Beliefs About Infants of Mothers Rearing 18~24 Month Old Infants and Their Practices (18~24개월 자녀를 둔 어머니의 영아에 대한 신념 및 신념에 따른 양육 실제)

  • Objective: This study aimed to understand mothers' beliefs about infants and their practices. Methods: As a qualitative study, the participants were 11 mothers and their 18-24 month old infants. Data were collected through in-depth interviews and observations. Results: The participants' maternal beliefs about infants were contrasted in terms of infants' abilities and respect for infants. First, the mothers that perceived infants as competent beings, believed that infants can understand daily routine, communicate and empathize, and concentrate. On the contrary, the mothers that perceived infants as incapable beings, believed that infants lack the understanding of daily routine and the abilities of communication, empathy, and concentration. Second, the mothers that perceived infants as subjective beings, respected their infants, and their infants' opinions when making decisions. On the other hand, the mothers that perceived infants as objective beings, believed that infants should be led by adults and their opinions as not being very important. Third, their maternal beliefs were closely connected to their practices. Conclusion/Implications: Since maternal beliefs about infants turn out to be significant in mothers' thoughts and practices with their infants, it is necessary to figure out maternal beliefs about infants and support them to have more positive beliefs.

Surgical treatent of esophageal atresia with tracheo-esophageal fistula (기관 식도루를 동반한 선천성 식도 폐쇄증의 외과적 치료)

  • Esophageal atresia is a rare congenital anomaly and it usually associated with tracheo-esophgeal fistula and other congenital anomalies. The first report of esophageal atresia with tracheo-esophageal fistula was done by Thomas Gibson in 1696. In 1941, Haight performed the first successful primary anastomosis for esophageal atresia. These accomplishments opened the gateway for clinical studies that have resulted in reinforcements and improvement in the care of infants born with this anomaly. From January 1986 to April 1994, 14 cases of esophgeal atresia with tracheo-esophgeal fistula were diagnosed in Kyung Hee Uinv. Hospital. There were 9 male and 5 female infants. 12 infants were Gross classification type C and 2 infants were type A. The average body weight was 2.7$\pm$0.4kg and Waterson Category A contained 4 infants, B contained 3 infants and C contained 7 infants. Among these infants, 9 infants were underwent anastomosis procedures. We performed retropleural approach in 6 infants, transpleural approach in 2 infants and 1 infant was performed colon interposition through substernal space.By the method of anastomosis, end-to-side anastomosis was performed in 5 infants, end-to-end anastomosis in 3 infants and esophagocologastrostomy in 1 infant.The former 8 infants were Gross classification type C and the latter was type A. Among the type C infants, 6 infants were anastomosed with one layer interrupted suture and 2 infants with 2 layer interrupted suture. Post- operative death was in 1 infant and 8 infants were discharged with good result and have been in good condition.

Lipid Metabolism of Korean Breast-Fed Formula-Fed Infants (모유영양아와 인공영양아의 지질 대사)

  • The difference in lipid metabolism between breast-fed(BF) and formula-fed(FF) infants were studied in 31 Korean infants at 1, 2 and 3 months postpartum. The formulas had more total lipids(TL), triglycerides(TG) and phospholipids and less cholesterol(CHOL) and free fatty acids(FFA) than the breast milk. The milk consumption of the FF infants was significantly greater than that of the BF infants with a wide individual variation. As a result, the FF infants appeared to consume more TL, TG and PL and less CHOL and FFA than the BF infants during 3 months. The lipid contents of the breast milk tended to decrease in due course of lactation, therefore the intake of lipids of BF infants was reduced during the first three month of lactation. The plasma TG and CHOL levels of infants at 3 month were not significantly different between the BF and FF infants, but the PL level of the BF infants was higher then that of the FF infants. The plasma levels of TG and CHOL were not significantly correlated with the intakes of TL, TG, CHOL, PL and FFA, respectively. The PL level, however, was positively correlated with the intakes of CHOL and FFA, respectively and negatively correlated with PL intake as well as 18 : 2w6 fatty acid content of the breast milk or the formulas. The fecal weight and fecal loss of TL of the FF infants were greater than those of the BF infants during 3 months, however, apparent lipids digestibility was not significantly different between the BF and FF infants. The fecal excretions of CHOL and bile acids of FF infants were substantially higher than those of BF infants during 3 months. New sterol balance showed a particularly large difference between the BF and FF infants. The value of the net sterol balance of the BF infants was negative, but that of the FF infants was positive. This study shows that the consumption pattern of various lipid components of BF and FF infants were different as a result of different lipid compositions between breast milk and formula. The significantly lower sterol balance of the BF infants than the FF infants may have been derived from the unique dietary characteristic of breast milk.

