무절개 수술을 이용한 맞춤형 지르코니아 지대주의 즉시 장착 증례 (Immediate Connection of Customized Zirconia Abutment Using Flapless Guided Surgery: A Clinical Report)

  • 임플란트 주위조직과 치아 주위조직은 조직학, 형태학적으로 유사한 면이 있다. 특히 functional ankylosis라 불리는 티타늄과 연조직의 직접적인 상피부착은 치아주위 조직의 접합상피처럼 염증세포의 치근단으로의 이행 및 그로 인한 골흡수를 막는 역할을 한다. 그러나 반복적인 지대주의 착탈은 임플란트 주위 연조직의 mucosal barrier를 파괴하여 골흡수를 야기할 수 있다. 만일 지대주의 반복적인 착탈 없이 수술 시에 보철 지대주를 채결한다면 골흡수의 양을 줄일 수 있을 것이다. 이는 기존의 수술방법으로는 한계가 있었으나 Cone Beam Computed Tomography(CBCT)를 이용한 guided surgery를 이용함으로써 임플란트 식립의 3차원적 정확성의 비약적인 향상으로 이런 술식이 가능하게 되었다. 본 증례는 정밀한 CBCT를 통한 분석 후 지르코니아 맞춤 지대주를 미리 제작하고 수술 시에 즉시 지대주와 임시보철물을 연결한 경우로 임상적으로 만족할만한 결과를 얻어 이를 보고하는 바이다.

다양한 마이크로쓰레드(Micro thread)의 개수를 가지는 임플란트의 상부구조물 형상과 하중조건에 따른 3차원 유한요소해석을 이용한 하악골의 응력분포에 관한 연구 (Three-dimensional Stress Analysis of Implant Systems with Micro Threads in the Maxillary Bone)

  • A comparative study of stress distributions in the maxillary bone with three different types of abutment was conducted. Finite element analysis was adopted to determine stress generated in the bone with the different implant systems with micro threads (Onebody type implant, Internal type implant, and External type implant). It was found that the types of abutments and the number of micro threads have significant influence on the stress distribution in the maxillary bone. They were due to the difference in the load transfer mechanism and the size of contact area between abutment and fixture. Also the maximum effective stress in the maxillary bone was increased with increasing inclination angle of load. It was concluded that the maximum effective stress in the bone was the lowest by the internal implant among the maximum effective stresses by other two types of implants and by appropriate number of micro threads, and that the specific number of micro thread was existed to decrease the maximum effective stress in the maxillary bone due to different implant systems and loading conditions.

Comparison of CAD/CAM abutment and prefabricated abutment in Morse taper internal type implant after cyclic loading: Axial displacement, removal torque, and tensile removal force

  • PURPOSE. The purpose of this study was to compare computer-aided design/computer-aided manufacturing (CAD/CAM) abutment and prefabricated abutment in Morse taper internal connection type implants after cyclic loading. MATERIALS AND METHODS. The study was conducted with internal type implants of two different manufacturers (Group Os, De). Fourteen assemblies were prepared for each manufacturer group and divided into 2 groups (n=7): prefabricated abutments (Os-P, De-P) and CAD/CAM abutments (Os-C, De-C). The amount of axial displacement and the removal torque values (RTVs) were measured before and after cyclic loading (106 cycles, 3 Hz with 150 N), and the tensile removal force to dislodge the abutments was measured after cyclic loading. A repeated measures ANOVA and a pattern analysis based on the logarithmic regression model were conducted to evaluate the effect of cyclic loading on the axial displacement. The Wilcoxon signed-rank test and the Mann-Whitney test was conducted for comparison of RTV reduction% and tensile removal forces. RESULTS. There was no significant difference between CAD/CAM abutments and prefabricated abutments in axial displacement and tensile removal force; however, significantly greater RTV reduction% after cyclic loading was observed in CAD/CAM abutments. The correlation among the axial displacement, the RTV, and the tensile removal force was not significant. CONCLUSION. The use of CAD/CAM abutment did not significantly affect the amount of axial displacement and tensile removal force, but presented a significantly greater removal torque reduction% than prefabricated abutments. The connection stability due to the friction at the abutment-implant interface of CAD/CAM abutments may not be different from prefabricated abutment.

