Evaluation on Utilization of the Health Care Service in One Urban Area in Korea (일개지역의 보건의료서비스 이용 평가;Y지역의 대학병원과 보건소 데이터베이스를 통하여)

  • Purpose: This study was to evaluate the utilization of health care service and to provide supportive data for health care policy making in one urban area in Korea. Method: This study tested the significance of public health service using the database of an university hospital and public health center from Feb. 2000 to Dec. 2004. Data were analyzed by multidimensional analysis and data mining technique and produced the information on the classification of utilization characteristics by main disease and the total cost of use and disease association with the users of the public health center. Results: The Results were as follows: 1) Top 10 diseases in the area accounted for 22.4% of total frequency for the most recent 5 years in university hospital, while 59.0% in public health center. 2) There were significant correlations between university hospital and public health center user's insurance type and place of residence: It showed higher use of public health center for free service beneficiaries residing in Seoul than residents in nearby or local area. The medical insurance types for hospital users were more various than those for public health center users. 3) The use of hospital for patients of hypertension, diabetes mellitus and hyperlipidemia was tended to concentrate in mostly autumn and winter since August 2000, while the cost of using public health center for those patients has been steadily reduced since July 2000. 4) As a result of cluster analysis, there were classified into three homogeneous groups according to the total cost of using public health service, age, and the frequency of use. 5) The association analysis on patients with chronic disease in public health center produced a detailed information on accompanying diseases related to the incidence rate of disease of high frequency due to aging, information on drug abuse and immune disease. Conclusion: The health care policy for local community should be evaluated continuously. And the policy to build an integrated data warehousing by public health indicator system and to enhance the faithfulness of data is required.

Potentials of Chenpi on Metabolic Syndrome: A Review

  • Objectives: Metabolic Syndrome (MetS) is strongly related with central obesity, hypertriglyceridemia, low high-density lipoprotein-cholesterol (HDL-C), hyperglycemia, and hypertension. This study reviewed the potential of Chenpi in treatment of MetS through amelioration of co-related diseases, such as diabetes mellitus, hyperlipidemia, obesity, hepatic steatosis, and inflammation. Methods: Six electronic databases (Oriental Medicine Advanced Searching Integrated System (OASIS), Korean Traditional Knowledge Portal, Korea Institute of Oriental Medicine (KIOM), Research Information Sharing Service (RISS), PubMed, and Embase) were used to search for in vitro, in vivo, and clinical research that discusses the potential effects of Chenpi (Citrus unshiu Markovich, Citrus reticulata Blanco) on diabetes mellitus, hyperlipidemia, obesity, hepatic steatosis, and inflammation. Results: This review suggests the potential of Chenpi as a candidate for the treatment of metabolic syndrome through improvement of co-related diabetes, hyperlipidemia, obesity, hepatic steatosis, and inflammation. However, comparison of the results of each study was limited by a lack of quantification of the experimental materials.

Scoping Review of Research on Mental Health of Nurses Working in Korean Medicine Hospitals (한방병원에 근무하는 간호사의 정신건강에 대한 연구동향: 국내연구를 중심으로)

  • Objectives: The aim of this scoping review was to analyze research trends about mental health of nurses working in Korean medicine (KM) hospitals in Republic of Korea. Methods: Searches were conducted using four electronic databases including Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System, Research Information Sharing Service, Korea Citation Index to collect relevant studies. The search date was March 4, 2021. All studies published up to the search date were considered. Observational studies reporting mental health outcomes of nurses working at KM Hospital were included. Results: A total of 11 cross-sectional observation studies were included. Four of them compared mental health of nurses working on KM and Western medicine (WM). Commonly reported outcomes related to mental health were job satisfaction, role conflict, and work stress. As a result of meta-analysis based on a 5-point Likert scale, the following factors were related to mental health of KM nurses: job satisfaction (2.844±0.067 points), role conflict (3.678±0.058 points), work stress (3.142±0.021 points), turnover intention (3.483±0.028 points), and burnout (3.180±0.033). Compared to WM nurses, KM nurses had significantly less work stress (p=0.000), role conflict (p=0.039), and job satisfaction (p=0.000). Conclusions: Mental health problems of nurses are known to be very common. Although improving them is an important social task, studies on mental health of KM nurses remain insufficient. Based on findings of this study, more cooperation between nurses and KM doctors should be made to improve the mental health of KM nurses, especially their job satisfaction in the future. The Korean Society of Oriental Neuropsychiatry needs to pay more attention to this topic.

Adoption of MFER and HL7 Standard for Shared Electronic Medical Record (공유 전자의무기록을 위한 MFER과 HL7 표준 적용)

  • Medical environments incorporate complex and integrated data networks to transfer vast amounts of patient information, such as images, waveforms, and other digital data. To assure interoperability of images, waveforms and patient data, health level seven(HL7) was developed as an international standard to facilitate the communication and storage of medical data. We also adopted medical waveform description format encoding rule(MFER) standard for encoding waveform biosignal such as ECG, EEG and so on. And, the study converted a broad domain of clinical data on patients, including MFER, into a HL7 message, and saved them in a clinical database in hospital. According to results obtained in the test environment, it was possible to acquire the same HL7 message and biosignal data as ones acquired during transmission. Through this study, we might conclude that the proposed system can be a promising model for electronic medical record system in u-healthcare environment.

