Understanding the Treatment Strategies of Intracranial Germ Cell Tumors : Focusing on Radiotherapy

  • Intracranial germ cell tumors (ICGCT) occur in 2-11% of children with brain tumors between 0-19 years of age. For treatment of germinoma, relatively low radiation doses with or without chemotherapy show excellent 10 year survival rate of 80-100%. Past studies showed that neoadjuvant chemotherapy combined with focal radiotherapy resulted in unacceptably high rates of periventricular tumor recurrence. The use of generous radiation volume which covers the whole ventricular space with later boost treatment to primary site is considered as standard treatment of intracranial germinomas. For non-germinomatous germ cell tumors (NGGCT), 10-year overall survival rate is still much inferior than that of intracranial germinoma despite intensive chemotherapy and high-dose radiotherapy. Craniospinal radiotherapy combined with cisplatin-based chemotherapy provides the best treatment outcome for NGGCT; 60-70% of overall survival rate. There is a debate on the surgical role whether surgery can contribute to improved treatment outcome of NGGCT when added to combined chemoradiotherapy. Because higher dose of radiotherapy is required for treatment of NGGCT than for germinoma, it is tested whether whole ventricular irradiation can replace craniospinal irradiation in intermediate risk group of NGGCT to minimize radiation-related late toxicity in the recent studies. To minimize the treatment-related neural deficit and late sequelae while maintaining long-term survival rate of ICGCT patients, optimized administration of chemotherapy and radiotherapy should be selected. Use of technically upgraded radiotherapy modalities such as intensity-modulated radiotherapy or proton beam therapy is expected to bring an improved neurocognitive outcome with longitudinal assessment of the patients.

Effect of Medicinal Plant By-products Supplementation to Total Mixed Ration on Growth Performance, Carcass Characteristics and Economic Efficacy in the Late Fattening Period of Hanwoo Steers

  • This study was conducted to evaluate the effect of medicinal plant by-products (MPB) supplementation to a total mixed ration (TMR) on growth, carcass characteristics and economic efficacy in the late fattening period of Hanwoo steers. Twenty seven steers (body weight [BW], $573{\pm}57kg$) were assigned to 3 treatment groups so that each treatment based on BW contained 9 animals. All groups received ad libitum TMR throughout the feeding trial until slaughter (from 24 to 30 months of age) and treatments were as follows: control, 1,000 g/kg TMR; treatment 1 (T1), 970 g/kg TMR and 30 g/kg MPB; treatment 2 (T2), 950 g/kg TMR and 50 g/kg MPB. Initial and final BW were not different among treatments. Resultant data were analyzed using general linear models of SAS. Average daily gain and feed efficiency were higher (p<0.05) for T1 than control, but there was no difference between control and T2. Plasma albumin showed low-, intermediate- and high-level (p<0.05) for control, T1 and T2, whereas non-esterified fatty acid was high-, intermediate- and high-level (p<0.05) for control, T1 and T2, respectively. Carcass weight, carcass rate, backfat thickness and rib eye muscle area were not affected by MPB supplementation, whereas quality and yield grades were highest (p<0.05) for T1 and T2, respectively. Daily feed costs were decreased by 0.5% and 0.8% and carcass prices were increased by 18.1% and 7.6% for T1 and T2 compared to control, resulting from substituting TMR with 30 and 50 g/kg MPB, respectively. In conclusion, the substituting TMR by 30 g/kg MPB may be a potential feed supplement approach to improve economic efficacy in the late fattening period of Hanwoo steers.

수도작에 있어서 물관리에 대한 연구 (The Study on the Irrigation Water Control in the Cultivation of Rice Plants)

