Comparative Efficacy Study of Pyeongwi-san(Pingwei-san) of Digestive Tract Disease Model in Rat (소화기 질환 동물모델에서 평위산(平胃散)의 염증 완화 효과)

  • Objectives : Pyeongwi-san is widely used in Korean medicine for acute indigestion or gastrodynia. As a therapeutic agent for digestive diseases of modern people, in order to confirm the mechanism of Pyeongwi-san on digestive tract disease and the difference of therapeutic efficacy between its formulation, a comparative efficacy test was conducted on digestive tract disease animal model. Methods : For LPS enteritis animal model, male SD rats were intraorally treated with different formulation types of Pyeongwi-san, and then intraperitoneally administered LPS one hour later to induce enteritis. After 5 hours, blood was collected and TNFα, IL-1β, IL-6 and PGE2 were confirmed by ELISA. For acute gastritis animal model, male SD rats were intraorally treated with different formulation types of Pyeongwi-san according to the prescribed concentration, and then intraorally administered 60% ethanol and 150 mM HCl one hour later to induce acute gastritis. After 5 hours, blood was collected and TNFα,IL-6 were confirmed by ELISA. Results : In the LPS-administered enteritis animal model, Pyeongwi-san decreased TNFα, IL-1β, PGE2 and especially IL-6. Pyeongwi-san also decreased IL-6 in acute gastritis animal model. In addition, there was no significant difference in efficacy between the two formulations when compared with inflammatory markers. Conclusions : The efficacy of Pyeongwi-san was confirmed in the inflammatory markers related to digestive inflammatory diseases, and the efficay between two formulations of Pyeongwi-san was relatively similar. Further studies are needed to investigate the new applicability of Pyeongwi-san on different inflammatory diseases that have similar inflammation markers identified in this experiment.

  • Primary tumors of the parapharyngeal space are rare and account for only 0.5% of head and neck neoplasm. About 80% of parapharyngeal tumors are benign and 20% are malignant. Parapharyngeal space is classified into the Prestyloid space and the Poststyloid space. The Poststyloid tumors are usually benign lesions such as neurogenic tumors, paragangliomas, vascular tumors, or aneurisms. The origins of prestyloid tumors are much more diverse pathology, the pleomorphic adenoma in parotid deep lobe is most common type. Several surgical approaches have been introduced for management of parapharyngeal tumor, such as transcervical, transparotid-transcervical and the transcervical-transmandibular approaches. This paper is aimed to present a large parapharyngeal space tumor removed via transoral approach. It is possible to remove easier by using microdebrider. The pathologic diagnosis was pleomorphic adenoma.

Versatility of Modified Nasolabial Flap in Oral and Maxillofacial Surgery

  • Background: To evaluate the versatility and reach of modified nasolabial flap used in reconstruction of defects created in and around the oral cavity. Methods: A total number of 20 cases were selected. Out of which 13 were males and 7 females. The age of these patients ranged from 24-63 years. 29 modified nasolabial flaps were raised in twenty patients. Based on clinical and histopathological examination, out of 20 patients, 14 patients were diagnosed with oral submucous fibrosis, 3 with verrucous carcinoma, 1 with squamous papilloma, 1 with oro-antral fistula and 1 with traumatic loss of lower lip. Results: Minimum preoperative interincisal distance (IID) was 0 mm and maximum was 15 mm with mean of $6.00{\pm}4.76mm$ in patients with oral submucous fibrosis and 12 months postoperatively minimum IID was 16 mm and maximum was 41 mm with mean of $28.00{\pm}8.96mm$. In one case, dehiscence (3.4%) was noted on the anterior tip for which tip revision was done. Bulky appearance of the flap intraorally was observed in 2 cases (6.9%). Five (17.2%) among the 29 flaps had visible scar at the donor site postoperatively up to 3 months. Conclusion: Numerous reconstructive techniques have been employed in the reconstruction of small to intermediate sized defects of oral cavity. Modified nasolabial flap is a versatile flap which has robust vascularity and can be successfully used with minimal complications. It can be rotated intraorally to extend from the soft palate to the lip. Thus, it can be used efficiently to treat the small defects of the oral cavity as well as recreating lost lip structure.

Color alterations of a PMMA resin for fixed interim prostheses reinforced with silica nanoparticles