  • In these days, HIS(Hospital Information System) raise the quality of medical services by effective management of medical records. As computing environment was developed, it is possible to search information quickly. But, standard medical data exchange is not completed between medical clinic and another organ so far. In case of patient transfer, past medical record was not efficiently transmitted. It be feasible treatment delay or medical accident. It is trouble that medical records is transferred by a person and communicate with each other. Extensible Markup Language (XML) is a simple, very flexible text format derived from SGML. Originally designed to meet the challenges of large-scale electronic publishing, XML is also playing an increasingly important role in the exchange of a wide variety of data on the Web and elsewhere. Form in system of company product, relative organs that handle bio-signal data is each other dissimilar and integration and to transmit to supplement bottleneck this research uses XML. In this study, it is discussed about sharing of medical data using XML web technology to standard medical record between hospital and relative organization The data structure model was designed to manage bio-signal data and patient record. We experimented about data transmission and all-in-one between different systems (one make use of MS-SQL database system and the other manage existent bio-signal data in itself form in file in this research). In order to search and refer medical record, the web-based system was implemented. The system that can be shared medical data was tested to estimate the merits of XML. Implemented XML schema confirms data transmission between different data system and integration result.

What Factors Make the Delay of the Information System Project Implementation? (무엇이 정보시스템 프로젝트 일정을 지연시키는가?)

  • Large-scaled hospital information system projects are often delayed than originally planned. Regarding project management, it is necessary to examine main factors triggering the schedule delay and to analyze the causality of such delay. This study tries to find the delay factors and causal relationship between them based on the case of the hospital information system development project by using the grounded theory method. It turned out that 'unrealistic scheduling' affects overall schedule delays like domino phenomenon, and 'poor analysis of existing systems' leads to poor quality of work analysis while 'poor subsystem integrated interface' triggers poor design quality. This study found out the factors affecting the delay of a project, analyzing cause-and-effect relationships between them in the academic side. From the practical view, it shows a solution to minimize delays in the project schedule for each of SI companies, hospitals and project managers.

  • Currently, many computer systems are used in many areas of medicine including radiation oncology. For the most part, the computer system has proved to be useful in radiotherapeutic planning and dose calculation. There has been attempts to develop computer system including information management of patients, patient tracing, and office automation in radiation oncology department. But some of these available commercial systems have shortcomings. We developed a management system of patients in our radiation oncology department that integrated most of items for the evaluation of patents. In particular, the data were stored in a natural language (noncoded) and made themselves easily understandable by all clinical groups. In addition , the data could be isolated in files from which the computer could generate graphs and static data by the use of some simple commands. The system provided us with not only the functions of case review but functions of preparation of conferences, lectures and resident teaching.

  • The characteristics of today's health care industry, based on the state of the art IT can be represented as a paradigm of human-oriented ubiquitous and accessible as possible by U-Health care. In addition, the healthcare industry is information and communication technologies (ICT) developments regarding the many advances and applications based on the research being carried out actively. Medical information system has been developed toward combining information systems of medical IT and it sets its sights on the fusion of developed IT and u-healthcare system. So changing distributed medical information systems into a safe PHR integrated system based on IaaS cloud computing is suggested in order to forge u-healthcare system with the times in this paper. Our experimental results show that our proposed system increased the data access time by about 24% and reduces the waiting time for processing service by about 4.3% over the web-based PHR.

  • IPv6 and Ubiquitous Healthcare Environment (UHE) has become a main stream of the next generation technologies. IPv6 is designed in many ways with enhanced features such as a routing, mobility, scalability, QOS and security as a replacement of IPv4. Also, UHE is developed to provide patients with convenience and efficient healthcare services using the remote home healthcare system. However, IPv4 currently used as an Internet protocol does not have enough capability to fully support UHE. It may result in a restricted implementation of UHE. As a result, research on IPv6 implementations in UHE is increasingly becoming an issue within the healthcare industry. IPv6 has enhanced features to implement the remote healthcare system such as Neighbour Discovery process and address auto-configuration. In this paper, a basic of IPv6 and UHE will be firstly introduced and secondly, benefits brought by IPv6 in UHE will be discussed. In addition, security issues in IPv6 will be analysed to conclude this paper.

  • In the paper. the health card system to integrate several cards into one card for protecting patient's privacy and security problems is proposed. Firstly, it is defined the minimal data set for integrating several patient cards into one card using IC card, and developed the issuing system to issue the integrated patient IC card. In order to secure and certificate a patient's personal information. the integrated patient IC card has applied 3-DES and the PKI certificate authority based Windows 2000 is established. The receipt and reservation system for taking care of a healthcare has developed to cooperate with the existing hospital computer system. The integrating patient IC card system proposed in this paper is implemented to 11 hospitals and used for 1.000 patients. On the result of the simulation. the proposed system can receive or reserve for a patient to take care of healthcare in the simulated hospitals and also establish the basis of the mechanism to share a medical information.
