중심성 비만 분석을 위한 새로운 임피던스 해석법 (Novel Impedance Method for Analyzing Truncal Obesity)

  • Truncal obesity associated with insulin resistance and metabolic syndrome increase the likelihood of hypertension, various cardiovascular diseases, hyperlipidemia and coronary heart diseases. International Diabetes Federation (IDF) experts recognized that it is necessary to develop the simple diagnostic tool which is applicable to diagnose truncal obesity worldwide, and proposed the method using a waist circumference but there is a limit to estimate subcutaneous fat distribution. However, waist line is also influenced by total fat capacity less than the intra abdominal fat. The more having severe obesity, the more correlation coefficient between waist line and intra abdominal fat is low. Therefore, this thesis defines a new abdominal impedance measurement position and impedance-index to analysis central obesity. This proposes the new model to estimate abdominal obesity using the abdominal impedance-index and CT images acquired fro 160 Korean subjects. The proposed model shows that the abdominal fat distribution has a higher correlation than waist line. (Adj R2=0.809, 0.667 and 0.687 with abdominal fat area, visceral fat area and subcutaneous fat area respectively).

Increased Rate of Palmitate Oxidation in Adults Female: Comparison with Peri-pubertal Young Female Rats

  • Although estrogen is known to playa role in fatty acid metabolism, it remains unclear whether fatty acid oxidation in mature female rats differs from fatty acid oxidation in peri-pubertal young rats. In this study, we measured fatty acid metabolism in the skeletal muscles and livers of 5 and 50 weeks old male and female rats. The rate of palmitate oxidation in the liver and gastrocnemius red in the 50-week-old female rats were elevated as compared to the 5-week-old females, whereas there were no differences in the male rats. The rate of palmitate oxidation in the gastrocnemius red was correlated inversely with intra-abdominal fat mass in the 5-week-old male and female rats, whereas the palmitate oxidation rate was positively correlated with fat mass in the liver and gastrocnemius red in the 50-week-old rats. HOMA-IR and plasma insulin levels were positively correlated with intra-abdominal fat mass in the pooled 50-week-old male and female rats, but this correlation was not apparent in 5-week-old rats. In summary, the rate of fatty acid oxidation measured in the middle-aged adult female rats was significantly higher than those measured in the peri-pubertal young female rats. This difference may be attributed to the influence of ovarian hormones.

Comparison of Computed Tomography-based Abdominal Adiposity Indexes as Predictors of Non-alcoholic Fatty Liver Disease Among Middle-aged Korean Men and Women

  • Objectives: We compared the associations of 3 computed tomography (CT)-based abdominal adiposity indexes with non-alcoholic fatty liver disease (NAFLD) among middle-aged Korean men and women. Methods: The participants were 1366 men and 2480 women community-dwellers aged 30-64 years. Three abdominal adiposity indexes-visceral fat area (VFA), subcutaneous fat area (SFA), and visceral-to-subcutaneous fat ratio (VSR)-were calculated from abdominal CT scans. NAFLD was determined by calculating the Liver Fat Score from comorbidities and blood tests. An NAFLD prediction model that included waist circumference (WC) as a measure of abdominal adiposity was designated as the base model, to which VFA, SFA, and VSR were added in turn. The area under the receiver operating characteristic curve (AUC), integrated discrimination improvement (IDI), and net reclassification improvement (NRI) were calculated to quantify the additional predictive value of VFA, SFA, and VSR relative to WC. Results: VFA and VSR were positively associated with NAFLD in both genders. SFA was not significantly associated with NAFLD in men, but it was negatively associated in women. When VFA, SFA, and VSR were added to the WC-based NAFLD prediction model, the AUC improved by 0.013 (p<0.001), 0.001 (p=0.434), and 0.009 (p=0.007) in men and by 0.044 (p<0.001), 0.017 (p<0.001), and 0.046 (p<0.001) in women, respectively. The IDI and NRI were increased the most by VFA in men and VSR in women. Conclusions: Using CT-based abdominal adiposity indexes in addition to WC may improve the detection of NAFLD. The best predictive indicators were VFA in men and VSR in women.