Feeding Mode and Evacuation Pattern of Breast-Feeding and Formula-Feeding (모유 영양과 인공 영양의 수유 양식 및 배변 상황)

  • Feeding mode and evacuation pattern of 9 breast-fed(BF) infants and 22 formula-fed(FF) infants were monitored at 1, 2 and 3 months postpartum in Korea. The daily feeding volume to the BF infants was significantly less than that to the FF infants at 2 and 3 months postpartum. And the daily feeding voloume to the BF infants decreased significantly with age, whereas that to the FF infants increased significantly. These data indicated that the frequency of daily feeding decreased with age in bofy infants' intake per feeding increased whereas the BF infants' did not. As a result, the BF infants could not adjust their intakes. The frequency of daily evactation of the BF infants was higher than that of the FF infants, but the amount of daily evacuation of the BF infants was less than that of the FF infants.

Open Heart Surgery in Infants Weighing Below 10 kg. (영아 [10 kg] 개심술 환아의 임상적 고찰)

  • The principal aim of surgery for congenital heart anomalies is the establishment of normal hemodynamic function. Palliative and corrective operations are selected with time to attain this end with minimal risk. In recent years, as operative mortality after primary total correction is lower than the mortality after early palliation and delayed correction, corrective operations in infants have increasingly supplanted palliative ones. Two hundred and eighteen infants below 10 kg with congenital heart anomalies underwent primary surgical intervention at Yonsei Medical Center from March 1979 to June 1985. There were 155 infants with VSD, 35 Infants with TOF, 5 infants with ECD, 4 infants with TGV, 3 infants with DORV, 3 infants with Pulmonary atresia, 3 infants with ASD and PDA, 2 infants with DOLV, and the remainders were Sinus Valsalva rupture, residual mitral regurgitation after total correction of ECD, PAPVR, Cor triatriatum, Truncus arteriosus, and Tricuspid atresia. The overall surgical mortality was 15.1%. In the acyanotic group, 13 infants died among 168 infants, and mortality was 7.7%. But in the cyanotic group, the mortality rate was very high and 20 infants died among 50 infants raising the mortality to 40.0%. These poor surgical results in the cyanotic or complicated group was due to inaccurate diagnosis, improper surgical methods and inadequate post-operative care which should be improved.

The Effect of Physical Aspects of Quality Improvement in Medical Services on Premature Infants' Survival Rate (물리적 의료서비스 품질 개선이 미숙아 생존율에 미치는 영향)

  • This paper on an experiment, using System Dynamics, on the affect of increase in number of beds and medical instruments used for the care of premature infants, which constitute the physical requirements in quality of medical services, on changes in the survival rate of premature in ants that leads to demographic changes of Newborn infants. The model has four sectors: take-in capacity, survival rate of premature infants, demographics without newborn infants and demographics with newborn infants. The model simulates the changes in demographics of the newborn infants from 2002 to 2022. The study results show that the survival rate of premature infants can be increased by improving the physical aspects in the quality of medical services. An average of 1,900 premature infants can survive as a result of the physical quality improvements in medical services, adding up to an increase of 37,300 newborn infants by the year 2022.

  • Purpose: The purpose of this study was to evaluate and compared the growth and development of premature and full-term infants during the 2 years after birth. Method: The participants were 102 infants, 51 each for premature infants, and for healthy full-term infants. Participants in the premature group accounted for 17.5% of all premature infants who were registered at the public health center in G city. Developmental status was evaluated using the Korean Denver II. Results: The catch-up growth of the premature was 100% in weight and in height. Suspicious developmental delay according to the Korean Denver II was 3.9% in normal infants and 31.2% in premature infants. Factors related to the suspicious developmental delay in premature infants were their age and health state at birth. The rate of suspicious developmental delay was higher in infants over 6 months and infants unhealthy at birth. Conclusion: A premature follow-up program, which includes nutrition education to achieve catch-up growth and to prevention obesity, along with continuous developmental screening test for infants and children born prematurely is recommended. Provision for home visits and telephone counseling for premature infants and their families who do not to use the public health center should also be included.

