한국형 통합의료체계 모형 탐색 (In Search of Integrated Health Care System Tailored to Korea)

  • This manuscript treats a new paradigm for the Korean health care system. We give an account of innovative health care delivery and payment models widely discussed in the contemporary US accountable care organization and coordinated care organization. In doing so, we explore a new health care model amenable to foreseeable changes to the health care system. We propose creating an integrated health care system in which the network of health care providers delivers coordinated and comprehensive care for enrolled patients residing within the geographic boundaries served by the provider network; providers may participate voluntarily in one or more networks and assume shared responsibility for patient care and cost; provider networks compete with each other based on cost and quality; and consumers are allowed to choose a network. We expect that the new paradigm will create a financially-sustainable system that assures quality of care and improves patient experience, minimizing the existing system-wide inefficiency through cross-network competition and within-network care coordination.

Innovation and craft in a climate of technological change and diffusion

  • Industrial innovation in Britain, during the eighteenth and nineteenth centuries, stimulated the introduction of the factory system and the migration of people from rural agricultural communities to urban industrial societies. The factory system brought elevated levels of economic growth to the purveyors of capitalism, but forced people to migrate into cities where working conditions in factories were, in general, harsh and brutal, and living conditions were cramped, overcrowded and unsanitary. Industrial developments, known collectively as the 'Industrial Revolution', were driven initially by the harnessing of water and steam power, and the widespread construction of rail, shipping and road networks. Parallel with these changes, came the development of purchasing 'middle class', consumers. Various technological ripples (or waves of innovative activity) continued (worldwide) up to the early-twenty-first century. Of recent note are innovations in digital technology, with associated developments, for example, in artificial intelligence, robotics, 3-D printing, materials technology, computing, energy storage, nano-technology, data storage, biotechnology, 'smart textiles' and the introduction of what has become known as 'e-commerce'. This paper identifies the more important early technological innovations, their influence on textile manufacture, distribution and consumption, and the changed role of the designer and craftsperson over the course of these technological ripples. The implications of non-ethical production, globalisation and so-called 'fast fashion' and non-sustainability of manufacture are examined, and the potential benefits and opportunities offered by new and developing forms of social media are considered. The message is that hand-crafted products are ethical, sustainable and durable.

  • Internet environment and innovative ICT(information and communication technology) have brought about big changes to our lifestyle and industrial structure. In spite of the convenience of Internet, various cyber incidents such as malicious code infection, personal information leakage, smishing(sms + phishing), and pharming have frequently occurred. Information security must be recognized as a key and compulsory element for surviving in a global economy. Strategic roles of information security have recently been increasing, but effective implementation of information security is still a major challenge to organizations. Our study examines the influencing factors of information security and investigates the causal relationship between information security maturity level and organizational performance through an empirical survey. According to the results of our study, personal, organizational, technical, and social factors affect organizations's information security maturity level altogether. This result suggests that when dealing with security issues, the holistic and multi-disciplinary approaches should be required. In addition, there is a causal relationship between information security maturity level and organizational performance, and organizations aim to establish the efficient and effective ways to enhance information security maturity level on the basis of the results of this study.

Antimicrobial Effect of Low Temperature Atmospheric Plasma against Oral Pathogens

  • The purpose of this study was to investigate the antibacterial effect of the low temperature atmospheric plasma device with needle tip designed for easy approach to the oral cavity and root canal against Streptococcus mutans, Enterococcus faecalis and Candida albicans. The antibacterial activities evaluated by measuring clear zone of agar plate smeared with each bacteria after plasma treatment. To quantify antibacterial effects, dilution plate method was used. In addition, scanning electron microscope (SEM) was used for observation of changes in bacterial morphology. As treatment time of plasma increased, the clear zone was enlarged. The death rate was more than 99%. The SEM results showed that the globular shape of bacteria was distorted. These results suggest that needle tip plasma could be an innovative device for prevention of dental caries, and treatment of apical infection and soft tissue diseases.

  • 지식과 정보를 컨텐츠로 하는 디지털시대는 성공과 실패, 승자와 패자의 구분이 뚜렷해지는 변화에 도전하는 자만이 강자가 될 수 있고 주역이 될 수 있는 시대이다. 본 연구에서는 도서관의 궁극적 목적인 정보제공에 관련된 서비스패턴을 개혁시키기 위해 고안된 FISP(Forecasting Information Service Program) 모델을 제시한다. 이 모델은 아날로그시대의 소극적이고 피동적인 정보서비스패턴을 과감하게 탈피한 혁신전략으로 전문사서의 위상을 높이고 직제를 개편하는데 성공하기까지의 방법론을 밝히는 것이다.

