A Case study on improvement of ICE syndrome using Ortho-Cellular Nutrition Therapy (OCNT) (세포교정영양요법(OCNT)을 이용한 ICE 증후군 개선 사례 연구)

  • Objective: A Case study on improvement of ICE syndrome through the use of OCNT. Methods: OCNT was performed on a 45-year-old Korean female who had difficulty in opening the eyes widely due to painful and xerophthalmias, and suffered inconvenience in daily life due to myodesopsia. Results: After the implementation of OCNT, the intraocular pressure (IOP) level was restored to normal, and the pain, dryness and myodesopsia symptoms were alleviated. This resulted in improvement in depressive symptoms and quality of life. Conclusion: For patients suffering from problems such as elevated intraocular pressure, myodesopsia, and xerophthalmia, OCNT can be helpful in improvement and treatment of symptoms.

Biomechanical Properties of the Cornea Using a Dynamic Scheimpflug Analyzer in Healthy Eyes

  • Purpose: To investigate biomechanical properties of the cornea using a dynamic Scheimpflug analyzer according to age. Materials and Methods: In this prospective, cross-sectional, observational study, participants underwent ophthalmic investigations including corneal biomechanical properties, keratometric values, intraocular pressure (IOP), and manifest refraction spherical equivalent (MRSE). We determined the relationship of biomechanical parameters and ocular/systemic variables (participant's age, MRSE, IOP, and mean keratometric values) by piecewise regression analysis, association of biomechanical parameters with variables by Spearman's correlation and stepwise multiple regression analyses, and reference intervals (RI) by the bootstrap method. Results: This study included 217 eyes of 118 participants (20-81 years of age). Piecewise regression analysis between Corvis-central corneal thickness (CCT) and participant's age revealed that the optimal cut-off value of age was 45 years. No clear breakpoints were detected between the corneal biomechanical parameters and MRSE, IOP, and mean keratometric values. Corneal velocity, deformation amplitude, radius, maximal concave power, Corvis-CCT, and Corvis-IOP exhibited correlations with IOP, regardless of age (all ages, 20-44 years, and over 44 years). With smaller deformation amplitude and corneal velocity as well as increased CorvisIOP and Corvis-CCT, IOP became significantly increased. We provided the results of determination of confidence interval from RI data using bootstrap method in three separate age groups (all ages, 20-44 years, and over 44 years). Conclusion: We demonstrated multiple corneal biomechanical parameters according to age, and reported that the corneal biomechanical parameters are influenced by IOP.

Acupuncture for glaucoma: A systematic review and meta-analysis of randomized controlled trials (녹내장의 침치료 효과에 대한 체계적 문헌고찰 및 메타분석)

  • Objectives : This study aims to evaluate the effectiveness and safety of manual and electroacupuncture on glaucoma. Method : We searched 11 electronic databases using index words to identify randomized clinical trials. Meta-analysis of weighted mean difference (WMD) or standardized mean difference (SMD) were used to evaluate the outcomes. Cochrane bias risk assessment tool was used to assess the risk of bias in each clinical study. The collected data was analyzed using RevMan software (ver. 5.3). Results : At the initial stage of data retrieval, 549 papers were searched. After reviewing 37 full texts, a total of 10 RCT studies (426patients, 715 eyes) were selected and 8 RCT studies (357 people, 617 eyes) were involved in meta-analysis. Meta-analysis of 8 RCTs showed that acupuncture alone was more effective in reducing intraocular pressure(IOP) than conventional treatment (WMD = -5.73, 95% CI: [-12.30, 0.83], P = 0.09, I2 = 97%) The combination of acupuncture or electroacupuncture with conventional treatment was also effective in lowering IOP (WMD = -1.84, 95% CI: [-2.31, -1.37], P <0.00001, I2 = 45%). It was estimated that the combination of acupuncture with conventional treatment was also effective for improving visual field (VF) (WMD = -2.17, 95% CI: [-4.32, -0.02], P = 0.05, I2 = 89%) but improvement in visual acuity (VA) was not significant (MD = 0.06, 95% CI: [-0.03, 0.15], P = 0.23, I2 = 0%). Subgroup analyzes were performed only for the studies that used open glaucoma as the study's disease and combination of acupuncture or electroacupuncture with conventional therapy would have an effect on lowering intraocular pressure (WMD = -1.68)., 95% CI: [-2.46, -0.90], P <0.0001, I2 = 29%). Conclusion : This study suggests that acupuncture treatment for glaucoma may be effective in reducing intraocular pressure and helpful in improving visual field defects. However, due to the small sample size, high risk of bias and high heterogeneity in the methodology, it is expected that further studies will be needed to verify the results. Further studies in large-scale samples based on a minimized biased methodology would be necessary.

