북한이탈주민의 심신증상에 대한 M&L 심리치료와 한의치료 치험2례 보고 (Case Reports: Effect of M&L Psychotherapy and Korean Medical Treatment on Psychiatric & Physical Symptoms of North Korean Defectors)

  • Objectives: This study was undertaken by the department of Korean neuropsychiatry, to evaluate the effects of M&L psychotherapy and Korean Medical Treatments on hospitalized North Korean defectors.Methods: We treated two patients with indigenous Korean medical therapies (acupuncture and herb-medicine) and M&L psychotherapy. Beck's Depression Inventory (BDI), Insomnia Severity Index (ISI), Geriatric Depression Scale (GDS), Mini-mental state examination Korean Version (MMSE-K), and Visual Analogue Scale (VAS) were used to evaluate the patients.Results: Following observations were made after treatments. Case 1: the patient had significant improvement in Beck's Depression Inventory (BDI), Insomnia Severity Index (ISI), and Visual Analogue Scale (VAS). Case 2: similar improvements were observed. Here, the patient had undergone Geriatric Depression Scale (GDS), Mini-mental state examination Korean Version (MMSE-K), and Visual Analogue Scale (VAS). In addition, noticeable improvement was observed in other accompanying symptoms.Conclusions: These results suggest that M&L Psychotherapy and Korean medical treatments might be effective for treating the psychiatric and physical symptoms of hospitalized North Korean Defectors.

항생제 과민반응으로 인한 태음인 과민성대장증후군 환자 치험례 (Sasang Constitutional Treatment in a Taeeumin Patient with Irritable Bowel Syndrome(IBS) accompanied by Allergic Reaction to Antibiotics : A case study)

  • Objectives Allergic reaction to antibiotics is associated with increased use. And rate of Irritable Bowel Syndrome(IBS) is on the increase associated with stress, depression, anxiety, or previous intestinal problem. This case study reports significant improvement of patients with diarrhea accompanied by IBS who had suffered from allergic reaction to antibiotics after treatment with Sasang constitutional medicine. Methods This patient was diagnosed as Taeeumin type. Sasang constitutional medicine was taken by the patient, almost three times per day during treatment periods. We measured the degree of the main symptoms such as diarrhea, abdominal discomfort, insomnia, itching etc, using Visual Analogue Scale(VAS). Results The symptoms of diarrhea, abdominal discomfort, and insomnia were improved. VAS score was decreased from 4~5 to 0~1 for about two weeks. Conclusions This case shows that Sasang constitutional medicine treatment can contribute to improve main symptoms accompanied by IBS.

  • Objective: The purpose of this study was to report the effectiveness of modified Guibi-tang in a patient suffering from chemotherapy-induced peripheral neuropathy (CIPN). Methods: A 54-year-old Korean female patient diagnosed with recurrent ovarian cancer had CIPN with other symptoms, such as anorexia, dyspepsia, insomnia, etc. She was diagnosed with Simbiyangheo and hence treated with a modified Guibi-tang. Neuropathic symptoms were assessed using a numerical rating scale (NRS) and a sensory score. Quality of life was assessed using the Functional Assessment of Cancer Therapy/Gynecologic Oncology Group Neurotoxicity (FACT/GOG-Ntx). Results: After 14 days of treatment, the patients showed a decrease in NRS for bilateral limb pain and improvement in other symptoms, such as general weakness, insomnia, dizziness, and headache. Quality of life also increased. Conclusion: Modified Guibi-tang may be considered an optional treatment for CIPN if the patient is diagnosed with Simbiyangheo. Further studies are needed to confirm this finding.

