'Studies on diagnosis and Treatment of tumor.abscess.ulcer in intestinal carbuncle' (장옹(腸癰)에 있어서 종양(腫瘍).농양(膿瘍).궤양(潰瘍)의 진단(診斷)과 치료(治療)에 관(關)한 고찰(考察))

  • Studies on diagnosis and treatment of tumor . abscess . ulcer in intestinal carbuncle were carried out. The result of studies were summerized as follows: 1. By Nai-Gyung carbuncle-tumor arose from disharmony between nutrient and defensive because of cold, abscess arose from fever victory between cold and fever, ulcer arose from decreasing function of Bi-Kam year. By latter literature Bi-Kam year could be interpreted that spleen stomach was invaded evil influence from unattainable vital force of the earth. 2. Sites of intestinal carbuncle were large intestine, small intestine, intestinal inside or outside between large and small intestine, Intestinal carbuncle was common name of a disease about large intestinal carbuncle, small intestinal carbuncle, pelvic intestinal carbuncle, shrink leg intestinal carbuncle etc.. Pain appeared Chunchu-Hyul in large intestinal carbuncle, and Gwanweon-Hyul in small intestinal carbuncle. 3. On abdominal diagnosis tumor had indistinct pain of Gwanweon Chunchu, edema and heary feeling in low abdomen, no excessive pain by hand press and intestinal boiling sound. In abscess pain descended from right side of low abdomen to huckle, and there was rejection against press, feeling about fever,water sound with flank movement. In ulcer hand approach was difficult since excessive pain diffuse to whole abdomen, and perforating ulcer sometimes caused a serious symptom of umbilical pus. 4. On fecal and urinary diagnosis in tumor urine was yellowish red pollakiuria like gonorrhoea and occasional constipation. In abscess uncomfortable rough pain short red early urine like gonorrhoea appeared during urination, and constipation with stinging pain appeared during defecation. In ulcer red rough pyuria appeared, and stinging and pain with puruloid blood appeared during defecation. 5. On treatment in tumor Daiwhang-Tang Daisenggi-Tang Dangui-Jun by dissipation method, calming down method, interior reliance maturation method, in abscess Mokdan-San Euiiin-Tang Jeokduiin-Tang by the method of water repelling pus discharge, acute breaking, in ulcer Takridanggui-Tang Paljin-Tang Bojungikki-Tang were each used by the method of interior reliance, virulence astriction, supplement vital force and blood, supplement spleen stomach. 6. On treatment patient may have to be careful of excessive moving and suprising anxiety. Abuse of acupuncture and moxibustion made patient worse, misuse of analgesics purgative intestinal irrigation etc. could provoke difficult diagnosis and perforation. So you must treat after exact diagnosis. 7. Prognosis of ease tumor ease abscess ease ulcer and ease astriction was good. If the intestinal carbuncle were not to promote to abscess and ulcer for a long time, its prognosis was bad and it could metastasize to cancer because of dark purple with hardness. So tumor abscess ulcer in intestinal carbuncle may be significant of precancerous lesion.

Preparation of Active Fraction from Radish Water Extracts for Improving the Intestinal Functions and Constipation Activities (무(Raphanuse sativa var. nigra L.) 물 추출물로 부터 장기능 및 변비질환 개선을 위한 활성 분획의 제조)

  • The aim of this study was to develop an activator, 3-10 kDa fraction from radish water extracts, that will improve the intestinal function and bowel movement in the colons. Radish water extracts were investigated for their intestinal function effects according to the charcoal meal transit method, employing Balb/c mice: also, their anti-constipation activities were compared utilizing the loperamide-induced constipation method, employing SD rats. The result suggested that the effects of the charcoal meal transit increased remarkably in radish water extract administrated rats in comparison to loperamide administrated rats. Futhermore, the effects of various solvent extracts of radish on charcoal meal transit in Balb/c mice increased remarkably in radish water fraction administrated rats than in different solvent fraction administrated rats. Radish extraction was tested and isolated into 4 groups: below 3 kDa, 3-10 kDa, 10-300 kDa, and over 300 kDa. 3-10 kDa was the most effective on the intestinal function and bowel movement in the colons; also, 3-10 kDa fraction of radish water extraction was found to be the most effective charcoal meal transit. The dry weight and moisture content of feces remarkedly increased in the 3-10 kDa administrated rats group than in the loperamide only group. Experimental results revealed that 3-10 kDa fraction of radish water extract was the most effective on the intestinal function and bowel movement was the crypt epithelial cells that contained more MUC2 in the 3-10 kDa administrated group than the loperamide only group: in addition, the thickness of mucus layer stained with alcian blue was significantly thicker in 3-10 kDa administrated rats than in loperamide administrated rats. Crypt epithelial cells secreted more MUC2 in the 3-10 kDa administrated group than the loperamide only group and the stained cells clearly showed the MUC2 with antibody Biogenex AM358.

