Effects of Acupuncture at HapGuk$(LI_4)$ on Pain in Rats (합곡(合谷) 침자극(鍼刺戟)이 동통억제(疼痛抑制)에 미치는 영향(影響))

  • The purpose of this study was finding the pain inhibitory effect of acupuncture at $LI_4$. The pain at dentes incisivi was evoked by noxious electric stimulation and EMG changes based on the interval were measured. To do this, the opioid antagonist was administered intraperitoneally. Following are some of results. 1. The pain inhibitory effect of acupuncture at $LI_4$ was 94.35%, 84.56%, 57.62%, 54.40%, at 10, 20, 30 and 40 minutes respectively. All the data were calculated based on 100% at 0 minute. The effect was very significant at 40 minutes. 2. The continuous effect of acupuncture at $LI_4$ after taking out the needle was 58.58%, 57.62%, 66.22%, 73.18%, 83.70%, 92.68%, at 50, 50, 70, 80, 90 and 100 minutes respectively. The overall continuous effect was shown its maximum 20 minutes after taking out the needle and reached to the initial value at 60 minutes. 3. The pain inhibitory effect following the naloxone administration and acupuncture application at $LI_4$ was 95.96%, 96.04%, 94.86%, 94.92% at 10, 20, 30 and 40 minutes respectively. All values showed similar tendency to the initial data. 4. The continuous effect of acupuncture at $LI_4$ after taking out the needle which was preceded by the naloxone administration was 94.48%, 96.02%, 96.02%, 98.00%, 98.46%, 97.18% at 50, 60, 70, 80, 90 and 100 minutes respectively. This trend was not a significant fluctuation. It was concluded that effect of acupuncture at $LI_4$ was shown in conjunction with secretion of analgesic substance. Therefore it is implied that the acupuncture application will play a major role in treating many diseases with more revelation of scientific acupuncture mechanism. Further studies of acupuncture manipulation are needed in the future based on our study.

Effects of Acupuncture at Chok-samni(ST36) on Pain in Rats (족삼리(足三里) 침자(鍼刺)가 동통억제(疼痛抑制)에 미치는 영향(影響))

  • The purpose of this study was finding the pain inhibitory effect of acupuncture at ST-36. The pain at dentes incisor was evoked by noxious electric stimulation and EMG changes based on time interval were measured. To do this, the opioid antagonist was administered intraperitoneally. Followings are some of the results. 1. The pain inhibitory effect of acupuncture at ST-36 was 93.82%, 87.00%, 75.30%, 69.76% at 10 20, 30 and 40 minutes respectively. All the data were calculated based on 100% at 0 minute. The effect was very significant at 40 minutes. 2. The continuous effect of acupuncture at ST-36 after taking out the needle was 77.46%, 79.66%, 87.60%, 91.50%, 95.14%, 99.48% at 50, 60, 70, 80, 90 and 100 minutes respectively. The overall continuous effect was shown its maximum 20 minutes after taking out the needle and reached to the initial value at 60 minutes. 3. The pain inhibitory effect following the naloxone administration and acupuncture application ST-36 was 93.44%, 94.58%, 90.80%, 88.04% at 10, 20, 30 and 40 minutes respectively. All values showed similar tendency to the initial data. 4. The continuous effect of acupuncture at ST-36 after taking out the needle which was preceded by the naloxone administration was 91.26%, 91.90%, 92.06%, 93.66%, 94.12%, 93.50% at 50, 60, 70, 80, 90 and 100 minutes respectively. This trend was not a significant fluctuation. It Was concluded that effect of acupuncture at ST-36 was shown in conjunction with secretion of analgesic substance. Therefore it is implied that the acupuncture application will play a major role in treating many diseases with more revelation of scientific acupuncture mechanism. Further studies of acupuncture manipulation are needed in the future based on our study.