  • Glaucoma is characterized by a slow and progressive degeneration of the optic nerve, including retinal ganglion cell (RGC) axons in the optic nerve head (ONH), leading to visual impairment. Despite its high prevalence, the biological basis of glaucoma pathogenesis still is not yet fully understood, and the factors contributing to its progression are currently not well characterized. Intraocular pressure (IOP) is the only modifiable risk factor, and reduction of IOP is the standard treatment for glaucoma. However, lowering IOP itself is not always effective for preserving visual function in patients with primary open-angle glaucoma. The second messenger cyclic adenosine 3',5'-monophosphate (cAMP) regulates numerous biological processes in the central nervous system including the retina and the optic nerve. Although recent studies revealed that cAMP generated by adenylyl cyclases (ACs) is important in regulating aqueous humor dynamics in ocular tissues, such as the ciliary body and trabecular meshwork, as well as cell death and growth in the retina and optic nerve, the functional role and significance of cAMP in glaucoma remain to be elucidated. In this review, we will discuss the functional role of cAMP in aqueous humor dynamics and IOP regulation, and review the current medications, which are related to the cAMP signaling pathway, for glaucoma treatment. Also, we will further focus on cAMP signaling in RGC growth and regeneration by soluble AC as well as ONH astrocytes by transmembrane ACs to understand its potential role in the pathogenesis of glaucoma neurodegeneration.

Retinal Function Analysis and Histopathologic Assessment after Intravitreal Cidofovir Injection in Normal Beagle Dogs

  • This study was performed to evaluate the effects of an intravitreal injection of cidofovir in beagle dogs. Nine beagle dogs (18 eyes) were used and randomly assigned to the following three groups of various dosages: 100, 500 and $1000{\mu}g$. Aqueous paracentesis was followed by an intravitreal injection of cidofovir. Intraocular pressure (IOP) was measured twice a week and electroretinography (ERG) and ophthalmoscopic examination were performed every week during the study. At the end of the study, all eyes were enucleated for histopathologic evaluation after euthanasia. The IOPs in the 500 and $1000{\mu}g$ groups were statistically lower than baseline with no significant IOP changes in the $100{\mu}g$ group. Reduced amplitudes of ERG recordings were identified in the eyes injected with higher dose groups than the $100{\mu}g$ group. Histopathologic examination revealed that there were dose-related toxicities to the ciliary body and the retina. These results suggest that intravitreal cidofovir had dose-dependent IOP lowering effects associated with ciliary body destruction, but had the potential to cause retinal toxicity in beagle dogs.

Effectiveness and Safety Verification of Brinzolamide Combination Therapy on Primary Open-angle Glaucoma or Ocular Hypertension: A Systematic Review and Meta-analysis Study (브린졸라미드 복합제의 개방각 녹내장 또는 고안압증에 대한 효과 및 안전성 검증: 체계적 문헌 고찰 및 메타분석 연구)

  • Background: The treatment of primary open-angle glaucoma (POAG) or ocular hypertension (OHT) for intraocular pressure (IOP) reduction is recommended to proceed with the use of the compound. Therefore, this study aimed to evaluate the efficacy and safety of brinzolamide combination therapy on POAG and OHT following the subgroup analysis among types of brinzolamide combined medications. Methods: By June 2019, PubMed, EMBASE and Cochrane Library were searched to find a study that met our inclusion criteria. Based on randomized control trials (RCTs), we collected studies that tested the brinzolamide combination therapy in POAG and OHT patients, and analyzed the literature identified by the results of the study on IOP reduction and adverse reactions. Results: A total of 13 literature was collected to conduct an analysis including 2,197 patients. The intervention included brinzolamide combination therapies, combined with timolol, brimonidine, PGA or combined with both brimonidine and PGA. The analysis showed significant decreasing tendency for values at morning and end treatment per day in the use of brinzolamide combination therapy in the absolute IOP change (mean difference (MD) -1.41; 95% CI -1.92, -0.90; p<0.001 vs. MD -1.46; 95% CI -2.03, -0.89; p<0.00001, respectively). We could see higher adverse reactions in the brinzolamide combination group using intervention (odds ratio 1.43; 95% CI 1.20, 1.71; p<0.0001). Conclusion: Regarding IOP reduction in POAG and OHT patients, brinzolamide combination therapy is more effective but less safe than control treatment, which diverse among types of combined medications. Thus, more individualized therapy should be applied in real-world practice.

