일개 상급종합병원에 근무하는 간병인의 의료관련감염 관리 수행도 및 손의 미생물 오염도에 영향을 미치는 요인 (Influencing Factors on the Performance of Healthcare-associated Infection Control and Microbiological Hand Contamination among Caregivers at a Tertiary Hospital)

  • Purpose: This study was conducted to identify influencing factors on the performance of healthcare-associated infection control and microbiological hand contamination among caregivers at a tertiary hospital. Methods: The participants of this study were 59 caregivers woring at a tertiary hospital. Data were collected from July 1 to 30, 2018. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé test, Pearson's correlation coefficients and stepwise multiple regression by SPSS 23.0 Win program. Results: Multiple regression analysis revealed that factors influencing performance of healthcare-associated infection control were awareness (β=.63, p<.001) and the experience of infection-related education (β=-3.40, p=.042). Regression equations describing the performance of healthcare-related infection control were found to be appropriate (F=27.29, p<.001) and accounted for 68% of variance. Factors affecting the degree of microbiological hand contamination were work experience (β=-0.28, p=.026) and healthcare-related infection performance (β=-0.28, p=.029). A regression equation describing the microbiological hand contamination was appropriate (F=6.10, p=.004) and accounted for 42% of variance. Conclusion: The findings of this study suggest that it is necessary to increase performance of healthcare-associated infection control by caregivers. Also, educations for preventing healthcare-associated infection and guidelines for increasing compliance with healthcare-associated infection control are recommended to improve performance of healthcare-associated infection control.

종합병원 의료종사자의 병원감염관리 인지도 및 실천도 연구 (A Study on the Awareness and Practice of Hospital Infection Control among Medical Workers in General Hospitals)

  • 본 연구는 부산 시내에 있는 종합병원에 근무하는 종사자들을 대상으로 병원감염관리에 대한 인지도 및 실천도에 대해 파악하여 병원감염관리 방안 및 교육 등에 대한 기초자료를 제공하고자 설문조사를 실시하고 이를 분석하였다. 일반적인 변수의 차이를 보기 위한 t-test 및 일원분산분석(one way ANOVA)을 실시한 결과 대체적으로 개인 위생관리에 대한 인지도와 실천도가 낮게 나타났고, 장비 위생관리의 인지도와 실천도는 높게 나타났다. 남성의 개인위생관리를 제외한 전 영역에서 인지도가 실천도보다 높게 나타나 병염감염관리의 인지도에 비해 실천도는 부족하다는 것을 확인하였다. Pearson 상관분석을 통한 결과로는 감염관리에 관한 인지도가 높은 사람이 실천도도 높은 결과를 나타내 병원 종사자들의 감염관리에 대한 인지도를 높이는 것이 병원감염예방에 효과적인 것으로 생각되었다. 따라서 병원감염관리에 대하여 지속적인 교육으로 종사자들의 감염관리에 대한 인지도를 상승시키고, 이를 실천하기 위한 병원 종사자들의 노력도 필요한 것으로 사료된다.

요양병원 감염관리 현황 및 감염관리 담당자의 중요도 인식 조사 (Survey on Infection Control Status and Perceived Importance of ICP (infection control practitioner) in Long Term Care Hospital)

  • 본 논문은 요양병원의 감염관리 현황을 파악하기 위하여 2016년 5월 1일부터 7월 31일까지 영남지역의 인증평가를 받은 156개 요양병원을 대상으로 자료 수집을 하였다. 현황 파악에서 있으면 '예', 없으면 '아니오', 중요도 인식은 5점 척도의 총 85문항으로 구성된 설문지를 이용하였다. 자료 분석에 사용된 설문지는 107부이며 자료 분석은 SPSS/WIN 22.0 Program을 이용하였다. 감염관리위원회는 80.4%에서 있다고 하였으며 중요도는 3.79로 나타났고, 감염관리 담당자는 86.0%에서 있으며 대부분 겸임이다. 감염관리규정은 손위생을 포함한 7개 문항에서 100% 구비하고 있으며 규정에 대한 중요도는 손위생이 4.47로 가장 높게 인식하고 있다. 감염관리 활동은 직원교육이 시행 빈도와 중요도 인식에서 가장 높게 나타났으며, 감염관리 수행률은 멸균물품 유효기간 준수율이 96.3%이고 중요도가 4.64로 가장 높게 나타났다. 요양병원의 감염관리 규정은 어느 정도 갖추어져 있으나 감염관리 전담인력 부족과 요양병원에서 적용 가능한 감염관리 규정 미비로 감염관리 활동이 제대로 이루어지지 못하고 있는 것으로 나타났다. 그러므로 요양병원에 적용 가능한 감염감시기준 및 감염관리 가이드라인 개발과 이를 적용할 감염관리 전담인력이 필요하다.

