The Effects of Feuerstein's FIE Program on Gifted Children's Executive Function (Feuerstein의 FIE 프로그램이 영재아의 실행기능에 미치는 효과)

  • The purpose of this study was to assess the efficacy of Feuerstein's program on Gifted children's executive function. The subjects for this study were 40 Gifted children (3th year elementary students) from KAGE. Twenty were randomly allocated to the experiment group and the other 20 to the control group. The results of this study were as follows: The experiment group exhibited statistically significant improvement in attention control, cognitive flexibility and information process procedure compared to the control group. The results proved that FIE program had an effect on improvement of executive functions in gifted children.

  • Genome instability, primarily caused by faulty DNA repair mechanisms, drives tumorigenesis. Therapeutic interventions that exploit deregulated DNA repair in cancer have made considerable progress by targeting tumor-specific alterations of DNA repair factors, which either induces synthetic lethality or augments the efficacy of conventional chemotherapy and radiotherapy. The study of Fanconianemia (FA), a rare inherited blood disorder and cancer predisposition syndrome, has been instrumental in understanding the extent to which DNA repair defects contribute to tumorigenesis. The FA pathway functions to resolve blocked replication forks in response to DNA interstrand cross-links (ICLs), and accumulating knowledge of its activation by the ubiquitin-mediated signaling pathway has provided promising therapeutic opportunities for cancer treatment. Here, we discuss recent advances in our understanding of FA pathway regulation and its potential application for designing tailored therapeutics that take advantage of deregulated DNA ICL repair in cancer.

A Study of the Obesity Index and Psychosocial Factors Influencing Obesity among Adolescent Girls (여고생들의 비만도 및 비만과 관련된 사회심리적 요인 분석)

  • The purpose of this study was to investigate the dietary intake, obesity index and psychosocial factors influencing obesity among 200 high school girls in Seoul. The Social Cognitive Theory provided the Conceptual basis for this study. A cross-sectional survey was conducted to examine factors related to obesity, including self-efficacy for controlling overeating, social support for eating behavior, perception of body image and weight control, nutrition knowledge, and attitudes toward obesity. The data were analyzed using t-test and multiple logistic regression. The results of this study are as follows : 1) The mean age of the girls was 16.4 years, and the rate of overweight and obesity(measured by obesity index) was 27.0$\%$. 2) The mean energy intake of subjects was 1832.3$\pm$384.0kcal. The energy derived from carbohydrates, proteins and fats was 62.7$\%$, 13.8$\%$, and 23.5$\%$, respectively. There was no significant difference between the obese and the comparison group in energy intake. 3) The result of multivariate analysis indicated that obesity had a significant relation to the perception of ideal body image, social support for eating behavior, and self-efficacy for controlling overeating(p〈0.01). As subjects preferred thinner body images(OR=0.39) and received less social support(OR=0.93), the odds of being classified as obese increased. The odds of being obese were also associated with self-efficacy, however, the relation was not strong(OR=1.04). 4) Specific social support was related to obesity among adolescent girls. As subjects received more support from family member, the odds of being obese decreased. The emotional support as well as family member's positive nutrition behavior plays a significant role. In addition, instrumental support from friends was associated with obesity. With repect to self-efficacy, the odds of being obese were increased as subjects felt less confident in controlling overeating when tempting food was placed in front of them or after an argument. In contrast, the obese group felt more confident in controlling overeating for the rest of the specific situations examined. These findings suggest that educational interventions for weight control should incorporate strategies to help participants realize their degree of obesity, to reduce the discrepancy between current and ideal body image, to elicit and maintain social support from friends and family, and to increase the self-efficacy for changing eating behaviors. Comparisons of physical Fitness, Self Efficacy, Instrumental Activities of Daily Living, and Quality of Life between Institutionalized and Noninstitutionalized Elderly (가정노인과 양로원노인의 체력, 자기효능, 일상생활활동능력 및 삶의 질에 관한 연구)