임플란트 보철물의 연결 여부에 따른 유한요소응력분석 (Finite Element Analysis on the Supporting Bone according to the Connection Condition of Implant Prosthesis)

  • The purpose of this study was to compare the stress distribution according to the splinting condition and non-splinting conditions on the finite element models of the two units implant prostheses. The finite element model was designed with the parallel placement of two fixtures ($4.0mm{\times}11.5mm$) on the mandibular 1st and 2nd molars. A cemented abutment and gold screw were used for superstructures. A FEA models assumed a state of optimal osseointegration, as the bone quality, inner cancellous bone and outer 2 mm compact bone was designed. This concluded that the cortical and trabecular bone were assumed to be perfectly bonded to the implant. Splinting condition had 2 mm contact surface and non-splinting condition had $8{\mu}m$ gap between two implant prosthesis. Two group (Splinting and non-splinting) were loaded with 200 N magnitude in vertical axis direction and were divided with subdivision group. Subdivision group was composed of three loading point; Center of central fossa, the 2 mm and 4 mm buccal offset point from the central fossa. Von Mises stress value were recorded and compared in the fixture-bone interface and bucco-lingual sections. The results were as follows; 1. In the vertical loading condition of central fossa, splinting condition had shown a different von Mises stress pattern compared to the non-splinting condition, while the maximum von Mises stress was similar. 2. Stresses around abutment screw were more concentrated in the splinting condition than the non-splinting condition. As the distance from central fossa increased, the stress concentration increased around abutment screw. 3. The magnitude of the stress in the cortical bone, fixture, abutment and gold screw were greater with the 4 mm buccal offset loading of the vertical axis than with the central loading.

맞춤형 지르코니아 지대주와 완전도재관을 이용한 상악 전치부 임플란트 보철 수복 (Reconstruction of upper anterior by implant using customized zirconia abutment and all ceramics: a clinical report)

  • 상악 전치부에 임플란트를 이용해 심미적으로 수복 하는 것은 많은 어려움이 따른다. 이는 치조골 흡수와 치간 유두 수축 등과 같은 임플란트 주변 경, 연조직 문제와 금속지대주 및 금속도재관의 변연노출과 낮은 광투과도 등의 보철적 한계 때문이다. 이에 본 증례에서는 경, 연조직의 증대술 및 성형술, 발치 후 즉시 식립 등과 함께 임시수복물의 장기간 사용 및 수정으로 임플란트 주위 연조직의 형태와 건강을 형성한 후, 맞춤형 인상 코핑을 사용하여 이를 최종 모형에 재현하였다. 또한 맞춤형 지르코니아 지대주와 완전 도재관을 이용해 최종 수복함으로써 기능적이고 심미적인 결과를 얻었기에 이를 보고하고자 한다.

A STUDY ON SURFACE OF VARIOUS ABUTMENT SCREWS

  • Statement of problem: Regardless of any restoration, most of case, we used in screw connection between abutment and implant. For this reason, implant screw loosening has been remained problem in restorative practices. Purpose: The purpose of this study was to compare surface of coated/plated screw with titanium and gold alloy screw and to evaluate physical property of coated/plated material after scratch test in FESEM investigation Material and methods: GoldTite, titanium screw provided by 3i (Implant Innovation, USA) and TorqTite, titanium screw by Steri-Oss (Nobel Biocare, USA) and gold screw, titanium screw by AVANA (Osstem Implant, korea) - were selected for this study. Each abutment screw surface was observed at 100 times, and then screw crest, root, and slope were done more detailed numerical value, at 1000 times with FESEM. A micro-diamond needle was also prepared for the scratch test. Each abutment screw was fixed, micro-diamond scratch the surface of head region was made at constant load and then was observed central region and periphery of fine trace through 1000 times with FESEM. Results: The surface of GoldTite was smoother than that of other kinds of screw and had abundant ductility and malleability compared with titanium and gold screw. The scratch test also showed that teflon particles were exfoliated easily in screw coated with teflon. Titanium screw had a rough surface and low ductility. Conclusion: It was recommended that the clinical use of gold-plated screw would prevent a screw from loosening. CLINICAL IMPLICATIONS Clinical use of gold-plated screw would prevent a screw from loosening because it had abundant ductility and malleability compared with titanium and gold screw.