Intakes of Energy, Protein, Lipid and Lactose in Korean Breast-Fed and Formula-Fed Infants (모유 영양아와 인공 영양아의 에너지, 단백질, 지방 및 유당 섭취)

  • Korean breast-fed(BF) and formula-fed (FF) infants(n=31) were studied at their age of 1, 2 and 3 months to compare their intakes of energy, protein, lipid and lactose. Formulas had more energy, protein and lipid than breast milk, but the former had less lactose than the latter. The milk consumption of the FF infants was, however, greater than that of the BF infants with wide individual variation. As a result, the FF infants took in more energy, protein and lipid than the BF infants except lactose. The total daily caloric intakes(kcal/day) of the BF infants were 521.418 and 425 at their age of 1, 2 and 3 months respectively, and those of the FF infants were 676.752 and 723(at their age of 1, 2 and 3 months respectively). Daily protein intakes(g/days) of the BF infants were 9.5, 8.0 and 7.6 at their age of 1, 2 and 3 months respectively, and those of the FF infants were 13.9, 15.5 and 14.8(at their age of 1, 2 and 3 months respectively). The energy and protein of both the BF and the FF infants were deficient compared to the present Recommended Nutrient Allowances, and the deficiency of the BF infants was greater than that of the FF infants. But the energy intakes per kg of body weight of the FF infants at their age of 1 and 2 months were more than the present recommended energy allowances. And the protein intake per kg of body weight of the FF infant at the age of 1 month was the same as the present recommended protein allowances.

Health Problems and Health Services Utilization of Infants Born Prematurely in the U.S.

  • Purposes: The purposes of this study are to describe the health problems experienced by VLBW premature infants and their health care services utilization during the first year of life Method: Eighteen mothers of VLBW premature infants completed a survey questionnaire, asking socioeconomic/demographic information, health/developmental problems experienced by their infants, and their use of health care services. Results: Of the 18 infants, 78% experienced respiratory problems such as cold/running nose and wheezing during the first year and 33% experienced gastrointestinal problems such as vomiting, diarrhea and constipation. Twelve (67%) infants visited the emergency department at least one time and 10 infants (56%) were hospitalized at least once during their first year of age. Interestingly, infants without chronic lung disease visited the emergency department more than infants without chronic lung disease (p=.213). Infants living in non-disadvantaged neighborhoods were hospitalized more than infants living in disadvantaged neighborhoods (p=.000). Conclusions: Health care providers should initiate educating mothers, particularly those living in disadvantaged neighborhoods, about post-NICU discharge health care needs of their VLBW premature infants while their infants were still in the NICU so that unnecessary visits to the emergency department and rehospitalizations can be possibly prevented.

Status and Needs for Nutrition Services for Infants and Preschoolers among Public Health Center Workers and Infants Mothers (보건소 영유아 영양사업 실태와 보건소 종사자와 영유아모의 영양사업 요구도)

  • This study was carried out to investigate the present status of nutrition services for infants in public health centers and the need for nutrition services of health workers and infants mothers. The study subjects were 146 health workers and 197 infants mothers. The results were as follows : At present, the only major nutrition services for infants were vaccination and dental care. Proper nutrition management services were available to infants. Nutrition knowledge scores were 16.8 for health workers and 15.3 for mothers out of 20 possible points. Health workers strongly demanded a well-organized nutrition education program, government support, audio-visual materials and the employment of a community nutritionist. The public health workers, in particular, demanded the development of education programs for breastfeeding and weaning. The infants mothers demanded services of nutrition information and teaching of cooking and menu planning. Based on this, the results suggest that the employment of a community nutritionist and the development of practical nutrition service programs for infants are needed very urgently for public health centers.
