A Study on the Evaluation and Improvement of an Integrated Health Promotion Program in the Local Community (지역사회 통합건강증진사업 평가와 개선방안)

  • The purpose of this study was to investigate the effectiveness of public health center's Integrated Health Promotion Program for the public officials who provide health promotion services to local residents. The survey was conducted from September 10 to October 5, 2015 using a structured questionnaire. Each of the 254 public health centers were sent two questionnaires but I only received 348 and 339 were used for the final analysis. As a result of this study, the all subjects were showed the average level of efficiency, autonomy, accountability, and the support of the Ministry of Health and Welfare, and the related organizations. Also, the evaluation of all the variables were positive when there was a dedicated department for integrated health promotion program within the health center and when the proportion of essential program was high. However, after the Integrated Health Promotion Program was included in the Health centers, the workload has increased and the implementation of the essential program does not fully reflect the regional health problems or local conditions. Based on these results, it is necessary to find a plan for the successful operation of the Integrated Health Promotion Program.

Pilot Study Results of a Longitudinal Integrated Clerkship in a Single Institution (일개 의과대학 장기추적통합임상실습 시범과정 운영경험과 고찰)

  • The longitudinal integrated clerkship is an innovative approach in medical education that emphasizes continuity as a key principle to provide student-centered experiences and learning. Seoul National University College of Medicine decided to adapt longitudinal integrated clerkships to the new curriculum beginning in 2018, and therefore conducted pilot studies in 2016 and 2017. This study aimed to analyze the program evaluation results of the two pilot programs and discuss issues related to the successful implementation of longitudinal integrated clerkships in Korea. We conducted a focus group interview with nine students who participated in the 2016 pilot program and 13 students who participated in the 2017 pilot program. We also conducted a focus group interview with 11 faculty members who either participated in the pilot program or will participate in the main program. From the 2016 experience, we found that it is not appropriate to assign each patient to a single student and let the students contact their patients personally for feasibility and safety reasons. In the 2017 pilot program, we assigned each patient to a group of students, which made it more feasible for the students to follow-up with their patients. The students were satisfied with their new experience of longitudinal patient follow-up and regular meetings in the pilot program. Faculty emphasized the importance of establishing the course objectives and holding an orientation for the students and the faculty. Further study is planned to evaluate the early outcomes of the main longitudinal integrated clerkship.

A Community-Based Integrated Preventive Program of Depression and Its Effectiveness in Caring for Vulnerable Elderly (취약계층 노인의 우울예방을 위한 지역사회기반의 통합프로그램 개발 및 효과검증)

  • The purpose of the study was to develop an integrated prevention program to strengthen elders self-care capability and to examine its effectiveness on their psychological condition. This study used one group pre- and post-test design. Subjects were 85 elderly residents (over 65 years of age) who lived alone, and received free basic medical care and social welfare services in a rural community in Korea. Subject eligibility criteria for this study were to an elders who 1) is not currently taking any anti-depressant medication 2) is able to communicate, and 3) agrees to participate in this study. The integrated program was composed of horticulture, reminiscence, and friendship activities. Twelve sessions were provided for 12 weeks in community-based partnerships to achieve better outcomes. The intervention was case-managed by a public health nurse and aided by six volunteers. The main outcome variable was depression, which was assessed by using 15 items selected from the Geriatric Depression Scale-short form Korean version. Socio-demographic characteristics, functional status, and satisfaction with social support were used as covariates. Results showed that there was a significant intervention effect at post-intervention time point compared to pre-intervention time point(E.S. 0.94). Multiple linear regression analysis showed significant interaction effects between intervention and satisfaction with social support. These findings must be interpreted within the context that an effects of an integrated program could be more synergistically increased when social support factor is considered in the program. A community-based integrated prevention program of depression is effective for vulnerable rural elderly. It is suggested that randomized controlled trials within community setting for better methodological strength as well as multi-level outcomes on community need to be conducted in future.

