Prior to destructive testing, diagnostic tests were performed in eight high voltage motors. Diagnostic tests included polarization index, ac current, dissipation factor($tan{\delta}$) and partial discharge magnitude. The rewind of motor stator insulation at rated voltage is assessed by the results of these tests. After completing the diagnostic tests, the stator windings of motors were subjected to gradually increasing ac voltage, until the insulation punctured. No. 1 motor failed near rated voltage of 12.96 kV. The breakdown voltage of No. 4 motor was 6.99 kV which is lower that expected for good quality coils in 6.6 kV class motors. The failure was located in a line-end coil at the exit from the core slot. These two motors began operation in 1994. While testing No. 7 motor, flashover occurred between the stator winding and the stator frame at 15 kV. The relationship between the diagnostic test and the drop in insulation breakdown voltage was analyzed.

In the cases of severe arrythmia and conduction failure, a permanent implanted pacemaker is considered an essential treatment modality with less complication rate, broad indications and low morbidity. However, some pacemakers needs to be removed or replaced due to infection, in need of a lead upgrade, elective replacement, conduction failure or insulation failure. The most common indication for pacemaker extraction is infection. Conservative treatments such as administration of intravenous antibiotics and limitation of debridement are not effective and the removal of the entire pacing system is considered to be the best approach to pacemaker pocket infection. Although a locking stylet, a laser sheath and other newer methods of transvenous lead extraction have been proven to be effective, all leads cannot be removed. Moreover, major complications such as, cardiac tamponade and respiratory arrest during leads extraction procedure should not be ignored. We experienced two cases of exposed pacemakers in the subclavicular region. After removing the pacemaker body, exposed proximal lead was pulled out and cut off. The end of remnant external insulation tube was tied to prevent infection propagation between external insulation tube and inner metalic coil. Wounds were covered by local flap coverage. No other problems were detected during the one-year follow-up. Since there are few reports on lead-preserving method of treating limited infection of exposed pacemakers, we would like to present our new method for treating exposed pacemakers.

In this paper the problems associated with the switching surge of PWM inverter devices were analysed. As the application of ASD for efficient speed control of AC rmtors is increased, so is the rrntor and ASD-related failure increased. In the industrial rmtor awlications, the introduction of PWM drives can sometmes cause insulation breakdown between the phase windings due to high transient voltage peak. The motor cable length combined with the high switching frequency becomes more sensitive issue for proper drive operation. Effects of the cable length and high switching frequency influence both on rmtor and inverter. When the insulation level of rmtor winding is low, the failure probability caused by high peak voltage and fast voltage rise times(dv/dt) is high. Voltage refloctions are simulated far rmtor capacities, ASD and rise times and are presented graphically. The filtering techniques are discussed to reduce the rmtor terminal overvoltage and dv/dt in inverter fed AC rrntor drive systems. We confirmed that the lower motar capacity and rmtar insulation level, the shorter switching time and cable length and the higrer the probability of insulation breakdown .kdown .

After completing the diagnostic tests, the stator windings of motors were subjected to gradually increasing ac voltage, until the insulation punctured. No. 1 and No.2 motors(4.16 kV) failed near rated voltage of 12.3 kV and 14.2 kV, respectively. The breakdown voltage of No.3 and No.4 motors(6.6 kV) was 17.6 kV and 17.8 kV, respectively. These motors are higher that expected for good quality coils in 6.6 kV class motors.
In recent years, environmental concerns have been raised on the use of poorly biodegradable fluids in electrical apparatus in regions where spills from leaks and equipment failure could contaminate the surroundings. The newly invented vegetable insulating oil is highly biodegradable and have negligible impact on the environment, human health and the ecosystem. For development of the environmental-friendly pole transformer using vegetable insulating oil, the dielectric constructions of the pole transformer were discussed in this paper. Depending on the dielectric constructions, the AC breakdown characteristics of the Nomex insulating papers and the vegetable insulating oil were studied by simulated electrode systems. Based on the experimental results, the maximum design stress($E_{max}$) for insulation design of the environmental-friendly pole transformer were suggested.

Electrical stress makes insulating materials aged finally resulting in the dielectric failure. It becomes more and more important to recognize the degree of aging and the life time of the insulating materials since it is directly concerned with the reliability of the insulation system. In this paper, the life time of the Cross-linked Polyethylene(XLPE), the insulation layer in the 154kV-grade power cable, is expected and calculated using statistical and mathematical methods. The simulated results are compared with experimental ones and the life exponent of the material is obtained.
Recently, the insulation breakdown accident has been experienced at 6.6kV inverter driving motor. That brought about the reliability degradation of motor driving system.. We need to find the solution with system engineering level. The objectives of this study are developing the appropriate filter for the drive to protect the motor and the Insulation system of motor which Is driven by inverter. We clarify the cause of the deterioration of the inverter driving motor through the analysis of Insulation breakdown accidents.
