임플란트 고정성 보철물 사이에서의 인접면 접촉상실: 증례보고 (Interproximal contact loss between implant-supported prostheses: a clinical report)

  • 부분 무치악 환자가 임플란트 고정성 보철물 치료를 받은 경우 interproximal contact loss (ICL)은 가장 빈번하게 발생하는 합병증이다. 일반적으로 보고되는 임플란트 보철물의 ICL은 자연치의 이동에 따른 임플란트 보철물과 자연치 사이에서 발생하는 경우가 대부분이다. 임플란트 보철물과 자연치 사이에서 발생하는 ICL의 원인은 여러가지가 있다. 임플란트 보철물 사이에서 발생하는 ICL에 대한 보고는 거의 없으며, 이는 임플란트 보철물과 자연치 사이에서 발생하는 ICL의 원인만으로는 설명하기 어렵다. 본 증례에서는 임플란트 보철물 사이에서 발생하는 ICL을 증례보고하고, 이에 대한 원인 및 대처방법을 논의하고자 한다.

  • 골흡수가 심한 환자에서 임플란트를 이용한 전악 수복의 경우 치은과 치아의 외형을 심미적이면서도 기능적으로 회복하기 위해서는 보철물 제작에 있어 신중을 기해야 한다. 이러한 전악 수복 증례에서 CAD/CAM 시스템이 높은 정확성을 가지는 보철물 제작을 가능하게 함으로써 사용이 증가하고 있다. 최근에 임상에서 티타늄이나 지르코니아의 단점을 보완하면서 높은 생체적합성을 가지고 CAD/CAM 시스템으로 제작할 수 있는 팔라듐-은(Pd-Ag) 합금이 개발되어 사용되고 있다. 본 증례에서는 상악 전부 무치악 환자에서 임플란트를 식립하고 도재의 chipping이나 파절의 위험을 감소시키기 위해 Pd-Ag 합금을 이용해 제작된 임플란트 고정성 보철물로 수복한 환자에 대해 보고하고자 한다.

  • 부분 무치악 환자에서 구강 기능의 회복을 위한 보철 계획을 수립할 때, 잔존하는 치아에 따라 다양한 치료 방법을 선택할 수 있다. 특히 대합이 상실된 소수의 잔존치가 불안정한 교합을 야기하는 엇갈린 교합의 경우, 대합 되는 위치에 임플란트를 식립함으로써 교합지지를 회복할 수 있다. 이 증례에서는 발치 후 엇갈린 교합이 된 환자에서 의치의 하중을 견딜 수 있도록 상악 잔존치에 대응하는 하악 무치악 부위에 임플란트를 식립하고 자석유지장치(magnetic attachment)를 연결하는 국소의치를 제작하였다. 또한 상악 전치의 돌출과 하악 입술 지지도가 부족한 것을 개선하고 싶어한 환자의 요구사항에 따라 상악 전치부 보철적 수복 및 하악 의치를 통해 심미적 안모를 회복하였다. 5개월 정기검진 결과, 환자는 저작 기능 및 심미적인 면에서 만족하였고 의치의 안정성 또한 잘 유지되고 있기에 이를 보고하고자 한다.

Mechanical and biological complication rates of the modified lateral-screw-retained implant prosthesis in the posterior region: an alternative to the conventional Implant prosthetic system