Mediation analysis of chronotype, sleep-related factors, and depressive symptoms among workers: a cross-sectional study

  • Background: This study aimed to examine the impact of chronotype on depressive symptoms and explore the mediating effects of sleep quality, pre-sleep cognitive arousal, and social jetlag in a sample of wage earners. Methods: A total of 3,917 waged workers were surveyed online in July 2022. Logistic regression and mediation analysis were used to assess the relationship between chronotype (morningness, intermediate, and eveningness) and depressive symptoms (Patient Health Questionnaire ≥ 5), and the mediating effects of Insomnia Severity Index (ISI), Pittsburgh Sleep Quality Index (PSQI), and Pre-Sleep Arousal Scale (PSAS). All analyses were adjusted for age, education level, income level, marital status, coffee consumption, alcohol consumption, physical activity, occupation, employment status, and working hours to calculate odds ratios (ORs). Results: The chronotypes of all the participants were divided into morningness (4.7%), intermediate (93.5%), and eveningness (1.8%). Multiple logistic regression analysis showed an increased risk of depression in the eveningness chronotype (OR: 2.96; 95% confidence interval [CI]: 1.51, 5.86). Regarding the mediation analysis, ISI mediated 28.44% (95% CI: 16.39-40.5), PSQI for 31.25% (95% CI: 19.36, 43.15), and PSAS-Cognitive Score (PSAS-C) for 23.58% (95% CI: 10.66, 36.50) of the association between chronotype and depressive symptoms. However, social jetlag did not significantly mediate this relationship. (percentage mediated = 0.75%, 95% CI: -3.88, 5.39) Conclusions: Evening chronotypes exhibit an increased risk of depressive symptoms, which ISI, PSQI, and PSAS-C partially mediated. This suggests that interventions to improve sleep quality and maintain adequate sleep habits may effectively prevent and treat depression in employees with an eveningness chronotype.

  • Objective: This case report describes an 82-year-old man complaining of insomnia with pain after discontinuation of quetiapine. Methods: The patient was treated with the herbal extraction Gilchogeun-dan, and symptomatic improvement was assessed using the Korean version of the Pittsburgh Sleep Quality Index. Results: Following 28 days of Gilchogeun-dan treatment, the patient's symptoms improved. Conclusions: This case suggests that Gilchogeun-dan could be effective in treating insomnia with pain through central nervous depressant activity and analgesic effect.

  • Objective: This study addresses a case of fibromyalgia with insomnia and dry mouth. Methods: A patient diagnosed with fibromyalgia was treated with Korean medicine including herbal therapy with Guibiondam-tang-gagam, acupuncture, pharmacopuncture, moxibustion, and chuna for 16 days. To evaluate the therapeutic effects, sleeping hours, time taken to fall asleep, and surprise on waking were assessed, and a numeric rating scale (NRS) was used to monitor dry mouth. Results: After treatment, the patient's sleeping hours increased, time taken to fall asleep decreased, and the patient woke up surprised less often. The NRS score for dry mouth also decreased. Conclusion: These results suggest that Korean medicine therapies with Guibiondam-tang-gagam have a beneficial effect on insomnia and dry mouth as additional symptoms of fibromyalgia.

  • Objectives: The purpose of this study was to evaluate the effectiveness of insomnia treatment applied with Giungoroen-therapy in relieving adhesive capsulitis. Methods: There is a patient who has had insomnia symptoms for 2 years ago and complained of shoulder pain from 5 months ago. Korean medicine treatment like acupuncture, pharmacopuncture, herbal medicine and Giungoroen -therapy was applied to this patient. As evaluation tools, the numeral rating scale(NRS), a tool for pain evaluation and the Insomnia Severity Index(ISI), a sleep quality evaluation tool, were used. After 12 days of treatment, the range of motion of the shoulder joint increased and the NRS score improved from 8 to 5 and the ISI score improved from 22 to 16. Conclusion: For the treatment of adhesive capsulitis, it can be effective to relieve insomnia by applying Korean medicine treatments, including Giungoroen-therapy.