  • It is true that the new development in digital technology has Influenced on the definition of animation like it did on the other fields of an, but this doesnt mean this development drastically changes the traditional concept and replaces it with innovative new one. The more the technology is being developed, the mon it needs traditional base that can lead it to what it originally should be. As an example, computer animation companies are constantly seeking for people who have traditional animation background rather than well-trained computer oriented artists. This means digital technology will be no more a technological threat to the people who has traditional animation background. Consequently, when considering the extended concept caused by this new technology, we have to find its base on the traditional animation concept. Since we dont have any fundamental base in traditional animation and technological base in computer animation, we should consider those in advanced countries as a standard. However, in these days, there is a tendency to worship the words themselves such as "digital revolution", "information technology" etc., not the basics behind them. We cannot expect any achievement with this in mind. This study might be a short cut to the students and animators who want to learn and approach computer animation in a right wav.

Development of Facial Rejuvenation Procedures: Thirty Years of Clinical Experience with Face Lifts

  • Facial rejuvenation procedures can be roughly divided into face lift surgery and nonoperative, less invasive procedures, such as fat grafts, fillers, botulinum toxin injections, thread lifts, or laserbrasion. Face lift surgery or rhytidectomy is the procedure most directly associated with rejuvenation, due to its fundamental ability to restore the anatomical changes caused by aging. Various methods of face lift surgery have been developed over the last hundred years, thanks to advances in the understanding of facial anatomy and the mechanisms of aging, as well as the dedication of innovative surgeons. However, no generally applicable standard method exists, because the condition of each patient is different, and each operative method has advantages and disadvantages. Specific characteristics of the skin of Asians and their skeletal anatomy should be considered when determining the operative method to be used on Asian patients. Plastic surgeons should improve their ability to analyze the original aesthetic properties and problem areas of each patient, drawing on scientific knowledge about the aging process, and they should develop the skills necessary to perform various rejuvenative techniques. In the present article, we reviewed various face lift procedures and the current methods of modified double plane face lift, based on our clinical experience of over 30 years.

Potential drug targets in the GPCR-$G{\alpha}_{12}/G{\alpha}_{13}$ signaling pathways

  • GPCRs are large families of cell surface receptors that transmit signals through conformational changes upon ligand activation and an interaction with the heterotrimeric G-proteins. GPCRs regulate the cell-signaling pathways and participate in the regulation of physiological processes through the G-proteins defined by their ${\alpha}$ subunits. A family of 20 G protein-coupled receptors (GPCRs) that provide a large class of tractable drug targets for new anti-inflammatory drugs and, in certain instances, for the treatment of the inflammatory indications such as atherosclerosis, rhinitis, asthma, pulmonary disease and arthritis. In view of the important findings showing that $G{\alpha}_{12}/G{\alpha}_{13}$ regulate the various cellular processes such as actin-stress fiber formation, neurite retraction, platelet aggregation, gene induction, and apoptosis, we became interested in whether, after coupling to the activated GPCRs, the G-proteins and their downstream molecules might be involved in the pathologic processes of chronic inflammatory diseases (e.g., liver fibrosis). In this symposium, the possible link of the G-proteins with the pathophysiology will be discussed with the aim of finding potential new drug targets.

의료급여 관리사의 실무 경험을 통해서 본 사례관리 과정 (Case Management Process identified from Experience of Nurse Case Managers)

  • Purpose: The purpose of this study was to develop a substantive theory of case management (CM) practice by investigating the experience of nurse case managers caring for Medical Aid enrollees in Korea. Methods: A total of 12 nurses were interviewed regarding their own experience in CM practice. Data were recorded and analyzed using grounded theory. Results: Empowerment was the core category of CM for Medical Aid enrollees. The case managers engaged in five phases as follows, phase of inquiring in advance, building a relationship with the client, giving the client critical mind, facilitating positive changes in the client's use of healthcare services, and maintaining relationship bonds. These phases moved gradually and were circular if necessary. Also, they were accelerated or slowed depending on factors including clients' characteristics, case managers' competency level, families' support level, and availability of community resources. Conclusion: This study helps understand what CM practice is and how nurses are performing this innovative CM role. It is recommended that nurse leaders and policy makers integrate empowerment as a core category and the five critical CM phases into future CM programs.

New design concept and damage assessment of large-scale cooling towers

  • The motivation of this paper is to introduce the modern technology of large-scale cooling tower design. Thereby the innovative design concept for the world's largest cooling tower with a height of 200 m is briefly presented (Harte & Kr$\ddot{a}$tzig 2002, Bush et al. 2002). The new concept was considered not only for safety, but also for preservation of the durability of the structure, because cracking damage in large cooling towers in general cause extremely high cost of maintenance and repair. The paper demonstrates numerically the damage process in large cooling towers (Kr$\ddot{a}$tzig et al. 2001), and describes some basics of the numerical finite element approach for damage propagation modelling of shell structure. A prototype is analysed to trace the progressive damage process, whereby the changes in the dynamical behaviour of the structure, as mirrored in its natural frequencies and the corresponding mode shapes, are presented and discussed. Finally, the example shows that such damage processes develop progressively over the life-time of the shell structure.
