Epidemiology and Histopathological Spectrum of Head and Neck Cancers in Bihar, a State of Eastern India

  • Head and neck cancers are amongst the commonest malignancies, accounting for approximately 20% of the cancer burden in India. The major risk factors are tobacco chewing, smoking and alcohol consumption, which are all preventable. This retrospective study presents data from the histopathology register for a five year period from 2002-2006 at Patna Medical College and Hospital, a tertiary care hospital drawing patients from the entire Bihar state, the 3rd most populous state of India with the majority of the population residing in rural areas. Incidence rates based on sex, age, site of lesion, including age standardized incidence rates for males and females, with mean age of presentation, distribution of histological variants and year wise trend were calculated. Out of 455 head and neck neoplasias, 241 were benign while 214 were malignant. The most common age group for all malignant biopsies was 7th decade for males and the 5th decade for females. Malignant cases were commoner in males than females with the male:female ratio of 3.1:1, which was found to be statistically significant by the chi-square (${\chi}^2$) test. The crude rate and age standardized incidence rate was 0.05 and 0.06 per 100,000 population respectively. Squamous cell carcinoma (SCC) contributed about 96% of all cases, with grade I being the most common. Larynx was the most common site for malignancy, the supraglottic region being its most commonly affected sub-site. This observed incidence patterns in the region are a reminder of widespread unawareness, low healthcare utilization with virtually non-existent cancer programs. It also underlines the need to advocate for reliable cost-effective programs to create awareness, for early detection and plan appropriate management strategies. There is a compelling demand for a cancer registry in this region as well as proper implementation of preventive measures to combat this growing threat of cancer, many of whose risk factors are preventable.

Incidence, mortality and survival of gallbladder, extrahepatic bile duct, and pancreatic cancer using Korea central cancer registry database: 1999-2019

  • Backgrounds/Aims: In Korea, pancreatic cancer and "gallbladder and extrahepatic bile duct cancer" were ranked the 8th and 9th most frequent cancers in 2019 and the 4th and 6th most common causes of cancer deaths in 2020, respectively. Methods: This review provides national cancer statistics and secular trends of 207,521 patients with gallbladder (n = 44,178), extrahepatic bile duct (n = 61,856), and pancreatic cancer (n = 101,487) between 1999 and 2019 in Korea. Results: The crude incidence rate in both sexes increased in the gallbladder (2.8 to 5.4 per 100,000), extrahepatic bile duct (3.6 to 9.0), and pancreatic cancer (5.5 to 15.8). The age-standardized incidence rate in both sexes significantly increased in the extrahepatic bile duct (3.7 to 4.1) and pancreatic (5.6 to 7.6) cancers but decreased in gallbladder cancer (2.9 to 2.4). The overall 5-year relative survival rate increased in the gallbladder (21.8% to 30.6%), extrahepatic bile duct (23.1% to 27.5%), and pancreatic (8.5% to 13.3%) cancers. Between 2006 and 2019, the proportion of localized or regional stages remained stable. The proportion of surgical treatment within the first 4 months after diagnosis was relatively higher in the gallbladder (42.2%) and extrahepatic bile duct (45.9%) cancers than in pancreatic cancer (22.2%). Conclusions: The crude incidence and mortality rates of the gallbladder, extrahepatic bile duct, and pancreatic cancer are steadily increasing in Korea, and the prognosis remains poor. Early detection, active application of surgical treatment, and minimization of the proportion of untreated patients are required to improve the survival rates of these cancers.

Spatial Analysis of Skin Cancer Incidence in Iran

  • Skin cancer is one of the most common cancers worldwide, including in Iran. Variations in its incidence rate among geographical areas are due to various contributing factors. Since there has been a lack of studies on this topic in our country, the present spatial analysis of skin cancer incidence in Iran in 2009 was conducted using data from the cancer registry system for the country. The reported incidences of the disease were standardized on the basis of the World Health Organization population and the direct method. Then the data were inserted into the GIS software, and finally, using the analysis of hot spots (Getis-Ord Gi), high-risk areas were drawn. Provinces that were 1.9 SD higher or lower than the national average were considered hot spots or cold spots, with significance at the level of 0.05. In 2009, a total of 9,964 cases of skin cancer occurred, 3,696 in women and 6,268 in men (standardized incidence rates of 15.8 and 22.6, respectively). The results of the study showed that in men and women, the disease demonstrated high incidence in the central provinces and desert regions. In women, Yazd Province and in men, Qom Province had significant hot spots (p <0.05). While Isfahan, Markazi, Tehran and Kurdistan provinces were expected to be hot spots, the differences from the national average were not significant at the 0.05 level. As well, the provinces of Sistan Va Baluchistan, Kerman, and Hormozgan were identified as cold or low-risk disease regions (p <0.05). The central provinces of the country due to hot weather conditions, more solar radiation, and closer vicinity to the central desert of Iran demonstrated higher incidence rates for skin cancer, so further epidemiological studies into the etiology and early detection are essential in these areas.

