서로 다른 스캔 방식을 이용하여 CAD/CAM에 의해 제작된 코핑의 변연 및 내면의 적합성 (Marginal and Internal Fit of Copings Made by CAD/CAM using Different Scanning Methods)

  • 구강내 스캔(Group 1), 모델 광학 스캔(Group 2)과 모델 접촉식 스캔(Group 3)방식으로 각각 zirconia 코핑을 제작하여 광학 현미경으로 적합성을 관찰하였다. 측정항목은 변연오차(absolute marginal discrepancy;AMD), 변연간극(marginal gap; MG), 측방간격(gap of axial wall; GA), 선각간격(gap of line angle; GL)와 교합간격(gap of occusal surface; GO)이었으며, 결과는 아래와 같았다. 1. Group 1, Group 2, Group 3의 평균 AMD는 각 각 $141.21{\pm}42.94{\mu}m$, $140.63{\pm}31.64{\mu}m$, $109.37{\pm}28.42{\mu}m$이고, Group1, Group 2, Group 3의 MG는 각 각 $82.52{\pm}43.99{\mu}m$, $90.28{\pm}27.93{\mu}m$, $66.55{\pm}28.77{\mu}m$였다. AMD와 MG는 각 그룹에서 통계학적으로 유의한 차이를 보여주지 않았다(Anova test, P>0.05). 2. Group 2의 GA가 Group 1과 Group 3에 비해 통계학적으로 유의하게 적은 수치를 나타내었다(Anova test, P<0.05). 3. Group 1의 GL과 GO가 다른 그룹에 비해 통계학적으로 높은 수치를 나타내었다(Mann-whitney test(P<0.05). 세 가지 스캔 방식으로 제작된 zirconia 코핑은 세라믹 보철의 가장 중요한 평가 요소인 AMD와 MG에서 서로 통계학적 차이를 보이지 않았다.

마이크로 웨이브 소결 과정이 CAD/CAM 지르코니아 코아의 적합도에 미치는 영향 (The Influence of Microwave Sintering Process on the Adaptation of CAD/CAM Zirconia Core)

  • 본 연구의 목적은 전통적인 소결 방법과 마이크로 웨이브 소결 방법에 의해 제작된 지르코니아 코아의 내부 및 변연 적합도를 알아보는 것이다. 본 실험은 기존의 소결로를 이용하는 $3M^{(R)}$의 Lava(LAV), $Kavo^{(R)}$사의 Everest(EVE), $Cerasys^{(R)}$ CAD/CAM 시스템(CER)과 마이크로웨이브 소결로를 이용하는 Dent. $solution^{(R)}$사의 CAD/CAM 시스템(DEN)에서 시스템별로 12개씩의 코아를 제작하고 실리콘 접착제를 이용하여 금속 다이에 부착시켰다. $Skyscan^{(R)}$ 1076 미세 단층 촬영기를 이용해 각각의 시편을 촬영한 후 재구성하였다. 각 시편에 대해 B-L상, M-D상 절단면 및 치아 장축에 대한 횡단면을 구해내고 각각의 단면에 대해 변연 간격, 축면 간격, 교합면 간격을 측정하였다. 실험에 대한 결과는 다음과 같다. 1. 변연 간격은 EVE가 $36.20{\mu}m$로 가장 작았고, LAV는 $47.67{\mu}m$, DEN는 $52.47{\mu}m$, CER은 $54.63{\mu}m$를 나타내었다. 2. 축면 간격은 EVE가 $63.49{\mu}m$로 가장 큰 값을 나타내었으나 네군간에는 유의차는 없었다. 3. 교합면 간격은 DEN이 $77.06{\mu}m$으로 가장 작은 값을 나타내었으며, EVE와 CER은 $100{\mu}m$를 넘는 값으로 다른 두군과 유의차를 보였다. 본 연구에서 마이크로웨이브 소결을 통해 제작된 CAD/CAM 지르코니아 코아는 임상적으로 허용할 만한 변연 간격 및 축면 간격, 교합면 간격을 보여 주었다.