The Effect of zusanli(ST 36) Acupuncture on the Small Intestinal Motility (족삼리자침(足三里刺鍼)이 가토소장(家兎小腸)의 운동성(運動性)에 미치는 영향(影響))

  • This study was undertaken to determine effect of Zusan1i(ST36) acupuncture (AP) on the small intestinal motility in the rabbit. Changes in the motility in vivo of ileum was measured before and 20 min. after AP. In order to examine whether AP effect was affected by autonomic nerves, norepinephrine or acetylcholine was applicated. AP significantly increased the mean wave amplitude and motor index, but not the mean wave duration and motor activity. Infusion of norepinephrine ($2{\mu}m$) into an ear vein resulted in a decrease in the mean wave amplitude and motor index, which was not affected by AP. Acetylcholine ($10{\mu}m$) produced inhibition followed by stimulation of motility. The stimulation of motility was induced by an increase in the mean wave amplitude and motor index, but not the mean wave duration and motor activity. The effect of acetylcholine was not significantly different between before and after AP. These results suggest that Zusan1i(ST36) acupuncture stimulates the small intestinal motility, which is not mediated by autonomic nerves. Therefore, Zusan1i(ST36) acupuncture seems to be applicable to impaired small intestinal movement.

A Clinical Case Study of Paralytic Ileus Patient Improved by Euphorbiae Kansui Radix(Gan-sui) (마비성 장폐색환자의 감수로 호전된 증례)

  • Paralytic ileus is one of the gastro-intestinal symptoms of a patient who is in the post-symptom period resulting from stroke. The purpose of this study was to examine the efficacy of Euphorbiae Kansui Radix(Gan-sui) for a patient who has suffered from severe paralytic ileus as post-symptoms caused by 3rd stroke. The subject was a 70-year-old woman who had been troubled with dyspepsia, abdominal flatus and other pains during the past 10 years, and whose symtoms worsened because of her recent 3rd stroke. At the time of admission, she complained of abdominal flatus, conspitation, quadri weakness(Lt>Rt) and dysathria. For the first 10 days after admission, she was treated with Oriental' Western Medicine, which failed to relieve any symptom. However, after taking Euphorbiae Kansui Radix(Gan-sui), gastro-intestinal vermicular movement improved, so the symptoms of abdominal flatus and conspitation disappeared. As a consequence, the accompanying paralytic ileus condition also improved.

Pathological study on abdominal fat necrosis of adult cattle sampled from slaughterhouse in Korea (국내 도축우의 복강에서 관찰된 지방괴사의 병리학적 연구)

  • An abattoir study on the abdominal fat necrosis in adult cattle was performed pathologically. Grossly, masses of fat necrosis were leekgreen in colour, lobulated on the cut surface, and saponificated in the texture. These necrotic adipose tissues infiltrated usually into neighboring parenchymal organs including intestines and pancreas, leading to fibrosis or atrophy of them. Histopathologically, necrotic fat cells contained acidophilic, opaque, amorphous substance or basophilic fibrillar or granular minerals in their cytoplasms. The lesions of fat necrosis were divided by fibroconnective tissue. With increase of the severity, necrotic fat cells fused each other and then formed fat cysts. In this severe lesion, necrotic fat cells were partialy or completely replaced by macrophages. Multinucleated giant cells were scattered in this lesion. Interestingly, small artery in the lesion of fat necrosis revealed severe thickening of internal elastic membrane. Severe fibrosis was observed in or between the outer longitudinal and inner circular muscular externas causing segregation, degeneration and necrosis of muscle fibers. The nerve cells of Auerbach's and Meissner's plexuses surrounded by fibrosis were degenerated or necrotic. In addition, necrotic fat cells infiltrated into the pancreas, resulting in pancreas atrophy. From these results, it is speculated that fat necrosis might compromise intestinal movement due to necrosis of muscular externa and ganglion cells of Auerbach's and Meissner's plexuses.

