Short-term Associations of Air Pollution with Postneonatal Infant Death in Seoul, Korea, 1999-2003

  • Objective to assess whether exposure to air pollutants is associated with postneonatal infant death, using a timeseries methodology, between 1999 and 2003 in Seoul, Korea.. Methods We investigated the short-term effects of air pollution for 548,725 live births during the study period. The daily count of postneonatal infant deaths from all causes and from SIDS (sudden infant death syndrome) by birth order was analyzed by a Generalized Additive Poisson model, with controlling for the effects of seasonal trends, air temperature, relative humidity, barometric pressure, and day of the week as covariates. Results During the study period, we observed 699 deaths from all causes and 47 deaths from SIDS. We did not find any significant associations between daily mortality and ambient levels of air pollutants except for CO and $NO_2$. The estimated relative risk of postneonatal infant death from all causes was 1.17 (95% CI=1.04-1.32) and 1.16 (95% CI=1.03-1.29) by IQR (interquartile range) for CO and $NO_2$ respectively. Also, we observed no clear trend of the mortality effects of air pollution by birth orders. Conclusion In conclusion, our findings suggest that air pollution, in general, influenced adversely postneonatal infant death from all-cause and SIDS although it was not statistically significant. This study may support that the rationale.

산전 어머니역할교육 프로그램 개발과 그 효과 (Development and Test of Effectiveness of a Prenatal Parental Role Education Program)

  • Purpose: This study was conducted to develop and evaluate the effectiveness of a prenatal parental role education program. Methods: The participants were healthy primiparous women and their healthy newborn babies. 57 mother-infant diads(27 in the intervention group, 30 in the control group). For the intervention group, an additional 4 prenatal parental role education programs and 2 postnatal telephone calls(1st & 3rd week after birth) were provided. Data were analyzed by frequency, chi-square test, t-test and repeated measures ANOVA using SPSS PC+ 10.0 program. Results: Significant differences were found in self-confidence in maternal role performance, mother-infant interaction and infant physical growth between the two groups. This result indicate that the intervention program was effective in improving self-confidence in maternal role performance, mother-infant interaction and in facilitating infant physical growth. Conclusions: The prenatal parental role education program developed by the author was a very effective program in promoting maternal self-confidence, mother-infant interaction, and fostering infant's physical growth at 4 weeks after infant's birth.

  • Purpose: This study was done to investigate effects of Meridian massage on the growth of the infant and on mother-infant interaction, mother-infant attachment, and the mother's satisfaction with her mothering role. Method: The participants were 39 mothers of infants who were seen at one of two urban public health centers located in Yangju or Seoul. Data were collected from February 2008 to December 2008. Infants in the experimental group were given Meridian massage for 50 minutes (lecture 20 minutes, practice 15 minutes, preparation and arrangement 15 minutes) once a week for 6 weeks. Percentages, means with standard deviation, $X^2$-test, repeated measured ANOVA, and ANCOVA were used with the SPSS program to analyze the data. Results: Infants in the experimental group showed an increase in weight & height compared to those in the control group. Mother-infant interaction, mother-infant attachment, and satisfaction with mothering role were significantly higher in the experimental group than the control group. Conclusion: The above finding suggest that Meridian massage should be applied in clinical practice to improve growth of infants, interaction and attachment between infants and their mothers, and the mother's satisfaction with her mothering role.

Factors associated with healthcare utilization for infant falls in South Korea: a cross-sectional online survey

  • Purpose: Falls are a common cause of unintentional injuries in infants. This study was conducted to examine the patterns of healthcare utilization following infant falls in South Korea. Methods: This cross-sectional descriptive study utilized an online survey designed to gather information regarding the general characteristics of parents and infants, fall-related variables, and healthcare use. Results: The most serious falls identified by parents occurred at an average infant age of 6.97 months. Most fall incidents took place indoors (95.7%), and many occurred under the supervision of caregivers (68.0%). Following the fall, 36.4% of the participants used healthcare services. Logistic regression analysis revealed that healthcare use following an infant fall was significantly associated with being a firstborn child (odds ratio [OR]=5.32, 95% confidence interval [CI], 2.19-15.28) and falling from a caregiver's arms (OR=4.22; 95% CI, 1.45-13.68). Conclusion: To prevent and decrease the frequency of infant falls, improvements are needed in both the domestic environment and parenting approaches.

  • The purpose of this study is to investigate the effect on maternal perception of her infant and the infant's behavior development through maternal education designed to provide information of the capabilities and the individual characteristics of her infant. The subjects were 48 pairs of normal infants and mothers at three different hospitals, 24 pairs for the intervention group and 24 pairs of the control group. The survey was conducted from July 30 to October 6, 2001. The results of this study were as the following. A maternal perception of her infant was significantly different between the two groups, more positive in the intervention group than in the control group. The information and understanding of behavioral characteristics of the infant are an important factor influencing maternal perception for her infant and general development of the infant. On the basis of these results, the educational intervention method of this study may be worth utilizing in nursing practice. The practitioners of hospitals have to establish a system that it offers an education of understanding the infant's behavioral characteristics during the hospitalization after delivery.