  • This study was done to investigate levels of physical fitness, self efficacy(SE), instrumental activities of daily living(IADL), and quality of life (QL), and their relationships among the elderly Data were collected from 47 noninstitutionalized elderly dwell-ing in their own homes and 43 institutionalized elderly living in homes for older people. The ages of the subjects were 65 years and over. The data were col lected from January 20th to February 20th, 1995. Physical fitness was measured with T.K. K dyna mometer, grip dynamometer, stop watch, and Purdue pegboard. Structured questionnaires developed by Kim by Lawton & Brody, and by Ro were adopted to measure SE, IADL, and QL, respectively. The data were analyzed using t-test, ANOVA, and Pearson correlation coefficients. The results were as follows : 1. The noninstitutionalized elderly had higher scores in such measures of physical fitness as body weight, skinfold thickness, flexibility and coordination as compared to the institutionalized elderly. 2. On the following measures of physical fitness, men showed higher levels than women ; in height when standing, height when sitting, body weight, and muscle strength. Women had more trunk flexibility than men. 3. Muscle strength was positively correlated with height when standing, height when sitting, and body weight. The following measures of physical fitness, muscle strength, flexibility and coordi nation, were negatively correlated with "up and go”. 4. The noninstitutionalized elderly had higher scores in SE(t=2.28, p<0.05), IADL(t=2.24, p<0.05), and QL(t=2.41, p<0.05) as compared to the institutionalized elderly. 5. SE was positively correlated with both IADL(r=0.41, p<0.001) and QL(r=0.54, p<0.001), and the latter two variables were a positively correlated (r=0.30, p<0.001). 6. SE was positively correlated with the physical fitness measures of height when standing, body weight, strength of leg muscles, strength of back muscles, grip strength, and level of touching the floor, but negatively correlated with "up and go”. 7. Positive correlations were revealed between IADL and the physical fitness measures of height, strength of leg muscles, level of trunk muscle extension, level of touching the floor, and coordination. The IADL was negatively correlated with “up and go”. 8. QL was positively correlated with body weight (r=0.28, p<0.01) and skinfold thickness (r=0.26, p<0.05). 9. Age was negatively correlated with the physical fitness measures of height when standing, height when sitting, “up and go”, strength of leg muscles, strength of back muscles, grip strength, level of trunk muscle extension, and coordination. 10. Age was also negatively correlated with SE (r=-0.24, p<0.05) and IADL(r=-0.22, p<0.05). The above results suggest that caring elderly in their own homes were more effective and that nursing interventions to enhance physical fitness, SE, IADL, and QL especially for the institutionalized elderly are stressed.

A Study of Related Factors in the Bone Mineral Density of the Institutionalized Elderly (시설노인의 골다공증 관련요인 연구)

  • The purpose of this study was to measure and determine the relationship of femoral neck and lumbar bone mineral density with their and related factors. It were measured and determined the relationships among bone mineral density, bone mineral content in the lumbar and femoral neck, muscle strength (arm, back, leg), muscle endurance, instrumental activity of daily living (IADL), quality of life, cognitive perceptual variables(self efficacy, perceived health status), age, age at menopausal period. The twenty five subjects participating in this study consisted of twelve males and thirteen females at a C-institution in Chung Buk province. The mean age of subjects was 73.64 years. The data was collected from August, 1993 to September, 1993. The data was analyzed with $x^2-test$, t-test, Correlation, multiple regression using a SPSS pc+ program. 1. The mean femoral neck bone mineral density was $0.636g/cm^2$, 66.7% of young bone mineral density, the mean lumbar($L_2-L_4$) bone mineral density was $0.807g/cm^2$, 79.86% of young bone mineral density. The mean fermoral neck bone mineral content was 2.906g and the mean lumbar bone mineral content was 36.898g. 2. The mean muscle strength was 17.14kg(grip strength), 32.05kg(back lift strength), 17.14kg (leg lift strength) and the mean muscle endurance was 9.92times. 3. Men showed a significantly higher score (p<0.01) in muscle strength and muscle endurance than women, as well as a significantly higher score on self efficacy and perceived health status(p<0.05). 4. The femur neck bone mineral density had a significant correlation(p<0.0l) with leg lift strength, back lift strength, and their was a significant correlations (p<0.05) with arm strength and muscle endurance. Lumbar ($L_2-L_2$) bone mineral density had a significant correlation(p<0.05) with muscle endurance, grip strength and IADL. 5. With the multiple regression analysis the most significant predictor for lumbar bone mineral density were IADL, the most significant predictor for femoral neck bone mineral density was leg strength. This study concluded: As the mean bone mineral density and bone mineral content were low, the aged showed osteopenia. Bone mineral density, muscle strength and IADL were correlated. The aged could pro mote muscle strength, bone mineral density and IADL through Leg Press exercise which was safe and efficient for the aged. This Leg Press exercise contributed to prevention of osteoporosis and promoted the health of the aged.

