Caloric restriction and its mimetics

  • Caloric restriction is the most reliable intervention to prevent age-related disorders and extend lifespan. The reduction of calories by 10-30% compared to an ad libitum diet is known to extend the longevity of various species from yeast to rodents. The underlying mechanisms by which the benefits of caloric restriction occur have not yet been clearly defined. However, many studies are being conducted in an attempt to elucidate these mechanisms, and there are indications that the benefits of caloric restriction are related to alteration of the metabolic rate and the accumulation of reactive oxygen species. During molecular signaling, insulin/insulin-like growth factor signaling, target of rapamycin pathway, adenosine monophosphate activated protein kinase signaling, and Sirtuin are focused as underlying pathways that mediate the benefits of caloric restriction. Here, we will review the current status of caloric restriction.

Diabetes Management and Hypoglycemia in Safety Sensitive Jobs

  • The majority of people diagnosed with diabetes mellitus are in the working age group in developing countries. The interrelationship of diabetes and work, that is, diabetes affecting work and work affecting diabetes, becomes an important issue for these people. Therapeutic options for the diabetic worker have been developed, and currently include various insulins, insulin sensitizers and secretagogues, incretin mimetics and enhancers, and alpha glucosidase inhibitors. Hypoglycemia and hypoglycaemic unawareness are important and unwanted treatment side effects. The risk they pose with respect to cognitive impairment can have safety implications. The understanding of the therapeutic options in the management of diabetic workers, blood glucose awareness training, and self-monitoring blood glucose will help to mitigate this risk. Employment decisions must also take into account the extent to which the jobs performed by the worker are safety sensitive. A risk assessment matrix, based on the extent to which a job is considered safety sensitive and based on the severity of the hypoglycaemia, may assist in determining one's fitness to work. Support at the workplace, such as a provision of healthy food options and arrangements for affected workers will be helpful for such workers. Arrangements include permission to carry and consume emergency sugar, flexible meal times, selfmonitoring blood glucose when required, storage/disposal facilities for medicine such as insulin and needles, time off for medical appointments, and structured self-help programs.

대마 뿌리 및 줄기 추출물의 포도당 흡수 개선 효과 및 기전에 대한 비교 연구 (A Comparative Study on the Efficacy and Mechanism of Improving Glucose Uptake of Cannabis Root and Stem Extracts)

  • Objectives: Despite the pharmacological potential of the roots and stems of hemp based on literatures, active research has not been conducted for a long time. Comparative experiments were conducted on antioxidant and anti-inflammatory effects and improvement of glucose uptake using Cannabis root and stem extracts. Methods: Antioxidant contents in Cannabis root and stem extracts were examined with total phenolic, tannin, flavonoid assay. Anti-inflammatory properties were tested in lipopolysaccharides-treated RAW264.7 cells. Efficacy of Cannabis root and stem extracts on glucose uptake was investigated using fluorescent glucose analog (2-NBDG) in palmitate-treated HepG2 cells. The mechanism of action on metabolism was examined by western blot. Results: Antioxidant and anti-inflammatory efficacy were greater in stem extracts, but improvements in glucose uptake performed under various conditions were found to be greater in root extracts. It is assumed that Cannabis root extracts exhibited an improvement in glucose uptake through mechanisms such as AMP-activated protein kinase activation, not depending on general antioxidant and anti-inflammatory effects. Conclusions: Further research is needed on the mechanisms and substances that exhibit the anti-diabetic effects of Cannabis roots and stems.

트립톨라이드가 식이제한에 의한 수명연장과 노화관련 질환에 미치는 영향 (Triptolide Mimics the Effect of Dietary Restriction on Lifespan and Retards Age-related Diseases in Caenorhabditis elegans)

  • 뇌공등에 함유되어있는 트립톨라이드는 뛰어난 항염증, 항산화 효능을 가지고 있음이 보고되었다. 예쁜꼬마선충을 이용한 이전 연구에서 트립톨라이드의 섭취가 개체의 항스트레스 효능을 높이고, 수명을 연장시킴이 밝혀졌다. 본 연구에서는 트립톨라이드에 의한 수명연장에 관여하는 세포 내 기전과 트립톨라이드가 노화관련 질환인 당뇨병과 알츠하이머병에 미치는 영향을 평가하였다. 트립톨라이드는 인슐린/IGF-1-like 신호전달 저하에 의한 수명연장 돌연변이인 age-1과 미토콘드리아 전자 전달계 저하에 의한 수명연장 돌연변이인 clk-1의 수명을 유의적으로 증가시킨 반면, 식이제한 유도 돌연변이인 eat-2의 수명에는 유의적인 변화를 유도하지 못했다. 또한 박테리아 희석을 이용한 식이제한에 의해 연장된 수명을 추가적으로 더 연장시키지 못했다. 트립톨라이드 섭취는 고농도의 당 섭취에 의한 체내 독성과 사람 아밀로이드 베타 형질전환 유전자로 인한 체내 독성을 유의적으로 저하시켰다. 이러한 결과들은 트립톨라이드에 의한 수명연장이 식이제한에 의한 수명연장 기전과 중복되며, 트립톨라이드가 노화관련 질환을 저해하는 효능이 있음을 보여준다. 따라서, 트립톨라이드는 식이제한 효능을 대체할 수 있는 식의약품 개발에 활용될 수 있다.

LC15-0133, DPP IV 저해제: 여러 동물 모델에서의 효능 (LC15-0133, a DPP IV Inhibitor: Efficacy in Various Animal Models)

  • GLP 1 기반의 약(GLP-1 유도체와 DPP IV 저해제)과 인크레틴 유사체는 최근 가장 각광을 받는 2당뇨 치료제 계열 중 하나이다. GLP-1은 위장에서 분비되는 핍타이드 인크레틴 호르몬으로서 췌장으로부터의 당-의존적 인슐린 분비를 증진하고, 위장통과를 지연시키며, 췌장 베타세포의 증식을 촉진한다. DPP IV 저해제는 GLP-1 불활성화시키는 DPP IV 효소의 활성을 저해함으로써 인크레틴처럼 작용한다. LC15-0133은 경쟁적, 가역적 DPP IV 저해제 ($IC_{50}$ = 24 nM, Ki=0.247 nM)이며, DPP II, DPP 8, 엘라스타제, 트릴신, 유로키나제에 비해 DPPIV 에 대한 선택적 억제 효능이 우수하다. LC15-0133 랫과 개에서 긴 반감기와, 우수한 경구흡수율을 보였다. LC15-0133 랫과 개에서 각각 0.1 mg/kg 0.02 mg/kg 용량으로 경구투여하고 24시간이 지난 후에도 50%이상의 혈장 DPP IV 활성 억제효능을 유지하였다. C57BL/6 마우스에서 LC15-013301 경구당부하에 의한 혈당증가를 억제하는 최소유효용량은 0.01 mg/kg, 당에 의한 GLP-1 분비를 증가시키는 최소유효용량은 0.1 mg/kg 이다. Zucker 당뇨 랫에서 LC15-01331의 1개월간의 경구반복투여는 당뇨병으로의 진행을 지연시키고, 혈중 HbA1c 감소시켰다. 결론적으로, LC15-0133은 신규의 강력하고, 선택적인 경구 DPP IV 저해제이며, 여러가지 동물모델에서 탁월한 혈당강하효능을 보였다.
