인력수급 대책을 위한 치과기공사의 보건지소 진출에 관한 연구 - 2012년 틀니보험화 정책을 중심으로 - (The study on the entry of dental technicians in the public health center for a manpower supply and demand plane (Centering around a denture insurance policy in 2012))

  • Purpose: This study evaluated the manpower planning of dental technicians. Methods: Methods : Survey was conducted regarding subject's general characteristics, characteristics of dental technician's occupational view and working conditions, and reason to enter into branches of the public health enter. The survey sample consisted of 323 dental technicians (221 male/ 102 female). Survey was conducted for one month from August 1 to September 1 in 2010. Following results were obtained. Results: 1. Regarding general characteristics of the subjects, there was a slight male predilection with 68.4%. Subjects in their 40s occupied the highest proportion of 32.2%. Majority of subjects worked in the big cities (71.5%). Favored working places were dental laboratory (57.6%) and dental hospital or dental clinic (19.5%). Although no position is available in the public sector such as public health center or health care civil servant, 9.3% preferred working in the public health center. Public sector is more favored than 2-year technical colleges or dental supply and equipment companies. In respect to education, 57% of the subjects graduated college. 67.8% were married. Subjects who specialize in the porcelain or all-ceramic were 57.9%. With regard to current position, head of the dental laboratory was most common. 2. Following occupation characteristics were found. Economic reason took the highest proportion when deciding their occupation (39.9%) followed by gaining social experience (36.5%). Majority of the subjects (76.5%) wanted to work as dental technicians until their retirement. 71.5% pursuit to become a manager of the dental laboratory. 76.1% agreed on opening a dental technician position in the public health center. Regarding an authority to lead developing the dental technician position in the public sector, majority of the subject chose ministry of health and welfare (35.6%) and Korean dental technologist association (34.7%). Employment (average, 30%) and turnover rate (average, 36.5%) of dental technology college graduates ranged from 40% to 60%. Most important factors for the employment were practical experience (41.2%) and competency (34.4%). With regard to job satisfaction, so-so as 46.7% and satisfactory was 42.7% 3. In respect to the reason for opening a position in the public health center, 'It is needed to continue denture rogram for elderly patients' obtained the highest score (4.14 point). 'Institutional devices are required to open a position for dental technicians as a public healthcare provider' received high score (4.11 point). 4. Concerning the working conditions, 'professional knowledge is required' received the highest score (4.23) followed by 'too short maternity and parental leave' (4.21). 5. Relationship between general characteristics of the subjects and favor of working in the public health center was investigated. Significant differences were found according to the current and favored working area, favored occupation, education level, marital status, and specialty. Working in the public health center was favored by following subjects: working in mid- or small-sized cities (4.16 point, p<0.05); PhD degree-holder (4.59 point, p<0.01). 6. Among general characteristics of the subjects, significant difference of working conditions was found in the following factors: gender; working areas; favored working areas; favored working positions, and education level. Majority of subjects favored working in big cities and currently work in big cities although satisfaction was comparatively low (3.75 score). 7. Future plan to work in public health center was evaluated according to occupational characteristics. Subject's intention to work in the public healthcare center was significantly affected by opening of dental technician position, leading authority, average turnover rate, and factors affecting employment. Working in the public health care center was favored by the following subjects: Dental technicians who actively supported opening of the dental technician position (4.34 point, p<0.001); subjects who thought the Korean dental technologist association is responsible for the opening of positions in the public sector (4.26 point, p<0.001); and subjects who thought that attitude and character are important for the employment (p<0.001). 8. Concerning difference of working conditions according to the occupational characteristics, significant difference was demonstrated by factors such as a reason to choose to be a dental technician, work plan, pursuing position, responsible authority, average employment rate, and job satisfaction. High standard of working conditions was required in subjects who selected to be a dental technician for the leisure time after work (s.05 point, p<0.01), who planted to work until their marriage (4.25 point, p<0.001), and who pursuit to be a manager (3.98, p<0.05). 9. In respect to influence of general characteristics on the advancement to public health centers, the coefficient of determination (R2) was 0.068 and age affected the working condition with significant difference according to the pvalue. 10. Regarding influence of general characteristics on the advancement to public health centers, the coefficient of determination (R2) was 0.335 and work plan, opening of dental technician position in the public sector, and responsible authority had significant influence over the subject's intention to work in the public health center according to the p-value. 11. With regard to the influence of general characteristics on the advancement to public health centers, the coefficient of determination (R2) was 0.091 and reason to choose to be a dental technician, work plan, and responsible authority significantly affected subject's working conditions. Conclusion: Korean society is becoming a super-aged society according to several statistics. As aged population is rapidly increasing, national health insurance plans to cover denture for senior citizen over 75 years old from 2012. Therefore, dental technicians are urgently needed in the public health centers all over the nation. Many subjects in this study planed to work until their retirement and recognized dental technician's expertise. Ministry of health and welfare and Korean dental technologist association should co-operate each other to prepare foundation and institutional devices for dental technicians to advance into the public health center. This will improve oral health of the population. This study showed urgency of medical facilities and services which meets increasing number of aged population and welfare of the population.