Malignant Tumors of the Central Nervous System in Kazakhstan: Component Analysis of Incidence Dynamics

  • The paper presents the incidence rates of malignant tumors of the central nervous system assessed by the component analysis. The data on primary registered cases of malignant tumors of the central nervous system in the country were used as the material of the study for the period from 2004 to 2011. A general trend of increase in the number of patients with malignant tumors of the central nervous system in Kazakhstan was determined and the potential of their increase was evaluated, which can be due to changes in the morbidity risk and age specifics, as well as the increase in population.

  • Esophageal cancer (EC) incidence rates in Kazakhstan were assessed by component analysis based on primary registered cases in 2001-2010. It was found that despite an apparent general decrease in the number of EC patients in Kazakhstan, a potential increase should be evaluated, due to changes in aging as well as the increase in population. Some problems of EC patients' registration were broached with an emphasis on the importance of the expected absolute number and reasons for undercounting in the country. Based on these, ways of improving the recording and registration of such patients in the country were suggested.

Incidence of Fusarium and other Molds in Korean Field Crops

  • The incidence of total molds, Fusarium species, and the estrogenic mycotoxin,zearalenone, in Korean grain samples were investigated . The majority of molds infecting grain were identified as belonging to the genus Alternaria , with an average infection rate of kernels of 43% and 32% in rice and baley, respectively. Fusarium speciens were less common, with average infection rates of 13% and 19% in reice and barley, respectively. A number of field fungi including Curvularia and Dactylaria were also observed. Among the Fusarium speices, 71 of 94 Fusarium isolates were identified as F.semitectum. A few F. moniliforme and F. equiseti were observed linked immunosorbent assay (ELISA) or high-performance liquid chromatography(HPLC). In addition, deoxynivalenol was not deteted by ELISA . However, thepresence of molds, including Fusarium species, may pose possbile health hazards to persons consuming those grains.

  • The aim was to evaluate different results from using crude incidence rate (CIR) instead of agestandardized incidence rate (AIR) when comparing groups having different age structures. After selecting a report using CIRs, AIRs and their 95% confidence intervals(CI) was calculated from the raw data. The statistically significant difference between CIR and AIR was decided based on AIR's 95% CI. Comparing with CIRs, AIRs were under-estimated. In addition, there were no statistical significance of annual trends(P=0.59). These findings are an additional evidence using AIR when comparing level of occurring infectious diseases on groups with different age structures.

  • Objective: Treatment with sulfonylureas in combination with metformin improves glycemic control in type 2 diabetes mellitus (T2DM), but is associated with hypoglycemia and weight gain. This retrospective study aims to compare the effectiveness of dipeptidylpeptidase-4 (DPP-4) inhibitors and sulfonylureas as an add-on therapy to metformin in patients with T2DM. Methods: Data from medical records of 355 T2DM patients received therapy either DPP-4 inhibitors (DPP-4 inhibitor group) or sulfonylurea (SU group) in combination with metformin from 1 March 2009 to 30 September 2011 were retrospectively reviewed. Of total 355 patients, 231 patients were in DPP-4 inhibitor group and 124 patients were in SU group. Baseline Hemoglobin $A_{1c}$ ($HbA_{1c}$) level in SU group was higher than DPP-4 inhibitor group with a statistically significant difference (8.6% vs. 7.8%). Comparative analysis between DPP-4 inhibitor group and SU group was performed for $HbA_{1c}$ values, amounts of $HbA_{1c}$ changes, and rates of $HbA_{1c}$ changes from baseline at 6-month intervals and incidence rates of major cardiocerebral events. Results: SU group showed larger $HbA_{1c}$ changes in both amounts and rates compared to DPP-4 inhibitor group, although statistical significance was not found in all study periods. Proportions of patients with stable $HbA_{1c}$ <6.5% or 7% were significantly higher in DPP-4 inhibitor group than SU group (<6.5%: 30.4% vs. 13.4%, <7%: 72.3% vs. 41.2%). Time to achieve stable $HbA_{1c}$ <6.5% was not significantly different, but time to achieve stable $HbA_{1c}$ <7% was shorter in DPP4 inhibitor group than SU group with a significant difference. The incidence rate of cardiocerebral events in group of patients with or without previous events was 1.7%, not significantly lower than that in DPP-4 inhibitor group (4.0%). For newly encountered cardiocerebral events during the treatment, incidence rates of two groups did not differ significantly. Conclusion: DPP-4 inhibitors were as effective as sulfonylureas in achieving the $HbA_{1c}$ goal of less than 6.5% or 7% and cardiocerebral event rates did not differ between the two drugs.