An Algorithim for Converting 2D Face Image into 3D Model (얼굴 2D 이미지의 3D 모델 변환 알고리즘)

  • Recently, the spread of 3D printers has been increasing the demand for 3D models. However, the creation of 3D models should have a trained specialist using specialized softwares. This paper is about an algorithm to produce a 3D model from a single sheet of two-dimensional front face photograph, so that ordinary people can easily create 3D models. The background and the foreground are separated from a photo and predetermined constant number vertices are placed on the seperated foreground 2D image at a same interval. The arranged vertex location are extended in three dimensions by using the gray level of the pixel on the vertex and the characteristics of eyebrows and nose of the nomal human face. The separating method of the foreground and the background uses the edge information of the silhouette. The AdaBoost algorithm using the Haar-like feature is also employed to find the location of the eyes and nose. The 3D models obtained by using this algorithm are good enough to use for 3D printing even though some manual treatment might be required a little bit. The algorithm will be useful for providing 3D contents in conjunction with the spread of 3D printers.

Tissue Transglutaminase Antibody and Its Association with Duodenal Biopsy in Diagnosis of Pediatric Celiac Disease

  • Purpose: This study aimed to evaluate a possible association between the anti-tissue transglutaminase antibody (anti-tTG) titer and stage of duodenal mucosal damage and assess a possible cut-off value of anti-tTG at which celiac disease (CD) may be diagnosed in children in conjunction with clinical judgment. Methods: This observational study was conducted at a gastroenterology clinic in a tertiary hospital from April 2012 to May 2013. Seventy children between 6-months and 18-years-old with suspected CD underwent celiac serology and duodenal biopsy. Statistical analyses were done using SPSS 16. Diagnostic test values were determined for comparing the anti-tTG titer with duodenal biopsy. An analysis of variance and Tukey-Kramer tests were performed for comparing the means between groups. A receiver operating characteristics curve was plotted to determine various cut-off values of anti-tTG. Results: The mean antibody titer increased with severity of Marsh staging (p<0.001). An immunoglobulin (Ig) A-tTG value at 115 AU/mL had 76% sensitivity and 100% specificity with a 100% positive predictive value (PPV) and 17% negative predictive value (NPV) for diagnosis of CD (p<0.001, 95% confidence interval [CI], 0.75-1). Conclusion: There is an association between the anti-tTG titer and stage of duodenal mucosal injury in children with CD. An anti-tTG value of 115 AU/mL (6.4 times the upper normal limit) had 76% sensitivity, 100% specificity, with a 100% PPV, and 17% NPV for diagnosing CD (95% CI, 0.75-1). This cut-off may be used in combination with clinical judgment to diagnose CD.

Azithromycin as an adjunct to subgingival professional mechanical plaque removal in the treatment of grade C periodontitis: a systematic review and meta-analysis

  • The aim of this systematic review was to evaluate clinical and microbiological outcomes with the use of azithromycin as an adjunct to non-surgical subgingival professional mechanical plaque removal (PMPR) in the treatment of grade C periodontitis. Online database searches using high-level MeSH terms in a PICO structure were conducted along with hand-searching of relevant periodontal journals. Titles and abstracts of identified studies were independently reviewed by both authors and the full texts of studies meeting the inclusion criteria were independently reviewed. In total, 122 studies were identified through searches, of which 6 were included in the qualitative analysis and 4 in the meta-analysis. Three studies included in the meta-analysis were deemed at low risk of bias and 1 at serious risk. There were conflicting results on whether azithromycin reduced the number of subgingival pathogens or detectable subgingival Aggregatibacter actinomycetemcomitans between the included studies. The meta-analysis revealed a statistically significant probing depth reduction difference in favour of azithromycin compared to the control at 3 months (weighted mean difference [WMD]=-0.39 mm; 95% confidence interval [CI], -0.66 to -0.13 mm; I2=0%) and 12 months (WMD=-1.32 mm; 95% CI, -1.71 to -0.93 mm; I2=0%). The clinical attachment level change was also statistically significant in favour of azithromycin compared to the control at 3 months (WMD=-0.61 mm; 95% CI, -1.13 to -0.10 mm; I2=71%) and 12 months (WMD=-0.88 mm; 95% CI, -1.32 to -0.44 mm; I2=0%). Based upon these results, azithromycin offers additional improvements in some clinical parameters when used in conjunction with subgingival PMPR in patients with aggressive periodontitis over control groups. These improvements appear to be maintained for up to 12 months after treatment completion. However, due to a lack of well-designed studies, the conclusions that can be drawn from the available evidence are limited.