치과의사와의 관계에서 치과위생사와 간호조무사의 직종 간 갈등 연구 (A study on conflicts between different occupational categories of dental hygienists and nursing assistances in terms of relationships with dentist)

  • The purpose of this study was to examine the state of conflicts among dental health care workers. A survey was conducted on 266 dental hygienists and nursing assistants who worked in dental institutions from September 12 to November 13, 2017, and SPSS(statistical package for the social science) version 20.0 was employed to analyze the collected data. The findings of the study were as follows: 1. The most common reason of the dental hygienists for turnover was working hours and heavy workload(24.6%), followed by pay (22.6%), conflicts with dentists(16.0%) and conflicts with colleagues (11.3%). The most dominant reason of the nursing assistants for turnover was pay(31.1%), followed by working hours(24.4%), heavy workload(17.8%), conflicts with dentists(15.6%) and conflicts with colleagues(8.9%). 2. The largest reason for unsuccessful communication with dentists was that heavy workload reduced the opportunity to communicate well(54.5%). The second biggest reason was that they couldn't communicate well though they had the opportunity(24.0%), and the third greatest reason was that they tended to lag behind dentists in terms of professional knowledge(16.9%). 3. The biggest reason for unsuccessful communication among the dental health care workers was that they didn't have a lot of chances to communicate well on account of heavy workload(41.0%). The second largest reason was the differences in professional knowledge(24.9%), and the third greatest reason was that they couldn't communicate well though they had the chance(23.7%). 4. The most dominant reason for conflicts with dentists was the difference in power(24.0%), followed by poor communication skills(22.1%) and a lack of mutual respect(18.1%). But the opinions of the nursing assistants were different from those of the dental hygienists, as they cited poor communication skills as the most common reason, which was followed by the difference in power and a shortage of understanding of each other's work. 5. The most common reason for conflicts among the dental health care workers was a shortage of communication and communication skills(22.9%), and the second most dominant reasons were a lack of mutual respect and poor understanding of each other's work(17.5%), followed by a lack of mutual respect(17.2%). 6. As to the ways of resolving conflicts with dentists, the most common case was making some mutual concessions to compromise (28.9%), followed by delivering opinions through the staff meeting (23.9%), resolving conflicts by candidly exchanging opinions(15.8%), avoiding each other in moderation(11.7%) and following the opinions or assertions of dentists(1.3%). 7. Concerning the conflict resolution methods among the dental health care workers, the most prevalent way was making some mutual concessions to compromise(36.4%), followed by resolving conflicts by candidly exchanging opinions(23.0%) and conveying opinions through the staff meeting(18.5%). 8. Regarding communication among the dental health care workers, the dental hygienists(3.53±.729) considered themselves to be better at communicating than the nursing assistants(3.29±.745) did(p<0.05), and the dental hygienists(3.45±.809) who thought there was respectful treatment among workers who were different in occupational categories found themselves to be better than the nursing assistants(3.21±.952) who had the same thought did(p<0.05). As a result of analyzing whether frequent job-related meetings occurred among the workers whose occupational categories were different, the dental hygienists(3.05±.975) perceived that there were more frequent meetings than the nursing assistants(2.67±.955) did (p<0.01).

