had more childhood traumatic experiences. However, general adolescents' childhood traumatic experiences showed minimal yet significant correlation with various adaptation indicators and their parents responded in a similar way, indicating that general adolescents with childhood traumatic experiences need close care and attention even if they have not shown prominent delinquent behaviors.

One-dimensional nonlinear consolidation behavior of structured soft clay under time-dependent loading

  • This research investigated the nonlinear compressibility, permeability, the yielding due to structural degradation and their effects on consolidation behavior of structured soft soils. Based on oedometer and hydraulic conductivity test results of natural and reconstituted soft clays, linear log (1+e) ~ $log\;{\sigma}^{\prime}$ and log (1+e) ~ $log\;k_v$ relationships were developed to capture the variations in compressibility and permeability, and the yield stress ratio (YSR) was introduced to characterize the soil structure of natural soft clay. Semi-analytical solutions for one-dimensional consolidation of soft clay under time-dependent loading incorporating the effects of soil nonlinearity and soil structure were proposed. The semi-analytical solutions were verified against field measurements of a well-documented test embankment and they can give better accuracy in prediction of excess pore pressure compared to the predictions using the existing analytical solutions. Additionally, parametric studies were conducted to analyze the effects of YSR, compression index (${\lambda}_r$ and ${\lambda}_c$), and permeability index (${\eta}_k$) on the consolidation behavior of structured soft clays. The magnitude of the difference between degree of consolidation based on excess pore pressure ($U_p$) and that based on strain ($U_s$) depends on YSR. The parameter ${\lambda}_c/{\eta}_k$ plays a significant role in predicting consolidation behavior.