종골 관절내 골절의 수술적 치료 후 임상 결과 -전산화 단층 촬영에 따른 분류 및 관혈적 정복 및 내고정 치료- (Result of Surgical Treatment of Intra-Articular Fractures of the Calcaneus - Based on CT Classification and Open Reduction and Internal Fixation -)

  • Purpose: To report the clinical result of the intraarticular calcaneus fracture after open reduction and internal fixation with plate by lateral approach. Materials and Methods: Thirty-six calcaneal fractures of 33 patients(29 men and 4 women) were treated by open reduction and internal fixation using an lateral approach from March, 1997 to May, 2002 and were followed more than one year. The autogenous iliac bone graft was done in 2 cases but the others didn't. Radiographically B?hler angle and Gissane angle on simple lateral radiograph were measured and in the 15 cases, the step-off(gap) of posterior facet joint on post-operative CT images were followed. The Salama method was used for evaluation of clinical results. Results: According to Sanders classification, 19 cases of the 36 cases were classified as type II. Type III fracture were found in 12 cases and type IV in 5 cases. The following results were obtained: twenty-two cases(61.1%) out of 36 cases were estimated as good or excellent. The good results or more were obtained in 15 cases(78.9%) in type II and 7 cases(58.3%) in type ill, but no case in type IV. B?hler angles were improved from preoperative average 1.6?to postoperative average 23.4?, Gissane angle was improved from preoperative 107.2?to postoperative 122.8?, respectively. Among 36 cases, Computed tomography was carried out in 15 cases. The postoperative step-off (gap) of posterior facet joint on computed tomography was filled with cancellous bone. Satifactory results was obtained in 7 cases with 2mm gap or less and in 6 cases of 2-5mm. There were no satifactory results in 2 cases with 5mm gap or more. Conclusion: Open reduction and internal fixation for intra-articular fracture of calcaneus was thought to be a good treatment modality. It is thought that the lateral approach is one of the good one for surgical treatment, and that accurate reduction of the posterior facet, acceptable recovery of B?hler angle are more important to obtain best results.

Fitting accuracy of ceramic veneered Co-Cr crowns produced by different manufacturing processes

  • PURPOSE. The purpose of this in vitro study was to evaluate the fitting accuracy of single crowns made from a novel presintered Co-Cr alloy prepared with a computer-aided design and computer-aided manufacturing (CAD/CAM) technique, as compared with crowns manufactured by other digital and the conventional casting technique. Additionally, the influence of oxide layer on the fitting accuracy of specimens was tested. MATERIALS AND METHODS. A total of 40 test specimens made from Co-Cr alloy were investigated according to the fitting accuracy using a replica technique. Four different methods processing different materials were used for the manufacture of the crown copings (milling of presintered (Ceramill Sintron-group_cer_sin) or rigid alloy (Tizian NEM-group_ti_nem), selective laser melting (Ceramill NPL-group_cer_npl), and casting (Girobond NB-group_gir_nb)). The specimens were adapted to a resin model and the outer surfaces were airborne-particle abraded with aluminum oxide. After the veneering process, the fitting accuracy (absolute marginal discrepancy and internal gap) was evaluated by the replica technique in 2 steps, before removing the oxide layer from the intaglio surface of the crowns, and after removing the layer with aluminum oxide airborne-particle abrasion. Statistical analysis was performed by multifactorial analysis of variance (ANOVA) (α=.05). RESULTS. Mean absolute marginal discrepancy ranged between 20 ㎛ (group_cer_npl for specimens of Ceramill NPL) and 43 ㎛ (group_cer_sin for crowns of Ceramill Sintron) with the oxide layer and between 19 ㎛ and 28 ㎛ without the oxide layer. The internal gap varied between 33 ㎛ (group_ti_nem for test samples of Tizian NEM) and 75 ㎛ (group_gir_nb for the base material Girobond NB) with the oxide layer and between 30 ㎛ and 76 ㎛ without the oxide layer. The absolute marginal discrepancy and the internal gap were significantly influenced by the fabrication method used (P<.05). CONCLUSION. Different manufacturing techniques had a significant influence on the fitting accuracy of single crowns made from Co-Cr alloys. However, all tested crowns showed a clinically acceptable absolute marginal discrepancy and internal gap with and without oxide layer and could be recommended under clinical considerations. Especially, the new system Ceramill Sintron showed acceptable values of fitting accuracy so it can be suggested in routine clinical work.