  • Purpose: The purpose of this study was to investigate the effects of sensory stimulation conducted by primiparas on mother-infant interaction, maternal self-esteem and infant temperament. Method: A non-equivalent control group post-test study design was used. The data were collected from December 1, 2001 to March 30, 2003. The participants were 39 primiparas with normal birth weight infants. They were divided into two groups, 20 mothers in the intervention group and 19 in the control group. The mothers in the intervention group were given instruction on sensory stimulation by the researcher, and the mothers, in turn, used this technique on their infants twice a day during the first 2 months of life. Both groups were measured for maternal self-esteem and infant temperament using the maternal self-report inventory (MSRI) and What My Baby Is Like (WBL) and a film was made of the feeding situation when each infant was between 8 and 10 weeks of age. Results: Compared to the control group, the experimental group had significantly higher scores in mother-infant interaction, maternal self-concept and infant temperament. Conclusion: This suggests that sensory stimulation conducted by primiparas may improve the mother-infant interaction, maternal self-esteem and infant temperament.

효과적인 모유수유 개념 분석 (Concept Analysis of Effective Breastfeeding)

  • Purpose: This study was conducted to analyze and clarify the concept of the effective breastfeeding. Methods: This study used Walker and Avant's process of concept analysis. Results: Effective breastfeeding is how to give infant adequate milk, which fulfills needs of mother and infant, from her breast so that they can be satisfied with the process and results. It included the effective breastfeeding properties as follows: feeding behaviors (positioning, latch on, and suckling), transferring a breast milk to an infant, mother-infant interaction, and satisfying their desires (satisfaction, comfort, mother's self-confidence, infant's adequate weight gain and defecation, adequate breastfeeding interval). The antecedent of effective breastfeeding were anatomical and functional normal breast, breastfeeding knowledge and steady-state of mother, and feeding desire, rooting reflex and normal oral cavity of infant. The consequences of effective breastfeeding were exclusive breastfeeding, infant and maternal health and wellbeing, and achievement of mother and infant attachment. Conclusion: The meaning of effective breastfeeding defined in this study will contribute to develop the effectiveness breastfeeding assessment tool and the nursing intervention for ineffective breastfeeding.

South Korean first-time mothers' knowledge of digestive health problems in infancy and their utilization of health care facilities for digestive health concerns: a descriptive study

  • Purpose: This study aimed to assess the knowledge level of first-time mothers regarding digestive health issues in infancy and to examine the utilization of healthcare facilities for such problems. Methods: Data from 119 first-time mothers of infants under 6 months of age were analyzed. Descriptive statistics, t-test, and one-way analysis of variance (ANO-VA) were conducted using the SPSS software. Results: The average correct response rate for first-time mothers' knowledge of digestive health problems in infancy was 61.9%. The highest correct response rate was observed for infantile colic, while diarrhea had the lowest. Less than 50% of mothers received education on infant digestive health problems across all categories. Among digestive health problems in infancy, diarrhea exhibited the highest rate of healthcare utilization, whereas infantile colic had the lowest. First-time mothers' knowledge of digestive health problems in infancy varied based on maternal age (t=-3.66, p<.001), education level (t=-2.26, p=.026), and planned pregnancy (t=3.24, p=.002). Moreover, mothers who received education on infant digestive health problems demonstrated better overall knowledge of digestive health problems. Conclusion: The rate of education regarding digestive health problems during infancy among first-time mothers was < 50%. Furthermore, mothers educated on infant digestive health issues exhibited improved knowledge. Therefore, it is necessary to provide appropriate pre-education to primiparous common gastrointestinal health issues in infants.

  • Purpose: This study was conducted to evaluate the effectiveness of a mother/infant-toddler health program developed to enhance parenting knowledge, behavior and confidence in low income mothers and home environment. Methods: A one-group pretest-posttest quasi-experimental design was used. Sixty-nine dyads of mothers and infant-toddlers (aged 0-36 months) were provided with weekly intervention for seven session. Each session consisted of three parts; first, educating to increase integrated knowledge related to the development of the infant/toddler including nutrition, first aid and home environment; second, counseling to share parenting experience among the mothers and to increase their nurturing confidence; third, playing with the infant/toddler to facilitate attachment-based parenting behavior for the mothers. Results: Following the programs, there were significant increases in parenting knowledge on nutrition and first aid. A significant improvement was found in attachment-based parenting behavior, but not in home safety practice. Nurturing confidence was not significantly increased. The program led to more positive home environment for infant/toddler's health and development. Conclusion: The findings provide evidence for mother-infant/toddler health program to improve parenting knowledge, attachment-based parenting behavior and better home environment in low income mothers. Study of the long term effectiveness of this program is recommended for future research.
