This is a case study of Seoul St. Mary's Hospital applying a real-time enterprise (RTE) strategy to improve customer satisfaction and operational efficiency with the main process of medical institutions. The hospital is applying an RTE strategy to get real-time information on occurrences at each contact point of the main process of the medical institution from reservation to discharge through dashboard and to resolve issues through rapid decision-making. The RTE strategy of the hospital has some summaries: First, the hospital has linked a hospital management strategy to the RTE strategy to build a patient-centered treatment process. Second, the hospital has operated a control tower for change management and implementation monitoring in the process of implementing the RTE strategy. Third, the hospital has built systematic RTE-based environment as an application program in which the nU System is linked to Business Processor Renovation (BPR) promoted from 2006 on. Fourth, the hospital is applying a strategy to improve efficiency in operating the hospital by increasing customer satisfaction, removing inefficiency and variability, and managing medical resources efficiently through the RTE strategy. Fifth, it has established an information-sharing system through authority management for each user in terms of RTE information. Sixth, it has supplemented limitations of short-term information of the RTE strategy by linking the key performance index to the cost information system in order to improve performance of the RTE strategy. Seventh, it has improved customer satisfaction and achieved higher performance in improving operational efficiency, as compared with rival hospitals, through the RTE strategy.

Recently, hospital patients experience anxiety, confusion, and stress about wayfinding as the spacial layout and treatment circulatory system of hospitals have become complicated due to their oversized and complex structure. As part of finding a solution to the problem, this study seeks to examine what are the essential elements of the wayfinding planning of O.P.D. in general hospitals, to develop the model of wayfinding, and to suggest the methods of improving the wayfinding system. The research methods of this study adopted were literature review in wayfinding cognition, plan analysis of ten general hospitals, space analysis of these hospitals through space syntax, analysis of the system of visual-perceptual information through a field study, and analysis of surveys and follow-up surveys conducted to support the results. Based on these results, the proposals for finding decision points, providing the information, and developing a model planning are listed as follows. 1) The comprehensive understanding of O.P.D. spacial layout and the visual-perceptual information system is necessary to find the essential elements of wayfinding. 2) The decision points are found through the full understanding of spacial functions, circulation systems, and facility configuration, considering the spacial layout, the bound of the visual-perceptual information system, and the circulatory system. Furthermore, the information decision points could be confined by space syntax. 3) The checklist and color compound & color codes, developed through the planning of signage system and color system could be applied to the methods of providing the information. 4) The planning of wayfinding system according to the whole process of practices for outpatients was mentioned above. The system of visual-perceptual information developed through the process of this study should be integrated in the spacial layout of the whole O.P.D.

Recently, introduced a cloud computing technology to the IT industry, smart phones, it has become possible connection between mobility terminal such as a tablet PC. For dissemination and popularization of movable wireless terminal, the same operation have focused on a viable mobile cloud in various terminal. Also, it evolved Wireless Sensor Network(WSN) technology, utilizing a Body Sensor Network(BSN), which research is underway to build large Ubiquitous Sensor Network(USN). BSN is based on large-scale sensor networks, it integrates the state information of the patient's body, it has been the need to build a managed system. Also, by transferring the acquired sensor information to HIS(Hospital Information System), there is a need to frequently monitor the condition of the patient. Therefore, In this paper, possible sensor information exchange between terminals in a mobile cloud environment, by integrating the data obtained by the body sensor HIS and interoperable data DBaaS (DataBase as a Service) it will provide a base of mBodyCloud System. Therefore, to provide an integrated protocol to include the sensor data to a standard HL7(Health Level7) medical information data.

A review of randomized controlled trials (RCTs) using electroacupuncture (EA) to treat patients with foot drop was performed to analyze the effectiveness of EA for this condition. Relevant studies (n = 183) from 7 databases (Cochrane Library, Excerpta Medica Database, PubMed, China National Knowledge Infrastructure, Korean Studies Information Service System, Research Information Sharing Service, and Oriental Medicine Advanced Searching Integrated System) were selected based on the inclusion and exclusion criteria, and 12 RCTs met the selection criteria. In all 12 studies, EA showed significantly positive changes. In most indicators, positive changes were observed in the EA group compared with that in the control group. Significant increases were confirmed in muscle strength-related indicators such as the Fugl-Meyer motor scale, surface electromyography, active range of motion, and gait-related indicators such as the Tinetti score, maximum walking speed, and Berg balance scale. No notable adverse events were reported. EA is suggested as an effective treatment for post-stroke foot drop; however, more RCTs are required.

본 논문에서는 현재 대학 한방병원에서 운영하고 있는 통합의료정보시스템의 외래환자 인구학적 정보와 진료기록 정보를 이용하여 고객세분화를 실시하고, 그 결과를 활용하여 외래환자 고객만족도 증진을 위한 고객관계관리 시스템 구축 모형을 제안하였다. 제안된 고객 관계관리 시스템 모형은 최선 정보기술과 인프라를 이용하기 보다는 현재 구축된 병원정보시스템의 부분적 수정을 통해 구축이 가능하므로 즉시 실현이 가능한 실용적인 모델이 될 수 있다. 또한 마케팅 전략에 따라 적절한 변수와 세분화 방법을 활용할 경우, 외래환자 고객만족 증진뿐만 아니라 다양한 형태의 마케팅 전략을 지원할 수 있는 고객관계관리 시스템 구축이 가능할 것이다.

