Influence of Sperm Fertilizing Capacity on Embryonic Development and Pregnancy in In Vitro Fertilization (체외수정시술에서 정자의 수정능력이 배아의 발생능 및 임신율에 미치는 영향)

  • Objective s: To assess the fertilizing capacity using sperm penetration assay (SPA) to predict the outcome of the in vitro fertilization-embryo transfer (IVF-ET) outcome. Materials and Methods: Semen samples were provided by 129 patients undergoing IVF. We attempted to correlate the extent of sperm penetration under enhanced SPA protocol with the results of fertilization, cleavage, preimplantation embryo development, and pregnancy. Results: Univariate analysis demonstrated a statistically significant correlation between fertilizing capacity and motility, kinetics, fertilization, cleavage and embryo development, and pregnancy rate. By logistic regression analysis, fertilizing capacity was found to be the only variable that was statistically significant with respect to pregnancy rate. Fertilizing capacity, cleavage rate and pregnant rate were significantly higher in pregnant group. However, the fertilization rates was comparable with both group. Conclusions: Lower fertilizing capacity could denote a poorer prognosis for establishing a pregnancy, even after satisfactory fertilization rate is achieved.

  • The ultrasonographic examination with vaginal probe(5MHz) was undertaken in 101 patients at infertility clinic of Eul-Ji General Hospital. This study was performed to evaluate the number of mature follicles per menstrual cycle, the relationship of both ovaries for consecutive ovulatory cycle and the responsiveness of follicular growth followed by administration of ovulation inductant. The results were as follows; 1. The ovulation induction group with clomiphene citrate showed more follicles than natural menstrual cycle group. 2. Each means of numbers of follicles between ovaries showed no difference between natural and ovulatory induction groups. 3. The rate of follicular growth per one menstrual cycle showed higher in the clomiphene citrate induced cycle group. 4. Clomiphene citrate induced group tends to be easier for multiple follicular growth but had no significant difference in statistics. 5. The ipsilateral Vs. contralateral follicular growth rate for consecutive menstrual cycles in both ovaries showed no significant difference between two groups.

  • The generation of artificial gametes is a real challenge for the scientific community today. In vitro development of human eggs and sperm will pave the way for the understanding of the complex process of human gametogenesis and will provide with human gametes for the study of infertility and the onset of some inherited disorders. However, the great promise of artificial gametes resides in their future application on reproductive treatments for all these people wishing to have genetically related children and for which gamete donation is now their unique option of parenthood. This is the case of infertile patients devoid of suitable gametes, same sex couples, singles and those fertile couples in a high risk of transmitting serious diseases to their progeny. In the search of the best method to obtain artificial gametes, many researchers have successfully obtained human germ cell-like cells from stem cells at different stages of differentiation. In the near future, this field will evolve to new methods providing not only viable but also functional and safe artificial germ cells. These artificial sperm and eggs should be able to recapitulate all the genetic and epigenetic processes needed for the correct gametogenesis, fertilization and embryogenesis leading to the birth of a healthy and fertile newborn.

A Correlation of Testicular Size with Testicular Function in Non-Obstructive Infertile Male (비폐쇄성 남성불임증환자의 고환용적과 고환기능)

  • The testicular volume measured by a Prader orchidometer was compared with sperm count in semen, the levels of serum FSH, LH, testosterone, prolactin, estradiol and progesterone in 59 nonobstructive infertile men. 1. The causes of infertility were primary hypogonadisms in 50 patients (35 unknown, 9 Klinefelter syndromes, 6 varicocels) and secondary hypogonadism in 9 (5 isolated FSH deficiencies, 1 hyperprolactinerriia, 3 pituitary hypogonadisms). 2. Decreased levels of serum FSH (less than 4 mIU/ml) did not correlate with testis volume but increased level of serum FSH (more than 20 mIU/ml) were mostly noted in the testis less than 10ml. 3. Decreased level of serum testosterone (less than 3 ng/ml) were distinguishably noted in the atrophied testis less than 5 ml. 4. There was no correlation between the testicular volume and the levels of serum prolactin, estradiol, and progesterone. Coclusively, testicular volume less than 11 ml suggests poor spermatogenesis, but normal testicular volume dose not nessarily rule out poor spermatogenesis. Function of Leydig cell is relatively well preserved in atrophied testis of 5 to 10 ml comparing with that of seminiferous tubule.