  • More stable and higher yields in rice paddy depend mainly upon an adequately balanced supply of water for higher yield. Rice paddy is supplied naturally by rainfall but inevitably requires artificial supplenental irrigation for higher yields. Even though it may be true that the water requirement of rice plants is generally higher than those of other crops, the submerged condition is not necessarily required for rice. The moisture requirements of rice vary according to its growing stages and it is possible to increse the irrigation efficiency by means of lessening the loss due to percolation and evapolation in the field. This experiment was conducted on the effect of the various amount of water supply and different irrigation periods for yield and yield components, and was carried out to find out the most suitable irrigation method and to increase the irrigation efficiency for higher yields in rice paddy. Randomized block design with 3 replications was employed where the 3 levels of the amount of irrigation water; (120% moisture contents), unirrigated (90~100%) and more un irrigated candition (80~90% moisture content), and levels of the various irrigation periods; usual, initial, intermediate and final periods, being treated. The results obtained in this experiment are as follows: 1. As for the physical and chemical and soil properfies, and other characteristics, there are no differences among the treatments enough to be effective for the growth of rice plants. 2. Culm length was measured after harvest as shown in table 2. 3. Difference of the amount of irrigation water did not change the culm length and ear length, however it also indicated more apparent increase in final treatment plots thatn that of usual. 3. No difference in the number of ears and number of ears pers per hill was founded treatments both in the difference of water supply and in the various irrigation periods. 4. There is no difference in the maturing rate and 1000 grains weight. 5. The number of panicles and grains and more increased in 80~100% moisture contents soil than those of 120%. and it shows in un irrigated plots, more irrigated plots and control plots in turn. Other wise according to the period\ulcorner of irrigation the trend is appeared in turn initial, usual, final and intermediate treatments.6. Yield as shown in table 7. 8 was more increased in unirrigated plots(90~IOO% moisture content) than the control plots (120% moisture content) by up to 8.2% and also 3. 2% in more unirrigated plots than that of control by periods is shown: usual plots final, initial, inter mediate treatment plots in turn. 7. The above resutts lead to the conclusion that no remarkable, differences in yields and soil properties are made by the unirrigation. However, it is apparent that this treatment has .some advantages in the points that one could spare the amount of water supply for irrigation with more increase in yield. In addition, a higher temperature and a brisk oxygen supply would be possible throug h this treatments. Accordingly, these treatment would be a more reasonable and economical cultivation method of rice for the better harvest.

Molecular Cloning and Characterization of a P38-Like Mitogen-Activated Protein Kinase from Echinococcus granulosus

  • Cystic echinococcosis (CE) treatment urgently requires a novel drug. The p38 mitogen-activated protein kinases (MAPKs) are a family of Ser/Thr protein kinases, but still have to be characterized in Echinococcus granulosus. We identified a 1,107 bp cDNA encoding a 368 amino acid MAPK protein (Egp38) in E. granulosus. Egp38 exhibits 2 distinguishing features of p38-like kinases: a highly conserved T-X-Y motif and an activation loop segment. Structural homology modeling indicated a conserved structure among Egp38, EmMPK2, and H. sapiens $p38{\alpha}$, implying a common binding mechanism for the ligand domain and downstream signal transduction processing similar to that described for $p38{\alpha}$. Egp38 and its phosphorylated form are expressed in the E. granulosus larval stages vesicle and protoscolices during intermediate host infection of an intermediate host. Treatment of in vitro cultivated protoscolices with the p38-MAPK inhibitor ML3403 effectively suppressed Egp38 activity and led to significant protoscolices death within 5 days. Treatment of in vitro-cultivated protoscolices with $TGF-{\beta}1$ effectively induced Egp38 phosphorylation. In summary, the MAPK, Egp38, was identified in E. granulosus, as an anti-CE drug target and participates in the interplay between the host and E. granulosus via human $TGF-{\beta}1$.

세 가지 환경구배에 따른 신갈나무의 생태적 지위폭과 상수리나무, 굴참나무와의 생태적 중복역 (Ecological Niche Breadth of Q. mongolica and Overlap with Q. acutissima and Q. variabilis along with Three Environment Gradients)

  • 신갈나무의 생태적 특성을 밝히기 위하여 유식물의 주요 환경요인 중 광, 수분과 영양소를 각각 4구배로 처리하고, 2007년 3월~10월까지 양묘하여 17가지의 형태 및 생태형질을 측정하여 생태적 지위폭을 계산하였다. 그리고 이 지위폭과 상수리나무와 굴참나무와의 생태적 중복역을 비교분석하여 3종의 분포적 특성과 경쟁관계를 생태적으로 해석하고자 하였다. 신갈나무의 생태적 지위폭은 환경요인 중 영양소구배에서 가장 낮았고, 수분구배에서 가장 높게 나타났다. 광구배에서는 그 중간이었다. 신갈나무는 광환경에 대해서만 상수리나무나 굴참나무보다 생태적 지위폭이 큼으로써 이는 3종의 참나무가 경쟁시 광환경에서는 신갈나무가 우월하며, 또한 한반도에서 신갈나무가 우점하는 원인 중의 하나가 광환경에 대한 높은 적응력일 수 있음을 의미하는 것이다. 신갈나무-상수리나무와 신갈나무-굴참나무의 생태적 중복역은 수분구배에서 가장 넓었고, 영양소구배에서 가장 좁게 나타났다. 광구배에서는 그 중간이었다. 이는 참나무 3종이 영양소환경이나 광환경에 대해서는 서로 간 생태적 분화가 수분 환경보다 상대적으로 많이 일어났음을 뜻한다. 유집분석과 주성분분석을 이용한 참나무 3종의 환경구배에 대한 반응에서는 신갈나무-상수리나무 간의 공간적 위치는 신갈나무-굴참나무 간의 위치보다 서로 가깝게 배열되어 나타났다. 이상의 결과는 신갈나무와 상수리나무는 생태적 지위가 유사하여 환경요인, 특히 수분환경에 대한 경쟁이 신갈나무와 굴참나무에 비해 더 심하게 일어남을 의미한다.

