Clinical efficacy of activated irrigation in endodontics: a focused review

  • Root canal debridement, which includes the removal of infected tissues and microbial biofilms, is considered the corner stone of root canal treatment. Chemical adjuncts play a multitude of functions in this regard, as tissue solvents, antimicrobial agents and for removing the smear layer. These adjuncts (irrigants) are usually delivered using a syringe and needle. With increasing knowledge of the complexity of root canal anatomy and tenacity of microbial biofilms, the need for strategies that potentiate the action of these irrigants within the root canal system cannot be overemphasized. Several such activated irrigation strategies exist. The aim of this review is to comprehensively discuss the different irrigant activation methods from the context of clinical studies.

수종의 근관세척액과 Listerine(R)의 항균성 비교 연구 (COMPARISON OF ANTIBACTERIAL EFFECT OF Listerine(R) WITH VARIOUS ROOT CANAL IRRIGANTS)

  • 본 연구는 여러 가지 근관세척액(NaOCl, CHX, EDTA)과 구강세정제로 사용되는 $Listerine^{(R)}$을 근관감염균주인 Porphyromonas gingivalis와 Enterococcus faecalis를 상대로 항균효과를 비교하고 $Listerine^{(R)}$이 근관세척액으로 사용가능한지를 확인하고자 시행하였다. 본 연구에서는 Porphyromonas gingivalis ATCC 3327과 Enterococcus faecalis ATCC 29212의 표준균주를 사용하였다. 실험을 위한 근관세척제로 0.1%, 0.2%, 1%, 2% Chlorhexidine(CHX)과 0.5%, 1%, 2.5%, 5.25% NaOCl, 0.5M EDTA(18.6% EDTA). 그리고 $Listerine^{(R)}$원액을 이용하였다. 항균효과의 비교는 액체배지상에서 균주의 혼탁도와 한천 확산법을 이용한 억제대 비교로 하였다. 모든 실험군은 대조군과 비교시 근관내 균주에 항균성을 나타냈다(p < 0.001). 모든 농도의 NaOCl, CHX, 그리고 EDTA는 실험균주에서 높은 항균성을 보였다. 모든 실험에서 $Listerine^{(R)}$은 다른 근관세척제에 비해 낮은 항균성을 보였다. 결론적으로, $Listerine^{(R)}$이 E. faecalis와 P. gingivalis에 대해 항균성을 보이나, 일반적으로 사용되는 근관세척 액에 비해서는 현저히 낮은 항균성을 나타냄으로 근관세척 액으로 사용은 적합하지 않음을 나타냈다.

관절경 수술시 슬관절내 온도변화 (Changes in Temperature during Arthroscopic Knee Surgery)

  • 목적 : 관절경 수술 전,후에 발생되는 관절내 온도 변화를 측정하고, 이러한 온도 변화에 영향을 줄 수 있는 요인을 분석하고자 한다. 대상 및 방법 : 반월상 연골의 손상과 전방 십자 인대 손상으로 관절경 수술이 시행된 연속된 40명의 환자를 대상으로 42예의 슬관절에서 온도 변화가 측정되었다. 슬관절 온도의 측정은 3차례에 걸쳐 실시되었는데, 술 전 지혈대 팽창 전, 지혈대를 팽창시킨 후 그리고 마지막 측정은 수술이 끝난 후에 실시하였다. 결과 : 지혈대 팽창 이전의 슬관절 평균 온도는 $35.1{\pm}1.0^{\circ}C$이었고, 술 후 슬관절 온도는 $24.6{\pm}1.5^{\circ}C$로 측정 되었으며 평균 $10.5^{\circ}C$의 온도 감소를 보였다. 관절 종창이 없는 군과 관절 종창이 경도(+1)인 군 관절 종창이 없는 군과 중등도(+2)인 군간에서의 술 전 슬관절의 온도는 통계적으로 유의한 차이를 보였다(p<0.01), 단지 관주용액의 온도와 수술 시간을 술 후 슬관절 온도와 비교할 때, 관주용액의 온도가 낮을수록(p<0.01), 수술 시간이 길수록 술 후 슬관절 온도가 낮았다.(p<0.01). 결론 : 관주용액의 온도를 체온과 유사하게하고 관주용액을 사용하는 수술 시간을 가능한 줄이도록 노력하는 것이 좋을 것으로 사료된다.