간호사의 인플루엔자 감염관리 이행에 영향을 미치는 요인 (Factors Influencing Compliance for Influenza Infection Control by Nurses)

  • Purpose: This study was conducted to investigate factors influencing influenza infection control compliance in nurses, including knowledge about, and awareness of infection control. Methods: The study participants were 168 nurses who worked at a hospital in J Province. Data were collected by self-reported questionnaires in April 2015. Collected data were analyzed using SPSS/WIN 20.0 program. Results: Mean score for knowledge about droplet precaution was 12.16 out of 16.00. Mean score for awareness about droplet precautions was 3.49 out of 4.00, and mean score for droplet precaution compliance was 3.33 out of 4.00. There was a positive correlation among knowledge, awareness and compliance (p<.001). Awareness, knowledge and experience of seasonal influenza education were the significant factors affecting the level of compliance (37.8%). Conclusion: Study results indicate that an educational program focusing on strategies to change nurses' awareness would be effective in improving infection control of respiratory virus and droplet precaution compliance in hospitals.

간호대학생의 다제내성균에 대한 지식 정도와 감염관리 이행도 (Knowledge and Compliance Level of the Multidrug-resistant Organisms of Nursing Students)

  • Purpose: This study was undertaken to identify the knowledge and compliance level of nursing students. It was based on self-administered questionnaires about multidrug-resistant organisms (MDROs). Methods: The test group was 184 nursing students who had contact with MDROs during clinical practice. The study was conducted from May 1 to September 16, 2011. Results: The average knowledge level of MDROs was 16.20 (correct answer rate: 67.5%). The nurses' compliance level of MDROs infection control was on average 40.50 (range: 13-52). "Hand hygiene" ranked highest in the compliance level of MDROs infection control. The lowest level of MDRO infection control was the wearing of disposable aprons or gowns. Conclusion: This study demonstrated the nursing students' knowledge and the compliance level of infection control. We suggest that nursing students receive mandated systematic education of MDROs in the nursing curriculum. And clinical practice experience of MDROs infection control is required in hospitals. These steps will help prevent transmission of MDROs in nursing settings.

일부 치위생 학생들의 엑스선촬영에 있어서의 감염관리 실태 조사 (A study on Performance of Infection Control in X-ray Taking for Dental Hygiene Student)

  • 구강진료기관에서의 개인방호의 중요성을 일깨우기 위해 보건대학 치위생과 3학년 재학생을 대상으로 치면세마와 엑스선 촬영 실습 수행과정에서 감염관리 및 엑스선안전관리에 대한 실천 정도를 조사하여 아래와 같은 결과를 얻었다. 1. 치면세마 실습시에는 감염관리에 대해 높은 수준으로 인식하고 철저히 수행하고 있었다. 2. 엑스선촬영 실습시에 감염관리의 필요성에 대한 인식 정도는 비교적 낮았다. 3. 엑스선촬영 실습시에는 필름유지기구를 제외하고는 거의 감염관리를 하지 않고 있었다. 4. 엑스선촬영 실습시에 엑스선안전관리에 대해서는 높은 수준으로 인식하고 실천하고 있었다. 이상의 결과를 종합해 볼 때 엑스선촬영시에는 거의 감염관리가 이루어지지 않고 있다는 것을 알 수 있다. 완벽한 감염관리를 위해 엑스선촬영시에도 철저한 감염관리가 이루어질 수 있도록 필요성에 대한 인식제고와 실천율을 높일 수 있는 교육의 강화가 요구된다.

치과위생사의 근무환경이 감염관리활동에 미치는 영향 (Influence of working environment on infection control activities in dental hygienists)

  • Objectives: The objective of the study was to infection control by analyzing the influence of working environment on infection control activities in the dental hygienists. Methods: The cross-sectional study was based on a survey on a total of 377 dental hygienists working in dental settings. Multiple linear regression analysis was performed to examine the relationship of general characteristics and infection control activities. All statistical analyses were performed using the SPSS for Windows version 20.0, and p<0.05 was considered to be significant. Results: Predictive powers(=Adjusted $R^2$ of the investigated factors such as operation room, sterilization disinfection laundry, dental unit waterline, staff individual, infection control system, personal protective equipment, medical waste, hand hygiene, oral surgical procedures, clinical contact surfaces were adjusted $R^2=0.394$, 0.306, 0.277, 0.244, 0.241, 0.177, 0.165,, 0.154, 0.134, 0.124 respectively. Conclusions: In order to enhance infection control activities, the program development and implementation for the aseptic procedure is very important. The program should include periodic reinforcement of infection control education and regular monitoring of infection control activities.

건강신념모델 기반 임상간호사의 감염관리 수행도 영향요인 (Factors Influencing the Infection Control Practice of Clinical Nurses based on Health Belief Model)

  • 본 연구의 목적은 간호사의 감염관리 중요도와 건강신념이 자신들의 감염관리 수행도에 영향을 미치는 요인을 파악하고자 함이다. 본 연구대상자는 D시의 종합병원에서 근무하는 간호사 142명 대상으로 2016년 12월 10일부터 15일까지 수집되었다. 간호사의 감염관리 중요도와 건강신념 중 인지된 장애성이 대상자의 감염관리 수행도에 영향을 미치는 요인(F=43.61, p<.001)으로 확인되었으며, 감염관리 수행도에 대한 이 두 요인의 설명력은 38.0% 이었다. 따라서, 간호사의 감염관리 수행도를 높이기 위해서는 무엇보다 감염관리활동이 중요하다는 것을 지속적으로 인식시키는 교육이 필요하다. 다만 감염관리 실천이 간호사에게 업무 부담이나 고통으로 인식되지 않도록 하는 제도적 보완도 함께 고려되어야 할 것이다.