적층 가공방식에 따른 고정성 치과보철물의 변연 및 내면 적합도 평가연구 (Evaluation the clinical acceptability of the marginal and internal gaps of fixed partial denture fabricated with additive manufacturing technology)

  • Purpose: The purpose of this study was to evaluate the clinical acceptability of the marginal and internal gap of Co-Cr metal copings fabricated with stereolithography (SLA). Methods: Titanium master dies were milled after scanning of the prepared tooth (n=30). For group I, Co-Cr metal copings were made from conventional lost-wax technique(LWT, n=10). For group II, the master dies were scanned and designed with CAD system. Then, metal copings were milled with Co-Cr(SUB, n=10). For group III(ADD, n=10), the scanning and design procedures were same as group II and burn-out resins were fabricated with SLA device. The marginal and internal discrepancies were measured under an optical microscope(100x) on ten reference points and were statistically analyzed with one-way ANOVA(${\alpha}=.05$). Results: The mean total discrepancies were $53.76{\pm}12.42{\mu}m$ in the LWT group and $69.82{\pm}15.48{\mu}m$ in the ADD group. The SUB group showed the largest total mean value $110.33{\pm}13.77{\mu}m$. There was statistically significant difference between the SUB and the other groups(P<0.05). Conclusion : Co-Cr metal copings fabricated with SLA technology showed clinically acceptable value on marginal and internal gap and there was no statistically significant difference between conventional lost-wax technique and SLA.

Comparison of fit and trueness of zirconia crowns fabricated by different combinations of open CAD-CAM systems

  • PURPOSE. This study aims to clinically compare the fitness and trueness of zirconia crowns fabricated by different combinations of open CAD-CAM systems. MATERIALS AND METHODS. Total of 40 patients were enrolled in this study, and 9 different zirconia crowns were prepared per patient. Each crown was made through the cross-application of 3 different design software (EZIS VR, 3Shape Dental System, Exocad) with 3 different processing devices (Aegis HM, Trione Z, Motion 2). The marginal gap, absolute marginal discrepancy, internal gap(axial, line angle, occlusal) by a silicone replica technique were measured to compare the fit of the crown. The scanned inner and outer surfaces of the crowns were compared to CAD data using 3D metrology software to evaluate trueness. RESULTS. There were significant differences in the marginal gap, absolute marginal discrepancy, axial and line angle internal gap among the groups (P < .05) in the comparison of fit. There was no statistically significant difference among the groups in terms of occlusal internal gap. The trueness ranged from 36.19 to 43.78 ㎛ but there was no statistically significant difference within the groups (P > .05). CONCLUSION. All 9 groups showed clinically acceptable level of marginal gaps ranging from 74.26 to 112.20 ㎛ in terms of fit comparison. In the comparison of trueness, no significant difference within each group was spotted. Within the limitation of this study, open CAD-CAM systems used in this study can be assembled properly to fabricate zirconia crown.

Gap comparison between single crown and three-unit bridge zirconia substructures

  • PURPOSE. To compare marginal and internal gaps of zirconia substructure of single crowns with those of three-unit fixed dental prostheses. MATERIALS AND METHODS. Standardized Co-Cr alloy simulated second premolar and second molar abutments were fabricated and subsequently duplicated into type-III dental stone for working casts. After that, all zirconia substructures were made using $Lava^{TM}$ system. Marginal and internal gaps were measured in 2 planes (mesial-distal plane and buccal-palatal plane) at 5 locations: marginal opening (MO), chamfer area (CA), axial wall (AW), cusp tip (CT) and mid-occlusal (OA) using Replica technique. RESULTS. There were significant differences between gaps at all locations. The $mean{\pm}SD$ of marginal gap in premolar was $43.6{\pm}0.4{\mu}m$ and $46.5{\pm}0.5{\mu}m$ for single crown and 3-unit bridge substructure respectively. For molar substructure the $mean{\pm}SD$ of marginal gap was $48.5{\pm}0.4{\mu}m$ and $52.6{\pm}0.4{\mu}m$ for single crown and 3-unit bridge respectively. The largest gaps were found at the occlusal area, which was $150.5{\pm}0.5{\mu}m$ and $154.5{\pm}0.4{\mu}m$ for single and 3-unit bridge premolar substructures respectively and $146.5{\pm}0.4{\mu}m$ and $211.5{\pm}0.4{\mu}m$ for single and 3-unit bridge molar substructure respectively. CONCLUSION. Independent-samples t-test showed significant differences of gap in zirconia substructure between single crowns and three-unit bridge (P<.001). Therefore, the span length has the effect on the fit of zirconia substructure that is fabricated using CAD/CAM technique especially at the occlusal area.