Whether can Posner-Schlossman syndrome be treated through a physiotherapy, ciliary muscle training method?

  • A new physiotherapy named Ciliary Muscle Training method and one successful case had been reported first for treatment of Posner-Schlossman Syndrome (PSS), also known as Glaucomatocyclitic Crisis. When a person observed a back and forth moving object, the ciliary muscle would involuntarily do sometimes tense and sometimes relaxant exercise which can relieve the tensive state of the ciliary muscle of patients suffering PSS disease. Through this period of continued treatment, this ciliary muscle training method can improve the health status of the ciliary muscle and finally achieve the purpose of curing PSS disease. In this successful case, the patient's feedback result was very good after receiving our suggested ciliary muscle training method as an attempt to treat PSS, ill right eye of this PSS patient showed normal appearance without any differences from the other normal eye, his vision test values of naked eye were both 4.9 under nearly 200 degrees of myopia. High intraocular pressure (IOP) was mitigated, measured IOP value was in the normal (≦ 21 mmHg) and comfortable state without PSS relapse until now. Therefore, ciliary muscle training method could be a potential clinical treatment approach to PSS disease.

Fatty Acid Concentration of Serum Phospholipids in Normal and Glaucoma Patients (정상인과 녹내장 환자의 혈청 인지질 지방산 농도)

  • Glaucoma is one of the leading causes of blindness worldwide and is characterized by degeneration of the optic nerve. Elevated intraocular pressure (IOP) is the major contributing factor to optic nerve damage. However, some patients develop glaucoma even with normal IOP. Other factors, including age, race, myopia, and nutrition, can affect glaucoma risk. Dietary fat intake and serum fatty acid composition are closely related with the fatty acid profile of the retina and thus can modulate glaucoma risk. In this study, we collected serum samples from 34 glaucoma patients (26 primary open angle glaucoma (POAG) and 8 normal tension glaucoma (NTG)) and 45 healthy controls and analyzed their serum phospholipid fatty acid concentrations. The results show that the ratio of oleic acid to stearic acid (OA/SA), which is an indicator of stearoyl-CoA desaturase activity, was higher in POAG patients when compared to normal controls. The ratio of eicosapentaenoic acid to arachidonic acid (EPA/AA), which is an indirect marker of eicosanoid biosynthesis, was also higher in glaucoma patients (p=0.048). These results imply that dietary fatty acid intake and serum fatty acid profile can influence glaucoma risk. Additional study is necessary to identify the relationship between fatty acids and glaucoma.

Ahmed Valve Implantation for Early Glaucoma Treatment in Dogs (개의 초기 원발성 녹내장안에서 Ahmed valve 이식술의 적용)

  • The purpose of this study was to compare the long term prognosis of Ahmed valve implantation and medical therapy in dogs with early primary glaucoma. The medical records of dogs with early primary glaucoma from the Veterinary Medical Teaching Hospital of Seoul National University between January 2005 and May 2008 were reviewed. Five dogs underwent Ahmed valve implantation and medical therapy was performed in six dogs. Breeds of the dogs included Shih Tzu(n = 7), American cocker spaniel(n = 2), and miniature poodle(n = 2). Adequate intraocular pressure(IOP) was better maintained in the Ahmed valve implantation group than in the medical treatment group throughout the follow-up period. Mean time for vision loss to develop after treatment in the Ahmed valve implantation group($198.0{\pm}115.4$ days) was significantly longer than that in medical treatment group($43.2{\pm}30.4$ days, p < 0.05). Post-operative complications included occlusion of the tube by fibrin in the anterior chamber (n = 1), fibrous capsule about the episcleral base of the device(n = 1), and dislocation of the valve body(n = 1). It is considered that Ahmed valve implantation is more effective strategy to control IOP and maintain vision than medical treatment in early glaucomatous dogs.