A Study of Body Composition and Energy Intake for Intensive Training Period on Junior Rowing Team Players (조정 종목 상비군 선수의 훈련기 신체구성 및 영양섭취에 관한 연구)

  • The purpose of this study was to provide the basic information on nutritional condition for improving performance of the national junior rowing team players(male n=17, female n=14). To attain this purpose, their body composition and energy intake situations were investigated during their three-week winter training camp. Measurement of body composition measured by the interval, and two days, and dietary intake in this study to investigate the type and amount of food, the researchers measured directly, and then refer to the record subjects to investigate the dietary intake and the adoption of a Weighing Method. The result is as follows. During training, body composition due to the high intensity training to maintain a properly balanced diet is the consumption of energy, weight and body composition has a positive influence. The male players' average energy intake is 5,951.6 kcal(72.6 kcal/kgBW, 150%), the rate of the three main nutrients, carbohydrate, fat, protein, is 72.6 : 8.7 : 18.7 respectively. For the female, the average energy intake is 3,685.6 kcal(51.8 kcal/kgBW, 121%), and the rate of the three main nutrients is 76.3 : 8.5 : 15.2. Both of male and female were taken sufficient Vitamin, mineral and other nutrient according to %RI(recommended intake). Based on the results of a survey of elite athletes and nutrition management and the systematic introduction and review of the program in conjunction with a professional nutritionist would need, and can also be applied to expand the school to the scene, the reality of the program review and development of qualified need.

Drug Interactions between Cardiovascular Agents and Psychotropic Drugs (심혈관질환약물과 향정신성약물의 약물상호작용)

  • There are numerous drug interactions related to many psychotropic and cardiovascular medications. Firstly, the principles in predicting drug interactions are discussed. Cytochrome P (CYP) 450 plays a significant role in the metabolism of these drugs that are substrates, inhibitors, or inducers of CYP450 enzymes. The two most significant enzymes are CYP2D6 and CYP3A4. The ability of psychotropic drugs to act as inhibitors for the enzymes may lead to altered efficacy or toxicity of co-administered cardiovascular agents as a substrate for the enzymes. The following is also a review of the known interactions between many commonly prescribed cardiovascular agents and psychotropic drugs. Most beta blockers are metabolized by CYP2D6, which may lead to drug toxicity when they use in combination with potent CYP2D6 inhibitors including bupropion, chlorpromazine, haloperidol, selective serotonin reuptake inhibitors, and quinidine. Concomitant administration of lithium with angiotensin converting enzyme inhibitors, angiotensin receptor blockers, and diuretics may increase serum lithium concentrations and toxicity. Calcium channel blockers and cholesterol lowering agents are subject to interactions with potent inhibitors of CYP3A4, such as amiodarone, diltiazem, fluvoxamine, nefazodone, and verapamil. Prescribing antiarrhythmic drugs in conjunction with medications are known to prolong QT interval and/or inhibitors on a relevant CYP450 enzyme is generally not recommended, or needs watchful monitoring. Digoxin and warfarin also have warrant careful monitoring if co-administered with psychotropic drugs.

Associations between Vitamin D Receptor (VDR) Gene Polymorphisms and Colorectal Cancer Risk and Effect Modifications of Dietary Calcium and Vitamin D in a Japanese Population