Korean Medical Treatments for Subfertility 6 Cases, Mainly Managed by FCST, TMJ Balancing Medicine (턱관절 균형의학 치료를 위주로 한 한방 난임 치료 출산 6례)

  • It was observed in the Subfertility Cases that Customized yinyang Balance Appliance (CBA) and Balancing Upper Cervical Alignment Technique of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance are theraputic. Four Subfertility cases were managed with the CBA, combined with acupuncture and herb-medicine. The other cases were managed with the Balacing Upper Cervical Alignment Technique, combined with acupuncture and herb-medicine. Clinical outcome measurement was based on subjective measures and clinical observations. Patients showed positive changes in subjective symptoms after the treatment and these effects maintained over the follow-up period. And each patient gave a birth to a male/female child. Further clinical and biological research with FCST therapy on hormone-related disordres is expected.

  • Different Y mutation in Yq11 occurring de novo in sterile males were first described 19 years ago. Since the phenotype of the patients was always associated with azoospermia or severe oligospermia, it was postulated that these mutations interrupt a Y spermatogenesis locus in the euchromatic Y region (Yq11) called azoospermia factor (AZF). Recently, it became possible to map AZF mutations to different subregions in Yq11by molecular deletion mapping. This indicated that azoospermia is possibly caused by more than one Y gene in Yq11 and the Yq11 chromatin structure. The frequency of AZF mutations in idiopathic sterile males $(5{\sim}20%)$ may indicate a need for a general screening programme for its analysis in infertility clinic. We have experienced a case of deletion distal to Yq11 region in azoospermic patient. So we report this case with a brief review of literatures.

  • Oocyte in vitro maturation (IVM) is an assisted reproductive technology in which oocytes are retrieved from the antral follicles of unstimulated or minimally stimulated ovaries. IVM of human oocytes has emerged as a promising procedure. This new technology has advantages over controlled ovarian stimulation such as reduction of costs, simplicity, and elimination of ovarian hyperstimulation syndrome. By elimination or reduction of gonadotropin stimulation, IVM offers eligible infertile couples a safe and convenient form of treatment, and IVM outcomes are currently comparable in safety and efficacy to those of conventional in vitro fertilization. IVM has been applied mainly in patients with polycystic ovary syndrome or ultrasound-only polycystic ovaries, but with time, the indications for IVM have expanded to other uncommon situations such as fertility preservation, as well as to normal responders. In this review, the current clinical experiences with IVM will be described.

Herlyn-Werner-Wunderlich Syndrome with Central Precocious Puberty: A Case Report

  • Herlyn-Werner-Wunderlich (HWW) syndrome is a rare congenital anomaly of the genitourinary tract comprising uterus didelphys, obstructed hemivagina, and ipsilateral renal agenesis. Patients with HWW syndrome usually present symptoms such as dysmenorrhea, abdominal pain, pelvic mass, and purulent vaginal discharge. If not treated at an appropriate time, complications such as infertility, endometriosis, pyosalpinx, and subsequent pelvic adhesions may occur. Here, we report a case of HWW syndrome in a 7-year-old-girl who was also diagnosed as having central precocious puberty. She was brought to the pediatric department with chief complaints of lump in her breast and vaginal discharge. When she was around 2 months old, she was confirmed to have a single kidney on ultrasonography. We checked her past medical history and diagnosed her as having HWW syndrome based on the results of imaging studies, including abdominal ultrasonography and pelvic magnetic resonance imaging. She underwent treatment with gonadotropin-releasing hormone analogue for 2 years. During 24 months of follow-up, she showed no serious problems or complications. If renal anomalies are identified immediately after birth or in infancy, further screening tests should be conducted prior to menstruation for determining congenital abnormalities of the reproductive tract and vice versa.