영과잉 음이항회귀 모형을 이용한 보험설계사들의 이직횟수 적합 (Fit of the number of insurance solicitor's turnovers using zero-inflated negative binomial regression)

  • 본 연구는 계수자료 (count data)를 반응변수로 갖는 포아송회귀 모형, 음이항회귀 모형, 영과잉 포아송회귀 모형, 영과잉 음이항회귀 모형의 4 모형의 비교를 통해 보험 설계사들의 이직횟수 적합을 위한 최적모형을 찾고자 한다. 보험설계사 이직횟수의 분산이 평균보다 큰 과대산포가 존재하고 0인 경우의 비중이 높을 경우에 영과잉 음이항회귀 모형을 적합하는 것이 타당함을 보여주고 보험 설계사들의 이직횟수에 영향을 주는 요인을 규명하고자 한다. 로그우도값, AIC, SBC 등을 고려하여 보험설계사 이직횟수 적합을 최적의 모형은 영과잉 이항모형과 음이항회귀모형의 결합인 영과잉 음이항 모형이 선택되었다. 영과잉 이항모형에 포함된 변수로는 성별, 총 보험설계사 근무연월, 교차모집 설계사 등록, 보유고객 수, 소속회사 유형이었고, 음이항회귀 모형에 포함된 변수로는 직무만족, 조직몰입, 채널경영만족, 총 보험설계사 근무연월, 현 직장에서 근무연월, 소속회사 유형이었다. 영과잉 음이항회귀 모형의 적합결과, 이직횟수에 유의한 영향을 주는 요인으로는 현 직장에서 근무연월, 총 보험설계사 근무연월, 소속회사 유형, 채널경영만족, 직무만족 순으로 나타났다.

병원 간호인력의 간호활동량에 관한 연구 (An analysis on nursing activity in a hospital)

  • The management of nursing resource is very important. that is because nursing staff accounts for 30-40% of total staff in a hospital and nurses provide patoents with attentive service for hours daily. The continuous turnover of nursing staff. however, impedes the quality-oriented nursing care, which will consequently leads to the loss of human and material resource in competitive society. This study aimed to calculate nurse's activity and compare the activity amount based on career experience. Futhermore it aimed to find factors which would influence 'quality weighed direct nursing activity amount'. Questionnaires and check lists for this study were distributed to nurses in a suburban hospital outside of Seoul from October 20 to November 14. 1997. The nursing activities were calculated according to professionality. independency and working hour. And then it were accumulated by quality score. The collected data was analyzed by statistical methods as t-test, ANOVA, correlation, multiple regression. The results of this study were as follows ; Firstly, carrel' experience had no influence on the quality weighed nursing activity amount. But the quality level of service of skilled nurses was higher than that of new nurses. Secondly, career was a variable affecting the quality in nursing service. So career was positively related to the quality of nursing care. Patient's disease severity and number of patient were positively correlated with weighed nursing activity amount. But job satisfaction was negatively correlated v:ith the amount. Thirdly, the independent variables which had significant influence on the weighed nursing activity amount were disease severity and the number of patients, The severity score and number of patient were directly proportional to the weighed nursing activity amount. This results indicated· that weighed nursing activity amount was influenced by the number of patient and patient's disease severity. The quality score of nursing services for experienced members is higher than that of new staff. But both new and skilled staff showed no difference in the quality weighed nursing activity amount. Internal and. external environment influences nursing activities. The quality of nursing services is very important factor in nursing activity. Therefore nursing managers should make an effort to improve nursing care quality through continuous research. Also they should try to maintain experience nurses and assign nursing staff appropriately with patient's severity and other relevant factors being considered. The quality-improved nursing care in the hospital will strengthen hospital's competitiveness.

유휴간호사 재취업 의향에 영향을 미치는 요인 (Factors influencing the intent to return to practice (work) of inactive RNs)

  • 본 연구는 유휴간호사의 재취업 의향에 영향을 미치는 요인을 규명하고자 실시하였다. 2013년 대한간호협회와 한국보건사회연구원에서 실시한 '병원간호사 이직(사직)에 대한 조사' 연구를 위한 설문조사 자료 중 최종 381명의 자료를 활용한 이차자료 분석 연구이다. 재취업 의향이 있는 경우는 70.9%로 나타났고, 이들 중 선호하는 근무형태로는 시간선택제 47.8%, 낮번전담제 43.3%, 3교대제 6.3% 이었다. 대상자의 사직의 주된 이유는 '노동 강도가 높아서' (18.8%), '밤근무가 힘들어서' (16.7%), '임금 수준이 낮아서' (15.9%), '출산 및 육아 보육' (15.9%), '직장내 인간관계의 어려움' (11.1%)이었다. 재취업 의향에 유의한 영향을 미치는 요인은 기혼, 최종 근무직장이 종합병원 또는 요양병원인 경우, 선호하는 근무형태가 3교대제인 경우가 각 기준집단에 비해 높은 것으로 나타났다. 또한 '간호직무 자체에 대한 불만족'의 사직이유인 경우로 나타났다. 이에 재취업을 활성화하기 위해 다양한 형태의 근무방식 적용과 노동강도를 고려한 인력 배치와 간호전문직 사명감을 높일 수 있는 교육 프로그램을 제안한다.

