Direct detection of cariogenic streptococci in metal brackets in vivo using polymerase chain reaction (교정용 메탈 브라켓에서 자가중합효소연쇄반응을 통한 치아우식증 원인균의 탈출)

  • Streptococcus mutans and Streptococcus sobrinus are major etiological agents in enamel demineralization around orthodontic appliances. This study was designed to examine the prevalence of these streptococci on orthodontic brackets in vivo using polymerase chain reaction. Four incisor brackets in the upper and lower arches were removed and collected from 80 patients at the time of debonding. The genomic DMA of adhered bacteria was extracted and each dextranase gene of S. mutans and S. sobrinus was amplified using the specific oligonucleotide primers. The results showed that the maxillary incisor brackets were colonized by both cariogenic streptococci to a somewhat higher degree than that taken from the mandible. The prevalence of S. mutans was $50.0\%$ on the maxillary incisor brackets and $33.8\%$ on the mandibular incisor brackets, and that of S. sobrinus was $17.5\%$ and $15.0\%$, respectively. Both species were detected on the maxillary incisor brackets of 7 patients $(8.8\%)$ and the mandibular incisor brackets of 5 patients $(6.3\%)$. These results suggest that cariogenic streptococci can adhere to the incisor brackets and may be resident species on the incisor brackets.

A STUDY ON THE ANGLE OF ARTICULAR EMINENCE AND THE INCLINATION OF ANTERIOR TOOTH RELATED TO FACIAL TYPES (안모유형에 따른 악관절융기와 전치의 경사도에 관한 연구)

  • The purpose of this study was to evaluate the difference and relationship between the slope of articular eminence and the inclination of upper and lower incisor teeth, which are related to the functional stability of occlusion in mandibular protrusion, according to Ricketts' facial types, by the use of lateral cephalogram and SAM2 articulator in 68-adult normal occlusion without tooth missing, orthodontic treatment and occlusal equilibration. The results of this study were as follows : 1 . The angle of articular eminence slope to occlusal plane in brachyfacial type was steeper than that in dolichofacial type, but the angle of articular eminence slope to SN plane and FH plane was not different between facial types. 2. The upper incisor axis in dolichofacial type was steeper than that in brachyfacial type, but lingual surface slope of upper incisor was not different between facial types. 3. In all samples there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to SN plane and FH plane, and in mesofacial type there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to SN plane, FH plane and occlusal plane, and in brachyfacial type there was a positive correlation between the angle of articular eminence slope and the lingual surface slope of upper incisor to occlusal plane. 4. In all samples there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to SN plane, and in mesofacial type there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to occlusal plane, and in brachyfacial type there was a positive correlation between the angle of articular eminence slope and the angle of Dc-Gn to SN plane and FH plane. 5. In all samples there was a positive correlation between the angle of Dc-Gn and the lingual surface slope of upper incisor to SN plane and FH plane, and in mesofacial type there was a positive correlation between the angle of Dc-Gn and the lingual surface slope of upper incisor to SN plane. 6. In all samples and facial types there was a negative correlation between MP to 1 axis and condylar incisal angle.

  • Anatomic abnormalities of teeth development include the presence of a single maxillary central incisor at midline instead of two central incisors. The incidence of cases with a single maxillary central incisor is approximately 1 in every 50,000 live births. single median maxillary central incisor(SMMCI) may be a integral manifestation of any one of a number of syndromes. It has been reported an association with several midline disorders which have varied in severity. A SMMCI has also been reported as an isolated finding. But in some cases, it has been considered as an incomplete expression of autosomal dominant holoprosencephaly. So any patient who appears for treatment with a SMMCI should be referred for a detailed medical examination to exclude other associated systemic abnormalities and for mutation analysis to facilitate correct diagnosis and the provision of appropriate genetic counseling. Early orthodontic treatment is needed to minimalize emotional trauma of child. This case report was about three patients with a SMMCI as isolated finding. The purpose of these case reports was to present cases of single maxillary central incisor not associated with other disorders.

Customized maxillary incisor position relative to dentoskeletal and soft tissue patterns in Chinese women: A retrospective study

  • Objective: To provide reliable prediction models based on dentoskeletal and soft tissue variables for customizing maxillary incisor positions and to optimize digitalized orthodontic treatment planning. Methods: This study included 244 Chinese women (age, 18-40 years old) with esthetic profiles after orthodontic treatment with fixed appliances (133 in group I: 1° ≤ The angle between the nasion [N]-A point [A] plane and the N-B point [B] plane [ANB] ≤ 4°; 111 in group II: 4° < ANB ≤ 7°). Dental, skeletal, and soft tissue measurements were performed on lateral cephalograms of the participants. Correlation and multiple linear regression analyses were used to determine the influence of dentoskeletal and soft tissue variables on maxillary incisor position. Results: The ideal anteroposterior position of the maxillary incisor varied between sagittal skeletal patterns. The position of the maxillary incisor correlated with the sagittal discrepancy between the maxilla and the mandible (ANB), protrusion of the midface, nasal tip projection, development of the chin, and inclination of both the maxillary and mandibular incisors. Distance from the maxillary central incisor to nasion-pogonion plane predicted using multiple linear regression analysis was accurate and could be a practical measurement in orthodontic treatment planning. Conclusions: Instead of using an average value or norm, orthodontists should customize a patient's ideal maxillary incisor position using dentoskeletal and soft tissue evaluations.