A Modified eCura System to Stratify the Risk of Lymph Node Metastasis in Undifferentiated-Type Early Gastric Cancer After Endoscopic Resection

  • Purpose: The original eCura system was designed to stratify the risk of lymph node metastasis (LNM) after endoscopic resection (ER) in patients with early gastric cancer (EGC). We assessed the effectiveness of a modified eCura system for reflecting the characteristics of undifferentiated-type (UD)-EGC. Materials and Methods: Six hundred thirty-four patients who underwent non-curative ER for UD-EGC and received either additional surgery (radical surgery group; n=270) or no further treatment (no additional treatment group; n=364) from 18 institutions between 2005 and 2015 were retrospectively included in this study. The eCuraU system assigned 1 point each for tumors >20 mm in size, ulceration, positive vertical margin, and submucosal invasion <500 ㎛; 2 points for submucosal invasion ≥500 ㎛; and 3 points for lymphovascular invasion. Results: LNM rates in the radical surgery group were 1.1%, 5.4%, and 13.3% for the low-(0-1 point), intermediate- (2-3 points), and high-risk (4-8 points), respectively (P-fortrend<0.001). The eCuraU system showed a significantly higher probability of identifying patients with LNM as high-risk than the eCura system (66.7% vs. 22.2%; McNemar P<0.001). In the no additional treatment group, overall survival (93.4%, 87.2%, and 67.6% at 5 years) and cancer-specific survival (99.6%, 98.9%, and 92.9% at 5 years) differed significantly among the low-, intermediate-, and high-risk categories, respectively (both P<0.001). In the high-risk category, surgery outperformed no treatment in terms of overall mortality (hazard ratio, 3.26; P=0.015). Conclusions: The eCuraU system stratified the risk of LNM in patients with UD-EGC after ER. It is strongly recommended that high-risk patients undergo additional surgery.

The Expression of Adipogenic Genes in Adipose Tissues of Feedlot Steers Fed Supplementary Palm Oil or Soybean Oil

  • We hypothesized that supplementing finishing diets with palm oil would promote adipogenic gene expression and stearoyl-CoA desaturase (SCD) gene expression in subcutaneous (s.c.) and intramuscular (i.m.) adipose tissues of feedlot steers. Eighteen Angus and Angus crossbred steers were assigned to three groups of 6 steers and fed a basal diet (control), with 3% palm oil, or with 3% soybean oil, for 70 d, top-dressed daily. Tailhead s.c. adipose tissue was obtained by biopsy at 14 d before the initiation of dietary treatments and at 35 d of dietary treatments. At slaughter, after 70 d of dietary treatment, tailhead s.c. adipose tissue and i.m. adipose tissue were obtained from the longissimus thoracis muscle. Palm oil increased plasma palmitic acid and soybean oil increased plasma linoleic acid and ${\alpha}$-linolenic acid relative to the initial sampling time. Expression of AMP-activated protein kinase alpha ($AMPK{\alpha}$) and peroxisome proliferator-activated receptor gamma ($PPAR{\gamma}$) increased between the initial and intermediate biopsies and declined thereafter (p<0.03). SCD gene expression did not change between the initial and intermediate biopsies but declined by over 75% by the final period (p = 0.04), and G-coupled protein receptor 43 (GPR43) gene expression was unaffected by diet or time on trial. Soybean oil decreased (p = 0.01) $PPAR{\gamma}$ gene expression at the intermediate sample time. At the terminal sample time, $PPAR{\gamma}$ and SCD gene expression was less in i.m. adipose tissue than in s.c. adipose tissue (p<0.05). $AMPK{\alpha}$ gene expression was less in s.c. adipose tissue of palm oil-fed steers than in control steers (p = 0.04) and CCAAT enhancer binding protein-beta ($CEBP{\beta}$) gene expression was less in s.c. and i.m. adipose tissues of palm oil-fed steers than in soybean oil-fed steers (p<0.03). Soybean oil decreased SCD gene expression in s.c. adipose tissue (p = 0.05); SCD gene expression in palm oil-fed steers was intermediate between control and soybean oil-fed steers. Contrary to our original hypothesis, palm oil did not promote adipogenic gene expression in s.c. and i.m. adipose tissue.