The Effect of an Integrated Health-Promotion Program in Middle-Aged Women (중년여성의 통합적 건강증진 프로그램 적용의 효과)

  • Purpose: The purpose of this study was to test the effect of an integrated health promotion program in middle-aged women. Method: The research design was a quasi experimental, one-group pretest-posttest design. Data was collected from July 1st to August 31st, 2003. One group consisted of 30 subjects. The instruments used for the study were the Self Efficacy Scale and the Health Promotion Behavior Scale developed by Park(1995), Subjective health status developed Speake(1989) and menopause-related symptoms developed by Park(1995). The data was analyzed using the SPSS WIN 10.0 program. In order to compare the effects of the pre-post synthesized health promotion program, the ed t-test was employed. Result: There was not a significant increase in subjective health status after the integrated health promotion program, nor was there a significant decrease in menopause-related symptoms after the synthesized health promotion program. There was a significant increase in self efficacy after the integrated health promotion program(p=.029), and there was also a significant increase in health promotion behavior after the integrated health promotion program(p=.006). Conclusions: Through an 8 week education program for health promotion, self efficacy and health promotion behavior were effectively changed in middle-aged women.

Effect of an Integrated Breast Health Program for Pregnant Women on Knowledge, Attitude, and Early Screening Practice Related to Breast Cancer

  • Purpose: Breast cancer is becoming increasingly prevalent among young Korean women. During pregnancy, women's concern regarding their breasts heightens. Thus, pregnancy provides a window of opportunity for breast cancer prevention and management along with antenatal care. This study developed and evaluated an integrated breast health program for pregnant women. Methods: This study employed a non-equivalent control group and non-synchronized design (22 experimental, 29 control). Women pregnant for over 28 weeks participated. The two-session integrated breast health program focused on breast management during breastfeeding and education about breast cancer prevention and early screening. Results: During the early postpartum period (within three months after the program), there were statistically significant differences in knowledge and attitude about breast cancer and breast self-examination before and after the program. There were also statistically significant differences in BSE at 6 and 12 months after the program and mammography at 12 months after the program. However, there were no statistically significant differences in clinical breast examination and breast ultrasonography at 6 and 12 months after the program. Conclusion: The integrated breast health management program was effective in increasing knowledge and improving attitudes regarding breast cancer, BSE, and early screening practices among pregnant women. Further studies should consider providing breast health programs differently for each phase of pregnancy and continuing the same after delivery.

The Effects of STEAM-Based Integrated Subject Study on Elementary School Students' Creative Personality (STEAM 기반 통합교과 학습이 초등학생의 창의적 인성에 미치는 영향)

  • The purpose of this study is to improve creative personality of elementary school students with integrated subject study program based STEAM education. On this study, I analyzed integrated subject study and STEAM, developed contents using educational robot applied integrated subject study program model. And analyzed result after applied to elementary school students. After dividing two groups-experimental group, comparison group-for this study, tested t-test. Finally I got a meaningful result statistically. There was an postive effect to improving elementary school students' creative personality by appling STEAM-based integrated subject study program.

Effects of an Integrated Health Improvement Program Based on Primary Health Care Posts on Depression, Social Support and Quality of Life during the COVID-19 Pandemic (코로나-19 동안 보건진료소에서의 통합형 건강증진 프로그램의 우울, 사회적 지지, 삶의 질에 대한 효과)

  • Purpose: This study was conducted to identify the effects of an integrated health improvement program for the elderly based on primary health care posts during the COVID-19 pandemic. Methods: A single group pretest-posttest design (n=16) was employed to evaluate an integrated health improvement program that took place twice a week for 11 weeks. The program was conducted in a small group of no more than 10 people in compliance with the guidelines to prevent COVID-19 infection. The program consisted of various contents such as making letters using blocks, health education, and talking about one's memories. The data were analyzed using the Wilcoxon signed rank test. Results: Following the program, social support (Z=-3.50, p<.001) and quality of life of the elderly (Z=-3.74, p<.001) were significantly improved among depression, social support and quality of life. Conclusion: The integrated health improvement program based on primary health care posts, considered in this study, was useful to improve the social support and quality of life of the elderly in the community, and needs to be applied to a larger number of elderly people throughout the community.