  • The purpose of the present study was to evaluate the effect of incisor protrusion on the mucogingival parameters including the width of attached gingiva. Thirty-seven young adults with lower anterior crowding were selected for this study. From the study model, the degrees of relative and absolute protrusions were measured for each lower incisor. Clinical non height, the width of keratinized gingiva, probing depth and the width of attached gingiva were measured with digital vernier calipers and Florida Probe System. Through comparing the difference of the above measurements between protruded and non-protruded incisors, and correlation analysis between each measurement, following results were obtained: 1. The protruded incisor showed narrow width of keratinized and attached gingiva comparing to non-protruded incisor. 2. The protruded incisor showed greater clinical crown height comparing to non-protruded side while there was no difference in the probing depth between protruded and non-protruded side. 3. The difference in the width of attached gingiva between protruded and non-protruded incisors showed higher significance in the lateral incisor than in the central incisor. 4. The degree of relative protrusion showed higher correlation with the width of attached gingiva than the degree of absolute protrusion. 5. Clinical crown height showed higher correlation with the width of attached gingiva than the degree of protrusion.

  • The primary cause of missing teeth vary depending on the region. The loss of posterior teeth is mainly due to dental caries, whereas that of the anterior teeth occur primarily due to trauma at the age of standing and walking and secondarily from the rampant dental caries. Particularly, reduction of the arch length in the cases of premature loss of primary teeth may compromise the eruption of succedaneous permanent teeth. This may result in crowding and impaction of the permanent teeth, and asymmetry of arch, thus a careful consideration for space maintenance should be made in such cases. Space maintainer is required in the case of premature loss of primary posterior teeth, because space loss result from the approximate and centrifugal movement of the neighboring teeth. Generally, in the case of primary incisor, space loss occurs when 1) tooth contact is relevant, 2) crowding in primary dentition is present, and 3) a primary incisor is lost before the eruption of primary canine. Contrarily, in the case of primary dentition with interdental space, space loss will not be observed, mostly when a primary incisor is lost after the eruption of primary canine. Thus, using a space maintainer in cases of premature loss of primary incisor has been introduced primarily not for the purpose of space maintaining but for an aesthetic purpose, prevention of parafunctional oral habits such as tongue thrust, and of pronunciation. Additionally, few case studies have been reported of space loss in cases of premature loss of primary incisor. This study is to report cases of the space loss following the premature loss of primary incisors observed in children.

  • Tooth impaction is defined as a cessation of the eruption of a tooth at the level of the oral mucosa or alveolar bone by any causes. Any tooth in the dental arch can be impacted, but the teeth frequently involved in a descending order are the mandibular and maxillary third molars, the maxillary canines, the mandibular and maxillary second premolars, and the maxillary central incisors. In these teeth, impaction of maxillary incisor occurs in about 0.1-0.5% and major causes are trauma, supernumerary teeth and periapical inflammation of primary maxillary incisor. Delayed eruption of a maxillary central incisor results in midline shift, the space's being occupied by an adjacent tooth and different levels of alveolar height. Treatment options are observation, surgical intervention, surgical exposure and orthodontic traction, transplantation and extraction. These cases were about the patients with delayed eruption of maxillary central incisor. We surgically exposed impacted tooth and guided it into normal position by the orthodontic traction. At the completion of traction, the maxillary central incisor was positoned fairly within the arch and complications such as root resorption were not observed.

Correlations Between Mesiodistal Crown Diameters of Permanent Teeth (영구치 근원심 폭경의 상관관계에 관한 연구)

  • Casts of 180 Korean male and female with normal occulsion of early permanent dentition (from dental age of Hellman III C, to IV A) were studied to measure the mesiodistal crown diameters and to calculate the coefficients of correlation between the teeth. From the study, the following conclusions were made: 1. Mesiodistal dimension of maxillary central incisors, canines, first molars and mandibular canines, first premolars, second premolars and first molars of male are larger than that of female. 2. Korean teeth are roughly intermediate between those of American Caucasian and those of American Negro. 3. In both sexes, the relation between the first and second premolars appeared highly correlated not only in the maxillary arch but also in the mandibular arch, and the relation between the central incisor and lateral incisor appeared highly correlated in the mandibular arch. 4. The relation between the maxillary and mandibular first premolars appeared highly correlated in both sex, and the relation between the maxillary canine and mandibular canine in male as well as between the maxillary central incisor and mandibular central incisor in female appeared highly correlated.

  • For the purpose of an estimation of age based on the changes in the human dental cavity caused by increase in age, 1,208 extracted teeth in the parts from central incisors and lateral incisors and lateral incisors to second premolars of upper and lower, right and left side were evaluated and analized all of surface index of pulp cavity. The results are as follows : 1. The surface index of pulp caxities of upper and lower, central and lateral incisors, and tend to decrease regularly as the age increase. So above teeth are more applicable to age estimation than canine and premolars. 2 For the purpose of age estimation by surface index of pulp cavity of central and lateral incisor, linear equations are as follows. Upper central incisor: X=(16.301-Y)/0.12 Upper lateral incisor: X=(16.620-Y)/0.11 Lower central incisor: X=(20.963-Y)/0.16 X=Age Y=Surface index of pulp cavity Correlation coefficient between chronologic age and estimated age is 0.699 3.The least error(3.3 yrs of age)reveals in 41-45 age group, which shows the highest possibility of estimation of age. The highest error(4.1 yrs of age)reveals in 61-65 age group and 56-60 age group.