Effect of Seatangle Oligosaccharide Drink on Oxidation of Serum Lipid and Bleeding and Plasma Clotting Time in Rats Fed a Hyperlipidemic Diet (해조 올리고당 음료가 고지혈증 유도 쥐에 있어서 혈액 지질 산화 및 혈액ㆍ혈장 응고에 미치는 영향)

  • We investigated the effect of seatangle drink and seatangle extract on lipid oxidation, blood coagulation and intestinal movement in rats fed a hyperlipidemic diet. In the dietary hyperlipidemic induced group, the serum superoxide dismutase activity decreased and formation of hydroxy radical increased when compared to normal group, but these were controlled by seatangle drink treatment. The decreased of bleeding time and increased of tissue factor in the dietary hyperlipidemic rats were regulated by seatangle drink and seatangle extract, and especially the activity of tissue factor was remarkably decreased. Seatangle drink and seatangle extract were increased contraction on intestinal movement. Therefore, it can be concluded that seatangle drink or seatangle extract treatment depresses changing in absorption of gastrointestinal track in rats fed a hyperlipidemic diet.

Dietary Fiber and Large Bowel Cancer

  • Large bowel cancer correlates tightly to dietary factors such as dietary fiber and fat. Dietary fiber prevents the large bowel cancer in different modes of action which depend upon physicochemical and fermentable properties. Water-soluble fiber is fermented easily by intestinal microbes producing short chain fatty acids ; in contrast, water-insoluble fiber occurs effectively more rapid transit time due to greater bulk of gut content, though it is unfermentable. Not only short chain fatty acid is utilized in the proximal and distal colon as primary energy source, but also it lowers pH in the colon to normalize cellular differentiation and helps to stimulate peri staltic movement by acting as an osmotic laxative. In particular, butyric acid may also regulate gene expression and cell growth, though it is an important respiratory fuel for the colonocyte. Since dietary fiber and non-digestible oligosaccharides are the major source of butyric acid, this provides a possible link between dietary fiber and oligosaccharide and prevention of large bowel cancer. But, as with many links between dietary fiber and large bowel cancer, a direct casual association has not been established. In addition, RDA of dietary fiber which is 20~25g/day for adult Japanese, appears to be reasonable for the defecation once daily and the prevention of large bowel cancer.

Estimation Algorithm of Bowel Motility Based on Regression Analysis of the Jitter and Shimmer of Bowel Sounds (장음 특징 변수의 회귀 분석을 통한 장 운동성 추정법)

  • Bowel sounds (BS) are produced by the movement of the intestinal contents in the lumen of the gastro-intestinal tract during peristalsis and thus, it can be used clinically as useful indicators of bowel motility. We devised an estimation algorithm of bowel motility based on the regression modeling of the jitter and shimmer of BS signals measured by auscultation. Ten healthy males ($23.5\pm2.1$ years) were examined. Consequently, the correlation coefficient, coefficient of determination and standard error between the colon transit times (CTT) measured by a conventional radiograph and the values estimated by our algorithm were 0.98, 0.96 and 2.86, respectively. Also, through k-fold cross validation, the average value of the absolute differences between them was $5.0\pm2.5$ hours. This method could be used as a complementary tool for the non-invasive measurement of bowel motility.

Effect of Dietary Xylooligosaccharide on Indigestion and Retarding Effect of Bile Acid Movement Across a Dialysis Membrane (식이 Xylooligo당의 난소화성과 담즙산 흡수 지연효과)

  • The digestibility of xylooligosaccharide(XO) by juices of the digestive tract and retardation effect of XO on the adsorption of bile acids were compared with fructooligosaccharide(FO) and isomaltooligosaccharide(IO). In vitro digestion experiments showed that any hydrolyzed products of FO, IO and XO were not detected by HPLC after reaction with saliva, pancreatic, artifical intesteinal, and large intestinal luices, and artifical sera for 4 hours at 37$^{\circ}C$. However, IO were mostly digested by the small intestinal juice, and some quantity of FO were digested. XO were not digested at all by any enzyme of digestive tract. In order to investigate retardation effect of XO on the bile acid absorption. In vitro, permeability of bile acid against dialysis membrane was determined in the mixture which contained guar gum instead of XO was set 100%. The premeability of bile acid showed about 50% in the FO and IO mixture and 43% in the XO mixture. The activity of lactase in FO group and activity of sucrase and maltase in XO group in rat small intestinal mucosa were significantly decreased. Consequently, the present results indicate that XO is indigestible in digestive tract and has retarding effect of adsorption of bile acid compared with the other oligosaccharides. The disaccharidase activity of the XO dietary group was lower than that of the other oligosaccharides dietary group. Furthermore, it was suggested that hydrolysis of sugar may be retarded in digestive tract and glucose level in blood may be controlled effectively by the XO.