Evaluation of marginal and internal gaps in single and three-unit metal frameworks made by micro-stereolithography

  • PURPOSE. The purpose of this study is to compare single and three-unit metal frameworks that are produced by micro-stereolithography. MATERIALS AND METHODS. Silicone impressions of a selected molar and a premolar were used to make master abutments that were scanned into a stereolithography file. The file was processed with computer aided design software to create single and three-unit designs from which resin frameworks were created using micro-stereolithography. These resin frameworks were subjected to investment, burnout, and casting to fabricate single and three-unit metal ones that were measured under a digital microscope by using the silicone replica technique. The measurements were verified by means of the Mann-Whitney U test (${\alpha}=.05$). RESULTS. The marginal gap was $101.9{\pm}53.4{\mu}m$ for SM group and $104.3{\pm}62.9{\mu}m$ for TUM group. The measurement of non-pontics in a single metal framework was $93.6{\pm}43.9{\mu}m$, and that of non-pontics in a three-unit metal framework was $64.9{\pm}46.5{\mu}m$. The dimension of pontics in a single metal framework was $110.2{\pm}61.4{\mu}m$, and that of pontics in a three-unit metal framework was $143.7{\pm}51.8{\mu}m$. CONCLUSION. The marginal gap was smaller for the single metal framework than for the three-unit one, which requires further improvement before it can be used for clinical purposes.

스파크 점화 시스템의 방전 거동 및 특성에 관한 연구 (A Study on Behaviour and Characteristics of Spark Discharge in Spark Ignition System)

  • Time-resolved current and voltage measurements for an inductive automotive spark system were made. Also presented are measurements of the total energy delivered to the spark gap. The measurements were made in air for a range of pressures from 1-18atm, at ambient temperatures. The measured voltage and current characteristics were found to be a function of many ignition parameters; some of these include: spark gap distance, internal resistance of the spark plug and high tension wire, and pressure. The voltages presented were measured either at the top of the spark plug or at the spark gap. The measurements were made at different time resolutions to more accurately resolve the voltage and current behavior throughout the discharge process. This was necessary because the breakdown event occurs on a time scale much shorter than the arc and glow phases. The breakdown, are, and glow voltages were found to be functions of spark plug resistance, gas density, and spark plug gap as expected from the literature. Spark duration was found to decrease as either pressure or gap was increased. The transition from the arc to glow phase is usually distinguished by a sudden rise in the voltage across the gap. At pressures above about 7atm this transition was not observed suggesting that a glow phase was not present. Energy delivered to the gap increased with increasing pressure. The effective resistance of the spark gap during discharge was about twice as large for the glow phase as the arc phase.

간호서비스 질, 만족 및 병원 재이용 의도에 관한 소비자와 제공자간의 지각차이 비교 (A Comparative Study of Consumers and Providers on Nursing Service Quality, Satisfaction, and Hospital Revisiting Intent)

  • Purpose: The purposes of this study were to compare the nursing service quality, gap, overall nursing service satisfaction, overall medical service satisfaction and intent to revisit the hospital perceived by consumers and providers. Methods: Data were collected with self-administered questionnaire and analyzed using frequency, %, mean, standard deviation, t-test, Pearson correlation coefficient, and multiple regression analysis. Result: Nursing service gap perceived by consumers was smaller than that of providers. Consumer's overall satisfaction with nursing and medical service was higher than that of nurses. In consumers, nursing service satisfaction alone accounted for 62.9% of the variance in their intent to revisit the hospital, while explained 3% of the variance in providers. Conclusion: There are definitely perception gap between consumers and providers. Therefore nursing and hospital managers must recognize it, and carry out the internal marketing strategies for nurses.