소아 복부비만 진단을 위한 복부 전산화 단층 촬영의 유용성과 여러 지표와의 상관성 연구 (The Usefulness of the Abdominal Computerized Tomography for the Diagnosis of Childhood Obesity and Its Correlation with Various Parameters of Obesity)

  • 목 적 : 복부 비만은 고혈압, 인슐린 저항, 고인슐린혈증, 당뇨, 고지혈증과 같은 많은 심혈관계 질환의 위험 요인으로 제시되고 있다. 복부 비만을 측정하는 지표로 허리/둔부 비와 허리둘레가 사용되고 있으나, 관상동맥 질환의 예측인자로 복부 비만의 측정학적 cut-off 수치가 소아에서는 아직 제시되지 못하고 있다. 복부 전산화 단층촬영과 초음파를 이용하여 복부 지방을 측정하고, 여러 비만 지표들과의 상관관계를 연구함으로써 복부 비만의 합병증으로 초래되는 심혈관계 질환의 위험인자 평가 방법을 알아 보고자 본 연구를 실시하였다. 방 법 : 비만한 청소년 27명과 나이와 성별이 대등한 정상 체중을 가진 청소년 22명을 대상으로 하여 체질량 지수와 비만 지수, 상완둘레 및 삼각근에서 피부 두께를 비교하였다. 또한, 12시간 공복 상태에서 혈중 지질 및 혈당, 인슐린, 4가지 사이토카인을 측정하고, 인슐린 저항 지수를 구하였으며, 생체 전기저항법을 이용하여 체지방량, 체지방률, 복부지방률을 측정하여 비교하였다. 비만군에서는 복부 초음파로 배꼽부위 피하지방 두께와 복강 내 지방 두께를 측정하고, 복부 전산화 단층 촬영으로 얻은 횡단면 스캔에서 전체 지방 면적을 구하였고, 이 중 피하지방의 면적을 감산하여 복강 내 지방 면적을 구하였다. 결 과 : 소아에서 복부 CT로 측정한 총 지방 면적과 생체 전기 저항법으로 측정한 총지방량과 상관성이 매우 높았다(r=0.954, P<0.05). 복부 CT에서 측정한 복강 내 지방 면적은 상완둘레, 복부비만율, 초음파로 측정한 복부 내 지방 깊이 및 피하 지방량, 체질량 지수, 중성지방 등과 상관성이 높았다. 결 론 : 복부 CT는 복강 내 지방을 측정하는 가장 정확한 방법이지만 임상적으로 유용하지 못하므로, CT와 상관성이 높은 복부 초음파로 대치할 수 있다. 외래나 학교 신체검사에서 사용할 수 있는 선별 검사로는 생체 전기 저항법에 의한 체지방 측정, 상완 둘레, 체질량 지수, 허리/둔부 비 등이 있다. 소아 성인병의 심혈관 위험 예측 지표로는 공복시 leptin, 중성지방, 인슐린 농도 등을 사용할 수 있다.

Catch-up growth and catch-up fat in children born small for gestational age

  • Infants born small for gestational age (SGA) are at increased risk of perinatal morbidity, persistent short stature, and metabolic alterations in later life. Recent studies have focused on the association between birth weight (BW) and later body composition. Some reports suggest that fetal nutrition, as reflected by BW, may have an inverse programing effect on abdominal adiposity later in life. This inverse association between BW and abdominal adiposity in adults may contribute to insulin resistance. Rapid weight gain during infancy in SGA children seemed to be associated with increased fat mass rather than lean mass. Early catch-up growth after SGA birth rather than SGA itself has been noted as a cardiovascular risk factor in later life. Children who are born SGA also have a predisposition to accumulation of fat mass, particularly intra-abdominal fat. It is not yet clear whether this predisposition is due to low BW itself, rapid postnatal catch-up growth, or a combination of both. In this report, we review the published literature on central fat accumulation and metabolic consequences of being SGA, as well as the currently popular research area of SGA, including growth aspects.