Health-Related Quality of Life of Patients with Intermediate Hepatocellular Carcinoma after Liver Resection or Transcatheter Arterial Chemoembolization

  • Background: The aim of our present study was to compare quality of life (QoL) between intermediate-stage (BCLC-B) HCC patients who had undergone either liver resection or transcatheter arterial chemoembolization (TACE). Materials and Methods: A total of 102 intermediate-stage HCC patients participated in our study, including 58 who had undergone liver resection and 44 who had undergone TACE. Baseline demographic characteristics, tumor characteristics, and long-term outcomes, such as tumor recurrence, were compared and analyzed. QoL was assessed using the Short Form (SF)-36 health survey questionnaire with the mental and physical component scales (SF-36 MCS and PCS). This questionnaire was filled out at HCC diagnosis and 1, 3, 6, 12, 24 months after surgery. Results: For the preoperative QoL evaluation, the 8 domains related to QoL were comparable between the two groups. The PCS and MCS scores were significantly decreased in both the TACE and resection groups at1 month after surgery, and this decrease was greater in the resection group. These scores were significantly lower in the resection group compared with the TACE group (P<0.05). However, these differences disappeared at 3 and 6 months following surgery. One year after surgery, the resection group showed much higher PCS scores than the TACE patients (P=0.018), and at 2 years after surgery, the PCS and MCS scores for the resection group were significantly higher than those for the TACE group (P<0.05). Eleven patients (19.0%) in the resection group and 17 (38.6%) in the TACE group suffered HCC recurrence (P<0.05). Univariate and multivariate analyses indicated that tumor recurrence (HR=1.211, 95%CI: 1.086-1.415, P=0.012) was a significant risk factor for poorpostoperative QoL in the HCC patients.Conclusions: Due to its effects on reducing HCC recurrence and improving long-term QoL, liver resection should be the first choice for the treatment of patients with intermediate-stage HCC.

자외선(紫外線)-B 증가조사(增加照射)에 대한 양수(陽樹)와 중성수(中性樹) 유묘(幼苗)의 생장(生長)과 생리적(生理的) 반응(反應)에 관한 연구(硏究) (Growth and Physiological Responses of Shade Intolerant and Intermediate Tree Seedlings to Enhanced UV-B Radiation)

  • 본 연구는 증가되는 UV-B 환경에 대한 양수(陽樹)와 중성수(中性樹)의 생장 및 생리적 반응을 탐구하고자 자작나무(Betula platyphylla var. japonica Hara)와 물푸레나무(Fraxinus rhynchophylla Hance)의 유묘(幼苗)를 대상으로 14주 동안 3 수준의 UV-B(ambient UV-$B_{BE}$, ambient+3.2, +$5.2\;KJ\;m^{-2}s^{-1}$) lamp가 설치된 야외포장에서 실시되었다. 자작나무와 물푸레나무 유묘의 수고생장(樹高生長), 근원경생장(根元徑生長), 엽면적(葉面積) 및 건물생산(乾物生産)은 UV-B 증가에 의하여 감소되었다. UV-B 증가에 의해 물푸레나무 유묘 잎의 기공저항(氣孔低抗)은 커졌으며, 엽록소(葉綠素), carotenoid 함량의 감소는 자작나무에서 보다 물푸레나무에서 더 뚜렷하였다. UV-B 흡수물질(吸收物質)의 현저한 증가는 나타나지 않았지만 자작나무에 비해 물푸레나무의 경우 다소 증가하는 경향이었다. 이러한 결과들은 UV-B 증가 환경에 대한 공시 두 수종의 생리적(生理的), 생화학적(生化學的) 반응이 다르다는 것을 의미한다.

가공적성 향상을 위한 다시마 조직의 연화에 관한 연구 (Softening Studies of Raw Sea Tangle Texture for Improvement of Its Processing Compatibility)

  • 본 연구는 다시마 가공식품과 증간소재를 개발하기 위하여 다시마의 연화방법을 검토하였다. 그 결과 가열하면서 sodium triphospto를 0.3%첨가한 것이 연화에 가장 좋은 효과를 나타내었으며 이 때 다시마의 유연성과 퍼짐성도 좋은 것으로 나타났다. 다시마의 블랜칭은 microwave를 사용하는 것이 연화에 좋았으며, 그 정도는 microwave>스팀>끓는 물 순이었다. 또한 블랜칭할 경우 sodium triphosphate를 0.3% 첨가하면 연화에 시너지효과가 나타났다. 색깔의 변화는 p<0.05 수준에서 블랜칭 방법과 연화제 첨가량에 대한 유의성은 확인되었다.