  • PURPOSE. The aim of the present study was to evaluate the color changes of an autopolymerizing PMMA resin used for interim fixed restorations, reinforced with $SiO_2$ nanoparticles. MATERIALS AND METHODS. Silica nanoparticles were blended with the PMMA resin powder through high-energy ball milling. Four shades of PMMA resin were used (A3, B3, C3, D3) and total color differences were calculated through the equations ${\Delta}E_{ab}=[({\Delta}L*)^2+({\Delta}a*)^2+({\Delta}b*)^2]^{1/2}$ and ${\Delta}E_{00}=[(\frac{{\Delta}L^{\prime}}{K_LS_L})^2+(\frac{{\Delta}C^{\prime}}{K_CS_C})^2+(\frac{{\Delta}H^{\prime}}{K_HS_H})^2+R_T(\frac{{\Delta}C^{\prime}}{K_CS_C})(\frac{{\Delta}H^{\prime}}{K_HS_H})]^{1/2}$. Statistically significant differences between ${\Delta}E_{ab}$ and the clinically acceptable values of 3.3 and 2.7 and those between ${\Delta}E_{00}$ and the clinically acceptable value of 1.8 were evaluated with one sample t-test (P<.05). Differences among the different shades were assessed through One-Way ANOVA and Bonferroni multiple comparison tests. RESULTS. Significantly lower values were detected for all groups concerning ${\Delta}E_{ab}$ compared to the intraorally clinical acceptable values of 3.3 and 2.7. Significantly lower mean values were detected for groups B3, C3, and D3, concerning ${\Delta}E_{00}$ compared to the intraorally clinical acceptant value of 1.8. Color pigments in red-brown (A3) and red-grey (D3) shades affect the total color change to a greater extent after the reinforcement with $SiO_2$ nanoparticles compared to the red-yellow (B3) shade. CONCLUSION. Within the limitations of this in vitro study, it can be suggested that reinforcing PMMA with $SiO_2$ nanoparticles at 0.25 wt% slightly affects the optical properties of the PMMA resin without being clinically perceivable.

TREATMENT OF DENTIGEROUS CYST: REPORT OF A CASE (함치성양종의 치험예)

  • Dentigerous cysts are closed epithelium-lined sacs formed about the crowns of unerupted teeth. Most of them probably are the result of degenerative changes in the reduced enamel epithelium. The authors observed 1 3 year old girl with a swelling and buccal bony expansion of the left mandible. Roentgen examination showed a cystic area in the left side of mandible. Under general anesthesia by means of nasotracheal intubation, intraorally, the operation by enucleation consisted in surgical removal of dentigerous cyst and the left unerupted Ist molar of mandible. The cavity was partially sutured and packed with iodoform gauze. Healing was uneventful.

A STATISTICAL STUDY ON THE DENTAL CARIES INCIDENCE IN AUTISTIC PERSON (자폐인의 치아우식증에 관한 통계학적 연구)

  • The purpose of this study was to make a comprehensive study and to provide information about the dental caries of autistic persons, and to aid improvement of their oral health. The authors examined intraorally 114(male; 87, female; 27) autistic persons and 119(male; 68, female; 51) normal persons as control group. The obtained results were as follows: 1. Autistic persons were found to have lower dft rate than normal persons. 2. There was no significant difference in dental caries incidence between male and female autistic persons. 3. Institutionalized autistic persons were found to have lower dental caries incidence of deciduous teeth than non-institutionalized autistic persons. 4. Autistic persons whose parent's occupation is laborer were found to have higer dft index than those whose parent's occupation is private business or employee.

Frenectomy for improvement of a problematic conventional maxillary complete denture in an elderly patient: a case report

  • Maxillary labial and buccal frena are considered as normal anatomic structures in the oral cavity. However, they may exist intraorally as a thick broad fibrous attachment and/or become located near the crest of the residual ridge, thus interfering with proper denture border extension resulting in inferior denture stability, retention and overall patient satisfaction. This case report highlights the importance of clinical examination and treatment planning which may mandate preprosthetic surgery prior to fabrication of a new conventional complete denture. Adequate patient satisfaction with conventional complete dentures can be significantly increased after frenectomy.

  • A local anesthetic agent which is most commonly used for outpatients is lidocaine hydrochloride that contains epinephrine, which is for vasoconstriction in 1:100,000 concentration. This agent is known as a safe local anesthetic agent and has been used widely for topical use or injections. However, the allergic reaction that we will report in this case occurred when common local anesthesia was done intraorally, and the patient complained of hyperventilation, tachycardia, abdominal pain and unintentional tears. We experienced an allergic reaction after injecting the lidocaine hydrochloride and therefore report the case to suggest that local anesthesia should be always carried out very carefully.

  • Osteopetrosis is a rare skeletal disease of unknown etiology. Osteomyelitic changes in the jaw bones are frequently seen in this disease, especially in the mandible and may follow upon tooth extraction. A case is reported of a 31-year-old male who was seen because of intraorally exposed bony spicule and chronic dull pain. Radiographic and haematological investigations led to the diagnosis of osteopeirosis with osteomyelitis. The striking radiologic findings were pathologic mandibular fracture and generalized skeletal thickening. The present case could be grouped under the benign form, type II osteopetrosis. With the conservative and minimally invasive modes of treatment including administration of penicillin G sodium, curettage and closed reduction, we could successfully manage the osteomyelitis and pathologic fracture associated osteopetrosis.

  • Epidermal cysts in mouth floor are rare. There could be confusions in diagnosis due to location and low incidence. For diagnosis, imaging study and cytology are necessary. The treatment of choice is complete excision intraorally or externally and the recurrence is rare. Some authors prefer sclerotherapy, but the follow up is necessary for malignant change. We report two cases of epidermoid cyst in submental area excised externally with literature review.