  • PURPOSE. The modified lateral-screw-retained implant prosthesis (LSP) is designed to combine the advantages of screw- and cement-retained implant prostheses. This retrospective study evaluated the mechanical and biological complication rates of implant-supported single crowns (ISSCs) inserted with the modified LSP in the posterior region, and determined how these complication rates are affected by clinical factors. MATERIALS AND METHODS. Mechanical complications (i.e., lateral screw loosening [LSL], abutment screw loosening, lateral screw fracture, and ceramic fracture) and biological complications (i.e., peri-implant mucositis [PM] and peri-implantitis) were identified from the patients' treatment records, clinical photographs, periapical radiographs, panoramic radiographs, and clinical indices. The correlations between complication rates and the following clinical factors were determined: gender, age, position in the jaw, placement location, functional duration, clinical crown-to-implant length ratio, crown height space, and the use of a submerged or nonsubmerged placement procedure. RESULTS. Mechanical and biological complications were present in 25 of 73 ISSCs with the modified LSP. LSL (n=11) and PM (n=11) were the most common complications. The incidence of mechanical complications was significantly related to gender (P=.018). The other clinical factors were not significantly associated with mechanical and biological complication rates. CONCLUSION. Within the limitations of this study, the incidence of mechanical and biological complications in the posterior region was similar for both modified LSP and conventional implant prosthetic systems. In addition, the modified LSP is amenable to maintenance care, which facilitates the prevention and treatment of mechanical and biological complications.

  • 방사선치료를 받은 상악에 임플란트 지지 고정성 보철물을 이용하여 수복하는 것은 골치유능력이 낮기 때문에 상세한 치료계획이 필요하다. All-on-4 개념에 의한 임플란트 식립은 골이식을 피하면서 임플란트를 매식할 수 있어 유리하다. 일반적으로 경사된 임플란트에는 기성 경사형 지대주를 사용해왔다. 본 임상증례에서는 computer-aided design and computer-aided manufacturing (CAD/CAM)으로 제작된 지대주를 사용하였다. 본 증례는 all-on-4 개념에 의해 임플란트를 매식하고 CAD/CAM titanium 지대주를 제작한 다음 CAD/CAM zirconia 고정성 보철물로 수복하여 좋은 결과를 얻었기에 보고하고자 한다.

  • The term, 'crossed occlusion' implies clinical situation in which the residual teeth in one arch have no contact with those in the antagonistic arch, resulting in the collapse of occlusal vertical dimension. The treatment goal of this pathologic condition is restoration of the collapsed vertical dimension and stabilization of abnormal mandibular position. Previously, konus removable prostheses or tooth supported overdentures were suggested to solve crossed occlusion. Nowadays, dental implants have been used for definitive support to solve this problem. In this case report, a 65 years old female patient had a crossed occlusion, in which the maxillary posterior residual teeth and mandibular anterior residual teeth cross. Interim removable and fixed dental prostheses were used to confirm the proper vertical and horizontal jaw relation. After that, the mandibular posterior edentulous region was restored with implant-supported fixed dental prostheses. Computer tomography guided implant surgery was performed according to the concept of the restoration-driven implant placement. The maxillary anterior edentulous region was restored with Kennedy class IV removable prosthesis, considering the patient's economic status. The patient's jaw position and prostheses have been well maintained at the follow-up after 6 months of definitive restoration. The antero-posterior crossed occlusion problems appeared to be effectively solved with the combination of removable in one arch and implant-supported fixed prostheses in the other.

A COMPARISON OF THE MASTICATORY FUNCTION BETWEEN TWO DIFFERENT TYPES OF IMPLANT SUPPORTED PROSTHESES AND COMPLETE DENTURE FOR FULLY EDENTULOUS PATIENTS

  • STATEMENT OF PROBLEM: The improvement in oral function and comfort from the dental implant appears to depend on the particular type of implant support used with the denture. The number and positioning of implants have an influence on the force transfer and subsequent stress distribution around implants. Nevertheless, a quantitative comparison has not been made between the types of implant prosthesis used with different materials compared to conventional complete denture. PURPOSE: The objective of this study is to assess the masticatory performance, bite force and impact of two different type of implant supported prostheses on oral health-related quality of life compared to conventional complete denture with GOHAI, validated oral-specific health status measures, the sieving method, and the Prescale Dental System. MATERIAL AND METHODS: From the years 1999 to 2006, a total of 30 completely edentulous patients in a single arch were selected from the Yonsei University Dental Hospital, Department of Prosthodontics and Implant Clinic in Seoul, S. Korea. Patients were divided into 3 groups of 10 each. Group HR was restored with fixed-detachable hybrid prostheses with resin teeth. Group FP had fixed dentures with porcelain teeth while Group CD had a complete denture. The masticatory performance was compared between 3 groups. RESULTS: The results showed a significant improvement in oral health-related quality of life with dental implants compared to a conventional denture in GOHAI comparison. Overall, implant prostheses showed a higher masticatory performance ($S_{50}$) and maximum bite force compared with conventional dentures (P < .05) but no differences between different implant supported prostheses (P > .05). CONCLUSION: Within the limitation of this study, the numbers of implant and material of implant prostheses does not appear to impact patient satisfaction, masticatory performance or bite force.