Analysis of Studies on Samul-tang for Fundamental Establishment of Evidence Based Medicine (EBM 기반구축을 위한 사물탕 연구 문헌 분석)

  • To establish the fundament for EBM of Traditional Korean Medicine, the papers on Samul-tang which was frequently used in medical institutions of Traditional Korean Medicine were analyzed through researching domestic and international literatures. The papers were classified by the registration of domestic or international journals, by the year of publishment, by experimental methods, by laboratory animals used in biological experiment and by the kinds of studies on biological efficacy. Of total 67 papers on Samul-tang, 58 volumes were registered in domestic journals and 9 volumes were in international journals of which 8 volumes were in SCI journals. Since 1978, publishments of papers have continuously increased. The papers on instrumental analyses were 6, biological studies were 58 volumes, clinical studies were 3. Instrumental analyses were preceeded with standard compounds(gallic acid, albiflorin, paeoniflorin, benzoic acid, ferulic acid, 5-HMF). And biological studies showed improvement of cardiovascular function and circulation, antianemia, brain protection, immunoregulation, antistress, radioprotection, antifatigue, antiinflammation and antiallergy, antioxidative effect. Through clinical studies, antifatigue, improvement of insomnia and osteoporosis were reported. Samul-tang could be used to tonify and activate blood. And further study on clinical field need to be conducted in accordance with biological study.

The Analgesic Efficacy and Side Effects of Subarachnoid Sufentanil-Bupivacaine on Parturients in Advanced Labor (지주막하강 수펜타닐과 뷰피바케인의 혼합 투여가 분만 제 1 기 산모의 진통 효과에 미치는 영향)

  • Background : Previous studies have proven beneficial in labor analgesia to use subarachnoid sufentanil(alone or with adjuvant) on parturients in early first stage of labor. We designed this prospective study to evaluate analgesic efficacy and side effects of subarachnoid sufentanil plus bupivacaine in women with cervical dilatation of 7 cm greater. Methods : This was an open-label, nonrandomized trial of 32 parturients in late first stage labor who requested labor analgesia. After signing the consent form each patient received subarachnoid sufentanil (10 ${\mu}g$) and bupivacaine (2.5 mg). Patients were asked to rate their verbal pain score (0-10 scale) before regional anesthesia and 5 minutes after subarachnoid injection, and every 20 minutes thereafter until delivery or request for additional analgesia. Blood pressure, pruritus, Bromage motor block score, mode of delivery and need for supplemental analgesics were recorded. Results : Thirty women were included in the study. Mean pain scores (mean${\pm}$SD) were $8.7{\pm}1.0$ pre-spinal, $0.7{\pm}1.5$ 5 minutes post-injection, and remained less than 5 for 130 minutes after spinal injection. Of 30 patients, 24 had unassisted vaginal delivery, 4 instrumental vaginal delivery (vacuum), and 2 cesarean delivery. Of 28 patients who delivered vaginally, 19 did not require supplemental analgesics and had a delivery pain score of 5 or lower. Blood pressure decreased in three patients after spinal analgesia (p<0.05), which necessitated treatment. The Bromage motor block score was 0 in 26 patients and 1 in 4 patients. Pruritus was noted in 22 patients. Conclusion : Subarachnoid sufentanil-bupivacaine provides rapid analgesia for an effective duration of approximately 130 minutes in parturients in late first stage of labor.

The Clustering and Variables in Discriminating the Groups by the Level of the Loss Experience and Ego-integrity in the Elderly (노인의 상실경험과 자아통합감 수준에 따른 적응군집화 및 집단판별에 기여하는 변인)

  • The purpose of this study was to search the clustering and variables in discriminating the groups by the level of the loss experience and ego-integrity among the elderly. In addition, the study aimed to confirm the moderating effects of variables related to resilience in the relationship between a loss experience and ego-integrity. The subjects were 245 elderly aged 60 or above, most of whom were socially active with comparatively high education levels. Six individual variables(physical self-efficacy, general self-efficacy, social self-efficacy, existential spirituality, religious spirituality, optimism) and five environmental variables(emotional support, instrumental support, informational support, appraisal support, social activity) were used in the study. The results of the study are as follows. To data analysis, ANOVA, Stepwise Discriminant analysis, Cluster analysis, Regression analysis and Two-way analysis of variance was employed. First, the subjects were 4 cluster into overcome group, crisis group, stable group and vulnerable group according to the level of loss experience and ego-integrity. Second, optimism, physical self-efficacy, existential spirituality and emotional support were seen to be significant variables in discriminating the groups. Third, the ego-integrity became higher as the variables related to resilience were increased.