  • Much interest has been drawn to possible associations between vitamin D receptor (VDR) gene polymorphisms and colorectal cancer risk in conjunction with potentially protective effects of calcium and vitamin D. In a study of 685 cases of colorectal cancer and 778 community controls in Japan, we examined the associations of the FokI, BsmI, ApaI, and TaqI polymorphisms with colorectal cancer risk and effect modification by dietary calcium and vitamin D. Genotypes were determined by the PCR-RFLP method. The ApaI polymorphism seemed to be associated with a decreased risk of colorectal cancer, particularly of rectal cancer. The adjusted odds ratio of colorectal cancer for the ApaI AA and Aa genotypes combined versus the aa genotype was 0.83 (95% confidence interval [CI] 0.67-1.02), and the corresponding value for rectal cancer was 0.75 (95%CI 0.56-0.99). A decreased risk of colorectal cancer for the ApaI AA and Aa genotypes combined was more evident in individuals with high calcium intake (interaction p=0.055). The FokI polymorphism seemed to be associated with a decreased risk of colon cancer among those with high vitamin D intake (interaction p=0.09). The BsmI and TaqI polymorphisms were unrelated to colorectal cancer risk, and the null associations were not modified by calcium or vitamin D intake. In conclusion, the ApaI polymorphism may be associated with a decreased risk of colorectal cancer in Japanese, dependent on dietary calcium intake.

  • Pharmacologic coronary vasodilation in conjunction with myocardial scintigraphy has become an accepted alternative to dynamic exercise testing for the diagnosis of coronary artery disease. Although dipyridamole has traditionally been used for this purpose, it causes frequent side effect, which at times can be life-threatening. Moreover, dipyridamole dose not elicit maximal coronary vasodilation in a substantial number of patients receiving the usual i.v. dose. Adenosine is an endogenously produced compound that has significant effects as a coronary vasodilator and rapid onset action and extremely short half-life (< 10 seconds). The diagnostic accuracy and safety profile of adenosine $^{99m}Tc-MIBI$ myocardial scintigraphy were evaluated and comparison with exercise $^{99m}Tc-MIBI$ was performed. Twenty-eight subjects underwent $^{99m}Tc-MIBI$ imaging after adenosine infusion and exercise $^{99m}Tc-MIBI$ imaging. Adenosine was infused intravenously at a dose of 0.14mg/kg/body weight per minute for 6 min and MIBI was injected at 3 minute. Adenosine caused an incerease in heart rate ($64{\pm}12$ at baseline versus $74{\pm}16$ beats/min at peak effect, p<0.001), a mild decrease in systolic and diastolic blood pressure and a slightly increase in PR interval(p; NS). Side effects were reported in 92% of patients and were mostly mild in nature and promptly resolved within 1 or 2 minutes of termination of adenosine infusion. Facial flushing (53%), chest pain (36%), mild dyspnea (39%), headache (21%), throat discomfort (21%) were frequent symptoms. ST segment depression (> 1 mm) and second degree AV block in electrocardiography occured in 11% of the patients, respectively. The overall sensitivity and specificity for individual coronary stenoses in 16 patients underwent coronary angiography were 88% and 95%, respectively. The agreement ratio of segmental perfusion between adenosine and exercise images was 92% (Kappa index=0.82). In conclusion, $^{99m}Tc-MIBI$ myocardial perfusion scintigraphy with intravenous adenosine is a feasible, safe and highly accurate noninvasive technique for the detection of coronary artery disease and results are at least comparable with those of exercise $^{99m}Tc-MIBI$ scintigraphy.

Comparison of Load Ratio of Load-cell type Anemometer with Windswept Shape Variation (수풍부 형상에 따른 로드 셀형 풍향풍속계의 하중 비 비교)

  • Anemometer is a meteorological instrument that measures wind direction and wind speed in real time, and is mounted to the cranes that are used at ports, shipbuilding yards, off-shore structure, or construction sites that are influenced by wind, and it is used in conjunction with the safety system. Load cell-type anemometer measures the wind direction through the ratio of load between 4 positions by mounting the thin plate to 4 load cells, and measures wind velocity through the summation of loads. According to previous research, the load ratio between two adjacent windswept with respect to the wind direction has unstable value due to vortex around windswept. This causes the result that increases an error on the wind direction. In this study we compared and analyzed the difference between the load ratio with respect to three type windswept shapes in order to suggest the proper windswept shape to reduce this error. The computational fluid flow analysis is carried out with ANSYS CFX to analyze the load ratio between three windswept shapes. Wind direction was adopted as the design variable, and selected 9 wind direction conditions from $0^{\circ}{\sim}90^{\circ}$ with $11.25^{\circ}$ interval for computational fluid flow analysis.
