Effects of a breastfeeding coaching program on growth and neonatal jaundice in late preterm infants in South Korea

  • Purpose: This study examined the effects of a breastfeeding coaching program for mothers on growth and neonatal jaundice in late preterm infants (LPIs). Methods: This was a quasi-experimental study (non-randomized intervention) with a time-series design. The study was conducted among 40 LPIs who were admitted to the neonatal intensive care unit of a university hospital in Daegu, South Korea. In the order of admission, the first 21 infants were assigned to the experimental group, and 19 were assigned to the control group. The intervention program consisted of home- based and web-based practical breastfeeding support education for mothers across a total of 5 sessions. Infant growth was measured using body weight, length, and head circumference, and neonatal jaundice was assessed using transcutaneous bilirubin levels. Results: The likelihood of breastfeeding for infants in the experimental group at 4 weeks after discharge was the same as on the day of discharge, whereas it steadily decreased in the control group. There were significant differences in head circumference between the groups. However, weight, length, and transcutaneous bilirubin levels did not show a significant group-time interaction. Conclusion: A formal breastfeeding coaching program should be considered in clinical settings and at home within the first few weeks postpartum.

A Study on the child care system in Sweden (스웨덴의 아동보육제도 연구)

  • This study is to describl the child care system in Sweden and to understand how the progressive child care system has been developed. The sex categories of the child care issues were studied based on a review of the literature related to the Swedish child care, family welfare system and the socio-economic information on Sweden. Six categories were : (1) historical and socio-economic background of child care system, (2) supply of and demand for the child care service and the types of public and private child care, (3) child care program activities, (4) child care staff training system, (5) administration and financial support system for the child care, and (6) the family policy and the family support programs in Sweden. For the Korean child care system, this study on the Swedish child care implied that : (1) To provide a progressive system of the child care system in Korea will require serious discussion about concentration of the child care administration system which is currently distributed to Ministry of Education and Ministry of Health and Welfare. (2) The extensive family support programs such as the parental leave and flexible work conditions for women are needed for the infant care by parents at home. (3) to expand the child care services in Korea, public support should be primarily focused on the supply of the public day care centers even though the demands for the various child care services are to be met by public financial support. (4) Most of all, societal recognition that all children are the resposiblity of our society is needed to develop a progressive child care system in Korea.

An Analysis of Structural Relationships of the Cognitive and Communication Ability Related Variables on Infants (영아기 인지·의사소통 발달 관련요인 간의 구조적 관계 분석)

  • This study investigated the structural relationships among infant's cognitive and communication ability, temperament, teacher-infants interaction, and mothers' parenting stress. Subjects used in this study were 249 infnats whose mean age of 30.21 months, and who were attended in preschool in S city. Research instruments used in collecting data were 'K-ASQ(Korean Ages and Stages Questionnaires)', 'EAS(Emotionality, Activity, and Sociability-Temperament Survey for Children-Parental Ratings)', 'PSI/SF(Parenting Stress Index/ Short Form)', 'ECOI'. The collected data were analyzed through SPSS 18.0 and AMOS 18.0 program. Pearson's correlation and Structural equation model analysis were used to analyze the data. Findings are as follows. First, there were positive correlations between infant's cognitive and communication ability with social-ability temperament, teacher-infants interaction. There were negative correlations between infant's cognitive and communication ability with infant's emotionality temperament, mothers' parenting stress. Second, social-ability temperament directly affected disposition toward cognitive ability, but in-direct effect was not significant. Emotionality temperament partially mediated the effects of parenting stress toward cognitive ability. Third, social ability directly affected disposition toward language ability, but in-direct effect was not significant. There has no significant path of temperament to teacher-child interaction, but teacher-child interaction has direct effect to infant's cognitive ability. Finally, all path about infant's communication ability were not significant.

Effects of Maternal Empowerment Program on Stress, Anxiety, Depression and Parenting Confidence in Mothers of Preterm Infants in the Neonatal Intensive Care Unit (신생아집중치료실 입원 미숙아 어머니를 위한 임파워먼트 프로그램이 스트레스, 불안, 우울 및 양육자신감에 미치는 효과)

  • Purpose: This study aimed to evaluate the effects of an empowerment program on maternal stress, anxiety, depression and parenting confidence. Methods: A total of 44 mothers of preterm infants were assigned into an experimental or a control group (n=22 each). The experimental group received the usual nursing care and 7 sessions of an empowerment program. The control group only received the usual care. The program was implemented from June to December, 2016 in the neonatal intensive care unit of K university-affiliated hospital in Daegu, Korea. The outcome variables measured were parental stress (PSS: NICU), anxiety (STAI), depression (CES-D) and parenting confidence. Data were analyzed using t-test or repeated measures ANOVA. Results: Scores for both parental stress (t=3.07 p=.004) and depression (F=3.76, p=.26) were significantly lower in the experimental group than in the control group. However, there were no significant differences in anxiety between the groups (F=0.79, p=.505). Parenting confidence scores (F=9.05, p=.001) were significantly higher in the experimental group than in the control group. Conclusion: A maternal empowerment program can be an effective means of reducing parental stress and depression as well as enhancing parenting confidence, for mothers of preterm infants.