Smoking Is Associated With Abdominal Obesity, Not Overall Obesity, in Men With Type 2 Diabetes

  • Objectives: Abdominal obesity increases mortality and morbidity from cardiovascular disease and there is a possibility that smoking effects obesity. However, previous studies concerning the effects of smoking on obesity are inconsistent. The objective of this study was to examine whether smoking is positively related to abdominal obesity in men with type 2 diabetes. Methods: Subjects consisted of 2197 type 2 diabetic patients who visited Huh's Diabetes Center from 2003 to 2009. Indices of abdominal obesity were defined as visceral fat thickness (VFT) measured by ultrasonography and waist circumference (WC). Overall obesity was defined as body mass index (BMI). Results: Statistically significant differences in WC and VFT by smoking status were identified. However, there was no statistical difference in BMI according to smoking status. Means of WC and VFT were not significantly higher in heavy smokers and lower in mild smokers. Compared to nonsmokers, the BMI confounder adjusted odds ratio and 95% confidence interval for VFT in ex-smokers and current-smokers were 1.70 (1.21 to 2.39) and 1.86 (1.27 to 2.73), respectively. Conclusions: Smoking status was positively associated with abdominal obesity in type 2 diabetic patients.

  • 5 years Poodle was referred to the Veterinary Teaching Hospital, Chonnam National University in January 1997, with progressive swelling on the rear xiphoid process after cesarean section last year in local-vet clinic. Physical, radiographic, ultrasonographic and intraoperative findings that were obtained from this case were as follow; Physical findings were known as no pain, no fever in the hernia] sal at palpation. Radiographic findings revealed the decreased density of swelling contents more than peritoneum and did not identify the abdominal wall defect line. Ultrasonographic findings showed abdominal wall defect line of 3mm and intra-abdominal originated fat contents with mixed echo(hypoechoic). During the operation, falciform ligament into the hernial sac was observed and not excised, manipulated back into the cavity. The dog was given an herniorraphy with no recurrence and infection.

Chitinase 3-Like 1 (CHI3L1) Polymorphism Contributes to Visceral Obesity and Obesity-related Inflammation Induces Chi3l1 in Adipocytes

  • Abdominal obesity is considered as one of the most risky factors governing the development of metabolic diseases. Here we identify that human chitinase 3-like 1 (CHI3L1, also called YKL-40 in human) single nucleotide polymorphism (SNP), rs883125, is associated with abdominal obesity in Korean women. Korean women subjects with the rs883125 G/G or C/G genotype present higher waist-hip ratio than subjects with C/C genotype suggesting that human subjects who G nucleotide substitution at the rs883125 tended to more accumulate intra-abdominal fat at the abdominal cavity. In addition, Chi3l1 gene expression is increased in adipose tissue from obese mice and pro-inflammatory cytokine enhances Chi3l1 expression in adipocytes, indicating that Chi3l1 is greatly related with obesity and obesity-induced pro-inflammatory responses. Taken together, the minor allele of rs883125 is associated with a higher prevalence of abdominal obesity in Korean women. These findings suggest that genotype of rs883125 can be a biomarker of incident abdominal obesity and abdominal obesity-related metabolic diseases.

Effects of ᴅ-allulose on body fat accumulation in rats fed severely carbohydrate-restricted diets containing beef tallow or soybean oil