이염화이소시아뉼산나트륨 제재의 근관세척액 사용 가능성 평가;염소이온농도, 세포독성, 항균성 및 pH (EVALUATION OF SODIUM DICHLOROISOCYANURATE AS A ROOT CANAL IRRIGATION SOLUTION;Cl- CONCENTRATION, pH, CYTOTOXICITY AND ANTIMICROBIAL EFFECT IN VITRO)

  • The purpose of this study was to evaluate the clinical applications of the Sodium Dichloroisocyanurate effervescent tablet as a routine root canal irrigant by performing several in vitro tests such as $Cl^{-}$ content. cytotoxicity. antimicrobial effect as well as its pH level compared to the equivalent concentration of sodium hypochlorite solution. 1. Sodium Dichloroisocyanurate demonstrated lower level of $Cl^{-}$ concentration than each dilution of sodium hypochlorite solution. Both solution has increased level of $Cl^{-}$ as the concentration of each solution increased. There was no significant change of $Cl^{-}$ concentration in sodium hypochlorite as time goes by. However. $Cl^{-}$ concentration in Sodium Dichloroisocyanurate was increased. 2. The antimicrobial effects of both solutions were increased when their concentrations were increased. One day after dilution. antimicrobial effect of Sodium Dichloroisocyanurate was slightly higher than sodium hypochlorite. however. there was no difference in 1 week dilution solution. One month dilution solution of sodium hypochlorite still retain its activity. but antimicrobial effect of Sodium Dichloroisocyanurate was drastically decreased 1 month after dilution. 3. The cytotoxicity of Sodium Dichloroisocyanurate was rather higher than same concentration of sodium hypochlorite solution until 1 week after dilution. Then in 1 month. cytotoxicity of Sodium Dichloroisocyanurate was decreased than that of 1 week dilution solution. especially 4% Sodium Dichloroisocyanurate solution has almost no toxicity. However. 1% and 2% sodium hypochlorite solution has unchanged moderate degree of cytotoxicity after the dilution. Furthermore. 4% sodium hypochlorite solution showed high level of toxicity. 4. The pH level of Sodium Dichloroisocyanurate showed that the solution was weak acid (pH5). On the other hand. sodium hypochlorite was revealed as a strong alkaline solution (pH12). There was no change in pH following the dilution of each solution. As results. Sodium Dichloroisocyanurate solution fully satisfy the basic requirements as a root canal irrigation solution. However. we strongly recommend to use this solution clinically in low concentration and try to apply into the root canal within 1 week after dilution.

유치 치수 치료의 근관세척 (The Canal Irrigation in Paediatric Pulpal Treatment)

  • 유치나 영구치를 막론하고 근관내의 세균 감염으로 인한 치수 괴사 및 이에 따른 치근단 병소의 유발은 임상의로 하여금 근관치료나 발치와 같은 도전적인 치료에 직면하게 한다. 특히 유치에 근관치료를 필요로 하는 경우, 소아 환자의 행동조절이 한계가 있고, 치아의 해부학적 차이, 또는 계승치의 존재로 인해 근관치료가 아닌 발치와 같은 차선의 치료로 접근할 수 있다. 영구치나 유치를 막론하고 치수내 잔존 세균과 유기조직 잔사를 기계적 방법과 함께 적절한 근관세척액을 사용하여 가능한 많이 제거함으로써 치근단 주위조직의 치유를 유도하여야 한다. 차아염소산 나트륨은 우수한 항균효과와 조직용해성, 독소의 무력화 또는 도말층을 제거할 수 있는 성질 등으로 이상적인 근관세척제의 조건에 거의 합당한 약제이다. 그러므로 러버댐 및 추가적인 차폐 조치를 한 다음, 저농도의 차아염소산 나트륨을 사용하는 것이 바람직하다.

근관 소독제의 효과에 관한 임상적 연구 (A CLINICAL STUDY ON THE EFFECT OF INTRACANAL MEDICATION IN ENDODONTIC TREATMENT)

  • The purpose of this study was to clarify the effect of intracana1 medication on the clinical symptoms and the number of visits before canal filling in endodontic treatment The experimental teeth was divided into three groups. The teeth of group 1 was irrigated with saline solution and the teeth of group 2 was irrigated with sodium hypochlorite during biomechanical preparation. The intracanal medicament was not applied in the canals of group 1 and group 2. The teeth of group 3 was irrigated with sodium hypochlorite and hydrogen peroxide during canal enlargement and applied formocresol as intracanal medicament after drying the root canals with paper points. The incidence of interappintment pain and percussion pain after 1 st visit and the number of appointments before canal filling was examined, and the results was statistically compared. The following results were obtained. 1. The incidence of interappointment pain and percussion pain and the number of visits before canal filling was not effected by the types of irrigant and the use of the intracanal medicament 2. There was no significant difference in the incidence of interappintment pain arid percussion pain and the number of appointement before canal filling between vital teeth and non vital teeth, and also between single rooted teeth and multirooted teeth. 3. The use of formocresol as intracanal medicament was not able to aid the elimination of clinical symptoms and to decrease the number of appointment before canal filling.