A Cost Benefit Analysis of Individual Home Visiting Health Care (맞춤형 방문건강관리사업의 비용-편익분석)

  • Purpose: The purpose of this study is to evaluate the costs and benefits of individual home visiting health care using secondary data and literature review. Methods: The total number of subjects was 1,008,837. A specific program was classified into disease management, care of infant, child and women, or elderly care. The costs and effects of a program were identified from a societal perspective, and the effects were converted into monetary terms or benefits. The total cost was calculated in the way that medical expenses, travel costs and productivity losses were offset by the decrease in benefits and thus only the program budget was included in the total cost. Results: The total program cost was 47.6 billion won per year and the total annual benefit was estimated at 435.6 billion won. The benefits of arthritis management were the biggest among disease management programs. The net benefit was 388.0 billion won per year and the benefit/cost ratio was 9.16. Conclusion: Home visiting health care was validated to be economically effective. It made a positive contribution to improving the health status of vulnerable populations and reducing medical expenses. These results suggest that home visiting care should be extended more broadly to vulnerable populations.

The Evaluation of Midwifery Program Through the Midwifery Leadership Training Program (조산수습과정 지도자 강습회를 통한 조산교육 평가조사연구)

  • The purpose of the study was to evaluate the educational content which had been given by midwifery training program. It was hoped that this result would help. It was sponsored by com-munity health worker plan effective health education. College of Nursing Ewha Womans University and The Korean Nurses Academic Society during the November 19 thru 24, 1979. It was carried out on July through on September 1980, and involved 22 community health workers. The results were as follows: 1. Most of the community health workers came from Seoul & Pusan areas and have been working at the hospitals. There were 31.82% of Head Nurses, 27.2% of Staff Nurses, 22.73% Nurse Supervisons, 13.6% of Nurse Directors and 4.5% of educational coordinator for Nurses. These participant had nurse-midwifery lincences by 63.64%. None of there had just midwifery lincences. 2, Age structures of the study population shows 31.82% of whom are.26-30 years and 22.73% of whom are 36. 40 years of age. This shown that seniority proportion is higher than the younger. There are 31.82% of 1-5 years, 27.27% of 6-10 year and 11-15 years, respectively by work career. 3. There are 54.55% of the institutions have opened their own midwifery training course for their nursing staff members. Because of lack of the facilities, shortage of instructors, and problems of administrative process. 4. According to the institution which opened for midwifery training courses, the participant was responsible for “midwifery”“Infant care”“MCH”“practice of midwifery”“Nursing adjustment”and“F. P.”5. During the midwifery couse, there were 8 institution who used the textbook and 4 institution who did not. Least of there referned to content matinals which was given by the sponsored. 6. There are 7 insititues who kept their training courses with other professional helps such as physicians., professiors and nurses. Some problems are pointed out by respondents such as“conflict with residents”“poor suportive administration”and“lake of manpower”. 8. The participant showed that they learned new knowledge as trends during this programs for there quality work so it need (one or twice times) a year. But they suggested that it needed more emphasis on the“maternal health care”and“role of the nurse-midwifery”. 9. The analysis of the results are as follows within the 6 areas which are given by the sponsored: There are highest ranks between“basic theory & family planning”“role of midwifery & nursing practice”. In the prenatal care the highest rank ware related to“health risk”on“idenify of risk symtoms”. In the health care areas which related to delivery, the responsers were related to“general conditions”or“high risk criteria”. In the health care area which related to high risk maternity care. In the neonatal health care, the highest rank was related to”health assessment of normal infant”. In the infant health care the responses was related to“abnormal symptoms”and“risk symptoms”. Actually, the participants show that they are more interested in“role of midwifery”“health assessment”and “high risk maternity care”are which emphasised on health promotion, health maintenance & disease preventive. 1) The midwifery training program need higher education for midwifery on a regular basis. 2) Within the open institution of midwifery training program, the nurses must be supported by their own institution and administry of social welfare must give systematic support. Also non-open institution must be open very soon. 3) All health workers including the residents & other workers, must cooperate for their phased common good of impovement of the maternity health. 4) Administration agonies & education institutions must provide the curriculum facilitis and administration systems which are needed for training of nurse-midwifery.