양측성 후방연장 임플란트 보조 국소의치의 합병증과 관리: 증례보고 (Complication and management of implant-assisted removable partial denture with distal extension: a clinical report)

  • 임플란트를 이용한 임플란트 지지 국소의치(Implant supported removable partial denture)는 기존 가철성 국소의치의 생체역학적 한계를 보완할 수 있어 무치악 환자의 유용한 치료로 활용된다. 기계적, 생물학적 합병증을 예방하고 장기간 적절한 예후를 위해서는 정기적인 내원을 통한 관리가 매우 중요하다. 본 증례보고에서 치유 지대주를 이용한 양측성 후방연장 임플란트 지지 국소의치에서 발생한 기계적 합병증 및 관리에 대해 기술하고자 한다. 임플란트 국소의치 장착 36개월 후 치유 지대주의 마모 및 파절이 발생하였고, 이에 임플란트 유지 국소의치로 전환하기 위하여 임플란트의 치유 지대주를 $Locator^{(R)}$로 교체하였다. 환자는 기능 및 심미적으로 만족할 만한 결과를 얻었기에 이를 보고하는 바이다.

치은연하 변연을 가지는 맞춤형 지대주에서 디지털 중첩기술을 이용한 인상채득술의 임상 적용 증례 (Clinical accuracy of impression technique using digital superimposition of customized abutment with subgingival margin: A case report)

  • 임플란트 상부 보철물 제작 시 치은연하 변연을 가지는 맞춤형 지대주의 인상채득을 위해 전통적으로 치은 압배술이 실시되어 왔다. 하지만 치은 압배술은 치은 퇴축 등의 부작용이 있으며 출혈은 부정확한 인상채득의 원인이 될 수 있다. 본 증례에서는 이러한 문제점들을 해결하기 위해 보철물을 제작할 때 중첩 가능하도록 제작된 맞춤형 지대주를 이용하는 새로운 방법을 적용하였다. 지대주를 임플란트에 연결하기 전 형상데이터를 저장하고, 임플란트에 연결 후 치은 압배 과정 없이 광학 인상채득한 뒤 두 데이터를 중첩하여 상부보철물을 제작하였다. 치은 연하 변연을 가지는 임플란트 보철물 제작 시 심미성을 만족시키면서 기존의 인상채득 방식보다 빠르고 간편하며 우수한 변연 및 내면적합도를 보였기에 임상적 효용성에 대해 보고하고자 한다.

  • Statement of problem. Higher stresses at the cervical bone around dental implants have been seen as a primary cause of the bone resorption at the site. Purpose : To determine the possibility of stress reduction by assembly of different abutment and implant in diameters. Material and methods. Abutments of several different diameters assembled on the top of XiVE$^{(R)}$ implants were axisymmetrically modeled for a series of finite element analyses. Abutments of 3.4, 3.8, 4.5, and 5.5 mm diameters were assumed to be sit on implants of the same or bigger diameters. All the abutments with an exception of 3.4mm dia, are technically possible to be assembled on bigger implants. Main consideration was given to the stresses at the cervical cortical bone induced by loads of parallel to the implant axis. Results and conclusions. 1. Higher stresses were observed at the cervical area of all the models of the same diameters of abutment and future. The peak stresses, which were shown to be a function of the fixture diameter, were from 1-1.85MPa. 2. Difference in the diameters of the abutments and the implants actually reduced the cervical bone stresses. 3. Downsizing of the abutment by one step resulted in 0.1MPa (5%) reduction of the stresses. In light of the relatively lower bone stress, however, this amount of stress reduction was decided to be biomechanically insignificant.

Effect of abutment neck taper and cement types on the amount of remnant cement in cement-retained implant restorations: an in vitro study

  • PURPOSE. The present study aims to analyze the effect of abutment neck taper and types of cement on the amount of undetected remnant cement of cement-retained implant prostheses. MATERIALS AND METHODS. Three neck taper angles (53°, 65°, 77°) and three types of cement (RMGI: resin-modified glass ionomer, ZPC: zinc phosphate cement, ZOE: zinc oxide eugenol cement) were used. For each group, the surface percentage was measured using digital image and graphic editing software. The weight of before and after removing remnant cement from the abutment-crown assembly was measured using an electronic scale. Two-way ANOVA and Duncan & Scheffe's test were used to compare the calculated surface percentage and weight of remnant cement (α = .05). RESULTS. There were significant differences in remnant cement surface percentage and weight according to neck taper angles (P < .05). However, there were no significant differences in remnant cement surface percentage and weight on types of cement. No interaction was found between neck taper angles and types of luting cement (P > .05). The wide abutment with a small neck taper angle showed the most significant amount of remnant cement. And the types of luting cement did not influence the amount of residual cement. CONCLUSION. To remove excess cement better, the emergence profile of the crown should be straight to the neck taper of the abutment in cement-retained implant restoration.
