Buerger법 치료를 위한 국소 정맥내 Prostaglandin E1 주입 (Intravenous Regional Administration of Prostaglandin E1 for the Treatment of Buerger's Disease)

  • Buerger's disease is a chronic occlusive arterial disease in which a non-arteriosclerotic lesion involves medium-sized arteries, veins, and nerves of the distal leg or arm. Sympathetic interruption is indicated to improve blood flow to the involved extremity, although sympathetic blockade can provide temporally relief of vasospasm and pain. Chemical or surgical sympathectomy has been performed for this purpose and intravenous regional sympathetic block(IRSB) is an alternative. Guanethidine or reserpine has been administered for IRSB. Intraarterial or intravenous systemic administration of prostaglandin E1(PGE1) has been recommended for the treatment of Buerger's disease. We used PGE1 for intravenous regional administration as an IRSB with results as good as that of intraarterial injection. The advantages of the method include that it is less expensive than systemic administration, less invasive than intra-arterial injection, and simple in technical application.

黃芩芍藥湯 煎湯液이 血漿Cortisol 및 鎭痛에 미치는 影響 (Effects of Hwang Keum Jag Yag Tang Water Extract on the Plasma Cortisol Concentration and in the Rabbit and on the Analgesic Effect in the Mouse)

  • In order to investigate the effects of Hwang Keum Jag Yag Tang (HJT) water extract on the plasma cortisol concentration in the rabbit and on the analgesic effect in the mouse were administered. The results were summarized as follow: 1. Intravenous administration of HJT water extract at the dose of 0.2ml/kg remarkably increased plasma cortisol concentration after two hour. 2. Intravenous administration of HJT water extract at the dose of 0.4ml/kg significantly increased plasma cortisol concentration after one hour. 3. Intravenous administration of HJT water extract at the dose of 0.2ml/kg, all the experimental period, significantly increased plasma sodium concentration. 4. Intravenous administration of HJT water extract at the dose of 0.4m1/kg significantly decreased plasma potassium concentration. 5. Intravenous administration of HJT water extract at the dose of 0.2ml/kg and 0.4ml/kg, all the experimental period, significantly decreased plasma calcium concentration. 6. The analgesic effect of HJT water extract showed inhibitory effect more than at 0.1ml/20g. According to the results, Hwang Keum Jag Yag Tang water extract remarkably increased plasma cortisol concentration and showed analgesic effect.

  • The purpose of this study was to report dose-independent pharmacokinetics of KR-31543, a new neuroprotective agent for ischemia-reperfusion damage, after intravenous and oral administration and first-pass effects after intravenous. intraportal, intragastric, and intraduodenal administration in rats. After intravenous (10, 20 and 50 mg/kg) and oral (10, 20 and 50 mg/kg) administration, the pharmacokinetic parameters of KR-31543 were dose-independent. (omitted)

  • To evaluate effects of Pai Mo San (PMS) water extract on the plasma cortisol concentration and arterial blood $PCO_2,\;PO_2$ in the rabbit. The results obtained were as follows: 1. Intravenous administration of PMS water extract at the dose of 0.2 ml/kg remarkably increased plasma cortisol concentration from 1 to 2 hours. 2. Intravenous administration of PMS water extract at the dose of 0.4 ml/kg plasma cortisol concentration showed more significant increase than the control group after 1,2,4 hours. 3. No change observed in the arterial blood $PO_2$ after intravenous administration of PMS water extract at the dose of 0.2 ml/kg used. 4. No change observed in the arterial blood $PO_2$ after intravenous administration of PMS water extract at the dose of 0.4 ml/kg used. 5. Intravenous administration of PMS water extract at the dose of 0.2 ml/kg arterial blood $PCO_2$ showed more significant decreased than the control group after 1,3 hours. 6. Intravenous administration of PMS water extract at the dose of 0.4 ml/kg significantly decreased arterial blood $PCO_2$. These results suggest that the therapeutic action of Pai Mo San water extract for Yeol Su (熱嗽), Wha Su (火熱), Gu Su (久熱) and anti-asthma maybe related with the increase of plasma cortisol, decrease of $PCO_2$.

ICR계 마우스에서 Enrofloxacin과 Colistin 복합체의 단회 경구 및 정맥투여시 급성독성 (Acute Toxicity of Enrofloxacin-Colistin Combinations after a Single Oral and Intravenous Administration in ICR Mice)

  • The study was carried out to evaluate the acute toxicity of enrofloxacin-colistin combination via a single oral(p.o.)and intravenous(i.v.) administration in ICR mice. All procedures of the test were performed by the established regulation of Korean National Institute of Safety Research (1994. 4.14). The maximal dose of oral and intravenous routes was 5,000mg/kg and 90mg/kg, consisting with each 6 groups including control of male and female, respectively. As the results, $LD_{50}$m}'s of the combinations showed 3,075mg/kg (f)and 2,564mg/kg(m) after oral administrations, together with 48mg/kg(f) and 40mg/kg(m) after intravenous administration. These facts indicated that acute toxicitiy of enrofloxacin-colistin combination were different depending on the administration routes and sexes in ICR mice. In conclusion, the route of enrofloxacin-colistin combination must not choose as i.v. route administration in terms of acute toxicity based on $LD_{50}$.