한국(韓國)에 있어서의 요충감염상(蟯虫感染相) (Pattern on Enterobius vermicularis in Korea)

  • Enterobius vermicularis (pinworm) infestation in present day Korea has become one of nationwide problems. But, the incidence of pinworm infestation having been reported by many workers was almostly the data for specific groups. The incidence of pinworm infestation for 208 families of 760(male; 30, female;420) middle class inhabitant of Seoul, 238 subrural inhabitants in Gyeonggi Province and 150(male;77, female;73) rural primary students in three different districts were examined by modified Gralam's scotch-tape swab method. The results are summarized as follows; 1. The incidence of nationwide infestation a. The nationwide incidence of pinworm infestation of the 1,148 specimens examined was 28.7% (male; 27.5%, female; 29.6%). b. The highest incidence was found among 5-14 years of age. c. In comparison of the incidence by the area, the highest incidence was found among subrural area, next among rural area and the lowest among urban area. 2, Urban area a. The incidence of the 760 inhabitants examined was 23.2% (male; 24.1%, female; 22. 3%). b. The incidence of the parents group was 12.2% and children group was 30. 30.3%. c. In the parents who hold one or more pinworm poitive cases among children, the incidence of father group was 13.2% and mother group was 29.7%.3. Subrural area a. The incidence of the 238 inhabitants examined was 42.9% (male, 40.8%, female; 44.4%). b. The cumulative positive rate of anal swabs showed increasing tendency. c. Worm collection rates after chemotherapy of 98 inhabitants were 54, 0%. d. In scotch tape anal swab only one times, diagnostic value was 74.0%. e. Mother group infested with pinworm seemed to play an important role for the high incidence in individual family. f. In comparison of the incidence by the examination method, the highest incidence was found among scotch tape method. 4. Rural area The incidence of the 150 primary students examined was 34. 5% (male ; 24. 7%, female; 43.8%).

신경외과 병동에 적용한 Vancomycin 내성 장구균의 감염관리 전략효과 및 균집락의 위험인자 분석 (Effects of Infection Control Strategies & Analysis of Risk Factors for Vancomycin Resistant Enterococci)

  • Objectives: This study was intended to check if the "Creating Clean Wards" project, which is an innovative reinforced campaign activity targeting infection control strategies and active surveillance cultures for VRE (vancomycin resistant enterococci) high-risk patients to be admitted in the NS (neuro-surgery) wards, would be reduced the incidence rates of VRE acquisition, transmission rates. Methods: 75 subjects of the VRE high-risk patients were surveyed by carrying out active surveillance cultures of VRE colonization 11 times from January to March, 2012. And the retrospective study was conducted dividing them into two groups. Results: The incidence rates of VRE acquisition was reduced to 3.67 cases per 1,000 patients day in the control group and to 2.88 cases in experimental group, which was not statistically significant (p = .753). VRE transmission rates of 0.0015 per day before the project tended to increase to 0.0019, although not statistically significant (p = .650). As a result of multivariate analysis with regard to using glycopeptide antibiotics in order to find out risk factors of VRE colonization, the patients who had been treated with glycopeptide until VRE colonization showed 274.41 times higher rate. Conclusion : For effective VRE infection control in NS wards, We should carry out active surveillance culture regularly, especially patient of using glycopeptide. And block the spread of VRE by strengthening infection control through the strict isolation and the changed mind-set of members motivated by the "Creating Clean Wards" campaign.
