Purpose: The purpose of this study was to examine the effect of a maternal self-efficacy promotion program for Korean primiparas on maternal confidence and mother-infant interaction. Method: This study was non-equivalent quasi-experimental research with a control group selected by purposive sampling. Based on Bandura’s (1986) self-efficacy theory, the research team developed the maternal self-efficacy promotion program. The program included achievement experiences, verbal persuasion, and vicarious experiences. The program was provided through face-to-face teaching at the time of discharge, counseling sessions over the telephone, and a home visit. Subjects also learned about parenting skills using a videotape developed by the research team. Sixteen primiparas were recruited to the experimental group at one hospital in Kyongi province, and fifteen primiparas who were matched according to socioeconomic status were recruited to the control group at a postpartum care center in the same province. Results: Subjects in the experimental group demonstrated significantly higher maternal confidence and mother-infant interaction than those in the control group. Conclusion: The maternal self-efficacy promotion program appears to be an effective nursing intervention for parenting of first-time mothers in Korea.
In recent years, reductions in infant mortality have mainly been accomplished by improving the survival of premature and low birth weight infants, however premature infants still remain at great risk. The purpose of this study was to review the maternal child health service related to premature infants and to provide a future direction for improving maternal child health (MCH) in Korea. We reviewed two MCH services which are directly related to premature infants: 1) a registry and financial support program for families with a premature infant, and 2) financial support to build neonatal intensive care units in rural public hospitals. Suggestions are made for the development of a national vital signs record system to identify high risk infants and to monitor the trends in infant mortality due to prematurity. Prevention efforts and preconception care for childbearing women is also an important strategy to reduce the rate of preterm births. Finally, we need consider long-term follow-up plans for premature infants for a successful transit to the special education system. Developing MCH policy related to premature infants that decreases the occurrence of premature may decrease infant mortality, and also improve maternal and child health services.

The sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to December 14th in 1999. For the intervention group received programmed education that was consisted of discharge education, telephone counselling, and home visiting for maternal education. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program. This study was focused on the results of six months time point. The results were as follows: 1. There was significant difference in the mean score of mother-infant interaction(NCAST) between two groups. Intervention group showed higher scores in the subscales of sensitivity to cues, cognitive growth fostering, and caregiver total. 2. There was no significant difference in the mean score of child rearing environment (HOME) between two groups of six-month-infants. But when each subscale of HOME was examined, intervention group showed higher scores in the dimensions of opportunities for variety in daily stimulation, maternal involvement with child, and emotional, verbal response. 3. Six-month-infants of the intervention group showed significantly higher GQ in the Griffiths mental development scale. This study aimed at improvement of the educational program for posrpartal mothers and the newborn babies. Subjects were 40 primiparae who were admitted to the obstetric ward in a general hospitals in Seoul from August 20th to September 10th, 1995. Subjects were those who had no labor pain at the admission time, had no complications during labor and delivery, gave a birth to a healthy baby, and agreed to participate in this study. All subjects were well educated and were in well-to-do group. Most of them received antenatal care adequately. They were tested on knowledge and self-confidence in the area of postpartal self-care and infant care two times, one at the admission time and the other prior to discharge. After the first test, nurses in a maternity ward and nursery taught them on postpartal self-care and infant care. Education consisted of a 1 hour planned program and incident teachings done at bed side. Four tools were developed by authors based literature review. The test tool for knowledge of postpartal self-care consisted of 15 items which included the definition of postpartum, dangerous symptoms in postpartum, lochia, time to begin coitus, postpartum exercise, sitz-bath, and perineal care. The test tool for self confidence in the newborn baby care included umbilical cord care, vaccination, breast feeding, emergency care for baby. Analysis of demographic data were analyzed with calculation of percentage. Score differences between the first test and the second test were analyzed with paired t-test. SAS / PC (Release 6.04 for DOS) program are as follows. 1. Mothers' knowledge of postpartal self-care increased significantly after education (t=10.04, df=38, p=0.0001). 2. Mothers' self-confidence in postpartal self-care increased significantly after education (t=6.53, df=38, p=0.0001). 3. Mother's knowledge of the newborn abay care increased significantly after education (t=9.74, df=38, p=0.0001). 4. Mothers' self-confidence in the newborn baby care increased significantly after education(t=8.22, df=38, p=0.0001) Suggestions for further studies and nursing practice were as follows. 1. We suggest studies by randomized control-group pretest-posttest design or nonequivalent control group pretest-posttest design will be done. 2. We suggest follow-up studies to find if mothers's confidence will last or not after discharge. 3. We suggest general hospitals to establish a phone-counseling system.