4차 산업혁명 대비 보건의료 산업분야종사자를 위한 융합교육 프로그램에 관한 연구 (A Study on the Development of Convergence Education Program for workers in Health and Medical Industry in preparation for the forth industrial revolution)

  • 본 연구의 목적은 최근 활발하게 논의되고 있는 4차 산업혁명에 대한 인식을 조사하고 사회변화에 따른 대학의 신수요 창출을 위한 성인학습자 교육프로그램을 제안하고자 한다. 선행연구를 통해 관련 교육 분야와 이직분야, 교육요구 관련 문항을 도출, 보건의료산업분야 종사자를 대상으로 2017년 7월 1일부터 7월 31일까지 설문하였으며, 그 결과 바이오 의료기기, 고령친화산업, 환경 안전, 임상심리사, 외식산업(커피분야)등이 향후 필요한 교육 분야로 나타났다. 연구 결과의 타당성을 높이기 위해 분야별 관련 전문가의 심층인터뷰를 2017년 8월 19일부터 9월 22일까지 실시하였으며 4차 산업혁명의 도래와 분야별 변화 그리고 필요한 교육프로그램을 제시하였다.

재직자의 교육 훈련특성이 조직몰입을 통해 학습 전이에 미치는 영향 (Impact of Education and Training Characteristics of Incumbent on Learning Transfer through Organizational Commitment)

  • 본 논문에서는 IT 기업 재직자의 교육 훈련특성과 교육 성과에 대해 알아보고자 하였다. 빠르게 변화하는 시장환경에서 기업에게 인적자원은 시장 경쟁력확보의 중요한 요소로써 교육 훈련을 통해 재직자의 역량육성과 기업 성과를 도모하고 있다. 하지만 이러한 노력에도 불구하고 지속적으로 증가하는 퇴사율과 교육내용의 낮은 활용도로 기업들이 교육 훈련 투자를 망설이게 하고 있다. 이와 같은 이유로 인해 연구자들은 교육 훈련의 특성과 성과에 대해 많은 연구가 진행되고 있지만, 교육 훈련특성과 조직몰입의 하위 요소, 현장 활용에 대한 세분화 되어진 연구는 미비한 편이다. 이에 본 연구에서는 교육 훈련특성 요인을 기존 연구를 통해 도출하고 조직몰입을 통해 실제적으로 재직자들이 업무 활용도에 어떠한 영향을 미치는 지 보고자 한다. 그 결과 자기효능감은 조직몰입에 영향을 미치지 않은 것으로 나타났으며, 강사능력과 교육프로그램 적절성은 지속적 몰입과 정서적 몰입에 정(+)의 영향, 업무 관련성은 지속적 몰입과 정서적 몰입에 부(-)의 영향을 미치는 것으로 나타났다. 규범적 몰입은 학습 전이에 정(+)의 영향을 미치는 것으로 나타났다. 이와 같은 결과는 강사 능력과 교육프로그램에 대한 중요성을 높여야 한다는 의미로 보여지며, 교육 관련자들이나 관련 기업들에게 기초자료로 제공하고자 한다.

Assessment of Respiratory Problems in Workers Associated with Intensive Poultry Facilities in Pakistan

  • Background: The poultry industry in Pakistan has flourished since the 1960s; however, there are scarce data regarding the impact of occupational exposure on the pulmonary health of farm workers in terms of years working in the industry. The objective of the present study was to assess the effect of poultry environment on the health of occupationally exposed poultry farmers in countries of warm climatic regions, such as Pakistan. This study will also show the effect of exposure to poultry facilities on the health of poultry farmers in the context of low-income countries with a relatively inadequate occupational exposure risk management. Materials and methods: The lung function capacity of 79 poultry workers was measured using a spirometer. Along with spirometry, a structured questionnaire was also administrated to obtain information about age, height, weight, smokers/nonsmokers, years of working experience, and pulmonary health of farm workers. The workers who were directly involved in the care and handling of birds in these intensive facilities were considered and divided into four groups based on their years of working experience: Group I (3-10 months), Group II (1-5 years), Group III (6-10 years), and Group IV (more than 11 years). The forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and the FEV1/FVC ratio were considered to identify lung function abnormalities. Statistical analysis was carried out using independent sample t test, Chi-square test, Pearson's correlation, and linear regression. Results: Based on the performed spirometry, 68 (86 %) of workers were found normal and healthy, whereas 11 (14 %) had a mild obstruction. Of the 11 workers with mild obstruction, the highest number with respect to the total was in Group IV (more than 11 years of working experience) followed by Group III and Group II. Most of the workers were found healthy, which seems to be because of the healthy survivor effect. For the independent sample t test, a significant difference was noticed between healthy and nonhealthy farmers, whereas Chi-square test showed a significant association with height, drugs, and working experience. Linear regression that was stratified by respiratory symptoms showed for workers with symptoms, regression models for all spirometric parameters (FVC, FEV1, and FEV1/FVC) have better predictive power or R square value than those of workers without symptoms. Conclusion: These findings suggest that lung function capacity was directly related to years of working experience. With increasing number of working years, symptoms of various respiratory problems enhanced in the poultry workers. It should be noted that most of the poultry workers were healthy and young, the rationale being that there is a high turnover rate in this profession. The mobility in this job and our finding of 86% of the healthy workers in the present study also proposed healthy worker survivor effect.