  • In this case report, we describe the effects of Korean Medicine therapies, including Bunshimgi-eum, on a patient hospitalized with insomnia due to fibromyalgia in a Korean Medicine Hospital. We treated the patient with Bunshimgi-eum for 21 of 27 hospital days. Post-treatment, we used the Insomnia Severity Index (ISI) to measure the severity of insomnia. To determine the severity of multiple pain, including neck, low back, Lt. shoulder, Lt. elbow, Rt. knee, and hand, we used the Numerical Rating Scale (NRS). For evaluation of generic health status, we used the European Quality of Life-5 Dimensions (EQ-5D) scale. After the treatment, the patient's clinical symptoms improved, according to the ISI, NRS, and EQ-5D. The results of this case study suggest that Korean Medicine therapies, including Bunshimgi-eum, may have positive effects as a treatment for insomnia related to fibromyalgia.

  • Objectives: We describe the clinical course in a 53-year-old female patient admitted to a Korean Medicine Hospital with insomnia disorder accompanied by a herniated intervertebral disc and managed with Korean medical treatments. Methods: The patient was treated with herbal medicines, including Shihogayonggolmoryo-tang, acupuncture, pharmacopuncture, and Chuna manual therapy for 33 days. The therapeutic effect was evaluated based on the Insomnia Severity Index, Numerical Rating Scale score, Oswestry Disability Index, and European Quality of Life 5 Dimensions Scale. Results: Post-treatment, the Insomnia Severity Index decreased from 21 to 6, the Numerical Rating Scale score decreased from 7 to 3, the Oswestry Disability Index decreased from 60 to 18, and the European Quality of Life 5 Dimensions Scale increased from 0.35 to 0.85. Conclusions: Korean medical treatments improved the symptoms of this patient with insomnia disorder accompanied by a herniated intervertebral disc. Further clinical research on patients with insomnia disorder is needed.

설문지를 이용한 암환자의 동반 자각 증상에 관한 연구 - 홍삼투여 인체적용시험에 참여한 48명 환자를 중심으로 - (Research on Subjective Symptoms of Cancer Patients Using Questionnaire - Based on the 48 Patients Who Have Participated in Clinical Trial about Korean Red Ginseng -)

  • Objectives: This research was aimed 1) to find out various subjective symptoms cancer patients had, 2) to find out the quantitative difference of symptoms occurring according to the area of cancer, current treatment methods, and relapse of the cancer and 3) to analyze the relationship between the occurrence of symptoms. Methods: This study was a survey about the various subjective symptoms cancer patients had. The subjects were 48 cancer patients in a double-blinded randomized controlled trial about the effectiveness of Korean Red Ginseng (KRG) treatments. Surveys were examined before Korean Red Ginseng (KRG) treatments. The questionnaire consisted of demographic research, patient history and accompanying symptoms. The symptoms part consisted of 4 questions on general symptoms (vertigo, hot flush, rash, insomnia), 2 questions on digestion symptoms (anorexia, dyspepsia), 2 questions on excretions (diarrhea, constipation), and 5 questions on symptoms in different body parts (headache, dryness of lips, chest pain, aphthous ulcer, nose bleeding). The questionnaire was used to research presence of symptoms and relationships between the occurrence of symptoms. Results: A total of 48 patients took part in this survey. Dizziness was the most frequent subjective symptom patients had (64.58%), followed by dry mouth (43.75%), headache (43.75%), insomnia (41.67%), anorexia (37.5%) and chest discomfort (37.5%). The presence of these subjective symptoms was partially related to the patient's history details - type of cancer, method of cancer treatment (chemotherapy, radiotherapy), termination of treatment, presence of pain, fatigue and recurrence, etc. Factor analysis was conducted to analyze the relationship between the occurrence of symptoms. It suggested 5 factors as a result, but there was a limitation that only a low level of correlation was shown among them. Conclusions: We were able to analyze the occurrences and the relationships among them for accompanying subjective symptoms in cancer patients. However the results of the study are limited in that only 48 patients participated. The symptoms in cancer patients showed a tendency for clustering, occurring simultaneously or continuously rather than occurring alone. The study of these symptom clusters is worth further study, for it is similar to the Symptom Differentiation System in Traditional Korean Medicine. For effective approach to the treatment of cancer in Traditional Korean Medicine, further research on the Korean traditional view of cancer patients should be done based on this research, regarding various symptoms and those relationships on a larger scale.