Development of Integrated Science and Art Teaching-Learning Programs for the Improvement of Creative Brain Activity of Scientifically Gifted Elementary School Student (초등과학영재의 창의적 두뇌 활성화를 위한 과학과 미술 통합 교수-학습 프로그램 개발)

  • The purpose of this study was to develop science and art integrated program to improve the creativity of scientifically gifted elementary school students. This study was to develop science and art integrated program to enhance the creativity of these subjects. This program was consisted of 30 lessons covering 10 topics. It was developed of five stages including the observation stage reflecting the characteristics of the right hemisphere relevant to creativity, the interest and curiosity stage, the experiment design and performing stage, the internalization stage, and the stage of expressing arts. This program was applied to 20 senior gifted students in Y Elementary School in Gyeonggi province. Torrance Tests of Creative Thinking(TTCT) was used in order to investigate and measure the effectiveness of the program before and after its use in class. The results of this study are as follows: First, this program showed results of significant improvement of creativity of scientifically gifted elementary school students after its use in class(p<.05). Second, it was significantly effective in increasing their creativity, especially in the subdomains such as originality, abstractness of title, and territory of resistance on hasty conclusions after its use in class(p<.05). Third, it was significantly effective to increase the Creativity Index that represents creative potential(p<.01). In particular, emotional expression, internalized visualization, unique visualization, and richness of the imagery emerged. This study implies that the science and art integrated program was closely related to the right hemisphere of the features enabling the subjects to create new ideas, new things, and new reactions. In addition, this program is expected to contribute to activate the brain areas of creativity for gifted students in the science field.

Future Directions of the Integrated Health Promotion Programs in Health Center (보건소 통합건강증진사업의 발전 방향)

  • Objectives: This paper reviews the integrated health promotion program in health center to analyse the problems and to provide the future directions aiming to health promotion block grants. Methods: The guidebook for the integrated health promotion program published by the Minister of Health and Welfare were reviewed in this study. And also the empirical results and theoretical considerations were used in making future directions. Results and Conclusion: This paper suggested four improvement directions as follows; first, the legal review committee is needed to approve when the central government change the way of health promotion program in health center. Second, the required programs should be replaced by an integrated outcom evaluation indicators. Third, the central government should strengthen the capacity of local government. And finally, visiting training should be conducted each health center.

The Effects of an Integrated Management Program on Climacteric Symptoms and Depression in Middle-aged Women (통합 관리프로그램이 중년기 여성의 갱년기 증상과 우울에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of an integrated management program on climacteric symptoms and depression in middle-aged women. Methods: A nonequivalent control-group design was used. The subjects consisted of 65 middle-aged women with climacteric symptoms. The experimental group (n=33) participated in the program for six 60-min sessions. To measure the climacteric symptoms and depression level of the subjects, two self-reporting questionnaires were used. Data were analyzed using SPSS Statistics 17.0 program. Results: The women in the experimental group reported the average score of 13.80 in climacteric symptoms, 14.38 in depression. The hypothesis that the climacteric symptoms of experimental group would be reduced more than that of the control group was supported (t=3.92, p<.001): physical symptoms (t=4.04, p<.001); psychological symptoms (t=1.80, p=.077); sexual symptoms (t=2.85, p=.006). The second hypothesis that the degree of depression of the experimental group who used the integrated climacteric symptoms management program would be reduced more than that of the control group was also supported (t=2.30, p=.02). Conclusion: The integrated management program for middle-aged women with climacteric symptoms was effective in reducing their climacteric symptoms and depression level.