Comparison of Positron Emission Tomography(PET) imaging-based initial in vivo pharmacokinetics by administration routes of [18F]FDG

  • In this study, the initial in vivo pharmacokinetic changes according to the routes of drug administration were investigated using bioimaging techniques. The purpose of this study was to quantify the degree of distribution of each major organ in normal mice over time by acquiring Positron Emission Tomography/Computed Tomography images while administering routes F-18 fluorodeoxyglucose such as intravenous, intraperitoneal and per oral, a representative diagnostic radiopharmaceutical. Dynamic Positron Emission Tomography images were acquired for 90 minutes after drug administration. Radioactivity uptake was calculated for major organs using the PMOD program. In the case of intravenous administration, it was confirmed that it spread quickly and evenly to major organs. Compared to intravenous administration, intraperitoneal administration was about three times more absorbed and distributed in the liver and intestine, and it was showed that the amount excreted through the bladder was more than twice. In the case of oral administration, most stayed in the stomach, and it was showed that it spread slowly throughout the body. In comparison with intravenous administration, it was presented that the distribution of kidneys was more than 9 times and the distribution of bladder was 66% lower. Since there is a difference in the initial in vivo distribution and excretion of each administration method, we confirmed that the determination of the administration route is important for in vivo imaging evaluation of new drug candidates.

Sulphamethomidine의 젖소에 있어서의 유선과 신장을 통한 배출 (Mammary and renal excretion of sulphamethomidine in cows)

  • The mammary excretion of suphamethomidine after intravenous and/or oral administration was investigated in cow. The results show that sulphamethomidine is bound to plasma proteins to a great extent (80~90%). Ay a dosage of 60 mg./kg. maximal concenration in plasma of this sulphonamide was reached 7-10 hours after oral dosing. The sulphonamide concentration in plasma slowly declined after both oral and intravenous administration (fig. 1, 2, and 3) The concentration of sulphonamide in milk was very low and the excretion was completed in 7 days after a single oral dose and 5 days after intravenous injection while in the case of blood plasma it was 11 and 7 days, respectively. In addition, the renal excretion of sulphamethomidine was investigated while under continuous intravenous intravenous infusion. The excretion ratios varies according to self depression (table. 1). Blockade of the tubular secretion with diodone lowered the excretion of sulphamethomidine. It is concluded that the renal excretion of sulphamethomidine in cows occurs by filtration by slight tubular secretion and also by a high rate of back diffusion.

Current trends in intravenous sedative drugs for dental procedures

  • Anxiety and phobia in dental procedures are common deterrents for patients visiting the dental care unit. For these individuals, procedural sedation may aid in completion of dental treatments. In most cases, the patients are conscious during sedation, thereby allowing spontaneous ventilation. Intravenous sedation (IVS) is widely used during dental treatment to relieve patient anxiety. IVS is the most effective route of administration to achieve this goal, but it requires advanced training, more than that provided during undergraduate education. During IVS, rapid onset, repetitive drug administration, easy titration, and rapid recovery from sedation can be achieved. However, conscious sedation during IVS can result in deep sedation that can cause respiratory and cardiovascular depression. Therefore, the characteristics of intravenous sedatives should be known. The purpose of this review is to discuss the characteristics and usage of intravenous sedatives currently used for dental procedures.

  • 환아의 연령, 과다체중, 환아의 약물 거부 등으로 인하여 chloral hydrate를 이용한 수면치료가 불가능한 경우, Midazolam을 이용한 정주진정요법이 전신마취를 대신하는 대안으로 사용될 수 있다. 본 환아의 경우, 0.3mg/kg의 midazolam을 근주하고, 70% 아산화질소 가스를 이용하여 초기 수면상태를 유도하고 정맥천자를 실시한 후, 0.2mg/kg midazolam을 정주하여 50분간 별다른 부작용 없이 성공적으로 치료할 수 있었다.

제왕절개술후 통증치료로 지속적 정주방법과 고식적 근주방법의 비교 (A Comparison of Continuous Intravenous Infusion and Traditional Intramuscular Injection for Postoperative Pain following Cesarean Section)

  • Background: Recently, improvements in drug administration technology have intensified interest in the treatment of postoperative pain. this has resulted in increased use of continuous intravenous infusion of opioid and epidural opioid as alternative to traditional intramuscular administration of opioid. The goal of this study, therefore, was to document the effects of pain control and side effects following continuous intravenous infusion of morphine or meperidine and intramuscular meperidine following cesarean section. Methods: The vital signs, pain score, oxygen saturation and side effects were compared in 150 patients receiving continuous intravenous infusion of morphine, 30 ${\mu}g/kg/hr$ (n=50, group 1); continuous intravenous infusion of meperidine, 150 ${\mu}g/kg/hr$ (n=50, group 2); or intramuscular meperidine, 50mg/every 6hrs (n=50, group 3). Results: VAS (Visual Analogue Scale) was significantly decreased after 30 minutes of administration in all three groups and was significantly lower at 1 hour, but higher at 6 hours in group 3 than two other groups. Severe desaturation episode, defined as $SpO_2$<90%, occurred in the group 3(0.2%). Moderate desaturation episodes, defined as $SpO_2$ 91~95%, occurred more in group 3 than in group 1 and 2 (17.4% vs. 10.4%, 8.2%). The incidence of side effects were similar among three groups. Conclusion: The continuous infusion of opioid was more effective and safe method of postoperative pain control than traditional intramuscular injection.