임상간호사의 병원감염관리에 대한 중요성 인지도, 실천정도 및 만족도에 관한 연구 (A Study on Clinical Nurses Level of Perception of Importance, Performance and Satisfaction in the control of Nosocomial Infection.)

  • Hospitals accomodate patients who have a high risk of infection due to reduced immunity as well as people who require surgical, medical or other treatments. Consequently, the role of clinical nurses, who come into close contact with these patients is very important in the control of nosocomial infection. This study was done to investigate and compare the level of perception of the importance of the control of nosocomial infections as well as the level of actual performance, and the level of satisfaction with the control of nosocomial infection by the clinical nurses. Thus, the purpose of this study is to contribute basic data for improving policies and educational programs to control nosocomial infection. A summary of the survey results is as follows. 1) The means of scores on all categories of the inquiry were 4. 51 for awareness 4.42 for actual performance, and 3.20 for satisfaction, of a possible high score of 5.00. 2) Correlations of the level of perception of importance between characteristics of nurses and hospital control of nosocomial infection differed significantly according to the type of hospital establishment type (p=.005), age(p=.000), career(p=.000), position (p=.002), and regular conferences on infection control in working departments(p=.003), Correlation of the level of actual performance between characteristics of nurses and hospital control of nosocomial infection diffesed significantly according to type of hospital(p=.000), hospital size (p=.009), working department(p=.000), age(p=.000), career (p=.000), school career(p=.040), position (p=.000), education experience on nosocomial infection(p=.020), and regular conferences on infection control in working department(p=.000). Correlation of degree of satisfaction between characteristics of nurses and hospital control of nosocomial infection also differed significantly according to the type of hospital establishment (p=.003), working department(p=.000), age (p=.000), and regular conferences on infection control in working department (p=.000). 3) Correlation between clinical nurses, level of perception of importance and actual level of performance for the control of nosocomial infection was relatively positive (r=.57, p=.000). Correlation between clinical nurses degree of satisfaction and level of actual performance for control of nosocomial infection was relatively positive (r=.47, p=.000). Correlation between clinical nurses, level of perception of importance and degree of satisfaction degree with the control of nosocomial infection was also relatively positive (r=.27, p=.000).

119구급대원의 감염관리에 대한 인지도와 수행도 관계 연구 (A Study on the level of Recognition and Practices of 119 Emergency Medical Technician about Infection Control)

  • In order to protect Rescue 119 workers exposed on the spot from potential infection, this study identified their awareness and practices of infection control so that it could help preventing them from infection and also provide basic materials necessary for pre-hospital infection control. This study applied questionnaire survey to total 215 Rescue 119 workers at fire stations in Jeonbuk province, Jeonnam province and Gwangju city from July 14 to Sept. 14, 2006 for the benefit of data collection. The questionnaire about possible associations between awareness and practices of infection control consisted of total 46 times across 6 categories such as washing hands during emergency activities ; fluid therapy and injection ; respirator maintenance; individual hygienics ; disinfectant supplies and equipments maintenance ; and control of infectious wastes. And collected data were processed using SPSS statistic program to analyze frequency and percentage, mean and standard deviation, Pearson's correlation coefficient, t-test and one-way ANOVA. As a result, this study came to the following conclusions : In terms of awareness about infection control, our respondents showed highest awareness about infectious waste control, and also showed highest level of practices in washing hands during emergency activities. Throughout all domains, awareness means were higher than practice means. In particular, infectious waste control was the domain of significant differences between awareness and practices. In terms of associations between awareness and individual characteristics, it was found that female rescue worker group and hospital/general hospital career group (before joining the Rescue 119) showed significantly higher awareness on statistic level. In regard to associations between individual characteristics and practices, it was found that female rescue worker group showed higher level of practices than male group on statistic level. This study also analyzed correlations between rescue workers' awareness and practice of infection control. As a result, it was found that the higher awareness was in correlations with the higher practices across all 6 domains including washing hands. In addition, the higher awareness of a questionnaire item was in significantly positive correlations with the higher practice of other items. However, our respondents showed high awareness about anti-infection, but low practices in reality. This indicates necessity of devising possible solutions to improve the practices as much as awareness. Especially, it was noted that major reasons for insufficient practices of infection control guideline come from unhabituated practices and lack of supports for infection-preventing supplies and protective device (mask, etc). Hence, it is necessary to provide more infection-preventing supplies for local rescue workers sufficiently, in parallel with steady habituation of infection control. Furthermore, it is required to manage and study infection control policies even at pre-hospital step in efforts for effective infection control, education and activities.