  • Purpose: The carbohydrate-restricted diet has been recognized to be effective into preventing and alleviating lifestyle-related diseases, such as obesity and type 2 diabetes. The rare sugar ᴅ-allulose is a functional monosaccharide with anti-obesity effects. In the present study, we examined the effects of dietary ᴅ-allulose on body fat accumulation in rats fed severely carbohydrate-restricted diets containing high concentrations of different fats, beef tallow, or soybean oil. Methods: Male Wistar rats (n = 35, 3-week-old) were divided into 5 groups: One chow-fed control (C) group, and four carbohydrate-restricted groups, namely, beef tallow (B), beef tallow + ᴅ-allulose (BA), soybean oil (S), and soybean oil + ᴅ-allulose (SA), with free access to the diet and water for 8 weeks. The B and BA diets contained 23% beef tallow and 2% soybean oil, whereas the S and SA diets contained 25% soybean oil. Furthermore, the BA and SA diets contained 5% ᴅ-allulose. Results: The final body weight, weight gain, and food intake were significantly higher, and food efficiency was significantly lower in the control group compared to the other carbohydrate-restricted groups. Intra-abdominal adipose tissue, carcass fat, and total body fat weights were not influenced by dietary fat type or ᴅ-allulose supplementation, except for the epididymal adipose tissue weight. In contrast, carbohydrate restriction suppressed body weight gain in rats, but remarkably increased body fat accumulation. Conclusion: Under carbohydrate-restricted conditions, no anti-obesity effects of dietary ᴅ-allulose were observed, regardless of the dietary fat type. The causes of these effects are unknown. However, they may be influenced by a very low carbohydrate and high protein diet. Further research is required to elucidate the effects of ᴅ-allulose under various nutrient compositions with different fat, carbohydrate, and protein energy ratios.

신체장애 평가에서 유방의 법적 지위 - 장기 해당 여부, 수유장애, 노동력상실에 대하여 - (The legal status of the breast in assessing physical disability)

  • 최근 들어서면서 유방에 대한 수술이 종류가 확대술 축소술 재건술로 대별되고 다양해지면서 이에 따른 원치 않은 수술성과에 대한 분쟁이 증가상태에 있으며, 이로 인한 배보상 문제가 최종적으로 대두되면서, '원치 않은 수술성과'를 '수술 후의 문제점' 보다는 "장애'로 보는 관점이 증가되면서, 신체장애 평가에서 유방의 흉부 장기 해당 여부에 따른 노동력상실률이 직접적으로 거론되기에 이에 대한 견해를 발표하고자 한다. 이를 위해, 법령에서 나오는 용어의 사전적 정의 및 의학적 제반 사항을 살펴보았다. 흉부 장기에 유방 포함 여부의 혼란은 용어 선정 후 그 해석이 시대 변화와 맞물려 유발된 것으로 생각된다. 법령에서의 자구 해석을 살펴보면, 유방은 흉부 장기에 해당되지 못함으로 규정된 것으로 사료되며, 수유장애는 낮은 출산율과 실제로 모유수유하는 경우가 현저하게 줄어들고 있는 상황에서 다수의 의견이라고 주장 강조함이 약화되고 있으며, 모유수유를 노동으로 볼 수 있는 유모로서의 직업도 현재 존재하는지 조차 모르는 실정이다. 이런 이유로, 유방을 흉복부 장기 해당 여부에 대한 논란을 잠재우기 위해서, 포함시키려면, (1) 법령집의 용어를 교체하거나(흉복부 장기에서 (ㄱ) 흉복부를 '흉복부 및 흉복강'으로 그리고 (ㄴ) 장기를 '장기 및 기관'으로), (2) 여러 법령에서 용어 변경은 어렵기 때문에, "산업재해보상보험법 시행규칙 [별표 제5호] 신체부위별 장해등급 판정에 관한 세부기준(제48조 관련)"에 유방을 예외적으로 포함시키면 될 것이라고 생각해 본다. 그러나 피부부속기관인 유방을 흉복부 기관 혹은 예외적으로 포함시키면, 당연히 다른 피부부속기관도 포함시켜야 하는 모순관계가 도출되는 문제가 있다고 사료된다. 반면 완전히 배제시키려면, (1) '흉복부'를 '흉복강'으로 하거나, (2) "유방은 제외된다"라는 단서 조항만 삽입하면 될 것이라고 생각해 본다. 법 앞에 만인이 평등하다 하지만 만인 앞에 법이 평등하기 위해서는, 즉 새로운 해석과 판결을 위해서는 사회적인 합의가 있어야 할 것으로 생각된다.