방사선사를 위한 교육 및 영상 정보에 대한 정확한 판단에 유용한 주석-기반 방사선 의료영상 검색 시스템을 설계 및 구현하고, 방사선 의료영상에 대한 단순 속성정보, 부가적인 정보인 텍스트 설명정보로부터 추출한 중요 키워드에 대한 효율적인 검색을 위해 $B^+$-트리와 역화일 기법을 이용한 색인기법을 제안하고자 한다. 윈도우즈 XP에서 Delphi를 이용하여 구현하였으며, 방사선사는 방사선 의료영상에 대한 속성 정보, 부가적인 설명정보, 이미지 정보를 저장하도록 하고, 구축된 영상 데이터베이스로부터 속성정보와 텍스트 키워드 정보를 이용하여 검색 가능하도록 하였다. 임상방사선사가 단순속성정보 및 텍스트 설명정보를 찾아냄으로써 임상현장에서의 체계적인 교육뿐 만 아니라 지식을 구조화함으로써 교육시간의 단축과 방사선 의료영상에 대해 정확한 판단을 내릴 수 있다. 구현되어진 방사선 의료영상검색 시스템은 차후에 일반촬영, 특수조영영상을 포함한 통합화상시스템으로의 확장이 요구되며, 아울러 웹을 통한 서비스를 구축함으로써 의사결정시스템으로 발전 할 수 있는 기반기술로 기대된다.

의료시장의 무한경쟁과 의료 경영환경을 극복하기 위한 최선의 방안은 병원 업무의 전산화를 통한 시스템 간의 효율적인 정보교환의 필요성이 대두되고 있다. 또한, 처방전달시스템, 전자의무기록시스템, 검사정보시스템, 의료영상 저장 및 전송시스템 등의 이기종간 의료정보 시스템의 통합 시스템을 구축하여 진료환경을 구축하고 진료의 효율성을 개선해야 할 것이다. 본 논문에서는 업무 시스템 간에 상호 연계가 보장된 표준정보연계체계 구축 및 최적화된 응용시스템 구축으로 환자 서비스 개선과 진료 및 경영 효율을 증대시킨 통합의료정보시스템의 구축방안에 대하여 연구하였다.

본 연구는 통합병원정보시스템 (Integrated Hospital Information System) 에서 의료용 데이터 웨어하우스 (Medical Data Warehouse) 부분의 보안 프로토타입 시스템을 메타데이터 기반으로 설계하고 구현하는 데 주안점을 두었다. 특히, 의료용 데이터 웨어하우스 중에서도 임상 데이터 웨어하우스 (Clinical Data Warehouse) 에 초점을 두었으며 이에 대한 프로토타입은 ㅈ 병원에 적용되어서 개발되었다.

Filmless full-PACS in korea has rapidly been growing, since government had supported collaborative PACS project between industry and university hospital in late of 1995. At the same time, a small company had started PACS business, while the Korea PACS society was being formed. In the beginning, PACS societies had focused on developing peripheral solutions such as DICOM gateway for image acquisition, x-ray film digitizer, and viewing software for research or management of personal image data, while Samsung Medical Center had started installing an imported partial PACS system which had recently upgraded with a new system. In similar time frame, a few hospitals had started developing and installing domestic large scale full-PACS system. Several years later, many hospitals have installed full-PACS system with national policy of reimbursement for PACS exams in November 1999. It is believed that Korea is the first country that adopted PACS reimbursement for filmless full-PACS as a national policy. Both experiences of full-PACS installation and national policy generated tremendous intellectual and technological expertise about PACS at all levels, clinical, hospital management, education, and industrial sectors. There are currently three types of PACS system which includes domestic, imported, and hybrid PACS system with imported solution for core system and domestic solution for peripheral system. There are more than 20 domestic PACS companies and they have now enough experiences so that they are capable of installing a truly full-PACS system for large-scale teaching hospitals. PACS societies in Korea understand how to design, implement, install, manage, sustain, and provide good services for large-scale full-PACS. PACS society has also strength for the highest integration technology of the Hospital Information. However, further understanding and timely implementation of continuously evolving international standard and integrated healthcare enterprise concepts may be necessary for international leading of PACS technologies for the future.

해외병원 프로젝트는 정보통신기술과 첨단의료기기의 발전으로 원격진료, Smart-health, City Like Hospital 개념 확산과 IT 비중이 증가하는 디지털병원으로 변화하는 추세이다. 디지털병원에서 IT역할은 첨단화 되어가는 시공, 기계, 전기, 시스템, 의료장비를 연계하여 통합함으로써 병원운영의 효율성을 극대화 시키는 것이 핵심 요소이다. 본 연구에서는 해외에서 발주되었던 5개의 디지털병원 프로젝트를 대상으로 입찰요구서를 IT 관점에서 분석하고 디지털병원 성공요인을 제안한다. 연구방법은 문헌조사를 통해서 디지털병원 개념과 발전과정을 분석하며, 디지털병원 시스템은 입찰요구서를 분석해서 디지털병원에 적용된 시스템과 기술수준을 분석하고, 최종적으로 디지털병원 성공요인은 프로젝트 성공관리기법에서 제안하는 원가, 공기, 품질 측면에서 분석한다. 연구 성과는 원가측면에서는 IT 및 병원 인프라 운영관점에서 가치를 증대하는 가치공학(Value Engineering) 전략 수립이 필요하고, 공기측면에서는 병원 내 시스템을 통합 테스트하는 Integrated Test & Commissioning 단계에서의 공사기간 준수가 필요하며, 품질측면에서는 사용자 요구사항이 반영된 System Integration을 통해 디지털병원이 지향하는 서비스 성능을 보장하는 것이 디지털병원 수출사업의 성공요인이라고 제시한다.