  • 최근 경기침제는 건설 산업의 전반적 건설기업환경을 악화시키고 있다. 2011년 대한건설협회 발표에 따르면 2008년 이후 워크아웃 및 법정관리 중인 건설사는 25개사에 이른다. 이는 기존 건설기업을 운영하는 틀과 사고((思考)에 대한 근본적인 변화를 요구하는 것이다. 지금부터 건설 산업은 서비스업이 되어야 하며, 또한 미래 성장산업으로 뻗어나가려면 서비스업으로 변신해 나가지 않으면 안 된다. 이에 본 연구는 기존 서비스산업분야의'외부고객의 만족을 위해서는 내부고객인 종사원의 만족이 우선이다.' 라는 내부마케팅을 건설기업에 시행(도입, 적용) 및 활동에 앞서 내부마케팅관련 구성요인을 선정하려한다. 또한 각 구성요인들 간에 어떠한 영향을 미치며, 시공사에 기업성과가 있는지 검정하려한다. 그리고 그동안 등한시 하였던 건설기업 구성원의 고객 지향(의식, 인식)적 사고의 전환 도구로, 내부 소통의 도구로 또한 현재 처하고 있는 건설 산업의 위기를 극복하기 위한 방편으로 건설기업에도 내부마케팅의 시행 및 활동을 제안 하고자 한다.

유아교사의 심리적소진과 관련된 요인에 대한 메타분석 (A Meta-analysis of the Factors Related to Psychological Burn-out of Early Childhood Teachers)

  • 본 연구는 유아교사의 심리적소진과 관련하여 지난 20여년 간 국내에서 연구된 석 박사 학위논문과 학술지 논문을 중심으로 심리적소진 관련요인들의 효과크기를 메타분석을 통해 검증하였다. 검증결과는 다음과 같다. 첫째, 유발요인과 억제요인들의 전체효과크기와 요인군, 하부요인, 정적 부적효과요인 모두 유의미한 큰효과크기를 나타냈다. 둘째, 유발요인군은 큰효과크기를 가진 업무상문제요인군이 중간효과크기를 가진 부정적관계요인군 보다 상대적으로 큰 효과크기를 나타냈다. 셋째, 부정적관계요인군의 하부요인 중, 큰효과크기를 가진 원장과의 부정적 관계를 제외한 모든 하부요인이 중간효과크기를 나타냈다. 넷째, 업무상문제요인군의 하부요인은 상대적으로 업무스트레스가 가장 큰 효과크기를 그 다음 역할갈등, 업무과다, 역할모호 순으로 나타났다. 다섯째, 억제요인군 별 효과크기는 상대적으로 회복탄력성요인군이 가장 큰효과크기를 그 다음 조직지원요인군, 교사효능감요인군, 사회적지지요인군, 감정노동요인군 순으로 나타났다. 여섯째, 억제요인군 중 중간효과크기를 보인 감정노동요인군의 내면적, 자연적, 표면적행동요인, 조직지원요인군의 하부요인인 조직지원요인과 교사효능감요인군의 일반교사효능감요인, 사회적지지의요인군의 직장 내 외요인, 사회적지지전체요인을 제외한 모든 요인군이 큰효과크기를 나타냈다. 일곱째, 효과요인에 있어, 직무만족, 우울, 이직의도요인은 모두 큰효과크기를 보인 것으로 나타났다. 결론적으로 본 연구는 선행된 연구들을 통합적으로 정리하여 객관적 결과를 살피기 위한 수량적 통합을 시도했다는데 의의가 있다.