Cell Type-Specific and Inducible PTEN Gene Silencing by a Tetracycline Transcriptional Activator-Regulated Short Hairpin RNA

  • Inducible and reversible gene silencing in desired types of cells is instrumental for deciphering gene functions using cultured cells or in vivo models. However, efficient conditional gene knockdown systems remain to be established. Here, we report the generation of an inducible expression system for short hairpin RNA (shRNA) targeted to PTEN, a well-documented dual-specificity phosphatase involved in tumor suppression and ontogenesis. Upon induction by doxycycline (DOX), the reverse tetracycline transcriptional activator (rtTA) switched on the concomitant expression of GFP and a miR-30 precursor, the subsequent processing of which released the embedded PTEN-targeted shRNA. The efficacy and reversibility of PTEN knockdown by this construct was validated in normal and neoplastic cells, in which PTEN deficiency resulted in accelerated cell proliferation, suppressed apoptosis, and increased invasiveness. Transgenic mice harboring the conditional shRNA-expression cassette were obtained; GFP expression and concurrent PTEN silencing were observed upon ectopic expression of rtTA and induction with Dox. Therefore, this study provides novel tools for the precise dissection of PTEN functions and the generation of PTEN loss of function models in specific subsets of cells during carcinogenesis and ontogenesis.

  • An Effect of Muscle Strength Training Program on Muscle Strength, Muscle Endurance, Instrumental Activities of Daily Living and Quality of Life in the Institutionalized Elderly Recent statistics shows that the aged are the fastest growing segment of our population by increasing life span. The age group of over 60 shows multiple health problems and the most serious handicapping problem of these. are related to the changes in muscular skeletal system. With aging, people lose. their muscle mass and muscle strength resulting from biological changes and physical inactivity. Studies documented a 30-50% loss of muscle mass in an advanced age and thus, ordinary life activities can be seriously affected due to weakened muscle strength. Preservation of muscle strength of lower limb is especially important in the aged. Since it is readily affected from reduced physical activity in old age, sometimes to the detriment of moving or walking. So muscle strength exercise program designed for the elderly to improve leg muscle strength and leg muscle endurance. The research design used was nonequivalent control group pretest - protest design. The purposes of this study were to test the effect of muscle strength exercise program utilizing Leg Press on muscle strength, muscle endurance, instrumental activities of daily living(IADL), cognitive perceptual variables and quality of life. Forty nine subjects participating in this study consisted of twenty four male and twenty five female. Twenty four experimental group subjects were selected from C-institution in Chung Buk province, and twenty five control group subjects were selected from O-institution in Chung Nam province. The mean age of subjects was 72.8 years. Muscle strength training program utilizing Leg Press for the experimental group was carried out three times a week for 9 weeks. The data was collected from August, 1993 to October, 1993. Data were analyzed with $X^2-test$, t-test, ANCOVA test, Kruskal Wallis 1-Way ANOVA test using SPSS PC program. Results were obtained as follows : 1) The experimental group showed significantly higher scores on muscle strength (leg lift strength, back lift strength and grip strength) and muscle endurance than control group after the experiment $\ulcorner$F=52.35(p=.001), F=54.07(p=.001), F=6.97(p=.011), F=18.17(p=.001)$\lrcorner$ 2) Experimental group were significantly higher scores on IADL than control group(F=7.51, p=.009). 3) Experimental group showed significantly higher scores on economical state and self esteem aspects of the quality of life scale than control group $\ulcorner$F=10.59(p=.002), F=6.97(p=.011)$\lrcorner$. But there were no differences in emotional state, physical and functional state and relationship with reatives in the quality of life between groups. 4) Scores on IADL showed a significant difference with the level of muscle strength and muscle endurance $\ulcornerx^2=7.73(p=.025),\;x^2=8.86\;(p=.011)\lrcorner$ 5) Scores on self esteem and physical and functional state aspects of the quality of life scale showed a significant difference with the level of IADL $\ulcornerx^2=11.39(p=.003),\;x^2=9.02(p=.011)\lrcorner$. 6) Scores of experimental group after the experiment in cognitive perceptual variables (perceived benefit on exercise, perceived health status, self efficacy, emotion on exercise) were significantly higher than those of control group $\ulcorner$F=32.09(p=.001), F=5.07(p=.029), F=20.63 (p=.001), F=30.38(p=.001)$\lrcorner$. As a result of this study, the effect of strength training exercise program with Leg. Press had improved muscle strength, muscle endurance, IADL, and the perception of self esteem, physical and funtional state, and economical state. Thus strength training program could be beneficially applied for the prevention of disablity and promotion of health and wellbeing in the aged easily and safely. The subjects in the experimental group have maintained their exercises till six months after the cessation of the experiment. This seem to illustrate that changes in cognitive perceptual variables and the improvement in health status have reinforced motivation for the continuation of voluntary exercises. A further study is necessary to determine the factors affecting maintainance of muscle strength exercise and to assess the effect of weight training program on bone density.