치과치료시 러버댐의 사용에 대한 환자의 인식도 조사 (Patients' perception on the use of rubber dam for dental treatment)

  • A questionnaire-based survey was done to evaluate the patients' perception on the use of Rubber Dam (RD) for the tooth isolation during dental treatment. Total 106 questionnaire were gathered and the data were analyzed according to the patients' gender, age, previous experience of RD. The 96% of total patients reported no experience of RD in their previous treatment. The first advantage of RD was selected as the protection of irrigant and medication into the oral cavity and the disadvantage was selected as saliva gathering during treatment. 98% of patients want to use RD for the next treatment and 99% remarked that the RD is essential for the dental treatment. Based on the surveyed data, majority of the patients highly satisfied with the RD use and they want to use RD and may consider the use of RD for the selection of dental clinic.

차아염소산(次亞鹽素酸)나트륨용액(溶液)의 치수조직(齒髓組織)에 대(對)한 용해효과(溶解效果) (THE EFFECT OF SOLVENT ACTION OF SODIUM HYPOCHLORITE SOLUTION ON PULP TISSUE)

  • Sodium hypochlorite solution has been widely used as endodontic irrigant due to its ability to dissolve pulp tissue debris and its antimicrobial action. This in vitro study was conducted to evaluate the solvent action of sodium hypochlorite solution on vital pulp tissue under various conditions include concentration, exposure time, and temperature. The percentage of weight loss due to pulp tissue dissolution was calculated with weight difference of lyophilized specimens before and after the exposure to test solutions. The results were as follows; Statistical analysis indicated that the ability of both 5.0% and 2.5% sodium hypochlorite solutions to dissolve pulp tissue was significantly greater than that of distilled water, but no significant difference was found between 5.0% and 2.5% sodium hypochlorite solutions. There was no significant increase in the pulp tissue dissolving ability of sodium hypochlorite solutions; as exposure time increased 2 minutes, 5 minutes, and 10 minutes. Of the given temperatures, no significant difference was found in the solvent aciton of sodium hypochlorite solution on pulp tissue between $20^{\circ}C$ (room temperature) and $37^{\circ}C$(body temperature).

유치의 근관 치료 중 차아염소산나트륨의 치근단 유입으로 인한 합병증 (Accidental Extrusion of Sodium Hypochlorite during Endodontic Treatment in a Primary Tooth)

  • 차아염소산나트륨은 가장 자주 쓰이는 근관 세척제이지만, 그 독성과 부작용에 대해서는 많이 인식되어 있지 않다. 본 증례는 근관 치료 중 차아염소산나트륨이 치근단 조직으로 압출되어 심각한 합병증이 발생하였기에 이를 보고하는 바이다. 만 5세의 남자 환자에서 근관 치료 중 차아염소산나트륨이 치근단 조직으로 유입되어 갑작스런 통증 및 부종, 주위 조직의 피하출혈 그리고 근관 내의 지속적 출혈을 보였다. 환자는 입원치료를 받았고, 진통제와 항생제를 처방받았다. 근단공이 넓은 미성숙 영구치와 유치, 치근흡수, 천공이 일어난 치아에서는 근관 치료 시 차아염소산나트륨이 압출되지 않도록 더욱 각별한 주의가 필요하다. 합병증이 발생하였을 때에는 적절한 환자의 관리와 약물치료가 필요하다.

Outcomes of the GentleWave system on root canal treatment: a narrative review

  • This study aimed to describe the outcomes of the GentleWave system (GW) (Sonendo) on root canal treatment. Published articles were collected from scientific databases (MEDLINE/PubMed platform, Web of Science, Scopus, Science Direct and Embase). A total of 24 studies were collected from August/2014 to July/2021, 20 in vitro and 4 clinical. GW System was not associated with extrusion of the irrigant, promoted faster organic dissolution than conventional syringe irrigation (CSI), passive ultrasonic irrigation (PUI) continuous ultrasonic irrigation (CUI) and EndoVac, reduced more bacterial DNA and biofilm than PUI and CUI, promoted higher penetration of sodium hypochlorite into dentinal tubules than PUI and CUI in vitro, and removed more intracanal medication than CSI and PUI. GW was able to remove pulp tissue and calcifications. Moreover, its ability to remove hard-tissue debris and smear layer was better than that of CSI, and its ability to remove root canal obturation residues was lower or similar to that of PUI, and similar to that of CSI and EndoVac. Regarding root canal obturation of minimally instrumented molar canals, GW was associated with high-quality obturation. Clinically, the success rate of endodontic treatment using GW was 97.3%, and the short-term postoperative pain in the GW group was not different from CSI. Further research, mainly clinical, is needed to establish whether GW has any advantages over other available irrigation methods.