Oral Health Practices of Some Local Child Care Teachers Oral Health Education Awareness and Need Survey (일부 지역 보육교사의 구강건강관련 실천도 및 구강보건교육 인식도·필요도 조사)

  • Purpose. Infant oral health care forms the basis of lifelong health, and the role of child care teachers in contributing to the quality level in the formation of correct habits at this time throughout the infant's health care and education is very important. The purpose of this study was to examine the relationship between the perceived oral health status of nursery teachers and the necessity of oral health education, and to emphasize that the oral health of teachers is closely related to the oral health of infants and young children. Methods. This study was conducted from May to June 2018 by surveying private nursery teachers in some parts of Gyeongsangbuk-do province by using the self-entry method. The research tools consisted of the general characteristics of subjects, subjective oral health status, oral health practice, oral health education awareness, and oral health education needs. We used shceffe as a post hoc test to compare the differences between groups after frequency analysis, independent t-test, and one-way ANOVA analysis. The linear regression analysis was conducted to determine the relationship between perceived oral health status, oral health practice and awareness, and oral health education needs. The statistical level was .05. Results. According to the results of this study, the oral health related practice according to the demographic characteristics is at the final education level, and the oral health related awareness is age, marital status and oral health education needs. The results showed statistically significant differences in final educational attainment and cohabitation. Regarding the relationship between perceived oral health status, oral health related practice and awareness, and oral health education needs There were significant differences in oral health practice(β =.329) and oral health education needs (β =.158). Conclusion. Based on the above results, the child care facility can emphasize the importance of the role of the child care teacher in the oral health care of infants, and the basic data for the establishment of the child care management program in the future by reflecting the role of the teacher in the development of the child care management program of the child care facility. Needs of Center-based Child Care Health Programs in Parents with Ill Children (급.만성질환 영유아 부모의 건강관리 특수보육에 대한 요구)

  • Purpose: This study was conducted to investigate the recognition on center-based child care health programs for parents with ill children. Methods: Cross-sectional descriptive study with convenient sampling method was used and 136 parents were recruited from two university hospital outpatient departments and hospitalized wards, and they completed a structured questionnaire. Results: Majority (57.4%) of parents experienced a leaving work early or being absent from their work due to children's illness. Most dissatisfying factors in child care centers were child health management. Most of parents agreed on the need for a center-based child care health program, and wanted a better management of health care by health professionals in child care centers. Conclusion: These results support that there is a great demand for center-based child care health programs. These special programs are needed for reducing a child rearing burden for working mothers and overcoming the low birthrate in Korea. Health-care programs in child-care settings can help parents meet the health needs of their children while reducing absenteeism from work, thereby contributing to job stability. These results suggest discussions of the related policy with child care centers responsible and national health and educational authorities.

The Relationship between Practice Level and Obstacle Factors for National Childcare Curriculum of Infant Teachers (영아교사의 표준보육과정의 실행 수준과 장애 요인과의 관계)

  • The purpose of this study was to analyze the relationship between practice level and obstacle factors for national childcare curriculum of infant teachers. The subjects were 204 infant teachers who are working in day care centers in Jeonnam. A questionnaire was made by the researcher on the basis of relevant materials. The data were collected from May 6 to June 13. The data were analyzed by statistical methods such as frequency, descriptive, ANOVA and Pearson-correlation with the SPSS Windows 18.0 program. The results are as follows: First, practice level for national childcare curriculum of infant teachers was relatively high level. There were no significantly different practice level of national childcare curriculum according to teacher's career, teacher's academic degree and teacher's workplace type. Second, obstacle factors for national childcare curriculum of infant teachers was normal level. There were no significantly different obstacle factors of national childcare curriculum according to teacher's career, teacher's academic degree and teacher's workplace type. There were negative correlation between practice level and obstacle factors for national childcare curriculum of infant teacher's.

Text Mining Analysis of the Online Counseling Contents of Nursery School Teachers (텍스트 마이닝을 활용한 어린이집교사 온라인 상담의 내용분석)

  • Objective: This study aimed to analyze the counseling contents of daycare center teachers by using text mining and semantic network analysis methods to find the necessary support directions for daycare teachers and to improve the quality of child-care. Methods: Five hundred thirteen cases of counseling recorded on the open bulletin board of online counseling (Naver Bands for Nursery Teacher Counseling) were collected, and frequency analysis, centrality solidarity analysis, and machine learning-based topic analysis were conducted using the NetMiner4.3 program. Results: First, 'teacher-to-child ratio' was highest in the frequency. Second, 'colleagues' were all high in all centrality analysis. Third, machine learning-based topical analysis shows that the topics were categorized as subjects about 'childcare and education', 'working environment that supports professional development' and 'working condition', and among them, 'first-time teacher concerns' accounted for 44% of the total counseling content. Conclusion/Implications: This study implied that it is necessary to provide high-quality child-care and education to infants by lowering the 'teacher-to-child ratio', and a systematic program is needed to help improve effective communication skills in interpersonal relationships such as between parents, fellow teachers, and principals. In addition, self-development and efforts to improve teachers expertise should be prioritized in order to improve infant care quality and quality of teachers.