  • The introduction of osseointegrated dental implants in dentistry brought about a new era in everyday dental practice. For the past 50 years, prosthetic restoration with implant-supported prosthesis has developed into a viable and predictable treatment option. Alongside the increasing use of dental implants is the occurrence of many complications during implant placement (surgery), in the mechanical or prosthetic problem, and in the biological aspect. In particular, abutment or screw fracture as one of the mechanical complications can put the dentist in a tight spot in a clinical situation. It is hard to remove the fractured abutment and screw to restore it properly. Therefore, it is very important that clinicians consider possible complications in advance and make an appropriate treatment plan. We discuss cases of abutment fracture and mechanical/prosthetic complications together with the causes and solutions.

Comparison of immediate complete denture, tooth and implant-supported overdenture on vertical dimension and muscle activity

  • PURPOSE. To compare the changes in the occlusal vertical dimension, activity of masseter muscles and biting force after insertion of immediate denture constructed with conventional, tooth-supported and Implant-supported immediate mandibular complete denture. MATERIALS AND METHODS. Patients were selected and treatment was carried out with all the three different concepts i.e, immediate denture constructed with conventional (Group A), tooth-supported (Group B) and Implant-supported (Group C) immediate mandibular complete dentures. Parameters of evaluation and comparison were occlusal vertical dimension measured by radiograph (at three different time intervals), Masseter muscle electromyographic (EMG) measurement by EMG analysis (at three different positions of jaws) and bite force measured by force transducer (at two different time intervals). The obtained data were statistically analyzed by using ANOVA-F test at 5% level of significance. If the F test was significant, Least Significant Difference test was performed to test further significant differences between variables. RESULTS. Comparison between mean differences in occlusal vertical dimension for tested groups showed that it was only statistically significant at 1 year after immediate dentures insertion. Comparison between mean differences in wavelet packet coefficients of the electromyographic signals of masseter muscles for tested groups was not significant at rest position, but significant at initial contact position and maximum voluntary clench position. Comparison between mean differences in maximum biting force for tested groups was not statistically significant at 5% level of significance. CONCLUSION. Immediate complete overdentures whether tooth or implant supported prosthesis is recommended than totally mucosal supported prosthesis.

교정적 치아 재배열을 포함한 다학제적 접근(multidisciplinary approach)을 통한 상악 전치부의 나사 유지형 임플란트 보철 수복 증례 (Cases of screw-retained implant prosthesis in the anterior maxilla through multidisciplinary approach, including orthodontic teeth alignment)

  • 상악 전치부에서 임플란트의 순구개측 식립 위치는 보철물의 심미성을 결정하는 중요한 요소이다. 그러나 상악 전치부의 얇은 순측골은 발치 후에 상당한 양이 흡수되어, 임플란트가 이상적 식립 위치보다 구개측에 식립될 수 있다. 본 상악 중절치 상실 증례들에서 다소 구개측에 식립된 임플란트의 보철 수복을 위하여 UCLA 지대주를 이용하였다. 또한 타과와 협진으로 발치 후 흡수된 치조골 외형과 치은의 형태 회복을 위하여 골유도재생술을 시행하였고, 인접전치를 재배열 하였다. 충분한 임시 보철물 수복 기간으로 치은 형태의 심미적 회복을 확인한 뒤에 최종 보철 시행하였으며, 임플란트 보철을 통한 치아 상실 부위의 기능, 심미 회복이 만족할 만한 결과를 보였다.
